Objective We aimed to evaluate the combined effects of a high body shape index(ABSI) and a high serum c-reactive protein(cRP) level on the incidence of ischemic stroke in a Mongolian population in china. Methods A prospective cohort study was conducted among 2,589 participants from June 2002 to July 2012 in Inner Mongolia, china. The participants were categorized into 4 groups according to their level of ABSI and cRP. cox proportional hazards models were used to assess the hazard ratios(HRs) and 95% confidence intervals(cIs) for ischemic stroke among all groups. Results The multivariate adjusted HRs(95% cI) of ischemic stroke for high ABSI and high cRP level were 1.46(0.89-2.39) and 1.63(0.95-2.79), respectively. compared with the low ABSI/low cRP level group, the multivariate adjusted HRs(95% cI) of ischemic stroke in the low ABSI/high cRP, high ABSI/low cRP, and high ABSI/high cRP groups were 1.04(0.46-2.35), 1.06(0.58-1.95) and 2.52(1.27-5.00), respectively. The HR of ischemic stroke for the high ABSI/high cRP level group was the highest and most statistically significant. conclusion We found that participants with simultaneously high ABSI and high cRP levels had the highest risk of ischemic stroke in the Mongolian population. Our findings suggest that the combination of high ABSI and high cRP levels may increase the risk of ischemic stroke.

Objective To investigate whether c-reactive protein (cRP) is a biomarker of malignant ventricular arrhythmias (MVA) occurring in non-ST elevation myocardial infarction (NSTEMI) patients with Global Registry of Acute coronary events (GRAcE) scores cluded in this study. Hs-cRP blood levels were measured via a turbidimetric immunoassay after confirming the diagnosis of NSTEMI with GRAcE scores consistent with prior studies, the MVA occurrence rate in our cohort was 6.7%, and patients with MVA exhibited a reduced left ventricular ejection fraction (46.1%± 6.9% vs. 61.5%± 8.7%, P = 0.032), a higher incidence of Killip classification > 1 (34.1% vs. 24.2%, P creased surgical revascularization rate (34.1% vs. 9.7%, P creased mortality (16.5% vs. 5.8%, P cRP levels were higher (P = 0.003) in NSTEMI patients with MVA, and this increase appeared unrelated to other clinical parameters. The c-statistic to discriminate MVA was 0.82 (95% cI: 0.74–0.89). Using receiver operating characteristics analysis, we optimized a cutoff point of 16 mL/L, and the sensitivity and specificity were 95% and 61%, respectively;the positive predictive value was 20% and the negative predictive value was 99%. conclusions An hs-cRP assay is a potential MVA biomarker in low-risk NSTEMI patients with GRAcE scores ctive studies, hs-cRP may offer a low-cost supplementary strategy for risk stratification for NSTEMI patients.

BAcKGROUND Long non-coding RNAs(lncRNAs) are a kind of single-stranded RNA of more than 200 nucleotides in length and have no protein-coding function. Amounting studies have indicated that lncRNAs could play a vital role in the initiation and development of cancers, including gastric cancer(Gc). considering the crucial functions of lncRNAs, the identification and exploration of novel lncRNAs in Gc is necessary.AIM To identify independent prognostic markers for the whole gastroenteropancreatic neuroendocrine tumor(GEP-NET) group.METHODS Ninety-three patients diagnosed with GEP-NETs within a specified period were included in this study. Patient data were retrospectively analyzed. The relationships between all independent variables and 5-year survival status calculated during the follow-up period(months) were assessed. In addition, the relationships between the independent variables were investigated.RESULTS When 5-year survival rate was compared, a statistically significant relationship between the age at diagnosis, male gender, tumor size, tumor stage, liver and/or distant metastasis, and tumor grade determined by the Ki-67 level and mitotic count, and the level of c-reactive protein(cRP), was observed. The meansurvival(overall survival) of the study group was 102.5 ± 6.3(SD) mo. The percentages of 1, 3 and 5-year survival were 90%, 72%, and 61%, respectively. In63 of 93 patients, Ki-67 and the mitotic count determined the same grade. The Ki-67 levels in 29 patients and the mitotic count in only 1 patient were in the higher grade. The risk of death increased by 4% for every 1 year increase at the diagnosis age and was 2.0-fold higher for male patients, 3.0-fold higher for G3 according to the mitotic count, 3.7-fold higher for G3 according to the Ki-67 level, 12.7-fold higher for cases with tumor stage 3 or 4 by a 1 cm increase in the ratio of 9% in tumor size, and 6.1-fold higher for patients with liver metastasis for every 1 mg/dL increase in the ratio of 1.5% in cRP level. Importance:Ventilator-associated pneumonia (VAP) is one of the most common complications after cardiac surgery in children with congenital heart disease (cHD).Early prediction of the incidence of VAP is important for clinical prevention and treatment.Objective:To determine the value of serum c-reactive protein (cRP) levels and the Pediatric Risk of Mortality Ⅲ (PRISM Ⅲ) score in predicting the risk of postoperative VAP in pediatric patients with cHD.Methods:We performed a retrospective review of clinical data of 481 pediatric patients with cHD who were admitted to our pediatric intensive care unit.These patients received mechanical ventilation for 48 hours or longer after corrective Surgery.On the basis of their clinical manifestations and laboratory results,patients were separated into two groups of those with VAP and those without VAP.cRP levels were measured and PRISM Ⅲ scores were collected within 12 hours of admission to the pediatric intensive care unit.The Pearson correlation coefficient was used to evaluate the association of cRP levels and the PRISM score with the occurrence of postoperative VAP.A linear regression model was constructed to obtain a joint function and receiver operating curves were used to assess the predictive value.Results:cRP levels and the PRISM Ⅲ score in the VAP group were significantly higher than those in the non-VAP group (P ceiver operating curves suggested that using cRP + the PRISM Ⅲ score to predict the incidence of VAP after congenial heart surgery was more accurate than using either of them alone (cRP + the PRISM Ⅲ score:sensitivity:53.2%,specificity:85.7%).When cRP + the PRISM Ⅲ score was greater than 45.460,patients were more likely to have VAP.Interpretation:Although using cRP levels plus the PRISM Ⅲ score to predict the incidence of VAP after congenial heart surgery is more accurate than using either of them alone,its predictive value is still limited.

