Association between PPARG genetic polymorphisms and ischemic stroke risk in a northern Chinese Han population: a case-control study

Two common polymorphisms of the peroxisome proliferator-activated receptor gamma(PPARG) gene, rs1801282 and rs3856806, may be important candidate gene loci affecting the susceptibility to ischemic stroke. This case-control study sought to identify the relationship between these two single-nucleotide polymorphisms and ischemic stroke risk in a northern Chinese Han population. A total of 910 ischemic stroke participants were recruited from the First Hospital of China Medical University, Shenyang, China as a case group, of whom 895 completed the study. The 883 healthy controls were recruited from the Health Check Center of the First Hospital of China Medical University, Shenyang, China. All participants or family members provided informed consent. The study protocol was approved by the Ethics Committee of the First Hospital of China Medical University, China on February 20, 2012(approval No. 2012-38-1). The protocol was registered with the Chinese Clinical Trial Registry(registration number: ChiCTR-COC-17013559). Plasma genomic DNA was extracted from all participants and analyzed for rs1801282 and rs3856806 single nucleotide polymorphisms using a SNaPshot Multiplex sequencing assay. Odds ratios(ORs) and 95% confidence intervals(CIs) were calculated using unconditional logistic regression to estimate the association between ischemic stroke and a particular genotype. Results demonstrated that the G allele frequency of the PPARG gene rs1801282 locus was significantly higher in the case group than in the control group(P < 0.001). Individuals carrying the G allele had a 1.844 fold increased risk of ischemic stroke(OR = 1.844, 95% CI: 1.286–2.645, P < 0.001). Individuals carrying the rs3856806 T allele had a 1.366 fold increased risk of ischemic stroke(OR = 1.366, 95% CI: 1.077–1.733, P = 0.010). The distribution frequencies of the PPARG gene haplotypes rs1801282-rs3856806 in the control and case groups were determined. Measurement invariance and latent mean differences of the Chinese version physical activity self-efficacy scale across gender and education levels

Background: Self-efficacy has been identified as an important determinant of youth's behavior change including physical activity(PA) participation. However, the dimensionality check of a PA self-efficacy scale has rarely been conducted in China. The current study aims to examine(1) the unidimensionality of a shortened Chinese version of PA self-efficacy scale(S-PASESC);(2) the measurement invariance of S-PASESC across gender and levels of education;(3) the latent factor mean difference between gender and levels of education;(4) the direct effects of self-efficacy on PA by different gender and education levels; and(5) the comparisons of the direct effects of self-efficacy on PA across gender and education levels.Methods: The participants were 5 th through 11 th grade public school students recruited from 7 cities located in different geographic regions of China. The final data include a total of 3003 participants(49.7% boys) who have completed the scales.Results: Confirmatory factor analysis(CFA) test supported the unidimensionality of S-PASESC. The S-PASESC is invariant across gender and 3 levels of education at both configural, full metric, and full scalar levels. Findings from latent mean comparisons showed that boys reported higher PA self-efficacy than girls. Students' perceived PA self-efficacy tend to decrease from elementary to high school. Finally, self-efficacy positively related to PA by groups of different gender and education levels and the relationship between self-efficacy and PA is stronger among middle school boys than girls.Conclusion: Findings suggest S-PASESC is a valid scale for measuring Chinese students' PA self-efficacy.

Objective:To evaluate the association between Chinese medicine(CM)therapy and disease-free survival(DFS)outcomes in postoperative patients with non-small cell lung cancer(NSCLC).Methods:This multiple-center prospective cohort study was conducted in 13 medical centers in China.Patients with stage I,II,or IDA NSCLC who had undergone radical resection and received conventional postoperative treatment according to the National Comprehensive Cancer Network(NCCN)guidelines were recruited.The recruited patients were divided into a CM treatment group and a control group according to their wishes.Patients in the CM treatment group received continuous CM therapy for more than 6 months or until disease progression.Patients in the control group received CM therapy for less than 1 month.Follow-up was 8nducted over 3 years.The primary outcome was DFS,with recurrence/metastasis rates as a secondary outcome.Results:Between May 2013 and August 2016,503 patients were enrolled into the cohort;266 were classified in the CM treatment group and 237 in the control group.Adjusting for covariates,high exposure to CM was associated with better DFS[hazard ratio(HR)=0.417,95%confidential interval(Cl):0.307-0.567)].A longer duration of CM therapy(6-12 months,12-18 months,>24 months)was associated with lower recurrence and metastasis rates(HR=0.225,0.119 and 0.083,respectively).In a subgroup exploratory analysis,CM therapy was also a protective factor of cancer recurrence and metastasis in both stage n-MA(HR=0.50,95%Cl:0.37-0.67)and stage IDA NSCLC postoperative patients(HR=0.48,95%Cl:0.33-0.71),DFS was even longer among CM treatment group patients.Conclusions:Lon ger duration of CM therapy could be 8nsidered a protective factor of cancer recurrence and metastasis.CM treatment is associated with improving survival outcomes of postoperative NSCLC patients in China.

