Calcium supplementation for the prevention of colorectal adenomas: a systematic review and meta-analysis of randomized controlled trials

AIM: To determine the efficacy of calcium supplementation in reducing the recurrence of colorectal ***: We conducted a systematic review and meta-analysis of published studies. We searched PubMed, Scopus, the Cochrane Library, the WHO International Clinical Trials Registry Platform, and the *** website, through December 2015. Randomized, placebo-controlled trials assessing supplemental calcium intake for the prevention of recurrence of adenomas were eligible for inclusion. Two reviewers independently selected studies based on predefined criteria, extracted data and outcomes (recurrence of colorectal adenomas, and advanced or “high-risk” adenomas), and rated each trial’s risk-of-bias. Between-study heterogeneity was assessed, and pooled risk ratio (RR) estimates with their 95% confidence intervals (95%CI) were calculated using fixed- and random-effects models. To express the treatment effect in clinical terms, we calculated the number needed to treat (NNT) to prevent one adenoma recurrence. We also assessed the quality of evidence using ***: Four randomized, placebo-controlled trials met the eligibility criteria and were included. Daily doses of elemental calcium ranged from 1200 to 2000 mg, while the duration of treatment and follow-up of participants ranged from 36 to 60 mo. Synthesis of intention-to-treat data, for participants who had undergone follow-up colonoscopies, indicated a modest protective effect of calcium in prevention of adenomas (fixed-effects, RR = 0.89, 95%CI: 0.82-0.96; random-effects, RR = 0.87, 95%CI: 0.77-0.98; high quality of evidence). The NNT was 20 (95%CI: 12-61) to prevent one colorectal adenoma recurrence within a period of 3 to 5 years. On the other hand, the association between calcium treatment and advanced adenomas did not reach statistical significance (fixed-effects, RR = 0.92, 95%CI: 0.75-1.13; random-effects, RR = 0.92, 95%CI: 0.71-1.18; moderate quality of evidence).CO

Can aspirin use reduce the risk of pancreatic cancer:an updated systematic review and meta-analysis

Background:chemoprevention effect of aspirin for pancreatic cancer(PC)remains *** we performed an updated systematic review and meta-analysis to investigate the real association between aspirin consumption and risk of ***:PubMed,Web of Science,EMBASE and the Cochrane Database were systematic searched to identify the potential *** ratio(ORs)with 95%confidence intervals(CIs)were applied to assess the strength of ***:Thirteen studies and approximately 28,440 participants were *** significantly reduced the incidence of PC(OR,0.82;95%CI,0.73-0.93)in case-control ***,the overall results did not reveal an obvious association(OR,0.92;95%CI,0.74-1.16).Both low-dose(OR=0.86)and high-dose(OR=0.80)aspirin intake showed prevention *** addition,low frequency(OR,0.87;95%CI,0.73-1.05)and high frequency(OR,0.84;95%CI,0.69-1.03)seemed to be equally associated with decreased risk for *** consumption longer than 10 years use seems to have better effect(OR,0.73;95%CI,0.51-1.04)than shorter aspirin use(OR,0.94;95%CI,0.77-1.15).Conclusions:Our study indicated that aspirin use might be associated with decreased risk of PC,especially at high *** we still need to be cautious when interpreting the results.

The unique topobiology of the transmembrane-bound Nrfl transcription factor with dynamic transient phases controls its multistep processing to yield distinct lengths of isoforms(***) during the maturation of this CNC-bZIP protein into an activator,enabling for its ER-to-nuclear translocation in order to gain access to cognate ARE/EpRE-driven genes before being ***-accumulating evidence demonstrates that Nrfl acts as an unique vital player in maintaining cellular(redox,lipid and protein) homeostasis and organ integrity during normal development and healthy growth throughout the whole life ***,loss-of-function of Nrfl results in severe oxidative stress,genomic instability,embryonic lethality,developmental disorders,and adult diseases such as non-alcoholic steatohepatitis,hepatocellular carcinoma,diabetes and neurogenerative ***,Nrfl is critically implicated in a variety of important physio-pathological processes by governing robust target genes in order to reinforce antioxidant,detoxification and cytoprotective responses to cellular ***,there also exists a proteasomal’bounce-back’ response mediated by Nrfl,insofar as to enhance the drug resistance to proteasomal inhibitors in preclinical treatment of neuroblastoma,multiple myeloma and triple-negative breast ***,several drugs or chemicals are found or re-found in some new ways to block the proteasomal compensatory pathway through inhibiting the multistep processing of ***,induction of Nrfl activity by some drugs or chemicals leads to cytoprotection from cell apoptosis and promotes cell *** is a good start of the constructive and meaningful studies to explore the mechanism(s) by which Nrfl is activated to protect neurons and other cells from malignant and degenerative ***,Nrfl has appealed attentions as a new attractive therapeutic strategy for human cancers and other diseases.