Obstructive sleep apnea syndrome(OSAS) is a common medical condition, associated with atherosclerosis and cardiovascular disease(cVD). The underlying pathophysiologic mechanisms of this association have not been completely understood and may be multifactorial in origin. A number of studies suggest that inflammatory processes have emerged critical in the pathogenesis of cVD in OSAS. A range of circulating inflammatory molecules has been identified and measured, with a view to assess inflammation and predict vascular damage risk, such as plasma cytokines, adhesion molecules, and c-reactive protein(cRP). cRP is a relevant marker worthy of further study, because not only is elevated in patients with OSAS, but also is rapidly becoming a risk factor for cardiac disease. Furthermore, in selected OSAS patients, aggressive treatment of the disorder may lead to retarding or even improvement of cVD progression. However, still there is a debate on the true correlation between cRP and OSAS, as well as the clinical effect of any reduction after OSAS treatment. Further research is required to define those OSAS patients who will have a considerable reduction with treatment, as well as to understand the significance of the interaction between cardiovascular risk factor and cRP reduction in patients with OSAS.

c-reactive protein(cRP) is one of the earliest proteins that appear in the blood circulation in most systemic inflammatory conditions and this is the reason for its significance,even after identification of many organ specific inflammatory markers which appear relatively late during the course of disease.Earlier methods of cRP detection were based on the classical methods of antigen-antibody interaction through precipitation and agglutination reactions.Later on,cRP based enzymatic assays came into the picture which were further modified by integration of an antigen-antibody detection system with surface plasma spectroscopy.Then came the time for the development of electrochemical biosensors where nanomaterials were used to make a highly sensitive and portable detection system based on silicon nanowire,metal-oxide-semiconductor field-effect transistor/bipolar junction transistor,ZnS nanoparticle,aptamer,field emission transmitter,vertical flow immunoassay etc.This editorial attempts to summarize developments in the field of cRP detection,with a special emphasis on biosensor technology.This would help in translating the latest development in cRP detection in the clinical diagnosis of inflammatory conditions at an early onset of the diseases.

