Efficacy evaluation of different forms of traditional Chinese health-preservation exercises for osteoporosis:a network meta-analysis

Objective To systematically evaluate the effects of different forms of traditional Chinese health-preservation exercises on osteoporosis(OP)using network *** A systematic search on Excerpta Medica Database(EMBASE),Springer Link,Allied and Complementary Medicine Database(AMED),PubMed,Cochrane Library,China National Knowledge Infrastructure(CNKI),Wanfang Academic Journal Full-text Database(Wanfang)and Chongqing VIP Database(CQVIP)targeted the randomized controlled trials(RCTs)studying traditional Chinese exercises for OP published up to January *** handbook was adopted to estimate the publication bias in the included studies,and statistical analysis was performed using Stata 14.0 and GeMTC 0.14.3 when data were *** Fifty RCTs were included in the network meta-analysis,comprising a total of 4505 OP *** network meta-analysis showed that in terms of visual analog scale(VAS)for pain,Tai Ji Quan(Tai Chi)was the most efficacious,followed by Yi Jin Jing(Sinew-transforming Qigong Exercises),Ba Duan Jin(Eight-sectioned Exercise),Wu Qin Xi(Five-animal Exercises),sports training,drug and blank control;in terms of bone mineral density(BMD)of femoral neck,Yi Jin Jing was the most efficacious,followed by Wu Qin Xi,Ba Duan Jin,Tai Ji Quan,sports training,blank control and drug;regarding the lumbar BMD,it was Yi Jin Jing,Tai Ji Quan,Ba Duan Jin,Wu Qin Xi,sports training,blank control and drug in the descending order of efficacy;in terms of serum alkaline phosphatase,it was Yi Jin Jing,Tai Ji Quan,sports training,Wu Qin Xi,Ba Duan Jin,drug and blank control in the descending order of *** The evidence to date suggests that the first choice for OP amongst the traditional Chinese exercises should be Yi Jin Jing,which can not only reduce the subjective pain,but also promote bone formation and increase BMD,though this conclusion requires more high-quality large-scale RCTs for further proof.

Therapeutic effect of heat-sensitive moxibustion plus medications for senile osteoporosis and its effect on serum BMP-2 and OPG levels

Objective:To observe the clinical efficacy of heat-sensitive moxibustion plus medications on senile osteoporosis(SOP),and to explore the related ***:A total of 70 elderly participants with osteoporosis were randomly divided into an observation group and control group,with 35 cases in each *** control group was treated with conventional drugs,and the observation group was treated with heat-sensitive moxibustion on the basis of the conventional *** groups were treated for 3 *** and after treatment,assessed the visual analog scale(VAS)and Oswestry disability index(ODI)scores,determined the bone mineral density of the participants'lumbar spine(L2-L4)and left femoral neck,and detected the participants'serum bone morphogenetic protein-2(BMP-2)and osteoprotegerin(OPG)***:After treatment,the VAS scores of both groups were lower than before treatment(both Pbone mineral density values of the lumbar spine and left femoral neck in both groups were significantly higher than before treatment(both Pbone mineral density values of the observation group were higher than those of the control group(Pbone density,which may be related to the improvement of the serum BMP-2 and OPG levels.

Gardner syndrome (also known as familial colorectal polyposis) is a rare autosomal dominant condition. It is characterized by osteomas, familial polyposis of the colon, cutaneous epidermoid cysts, soft tissue tumors, as well as certain dental anomalies. We report the case of a 29 years old male patient with Gardner’s syndrome who presented with no intestinal polyps but instead had an osteoma on the left side of the mandible and multiple impacted teeth. The case showed the possibility to manage the multiple impacted teeth without jeopardizing the health of the teeth in terms of mobility or significant root resorption taking into consideration the increased bone density in such patients.

