Clinical Application of Fast-Track Surgery with chinese medicine treatment in the Devascularization Operation for Cirrhotic Portal Hypertension

Objective： To investigate the clinical effect of fast-track surgery combined with chinese medicine treatment in devascularization operation for cirrhotic esophageal varices. Methods： Seventy-two patients with cirrhotic esophageal varices were selected from January 2009 to June 2013, and randomly assigned to a conventional group and a fast-track group （fast-track surgery combined with chinese medicine treatment） using a randomized digital table, 36 cases in each group. Operation and anesthesia recovery time, postoperative hospitalization and quality of life were recorded and compared between groups during the perioperative period. Results： Compared with the conventional group, the fast-track group had longer operation time （253.6±46.4 min vs. 220.6±51.0 min） and anesthesia recovery time （50.5 ± 15.9 rain vs. 23.5± 9.6 min; P〈0.01）; less bleeding （311.3±46.8 mL vs. 356.2 ± 57.5 mL; P〈0.01） and less transfusion （1932.3 ± 106.9 mL vs. 2045.6 ± 115.4 mL; P〈0.01）; as well as faster recovery of gastrointestinal function, shorter postoperative hospitalization and higher quality of life. There were no serious postoperative complications and no further bleeding occurred. Conclusion： Fast-track surgery combined with chinese medicine treatment is a safe and feasible approach to accelerate the recovery of patients with cirrhotic portal hypertension in perioperative period of devascularization operation.

Tourette syndrome is a common neuropsychiatric disorder that affects the physical and mental health of *** detection,diagnosis,and treatment are crucial to prevent serious impacts on the affected children,their families,and *** recent years,there has been an increasing trend towards using a combination of methods in the clinical treatment of children with Tourette *** approach has achieved remarkable results,leading to a reduction in the rate of Tourette syndrome symptoms in *** recent years,a combination of clinical methods has been used to treat children with Tourette syndrome,resulting in significant improvement in control *** article reviews the etiology of infantile Tourette syndrome and the progress made in chinese and Western medicine treatments,providing a reference for further treatment of the condition.

chinese medicine in the treatment of intracranial vascular stenosis in 1 case

Intracranial vascular stenosis is closely related to transient ischemic attack （TIA） and cerebral infarction, early prevention and treatment of intracranial vascular stenosis can prevent cerebral infarction and improve the prognosis of stroke. Western medicine employs interventional surgery or vasodilator, antiplatelet drugs, lipid lowering drugs, plaque-stabilizing agents and other drugs for treating intracranial vascular stenosis. No corresponding disease name with “intracranial vascular stenosis” is found in chinese medicine literature. Based on the characteristics of syndromes, symptoms and diseases, “intracranial vascular stenosis” is summarized as weakened body resistance and prevailed pathogenic factors, phlegm with blood stasis and “obstruction （Bi） syndrome” in the brain. The self-made chinese herbal “Naomaitong” prescription treating for bilateral middle cerebral artery stenosis in 1 case got satisfactory efficacy by transcranial doppler （TCD） and computed tomography angiography （CTA） examination.

Progress in the expression of P-glycoprotein in brain metastases of lung cancer andrelated TCM research

Brain metastases are common intracranial tumors, and their occurrence not only represents a high degree of malignancy, but also often is the major factor in treatment failure and poor prognosis. Primary site of brain metastases often occur in lung. P-glycoprotein is a member of the （ATP binding cassette） transporter superfamily,which is closely related to the development of lung metastases. It is the main reason for influencing the drug through the blood_brain barrier into the brain tissue, and it also is an important factor affecting the treatment of brain metastases. According to the theory of traditional chinese mddicine, the pathogenesis of brain metastases is due to phlegm, poison, stasis, virtual and so on. The principle of treatment is to promote blood circulation, remove phlegm turbidity. In recent years, the impact of chinese herbal medicine on P-glycoprotein is increasing. This paper analyzes the mechanism and components of the relevant chinese medicine on P-glycoprotein. It provides a reference for clinical rational drug use.

