Cardiology Discovery(CD)is the offcial journal of the Chinese Society of Cardiology(CSC)of Chinese Medical Association(CMA).It is an English language,peer-reviewed journal aims to publish high-quality materials on all aspects of cardiovascular medicine and surgery,including original clinical,epidemiological,basic and translational research,health services and outcomes studies,state-of-the-art reviews,technical evaluations,case reports,editorial,perspective,consensus and guideline papers.CD has a international outlook,that is to create a platform of international collaboration and exchange of cutting-edge information on cardiovascular research and education.

The novel coronavirus disease(COVID-19)has hit the healthcare system worldwide.The risk of severe infection and mortality increases with advancing age,especially in subjects with comorbidities such as cardiovascular disease,hypertension,diabetes,obesity and cancer.Moreover,cardiovascular complications such as myocardial injury,heart failure and thromboembolism are frequently observed in COVID-19 cases,and several biomarkers(troponin,NTpro BNP and D-Dimer)have been identified as prognostic indicators of disease severity and worst outcome.Currently,there is no specific therapy against SARS-Co V-2,although many medications are under investigation.The aim of this review will be to explore the intertwined relationship between COVID-19 disease and the cardiovascular system,focusing on elderly population.The available supportive treatments along with the related concerns in elderly patients,due to their comorbidities and polypharmacotherapy,will be explored.

Heart failure(HF)in the elderly,besides being a leading cause of mortality and morbidity,is rapidly increasing in prevalence with patients aged 65 and older accounting for more than 75%of heart failure hospitalizations.Elderly patients have historically been unrepresented in clinical HF trials and often present with multiple comorbidities,including frailty,depression,nutritional,functional and cognitive impairments.Additionally,pharmacologic challenges such as adherence to therapy,polypharmacy,altered drug pharmacokinetics and/or renal derangements make them less likely to receive guideline-directed medical therapies for HF.Recognition of these various interrelated domains is key and should prompt a multidisciplinary,holistic management approach so as to optimize prognosis in this vulnerable subset of the population.

Hypertension has become a major risk factor for cardiovascular disease(CVD)and premature mortality worldwide,with high prevalence and relatively poor control rate,especially in rural areas.[1]Blood pressure(BP)control in rural areas is the most vulnerable spot in hypertension control and is a key target to reduce cardiovascular and cerebrovascular events and total mortality.Medical workers globally have devoted considerable efforts to solving this problem and some progress has been made[Table 1].[2–6]Our recent study proved that a novel,village doctor-led,multifaceted intervention strategy for BP control was an effective,feasible,and safe method for controlling hypertension in rural China.[6]

cardiovascular disease(CVD)is the leading cause of mortality worldwide.Multiple factors are involved in CVD,and emerging data indicate that lipoprotein(a)(Lp(a))may be associated with atherosclerotic cardiovascular disease(ASCVD)independent of other traditional risk factors.Lp(a)has been identified as a novel therapeutic target.Previous studies on the influence of Lp(a)in CVD have mainly used in western populations.In this review,the association of plasma Lp(a)concentration with ASCVD was summarized,with regards to epidemiological,population-based observational,and pathological studies in Chinese populations.Lp(a)mutations and copy number variations in Chinese populations are also explored.Finally,the impact of plasma Lp(a)levels on patients with type 2 diabetes mellitus,cancer,and familial hypercholesterolemia are discussed.

cardiovascular disease is one of the leading causes of mortality worldwide.Recent studies have shown that circular RNAs(circRNAs)have emerged as important players in the prevention and treatment of cardiovascular diseases.circRNAs are a class of endogenous noncoding RNAs that are generated by back-splicing and are involved in many pathophysiological processes.In this review,we outline the current research progress on the regulatory roles of circRNAs in cardiovascular diseases.Further,new technologies and methods available for identifying,validating,synthesizing,and analyzing circRNAs,as well as their applications in therapeutics,are highlighted here.Moreover,we summarize the increasing insights into the potential use of circRNAs as circulating diagnostic and prognostic biomarkers.Finally,we discuss the prospects and challenges of circRNA therapeutic applications for cardiovascular disease therapy,with a particular focus on developing circRNA synthesis and engineering delivery systems.

The beneficial cardiorenal outcomes of sodium-glucose cotransporter 2 inhibitors(SGLT2i)in patients with type 2 diabetes mellitus(T2DM)have been substantiated by multiple clinical trials,resulting in increased interest in the multifarious pathways by which their mechanisms act.The principal effect of SGLT2i(-flozin drugs)can be appreciated in their ability to block the SGLT2 protein within the kidneys,inhibiting glucose reabsorption,and causing an associated osmotic diuresis.This ameliorates plasma glucose elevations and the negative cardiorenal sequelae associated with the latter.These include aberrant mitochondrial metabolism and oxidative stress burden,endothelial cell dysfunction,pernicious neurohormonal activation,and the development of inimical hemodynamics.Positive outcomes within these domains have been validated with SGLT2i administration.However,by modulating the sodium-glucose cotransporter in the proximal tubule(PT),SGLT2i consequently promotes sodium-phosphate cotransporter activity with phosphate retention.Phosphatemia,even at physiologic levels,poses a risk in cardiovascular disease burden,more so in patients with type 2 diabetes mellitus(T2DM).There also exists an association between phosphatemia and renal impairment,the latter hampering cardiovascular function through an array of physiologic roles,such as fluid regulation,hormonal tone,and neuromodulation.Moreover,increased phosphate flux is associated with an associated increase in fibroblast growth factor 23 levels,also detrimental to homeostatic cardiometabolic function.A contemporary commentary concerning this notion unifying cardiovascular outcome trial data with the translational biology of phosphate is scant within the literature.Given the apparent beneficial outcomes associated with SGLT2i administration notwithstanding negative effects of phosphatemia,we discuss in this review the effects of phosphate on the cardiometabolic status in patients with T2DM and cardiorenal disease,as well as the mechanisms by which SGLT2i counteract or overco

Aims: To determine the cardiovascular and cerebrovascular prognosis at 5 years of patients who underwent an ischemic stroke in Senegal. Methods: We performed a descriptive, retrospective and exhaustive study conducted at Principal Hospital of Dakar. We included all patients hospitalized at Principal Hospital of Dakar from 2013 to 2019 for an ischemic stroke confirmed by a cerebral CT-Scanner or by magnetic resonance imaging, and who survived the acute phase. Results: Overall, one hundred and seventy-one (171) patients were gathered. Ninety-six (96) male patients account for 56.14% and seventy-five (75) female patients account for 43.86%, with a sex ratio of 1.28. Patients’mean age was 66.33 ±13.99 years. Twenty-one (21) young patients (12.35%), aged less than 50 years, presented with an ischemic stroke. Myocardial infarction was found in a female patient (0.6%) 3 months after she experienced an ischemic stroke, nine (9) patients (5.29%) underwent a recurrence of the ischemic stroke with a mean recurrence period of 8 months and ex