Changes of bone mineral in steroid-induced osteonecrosis in rabbits

To investigate the significance of bone mineral changes in the pathogenic process of steroid-induced osteonecrosis in rabbits. Methods： Seventy adult female Japanese White rabbits were randomly divided into five groups Animals in steroid group received an intramuscular injection of methylprednisolone acetate （4 mg/kg） once a week, and killed in four different time after first treatment. Animals in control group received an intramuscular injection with the same volume of normal saline, while 150 kU penicillin was injected twice a week for preventing infection in all groups. The contents of serum calcium （Ca） and phosphorus （P） were determined and the bone mineral densities of the femoral head and neck were measured at the 2nd, 4th, 8th and 10th weeks after first treatment. Meanwhile the general condition such as weight, spirit and hair of rabbits was observed. Results： Compared with the control group, the weight of rabbits in steroid group lost since the first week, and progressively aggravated; the bone mineral density of the femoral head and neck was lowered since the 2nd and 4th week respectively （P〈0.05, P〈0.01）, and lowered more quickly in femoral neck than in femoral head. In steroid group, the bone mineral density of the femoral neck at the 10th week was significantly lower than that at the 8th week （P〈0.05） Compared with the control group, the serum levels of Ca and P in steroid group were lowered since the 2nd week （P〈0.05, P〈0.01）. In steroid group, the serum Ca level at the 10th week was lower than that at the 8th week, and the serum P level waslowered more quickly than Ca level was. Conclusion： Osteonecrosis appears in rabbits shortly after corticoids are first administered, and osteonecrosis in rabbits is chronologically associated with the onset of hypocalcaemia, hypophosphatemia and lowered bone mineral density. Steroid-induced osteonecrosis may be relevant to bone mineral loss and density lowering of the femoral head and neck.

The Effect of the Paleo Diet on People Suffering from Osteoporosis (Review Paper)

Osteoporosis is a disease that decreases bone mass and increases bone porosity, weakening bones. The Paleo diet is an eating plan that imitates the dietary patterns of the Stone Age. It excludes grains, dairy, and processed foods and emphasizes feeding on lean meats, fruits, vegetables, and nuts. Consumption of the Paleo diet has many positive sides, such as high protein intake and weight loss. Still, excluding dairy products risks calcium and vitamin D deficiencies, which are crucial for bone health. Statistics and simulations that have explored the relationship between the Paleo diet and bone health (especially for people suffering from low bone density) show mixed outcomes on bone health.). While the consumer does get lots of benefits from fruit and vegetable intake in a large sum due to them containing nutrients like magnesium, potassium, and vitamin K (which are also necessary for bone health), the lack of dairy products (gives the maximum amount of calcium and vitamin D) raises concerns about maintaining adequate bone mineral density (BMD). More information on this topic shows the negative impact of this diet on people suffering from osteoporosis due to a lack of nutrient intake that nourishes the bone. Although the Paleo diet can enhance overall health through nutrient-dense foods and reduced processed intake, it can’t be said the same for people suffering from osteoporosis.

Use of Extra-Narrow-Diameter Implants in Reduced Alveolar Ridge:A Case Report

Background: Narrow-diameter implants (3.0 - 3.5 mm range) have been introduced for the replacement of teeth with insufficient bone structure and/or limited mesiodistal or interimplant spaces, and appear to offer clinical results similar to those obtained with implants of greater diameter. Studies using extra-narrow-diameter implants (2.8 mm) are scarce. Case Presentation: A 59-year-old male patient received two extra-narrow-diameter implants, 2.8 × 11 mm in the region between elements 11 and 14. Together, two 3.5 × 8.5 mm SYSTHEX®platform 4.1 implants were installed in the region of elements 15 and 16 to provide greater stability in the occlusion. Of four previous implants on the maxillary left side, one in the region between the elements 23 and 24 that was located in a very apical position and vestibularized was removed. The provisional was already installed on the elements 11, 21, and 22 with the metal cores already prepared and with the Globteck®implants in the region of the elements 23, 24, and 27. The functional and esthetic results were satisfactory. Conclusions: Insertion of extra-narrow-diameter implants of 2.8 mm in the maxillary anterior region is a reliable option in a patient with absence of elements 12 and 13, restoring masticatory function and aesthetics in the upper arch.