The Prevalence and Trends of Obesity in Chinese Preschool Children from 1996 to 2019:A Meta-analysis

Purpose Understanding the prevalence and trends of obesity in preschool children is of critical importance for public health policy.Thus,the object of the present study was to assess the prevalence and trends of obesity in Chinese preschool children from 1996 to 2019,and to examine the differences among different population according to year,gender,age,urban/rural and geographical area.Methods Four online databases(Chinese National Knowledge Infrastructure,Wanfang Data Resource System,Weipu Chi-nese Journal Service Platform and PubMed)were searched for studies published from January 1990 to May 2019 using predefined inclusion/exclusion criteria.Random effects meta-analysis was used to calculate the prevalence of obesity.Results Forty-three studies with a total of 765,255 Chinese preschool children aged 3-6 years were included in this meta-analysis.The prevalence of obesity in the whole sample was 6.41%(95%CI:5.74%-7.78%,PChinese preschool children(P=0.001).Conclusion There was no significant difference of the obesity prevalence among any five-year periods from 1996 to 2019.The prevalence of obesity in boys was higher than girls and increased with age.Though the prevalence of obesity in rural area was lower than the urban area,the speed of the increasing of the prevalence after 2010 in rural area should be noted.Public health intervention such as physical activi

Observational Study of Chinese Medicine Syndrome Distribution in Patients with Acute Myocardial Infarction and Its Impact on Prognosis

Objective: To investigate the distribution of Chinese medicine (CM) syndrome in patients with acute myocardial infarction (AMI) on admission and its impact on prognosis. Methods: A total of 525 AMI patients were prospectively recruited and classified into 4 groups based on their clinical characteristics: excess-heat, excess-cold, deficiency-heat and deficiency-cold syndromes. Major adverse cardiovascular events (MACEs) were followed up. Results: The excess syndrome was more common than deficiency syndrome (72.95% vs. 27.05%;P<0.05). Totally 495 (94.29%) of 525 AMI patients were followed up (median 277 days). There were 59 (11.92%) MACEs. After adjusted with confounding factors in Cox regression models, the hazard ratio (95% confidence interval) of excess-heat, excess-cold, deficiency-heat and deficiency-cold syndrome groups were 1, 1.25 (0.63, 2.49;P<0.05), 2.37 (1.14, 4.94;P<0.05), 3.76 (1.71, 8.28;P<0.05), respectively. Conclusions: Excess syndrome was more common in AMI patients and had better prognosis, while deficiency-cold syndrome had the poorest prognosis. CM syndrome was of value in predicting long-term outcomes in AMI patients.

Evaluation on Effectiveness and Safety of Chinese Herbs inTreatment of Sub-health: A Systematic Review andMeta-Analysis of Randomized Controlled Trials

Objective: To evaluate the effectiveness and safety of Chinese herbs in the treatment of sub-health systematical y. Methods: Nine databases were systematicaly and extensively searched to col ect randomized control ed trials(RCTs) about Chinese herbs in the treatment of sub-health. The outcomes included overal effective rate, main symptoms, quality of life, etc. Literature screening, data extraction and quality assessment were conducted according to Cochrane Handbook 5.1. Meta-analysis was conducted to the included literature with Review Manager Software.Results: Seventy-two studies involving 9,296 patients with sub-health were included with 4,908 patients in experimental groups and 4,387 patients in control groups. The overal quality of included clinical research was not high. In the aspect of improving overall effective rate, relieving main symptoms, Pittsburgh sleep quality index(PSQI) score, Athens Insomnia Scale(AIS) score, Fatigue Scale-14(FS-14), Cornel Medical Index(CMI) score and discontinuation rate, the effects of experimental groups were better than that of control groups. According to available research reports, adverse reactions in Chinese herb groups were mainly mild gastrointestinal symptoms, which did not affect the treatment.Conclusion: Chinese herbs have a curative effect in the treatment of sub-health. However, there are no clear criteria for diagnosis and curative effectiveness judgment global y, which would affect the accuracy of curative effect evaluation.

