Efficacy and Safety of Ginkgo Biloba Tincture(银杏叶酊) Combined with Conventional Medication in the Treatment of cerebral infarction: Systematic Review and Meta-analysis

Objective:To analyze the efficacy and safety of Ginkgo biloba tincture combined with conventional medication in the treatment of cerebral infarction by systematic evaluation.Methods:Searched the randomized controlled trials of Ginkgo biloba tincture in treating cerebral infarction.English literatures were searched from PubMed,EMbase,Cochrane Library and Web of Science,and Chinese literatures were searched from Chinese Biomedical Literature Database(CBM),China National Knowledge Infrastructure(CNKI),China Science and Technology Journal Database(VIP)and Wanfang Medical Database.At the same time,we searched dissertations,conference papers,and found relevant unpublished research results reports.Literature quality was evaluated using a"bias risk assessment tool"and Meta-analysis using RevMan 5.4 software(from the Cochrane Collaboration).A total of 29 literatures were retrieved,and finally 4 literatures meeting the standards were included.There were 455 samples involved,including 244 cases in the experimental group and 211 cases in the control group.Results:①Effectiveness analysis:The total effective rate of Ginkgo biloba tincture combined with routine treatment was better than that of the control group[RR=1.26,95%CI(1.07,1.47),P=0.005].The hematocrit of Ginkgo biloba tincture combined with conventional treatment was significantly better than that of conventional treatment alone(P<0.05),The neurological deficits of Ginkgo biloba tincture combined with routine therapy was significantly improved(P<0.05).After Ginkgo biloba tincture combined with conventional therapy,the scores of mini-mental state examination(MMSE),activity daily living scale(ADL)and MOSSF-36 quality of life scale were significantly better than those of the control group(P<0.05).②Safety analysis:3 articles reported the safety of medication.There was no obvious change in blood routine,liver and kidney function results and no adverse reactions in the experimental group and the control group.Conclusion:The above results indicated that Ginkgo biloba tincture

ObjectiveTo understand the pathogenetic role and clinical significance of interleukin 6(IL -6),tumor necrosis factor(TNF)and soluble interleukin -2receptor(sIL -2R)in patients with cerebral infarctio n(CI ).MethodsThe IL -6,TNF and sIL -2R levels were measured in 46CI patient s with ELISA.ResultsThe serum IL -6,TNF and sIL -2R levels of CI group were much higher than those of controls group .The variation of IL -6,TNF and sIL -2R levels was c losely related to the size of cerebra l infarction.The serum IL -6,TNF and sIL -2R levels of recovery p eriod were much lower than those of acute period in patients with CI .Conclusions The IL -6,TNF and sIL -2R take part in the pathologi c process after CI .The measurement o f IL -6,TNF and sIL -2R is useful in determining the size of cerebral infarction.

Clinical treatment strategy of acute cerebral infarction complicated with pulmonary embolism: a case report

This article describes the clinical characteristics of a case of cerebral infarction complicated with pulmonary embolism(PE),and elaborates the therapeutic strategies of intravenous thrombolysis and anticoagulation.Treatment remedies:the medical history and examination data of a patient with aphasia and right hemiplegia were collected.After intravenous thrombolysis and pulmonary artery CTA(computed tomographic angiography),the patient was found to have low blood oxygen saturation and PE.And the patient was treated with anticoagulant therapy.Post treatment evaluating:cerebral hemorrhage was excluded by craniocerebral CT(computed tomographic),and left basal ganglia infarction was diagnosed.After thrombolysis,the symptoms of aphasia and hemiplegia were significantly improved.For PE,after anticoagulant therapy,the patient’s breathing was stable and blood oxygen saturation was normal.For deep venous thrombosis of both lower extremities,anticoagulant therapy was continued.Conclusion:patients with thrombophilia are prone to cerebral embolism,PE and deep venous thrombosis of lower limbs.Intravenous thrombolytic therapy in acute phase and heparin anticoagulant therapy in recovery period can effectively control the disease.