Yi Jin Jing （Sinew-transforming Qigong Exercises） for primary osteoporosis in the elderly： a clinical trial

Objective：To observe the clinical efficacy of Yi Jin Jing （Sinew-transforming Qigong Exercises） for primary osteoporosis in the ***：Eighty old patients with primary osteoporosis were randomized into a Yi Jin Jing group and a medication group,40 cases in each *** Yi Jin Jing group was intervened by Yi Jin Jing （Sinew-transforming Qigong Exercises） training,while the medication group was intervened by oral administration of alendronate sodium *** to and after 6-month intervention,the bone mineral density （BMD） of the proximal femur,visual analog scale （VAS） and activities of daily living （ADL） were ***：There were no significant differences in the BMD of the proximal femur,and VAS and ADL scores between the two groups before the intervention （P〉0.05）.After 6-month intervention,the above items all improved significantly in both groups （both P〈0.01）;the improvements in VAS and ADL scores in Yi Jin Jing group were more significant than those in the medication group （P〈0.01）,while the between-group difference in the BMD of the proximal femur was statistically insignificant （P〉0.05）.Conclusion：Practice of Yi Jin Jing （Sinew-transforming Qigong Exercises） can effectively ameliorate the BMD in the elderly with primary osteoporosis,and it can reduce the pain and improve ADL,with a better general effect compared to oral administration of alendronate sodium tablet.

The effects of soybean isoflavaones on the mRNA expression of OPG, ODF and M-CSF in bone tissue of ovariectomized rat

Objective： To observe the effects of the soybean isoflavaones on the rnRNA expression of osteoprotegerin（OPG）, osteoclast differentiation factor（ODF） and macrophage colony stimulating factor（M-CSF） in bone tissue of ovariectomized rat, and to investigate the possible pathway to prevent and treat postrmenopausal osteoporosis using soybean isoflavaones. Methods： Thirty healthy adult SD rats were randomly divided into 3 groups： sham-operated group, ovariectomized group and soybean isoflavaones treated group. All rats were ovariectomized except those in sham-operated group. The bone density of the 3-6th lumbar vertebrae was detected after 12 weeks. Total RNA was extracted from femur bone and the mRNA expression of OPG, ODF and M-CSF was examined by real time PCR. Results： Soybean isoflavaones increased the bone density of the lumbar vertebrae in ovariectomized rat and up-regulated the expression of OPG, whereas down-regulated the expression of M-CSF and the ratio of ODF to OPG. Conclusion： The effects of soybean isoflavaones on postmenopausal osteoporosis are tightly correlated with OPG, M-CSF and the ratio of ODF to OPG.

Early period of fracture healing in ovariectomized rats

Objective: To evaluate the effect of osteoporosis on fracture healing through observing the histomorphological changes, bone mineral density of callus and expression and distribution of transforming growth factor beta 1 (TGF β1),basic fibroblast growth factor (bFGF) and bone morphogenetic protein 2 (BMP 2) in ovariectomized rats. Methods: Sixty female Sprague Dawley rats (aged 12 weeks and weighing 235 g on average) were randomly divided into an ovariectomized (OVX) group (n=30) and a sham operated (SO) group (n=30). Ovariectomy was performed in the OVX rats and same incision was made in the SO rats. Three months later, fracture of femoral shaft was made on all the rats. Then they were killed at different time points. Callus formation was observed with histological and immunohistochemical methods. Results: A reduction in callus and bone mineral density in the healing femur and a decrease of osteoblasts expressing TGF β1 near the bone trabecula were observed in the OVX rats 3 4 weeks after fracture. Histomorphological analysis revealed a higher content of soft callus in the OVX rats than that in the SO rats. Immunohistochemistry results showed that no remarkable difference in expression and distribution of BMP 2 and bFGF between the OVX and SO groups was found. Conclusions: Osteoporosis influences the quantity and quality of callus during the early period of fracture healing. The effect of osteoporosis on fracture healing has no relationship with the expression of BMP 2 or bFGF. The decreased expression of TGF β1 in osteoblasts may cause a decrease in quality of facture healing after osteoporosis.
