Magnetic resonance imaging characteristics of postoperative intracranial dissemination of recurrent gliomas

Ten intracranial gliomas cases, that had postoperative intracranial dissemination, underwent magnetic resonance imaging （MRI） examinations, including T1 weighted imaging, fat-suppressed T1 weighted imaging, T2 weighted imaging and fluid attenuated inversion recovery （FLAIR）. Results showed that tumor metastasis had occurred via the cerebrospinal fluid, the brain white matter fibers and the surgical access site alone. On the plain MRI scans, 1/7 cases were linearly thickened with isointensity and 5/7 cases exhibited nodular foci on T1 weighted imaging; the cerebral sulci and cisterns in 2/7 cases had become shallow and five cases had nodular foci on T2 weighted imaging. FLAIR imaging revealed that the cerebral sulci and cisterns in 2/7 cases had become shallow and that six cases had affected nodular foci. The contrast-enhanced MRI scans revealed linear thickening in seven cases, nodules in seven cases, similarities to ＂mould-like＂ signs in six cases and hydrocephalus in six cases. These findings suggested that MRI with different sequences can diagnose glioma metastasis.

BACKGROUND Enteropathy-associated T cell lymphoma(EATL)is an aggressive intestinal T cell lymphoma derived from intraepithelial lymphocytes,which occurs in individuals with celiac disease(CD).Cerebral involvement is an extremely rare condition and as described so far,lymphoma lesions may present as parenchymal predominantly supratentorial or leptomeningeal *** describe a case of EATL with multifocal supra-and infratentorial brain involvement in a patient with refractory celiac disease(RCD).CASE SUMMARY A 58-years old man with known CD developed ulcerative jejunitis and was diagnosed with RCD type *** months later he presented with subacute cerebellar symptoms(gait ataxia,double vision,dizziness).Cranial magnetic resonance imaging(MRI)revealed multifocal T2 hyperintense supra-and infratentorial *** studies of blood and cerebrospinal fluid were inconspicuous for infectious,inflammatory or autoimmune diseases.18Ffluorodeoxyglucose-positron emission tomography/computed tomography(18FDG-PET/CT)scan showed a suspect hypermetabolic lesion in the left upper abdomen and consequent surgical jejunal resection revealed the diagnosis of *** the diagnostic work-up,neurological symptoms aggravated and evolved refractory to high-dosage *** MRI scans showed progressive cerebral lesions,highly suspicious for lymphoma and methotrexate chemotherapy was ***,clinically the patient responded only ***,cerebral biopsy confirmed the diagnosis of cerebral involvement of *** the poor prognosis and deterioration of the performance status,best supportive care was *** patient passed away three weeks after *** EATL with cerebral involvement must be considered as a possible differential diagnosis in patients with known RCD presenting with neurological symptoms.

PURPOSE: This study reported our experience with chemotherapy and/or radiotherapy in series of patients who had a clinical diagnosis of cranial germ cell tumors （GCTs） without surgical biopsy confirmation. METHODS: We retrospectively analyzed the outcome of patients who had typical MRI evidence of a pineal or suprasellar mass with or without elevated levels of beta-human chorionic gonadotropin （β-HCG） and/or alpha- fetoprotein （AFP）. The patients were grouped into chemotherapy（n=21 ）, radiotherapy（n=5）, and stereotactic radiotherapy （n=12） based on the primary treatment. RESULTS: Between May 2002 and August 2010, a total of 38 patients were treated. Median age of the patients at diagnosis was 16 years old （range, 4-30 years） with 33 males and 5 females. Twenty-six （68.4%） patients showed elevated levels ofβ-HCG and/or AFP in serum, cerebrospinal fluid, or both. Twelve patients had received prior ventricular- peritoneal shunt. Twenty-one patients received primary chemotherapy resulting in 9 complete response （CR）, 8 partial response （PR）, 2 stable disease （SD） and 2 not evaluate（NA）. The objective response rate（ORR）was 81%. In which, 17 patients followed by adjuvant radiotherapy. Five patients received primary radiotherapy resulting in 3 CR and 2 PR. The ORR was 100%. In which, 1 patient followed by adjuvant chemotherapy. Twelve patients received primary stereotacticradiotherapy（SRS） resulting in 2 CR, 9 PR and 1 SD. The ORR was 91.7%. In which, 4 patients followed by adjuvant chemotherapy after SRS and 8 of 12 patients received salvage chemotherapy when disease recurred. Six （out of 12） patients received whole brain or cranio-spinal radiotherapy after chemotherapy. Chemotherapy was consisted of cisplatin, VP-16/VM-26 and bleomycin（PEB regimen）.The median follow-up time is 29 months （range, 2-98 months）.