Objective:The purpose of this study is to investigate the application of chinese and Western medicine in the health management of school-age children with asthma and its research progress,with a view to providing a more comprehensive and effective management strategy for the ***:Using the method of literature review,this study systematically searched and analyzed the relevant studies on the combination of chinese and Western medicine in the health management of school-aged children with asthma in recent years,focusing on chinese medicine treatment,Western medicine treatment,and the combined health management of chinese and Western ***:chinese medicine treatment,Western medicine treatment,and the health management mode of combining chinese and Western medicine showed unique advantages in school-age asthmatic *** the comprehensive use of chinese medicine’s diagnosis and treatment,chinese medicine conditioning,chinese medicine characteristic therapy,and Western medicine’s modern medical methods,it not only effectively relieved asthma symptoms but also significantly improved the quality of life of the *** the same time,this management mode also focuses on the psychological guidance and family care of the children,forming an all-round and multi-level health management ***:The health management strategy of combining chinese and Western medicine has a broad application prospect in school-age asthmatic children,and its specific interventions and mechanisms of action should be further studied to provide more scientific and systematic guidance for clinical practice.

不孕症的发病率占已婚妇女的10%左右。历代医家对该病证的病因病机与辨证论治做了各自的阐发论述,从理、法、方、药各方面积累了大量相关资料,有些医家将西方医学的相关技术与研究方法,应用于中医诊治不孕症过程中,使不孕症的诊断与治疗水平有了进一步的提高。本文拟以古今医家诊疗经验作为切入点,全面性搜集历代有关治疗肾虚型不孕症的文献,运用中医文献学“辨章学术,考镜源流”的研究分析方法,偏重其辨证论治的探讨,给以系统性整理归纳、总结比较古今有关肾虚型不孕症治疗的临证诊疗经验,为不孕症的现代临床治疗提供新思路,为探索高效方药提供经验依据。女人的生长、发育、生殖、衰老各阶段的生理过程与肾气盛衰紧密相关,女人一些特有的生理现象-经(月经)、带(白带)、胎(胎孕)、产(分娩)、乳(哺乳)等也与肾中精气关系密切。中医学认为肾为先天之本,元气之根,主藏精气,是生长发育生殖之动力。肾中精气的盛衰,主宰着人体的生长发育及生殖功能的成熟和衰退,故肾气盛,天癸至,月经来潮;同时,生殖之精来源于肾,在肾精天癸的滋养下,才能正常孕育。尤其是子宫发育不全之不孕症,与肾的关系更为密切,况且冲任二脉受肾中精气滋养。若肾气不足,必然导致机体发育不良。由此可见子宫发育不良是与肾气不足有着直接的关系,也是导致原发性不孕的重要原因。肾气乃妇女生发之本,对冲任通盛有着密切的关系。冲脉为月经之本,而冲任之本在肾。女子以血为本,以气为用,气血是月经、孕育、哺乳的物质基础。肾藏精,精化血化气,是经、带、胎、产、乳下的先决条件,只有肾气旺盛,经、带、胎、产、乳功能才会正常。古代不孕症文献数据研究显示,在病因病机方面,对体质因素、六淫七情致病、生活所伤的论述至深,而古代医家认为不孕症病因诸多,病理复杂,但责于肝郁、肾虚不孕者居多。因为肝肾同司下焦,肝藏血,肾藏精为冲任之本,肝肾之变,冲任应之,冲任损伤,亦可损及肝肾,如果妇人多郁,情绪易于激动,情志不遂,肝失条达,抑郁不伸,肝气郁结,使气血失调,致冲任不能相资,则久婚不得孕育,故养精血、补肝肾、疏肝气即益冲任之源,源盛自畅,不孕之症而自治。明清以后文献的研究,分成理论研究、临床研究和实验研究三部分进行了归纳整理。现代医家认为,补肾药可增高雌性激素,并可兴奋下视丘、脑下垂体的功能,增加卵巢血液供应,提高卵巢子宫的代谢,从而促进生长发育。另外益肾温煦法有促进排卵、健全黄体的作用。而不孕症与月经病有密切关系,故有“种子先调经,经调孕自成”的理论,因而创立中医周期疗法,提出按月经周期中不同阶段的生理特点而周期性的用中药,使之出现周期性的月经。