Risk Factors for Prevention Stroke (IS or TIA) Due to cerebral infarction in Young Adults: A Meta-Analytical Study

Ischemic strokes (IS), also referred to as cerebral ischemia or brain ischemia, is a significant cause to the brain cells damage or death. Approximately, 10% - 14% of ischemic strokes cases occurred in young adults. Hence, we conducted a meta-analysis to find the effective interventions to prevent the best strokes caused by cerebral infarction in young adults. The search was done in different databases, including Google scholar, PubMed, Embase, Medline, Cochrane Central Register of Controlled Trials, Cochrane Database, Scopus, and Web of Science from January 2016 to April 2020, and only English published articles were considered. Our analysis included studies that stratified the risk of ischemic stroke by CHA2DS2-VASc score for patients with nonvalvular atrial fibrillation. Further, random effects model was used to estimate the summary annual rate of IS. Pooled relative risks and odds ratios, with their 95% confidence intervals, were calculated, respectively. The analysis was conducted using STATA (version 12), pooled effect sizes were calculated using the random-effects model and heterogeneity was tested for using the I2 statistic. The analysis included 13 studies. The analysis shows that diabetes, high blood pressure, ischemic heart disease, atrial fibrillation, hypercholesterolemia, alcohol consumption and smoking are significant risk factors. In Caucasian and Chinese ischemic stroke patients, the risk factor associations associated with ischemic stroke subtypes are similar. Compared to all other ischemia subtypes, diabetes is more familiar with aortic stroke, atrial fibrillation, ischemic heart disease (with obstruction), hypertension and diabetes. Our research shows that atrial fibrillation, ischemic heart disease, and hypercholesterolemia are low in patients with ischemic stroke and the risk factors are higher. Further analysis of each patient’s data is required to enable confounders’ adjustments to confirm and expand these findings.

Background: Early detection of cerebral infarction (CI) by home acupuncturists might be helpful in reducing stroke mortality. We compared vital signs of patients who developed CI and those who did not to ascertain if blood pressure balance is a useful marker for the early detection of CI and for distinguishing cases with potentially effective/ineffective acupuncture treatment. Methods: In this retrospective study, vital signs such as body temperature, pulse, and blood pressure, were simultaneously obtained from the right and left arms of patients who received weekly acupuncture treatment. We focused on the differences between values obtained from the right and left arms (the balance) and compared the values of patients who developed CI [CI (+)] (n = 18) and those of age-/gender-matched patients who did not [CI (−)] (n = 18). Results: Left/right blood pressure was significantly more unbalanced in the CI (+) group than in the CI (−) group, especially immediately before the onset of CI. Moreover, all CIs occurred on the side with a lower blood pressure. Discussion and Conclusions: Blood pressure balance might aid in the early detection of CI and to determine whether treatment with traditional or modern medicine is more appropriate. Acupuncture may thus help maintain patients’ quality of life.

Effect of Cell Cycle Inhibitor Olomoucine on Astroglial Proliferation and Scar Formation after Focal cerebral infarction in Rats

Background:Astrocytes become reactive following many types of CNS injuries.Excessive astrogliosis is detrimental and contributes to neuronal damage.We sought to determine whether inhibition of cell cycle could decrease the proliferation of astroglial cells and therefore reduce excessive gliosis and glial scar formation after focal ischemia.Methods:cerebral infarction model was induced by photothrombosis method.Rats were examined using MRI,and lesion volumes were estimated on day 3 post-infarction.The expression of glial fibrillary acidic protein(GFAP) and proliferating cell nuclear antigen(PCNA) was observed by immunofluorescence staining.Protein levels for GFAP,PCNA,Cyclin A and Cyclin B1 were determined by Western blot analysis from the ischemic and sham animals sacrificed at 3,7,30 days after operation.Results:Cell cycle inhibitor olomoucine significantly suppressed GFAP and PCNA expression and reduced lesion volume after cerebral ischemia.In parallel studies,we found dense astroglial scar in boundary zone of vehicle-treated rats at 7 and 30 days.Olomoucine can markedly attenuate astroglial scar formation.Western blot analysis showed increased protein levels of GFAP,PCNA,Cyclin A and Cyclin B1 after ischemia,which was reduced by olomoucine treatment.Conclusion: Our results suggested that astroglial activation,proliferation and subsequently astroglial scar formation could be partially inhibited by regulation of cell cycle.Cell cycle modulation thereby provides a potential promising strategy to treat cerebral ischemia.