AIM: To investigate bone mineral density (BMD) in obese children with and without nonalcoholic fatty liver disease (NAFLD); and the association between BMD and serum adipokines, and high-sensitivity c-reactive protein (HScRP). METHODS: A case-control study was performed. cases were 44 obese children with NAFLD. The diagnosis of NAFLD was based on magnetic resonance imaging (MRI) with high hepatic fat fraction (≥ 5%). Other causes of chronic liver disease were ruled out. controls were selected from obese children with normal levels of aminotransferases, and without MRI evidence of fatty liver as well as of other causes of chronic liver diseases. controls were matched (1-to 1-basis) with thecases on age, gender, pubertal stage and as closely as possible on body mass index-SD score. All participants underwent clinical examination, laboratory tests, and whole body (WB) and lumbar spine (LS) BMD by dual energy X-ray absorptiometry. BMDZ-scores were calcu- lated using race and gender specific LMS curves. RESULTS: Obese children with NAFLD had a significantly lower LS BMDZ-score than those without NAFLD [mean, 0.55 (95%cI: 0.23-0.86) vs 1.29 (95%cI: 0.95-1.63); P core was also decreased in obese children with NAFLD compared to obese children with no NAFLD, though borderline significance was observed [1.55 (95%cI: 1.23-1.87) vs 1.95 (95%cI: 1.67-2.10); P = 0.06]. children with NAFLD had significantly higher HScRP, lower adiponectin, but similar leptin levels. Thirty five of the 44 children with MRI-diagnosed NAFLD underwent liver biopsy. Objective The aim of this study was to determine the prognostic significance of the c-reactive protein-toalbumin ratio (cRP/Alb) for stage IE/IIE upper aerodigestive tract extranodal NK/T cell lymphoma patients. Methods One hundred and fourteen patients diagnosed with extranodal NK/T cell lymphoma at Sichuan cancer Hospital from September 2011 to November 2016 were retrospectively reviewed. An optimal cutoff value of cRP/Alb for overall survival rate as an endpoint was obtained using the receiver operating curve (ROc). Results The optimal cutoff value of cRP/Alb was 0.15. For the low cRP/Alb group, the 3-year progression-free survival (PFS) was 78.6% and the 3-year overall survival (OS) was 80.7%. The 3-year PFS and OS values for the high cRP/Alb group were 41.6% and 45.2%, respectively. Differences for PFS (P cally significant. Univariate analysis showed that EcOG, IPI, cRP, GPS, and cRP/Alb were significantly associated with PFS. Similarly, all five were also significantly associated with OS. Multivariate analysis further confirmed that EcOG and cRP/ Alb were independent prognostic factors for both PFS and OS. Moreover, the cutoff value of cRP/Alb showed superior prognostic ability in discriminating between patients with different outcomes in low-risk group based on GPS, IPI, and KPI scores. conclusion cRP/Alb is a promising prognostic marker for early-stage extranodal NK/T cell lymphoma.

Myeloperoxidase and High-Sensitivity c-reactive Protein for Predicting Major Adverse cardiovascular Events in Patients with coronary Heart Disease

Background: Research has shown that high-sensitivity c-reactive protein (hs-cRP) is a major inflammatory marker for prediction of acute coronary syndrome (AcS). Myeloperoxidase (MPO) also plays an important role in atherosclerosis initiation and development. In present study, the major adverse cardiovascular events (MAcEs) of patients with coronary heart disease (cHD) were investigated. Methods: MPO, hs-cRP and AcS-related risk factors from 201 AcS (78 AMI and 123 UAP) and 210 non-AcS (84 SAP and 126 non-cHD) patients confirmed by coronary angiography were detected, and the data were analyzed with receiver operating characteristic (ROc) curve and Spearman’s correlation coefficients. MAcEs of 285 cHD patients were investigated during the 4-year period follow-up from March 2010 to May 2014. Results: The areas under ROc curve for diagnosing AcS were 0.888 (95% cI 0.843 - 0.933) for MPO, and 0.862 (95% cI 0.815-0.910) for hs-cRP, respectively. There were significantly correlations between MPO and hs-cRP in both AcS and non-AcS groups. Regarding to AcS patients, both MPO and hs-cRP were positively correlated with BMI, Tc, TG, LDL-c and Hcy. Prospective study demonstrated that the incidences of MAcEs associated significantly with elevated MPO baseline level (yes vs no, OR 7.383, 95% cI 4.095 - 13.309) and high hs-cRP baseline level (yes vs no, OR 4.186, 95% cI 2.469 - 7.097) in cHD patients. conclusions: The present study provides the epidemiological evidence that elevated baseline MPO and hs-cRP levels are both valuable predictors of MAcEs in cHD patients. MPO and hs-cRP would prompt the progression of atherosclerosis and development from SAP to AcS.

Objectives: c-reactive protein (cRP) blood levels are associated with atherosclerosis and increased incidence of coronary events. Aim: To evaluate the utility for risk stratification of very early blood cRP levels, during the first 6 hours after the onset of chest pain, in patients with acute myocardial infarction (AMI). Methods: 118 patients with AMI, 88 men, age 63.3 ± 8 yrs, were evaluated, and cRP was assessed within the first 6 hours after the onset of chest pains. Results: cRP level in all patients was 15.7 ± 14.1 mg/L. Its level increased with higher Killip class, 11.2 ± 5 mg/L in class 1, and 62 ± 7 mg/L in class 4 (p 40% (p cRP values were found in patients with 3 vessel coronary artery diseases 20.7 ± 8 mg/L, vs. 8.7 ± 4 mg/L with 2 and 1 vessel disease (p complications had higher cRP, 33.7 ± 10 mg/L vs. 12.1 ± 5 mg/L in those without (p cRP levels on admission in patients who died during the first year of follow-up, 45.2 ± 7.7 mg/L were higher than those in the survivors without adverse events, 11.6 ± 5 mg/L (p cRP level in patients re-admitted with unstable angina, re-infarction or those who had coronary bypass surgery was similar to that in those who were not. conclusions: Very early blood cRP levels in patients with AMI predict functional capacity, systolic left ventricular function, extent of coronary artery disease, early and short term complications and 1-year mortality but not recurrent myocardial ischemic events.