目的:通过对槐杞黄颗粒治疗特发性膜性肾病的临床观察,评价其对患者免疫功能的影响,验证槐杞黄颗粒治疗气阴两虚型特发性膜性肾病的有效性和安全性,为特发性膜性肾病的中西医结合治疗提供依据。方法:筛选符合纳入标准的气阴两虚型特发性膜性肾病患者60例,按随机数字表法分为对照组和治疗组,每组各30例,对照组予基础治疗,治疗组在基础治疗上加槐杞黄颗粒治疗,疗程8周,详细记录患者治疗前(0周)、治疗中(第4周)以及治疗后(第8周)的外周血T细胞亚群(CD4+、CD8+、CD4+/CD8+)、免疫球蛋白(IgG、IgM、IgA)、补体C3、24小时尿蛋白定量(24h-Upro)、血清白蛋白(ALB)、血肌酐(Scr)和中医证候积分的变化,运用SPSS 25.0统计软件及相关专业知识分析试验结果,并得出结论。结果:1.一般资料:两组在性别、年龄分布、病理分期、治疗前的化验指标及中医证候积分方面比较,均无统计学差异(P>0.05),具有可比性。2.实验室指标:①T细胞亚群比较,两组在治疗过程中T细胞亚群的均值都呈现不同程度的升高,其中CD8+在治疗后升高更为明显,对照组的治疗中、治疗后分别与治疗前对比,仅治疗后较治疗前有统计学意义(P<0.05);治疗组的治疗中、治疗后较治疗前比较均有统计学意义(P<0.05)。治疗后,两组T细胞亚群组间比较有统计学意义(P<0.05),说明治疗组的疗效更好。②免疫球蛋白比较,两组治疗过程中IgG、IgM、IgA的均值都呈现不同程度的升高,对照组的治疗中、治疗后与治疗前比较,仅治疗后有统计学意义(P<0.05);治疗组的治疗中、治疗后分别与治疗前比较,均有统计学意义(P<0.05);治疗后,两组免疫球蛋白组间进行比较,有统计学意义(P<0.05)。③补体C3比较,两组的数值均有不同程度的升高,对照组的治疗中、治疗后与治疗前比较,仅治疗后有统计学意义(P<0.05);治疗组的治疗中、治疗后分别与治疗前对比,均有统计学意义(P<0.05);治疗后两组之间比较,存在差异(P<0.05),说明治疗组的疗效更好。④两组的24h-Upro 比较,治疗后与治疗前相比数值有所下降,其差异有统计学意义(P<0.05);治疗后,两组之间进行比较有统计学意义(P<0.05),表明在降低24h尿蛋白方面,治疗组的疗效优于对照组。⑤ALB 比较,治疗组、对照组ALB在治疗后较治疗前比较有所升高,存在差异,有统计学意义(P<0.05);治疗后,两组之间存在差异,有统计学意义(P<0.05),说明治疗组的治疗效果更好。⑥Scr 比较,两组治疗后与治疗前比较其相对水平有所降低,存在差异,有统计学意义(P<0.05);两组组间之间存在差异,有统计学意义(P<0.05)。3.中医证候积分及疗效比较:两组治疗后,组间、组内中医证候积分比较,均具有统计学意义(P<0.05)。两组疗效进行比较,与对照组(83.33%)相比,治疗组(86.67%)可明显提高IMN患者的中医疗效(P<0.05),且两组间的临床疗效有差异(P<0.05)。4.安全性方面:观察期间患者安全性指标正常,无不良事件。结论:槐杞黄颗粒能够改善气阴两虚型特发性膜性肾病患者的临床症状,改善T细胞亚群失衡状态,提高免疫球蛋白、血清白蛋白水平,减少24h尿蛋白。

Effect of Yangxue Antai Granule (养血安胎冲剂) in Prevention and treatment of Abortion and Short Life T-Suppressor Cell

Objective: To study the influence of T suppressor cell activity on habitual abortion (HA) and to observe the regulatory effect of Yangxue Antai Granule (YXATG) on T suppressor cell and its effect on fetus preservation. Methods: T suppressor cell activities of 56 pregnant women with HA history were tested with method of short life T suppressor cell activity (MTT method) during early pregnancy, and were followed up in middle and late pregnancy in 20 cases of them. All the 56 patients received YXATG treatment. Groups of normal early pregnant, normal non pregnant women and those non pregnant but with history of habitual abortion were involved in this study as control. Results: T suppressor cell activity in early pregnancy was lower than that of middle and late pregnancy ( P <0.01, P <0.001) and also lower than that of the control groups ( P <0.01, P <0.001). The successful rate of fetus preservation of YXATG was 98.2%, and 98.0% of the cases got normal delivery. Conclusion: Low T suppressor cell activity might be the immunologic etiology of HA. The fetus preservative effect of YXATG might be due to its regulatory effect on T suppressor cell activity.