Feasibility of Recruiting and Assessing Health-Related Quality of Life and Physical Function in Older Chinese Adults with cerebral infarction

Objective:While it is known that exercise therapy can improve physical and emotional function in cerebral infarction(CD)patients,few studies have examined how well this would be accepted by older adults in China.Methods:In this study,the feasibility of recruiting and asessing healh-related quality of life and physical function in older Chinese adults with cerebral infarction was asessed.Specific aims of the studly were to evalhuate the feasibility of recnuiting older adults,with and without CI,from three different locations/settings in China;Compare the quality of life and physical function measures between CI and control subjects;Propose future larger randomized controlled studies of aerobic and resistance exercise training in both human and animal models after CI.Results:Overall,66/275(24.0%)surveys that were sent to older Chinese adults were retumed and evaluated.Of those surveys returned,18(27%)met the study inclusion and exclusion critenia.Consequently,the results of this feasibility study indicate there is a recruitment yield(number of subject contacted/number of subjects who qualified for study)of 6.5%.These number varied at the different sites/settings.but the highest recruitment yield was seen in hospitalized patients.Despite small sample sizes,there were statistically sigmificant differences in health-related quality of life and physical function between CI patients and control subjects.Conclusion:This feasibility study demonstated that it is possible to successfully recruit CI patients for an exercise intervention study as well as to perform important assessments of health-related quality of life and physical fumction.Further randomized controlled trials,in humans and animal models,will be needed determine if aerobic and/or resistance exercise training can improve health and physical function in older CI patients.Additional studies will be needed to determine the specific mechanisms responsible for the benefits see with aerobic and resistance training.

Exploring the prescription in rules and mechanism for cerebral infarction based on data mining and Network Pharmacology

Objective: This study intends to explore the prescription rules for cerebral infarction treatment based on the famous prescription book Qian Jin Yao Fang, and further mined the core prescription and predicted the main active components and targets. Methods: The prescriptions for cerebral infarction in "Qian Jin Yao Fang" were collected. The data were analyzed using the auxiliary platform of traditional Chinese medicine V2.5, to explore the core herbs and summarized new prescriptions. At last, we analyzed the mechanism of core prescriptions of for cerebral infarction treatment using network pharmacology methods. Results: 100 prescriptions and 152 kinds of herbs were obtained by data mining. The results showed that deficiency-tonifying drugs account for a large proportion of drug classification;The core prescriptions for cerebral infarction in "Qian Jin Yao Fang" were composed of Herba Ephedrae(Mahaung), Cinnamomum cassia Presl(Rougui), Panax ginseng C.A.Meyer (Renshen), Glycyrrhiza uralensis Fisch(Gancao), Saposhnikovia divaricate(Trucz) Schischk(Fangfeng), Aconitum carmichaeli Debx(Fuzi) and Ligusticum chuanxiong Hort(Chuanxiong). Using network-based systems biology analysis, we predicted that 194 potential targets in the core prescriptions for cerebral infarction. 45 key targets of the core prescriptions were obtained in the treatment of cerebral infarction, suggesting that the related mechanism maybe closely related to TP53, STAT3, AKT1, MYC, FOS. Through Gene Oncology and Kyoto Encyclopedia of Genes and Genomes analysis, we found that the related signaling pathways mainly involved in PI3K-Akt signal pathway, TNF signal pathway, AGE-RAGE signal pathway, and so on. Conclusion: It takes expelling wind and relieving surface and warming spleen and kidney as the treatment method of cerebral infarction in "Qian Jin YaoFang". The core prescriptions were used for cerebral infarction treatment for the multi-component, multi-target and multi-channel interaction, such as PI3K-Akt signal pathway, TNF signal pathway, AGE-RA

Fluorescent histochemical observation and estimation of the content ofcatecholamine(CA)in cerebral cortex of twenty rats with unilateral occlusion ofmiddle cerebral artery were performed in this present study.Three d after localcerebral infarction,the CA granules which were of different fluorescent featurefrom those of normal cortex were seen in the marginal area of infarct. The contentof norepinephrine in bilateral cortexes was significantly reduced, but that ofdopamine was unchanged.