Objective: To assess the ef?cacy and safety of the Chinese medicine Dingkun Pill(定坤丹,DKP) on insulin resistance in women with polycystic ovary syndrome(PCOS).Methods: A total of 117 women with PCOS were randomly assigned to Group A(38 women),Group B(40 women),or Group C(39 women) in a randomization sequence with SAS software and a 1:1:1 allocation ratio using random block sizes of 6,and were given 7 g of oral DKP daily(Group A),1 tablet of Diane-35 orally daily(Group B),or 7 g of oral DKP daily plus 1 tablet of Diane-35 orally daily(Group C).Patients took all drugs cyclically for 21 consecutive days,followed by 7 drug-free days.The treatment course for the 3 groups was continued for 3 consecutive months.Oral glucose tolerance tests(OGTT)were performed before treatment and again after 2 and 3 months of therapy,respectively,and homeostasis model assessment for insulin resistance(HOMA-IR) and quantitative insulin sensitivity check index(QUICKI) were calculated.Results: Of 117 women with PCOS,110 completed the entire course of therapy: 35 in Group A,36 in Group B,and 39 in Group C.After treatment,all three groups showed signi?cant decreases in fasting glucose: at1 h glucose decreased signi?cantly in Group A(by 0.5±1.4 mmol/L,P=0.028) and Group C(by 0.5±1.2 mmol/L,P=0.045); while showing a tendency to increase in Group B(by 0.4±1.9 mmol/L,P=0.238).HOMA-IR decreased signi?cantly in Group C [by 0.5(–2.2 to 0.5) m IU mmol/L2,P=0.034].QUICKI was signi?cantly increased in Groups A and C(by 0.009±0.02,P=0.033 and by 0.009±0.027,P=0.049,respectively),while no change was observed in Group B.Repeated-measure ANOVA showed that the absolute changes in all parameters(except for glucose at1 h),including glucose and insulin levels at all time-points during OGTT and in HbA1 c,HOMA-IR,and QUICKI,were not signi?cantly different among the 3 groups after treatment(P>0.05).Conclusion: DKP or DKP combined with Diane-35 produce a slight improvement in insulin sensitivity compared with Diane-35 alone in PCOS patients(Trial Registration: Cli

Effect of Apolipoprotein E Genotypes on Huntington’s Disease Phenotypes in a Han Chinese Population

Huntington's disease(HD)is an autosomal dominant degenerative disease that mainly encompasses movement,cognition,and behavioral symptoms.The apolipoprotein E(APOE)gene is thought to be associated with many neurodegenerative diseases.Here,we enrolled a cohort of 223 unrelated Han Chinese patients with HD and1241 unrelated healthy controls in Southeastern China and analyzed the correlation between APOE genotypes and HD phenotypes.The results showed that the frequency of the E4 allele(7.1%)in HD patients was statistically less than that in controls(12.0%)(P =0.004).In addition,we divided patients into motor-onset and non-motor-onset groups,and analyzed the relationship with APOE genotypes.The results,however,were negative.Furthermore,the age at onset(AAO),defined as the age at the onset of motor symptoms,was compared in each APOE genotype subgroup and multivariate regression analysis was used to exclude the interference of CAG repeat length on AAO,but no association was found between APOE genotypes and AAO.Finally,we analyzed adult-onset HD to exclude the interference caused by juvenile HD(n = 13),and the results were negative.Therefore,our study suggests that APOE may not be a genetic modifier for HD,especially for adult-onset HD among Chinese of Han ethnicity.To the best of our knowledge,this is the first study of the correlation between APOE genotypes and HD phenotypes in a Han Chinese population.

A New Perspective for Chinese Medicine Intervention for Coronary Artery Disease: Targeting Inflammation

Inflammation, which plays a critical role in atherosclerosis and the occurrence of acute cardiovascular events, may be a new target for treatment of coronary artery disease(CAD) to reduce residual cardiovascular risk. Recently, Antiinflammatory Therapy with Canakinumab for Atherosclerotic Disease(CANTOS), the largest scale clinical trial that targeted inflammation but not lipids, has affirmed for the first time the inflammatory hypothesis of artherosclerosis and marked the advent of an exciting era of targeting inflammation for the prevention and treatment of cardiovascular diseases. Chinese medicine(CM) is a promising adjuvant therapy for CAD in light of its safety and pleiotropic effect of anti-inflammation, anti-platelet, lipid-regulating, endothelium-protection, microcirculation-improving, etc. In recent years, exploration of anti-inflammatory treatment of CAD with CM has been going on from theory to practice. Taking CANTOS as an example, the design strategy to combine CM and Western medicine to inhibit inflammation were discussed in this paper, which might provide a new perspective for CM intervention on CAD.
