c-reactive protein aggravates myocardial ischemia/reperfusion injury through activation of extracellular-signal-regulated kinase 1/2

Background Ischemia/reperfusion injury （IRI） is an inflammatory response that occurs when tissue is reperfused following a prolonged period of ischemia. Several studies have indicated that c-reactive protein （cRP） might play an important role in inducing IRI. However, the effects of cRP on myocardial IRI and the underlying mechanisms have not been fully elucidated. This study aimed to investigate the association between cRP and myocardial IRI and the underlying mechanisms. Methods We simulated ischemia/reperfusion using oxygen-glucose deprivation/ reoxygenation （OGD/R） in neonatal Sprague-Dawley rat cardiomyocytes; reperfusion injury was induced by three hours of hypoxia with glucose and serum deprivation followed by one hour of reperfusion. cell viability was tested with MTS assays, and cardiomyocyte damage was evaluated by lactate dehydrogenase （LDH） leakage. Mitochondrial membrane potential was measured using tetramethylrhodamine ethyl ester （TMRE） and mitochondrial permeability transition pore （mPTP） opening was measured using calcein/AM; both TMRE and caocein/AM were visualized with laser scanning confocal microscopy. In addition, we studied the signaling pathways underlying cRP-mediated ischemia/reperfusion injury via Western blot analysis. Results compared with the simple OGD/R group, after intervention with 10 pg/mL cRP, cell viability decreased markedly （82.36 % ± 6.18% vs. 64.84% ± 4.06%, P = 0.0007）, and the LDH leakage significantly increased （145.3 U/L ± 16.06 U/L vs. 208.2 U/L ± 19.23 U/L, P = 0.0122）. cRP also activated mPTP opening and reduced mitochondrial membrane potential during myocardial ischemia/reperfusion. Pretreatment with 1 pM atorvastatin （Ator） before cRP intervention protected cardiomyocytes from IRI. Objective c-reactive protein(cRP)/albumin ratio(cAR)is a new inflammation-based index for predicting the prognosis of various *** cAR determined on admission may help to predict the prognostic value of multiple trauma *** A total of 264 adult patients with severe multiple trauma were included for the present retrospective study,together with the collection of relevant clinical and laboratory ***,cRP,albumin,shock index and ISS were incorporated into the prognostic model,and the receiver operating characteristic(ROc)curve was ***,the shock index for patients with different levels of cAR was ***,univariate and multivariate logistic regression analyses were performed to identify the independent risk factors for the 28-day mortality of multiple trauma *** A total of 36 patients had poor survival outcomes,and the mortality rate reached 13.6%.Furthermore,after analyzing the shock index for patients with different levels of cAR,it was revealed that the shock index was significantly higher when cAR was≥4,when compared to cARcARc analysis helped to identify the independent association between the variables cAR(P=0.029)and shock index(P=0.019),and the 28-day mortality of multiple trauma *** cAR is higher in patients with severe multiple ***,cAR serves as a risk factor for independently predicting the 28-day mortality of multiple trauma *** shock index was significantly higher when cAR was≥4 in multiple trauma patients.

The gallbladder is an important component of the hepatobiliary system whose primary function is to aid in digestion of foodstuffs, excretion of drugs and facilitate removal of waste products from the *** gall bladder is principally a storage organ for bile, chemically modified salts and acids of cholesterol which are synthesized in the liver and which function as surfactants to solubilize fatty substances of limited aqueous solubility. Any disruption in the amount or activity of bile surfactant can lead to an accumulation of insoluble molecular clumps(e.g., gallstones) that deposit in and obstruct fluid movement within the gallbladder, and which can lead to pathological congestion and tissue *** natural host defense response to any event that causes tissue damage is to stimulate inflammation,which non-specifically but aggressively reacts to the stimuli so to remove the cause and repair damaged tissues. The c-reactive protein(cRP) is a primarily hepatically produced serum protein whose blood levels increase within 6–10 h of any tissue-damaging event. The extent with which it increases correlates with the level of tissue damage and associated inflammation. cRP levels are reported to be of value in diagnosing acute cholecystitis severity, in predicting the outcome and prognosis of cancerassociated gallbladder resection, and in helping identify cystic structures during emergency laparoscopic cholecystectomies. As an understanding of distinctive cRP structural isoforms is evolving, its role not only as a biomarker but as regulator of both physiologic and pathophysiologic processes of inflammation may be relevant in the understanding of and treatment approaches for gallbladderassociated disease.
