bleomycin肺炎的临床病理学研究胸部 X 线为重点的鉴别诊断

Pulmonary fibrosis(PF) is a chronic, progressive, fatal interstitial disease. The aim of the present study was to explore the possible discourse of DXM by intraperitoneal injection against the prevention of BLM-induced lung fibrosis in rats through modulation of TGF-β/Smad, PI3 K/Akt/mTOR and NF-κB signaling pathway. Methods Lung fibrosis was persuaded in Sprague-Dawley rats by a single intratracheal BLM(5 mg/kg) injection. Then, these rats were divided into three groups and named as control, model and DXM group. The lung function and biochemical, histopathological, and molecular alterations were studied in the lung tissues. Results These results demonstrated that the progression of BLM-induced PF in rats can be delayed by DXM treatment, which appeared to alleviate not only inflammation but also lung damage and fibrogenic activity trough TGF-β/Smad, PI3 K/Akt/mTOR and NF-κB signaling pathway. Conclusion All indicated a possible new role for DXM in the treatment of fibrosis.

抗癌药bleomycin对鳞状上皮癌有明显效果,其副作用为肺纤维化而导致病人死亡。同时也常有合并细菌性肺炎,或两者并存。因此对梗阻性肺炎、病毒性肺炎等引起之间质性肺炎,在鉴别上比较困难。著者对55例肺癌进行观察,其中使用Bleom-ycin100mg以下者15例,有3例合并细菌性肺炎。用量在300mg以下者20例,其中出现肺纤维症1例,细菌性肺炎2例,两者合并存在者2例。用药600mg以下者20例,其中肺纤维化3例,细菌性肺炎2例,两者合并存在者1例。以上均经解剖证实。

Objective:To initially explore traditional Chinese medicine patterns in a bleomycin-induced pulmonary fibrosis mouse ***:Thirty-six C57BL/6 mice were divided by the random number table method(with 12 rats per group)into three groups:a blank group,a model group,and a number 2 Feibi recipe(FBR-2)*** pulmonary fibrosis mouse model was established by intratracheal instillation of *** FBR-2 group was treated with FBR-2 for 4 *** in the mice such as mental behavior,food/water intake,body weight,body temperature,respiratory rate,and tongue image were *** samples were collected on the 14th day and 28th day after modeling,and lung tissues were visually assessed and microscopically evaluated by staining with hematoxylin-eosin and *** expression levels of hydroxyproline,interleukin(IL)-33,IL-37,tissue plasminogen activator,and plasminogen activator inhibitor-1 were determined by enzyme-linked immunosorbent ***:Mice in the model group were poor in spirit,less active,slow in response,showed reduced food/water intake,body temperature,and body weight,increased respiratory rate,and their tongue color had changed from light red to dark ***,treatment with FBR-2 significantly improved these *** inflammatory cell infiltration and collagen fiber deposition were observed in the lung tissues of the model *** with the blank group,the levels of hydroxyproline,IL-33,and plasminogen activator inhibitor-1 in the model group significantly increased(all Pbleomycin-induced pulmonary fibrosis mouse model exhibited traditional Chinese medicine patterns of qi deficiency,blood stasis,and heat retention.

Pulmonary fibrosis, a chronic inflammatory disease that occurs mainly in older adults, is a serious health threat with few effective treatment options. The etiological aspects of pulmonary fibrosis remain unknown, though some factors such as cigarette smoking, viral infections, surfactant protein polymorphisms, and chronic or high doses use of certain drugs are considered to be risk factors for the progression of pulmonary fibrosis. No standard treatments have been introduced in clinic yet. Although glucocorticoids and antioxidant drugs have been administered, the severe and broad-spectrum adverse effects of glucocorticoids limit their use. Efforts to identify novel therapeutic agents with improved safety profiles are therefore ongoing. In this review, the authors have described the effects of herbal extracts and compounds and certain pharmacological agents on pulmonary fibrosis in animal models. These effects indicate that herbs are a promising source of compounds that can play pivotal roles in the treatment of lung fibrosis.

Idiopathic pulmonary fibrosis(IPF)is a chronic and fatal lung disease characterized by pulmonary inflam-mation,oxidative stress,and excessive extracellular matrix(ECM)*** anti-fibrotic drugs for IPF treatment in the clinic lack selectivity and demonstrate unsatisfactory efficacy,highlighting the urgent necessity for a novel therapeutic ***(TA),which has biological activities against lung injury induced by various factors,is a potential anti-IPF drug due to its anti-inflammatory,antiox-idant and lung-protective ***,the protective effect of TA on IPF has not been confirmed,and its clinical application is limited due to its poor aqueous *** this study,we demonstrated that TA could inhibit epithelial-mesenchymal transition(EMT)and migration of A549 cells by inhibiting the transforming growth factor-β1(TGF-β1)/Smad signaling *** improve the aqueous solubility and pulmonary administration performance of TA,we prepared TA loaded methoxy poly(ethylene glycol)-poly(d,l-lactide)(mPEG-PLA)/d-α-tocopheryl polyethylene glycol succinate(TPGS)mixed polymeric mi-celles(TA-PM).Then a MicroSprayer^(R) Aerosolizer was used to deliver TA-PM once every two days for three weeks to evaluate their therapeutic effects on bleomycin(BLM)-induced IPF *** results demonstrated that inhaled TA-PM significantly inhibited BLM-induced inflammation,oxidative stress and fibrosis in lung ***,TA-PM exhibited high pulmonary deposition and retention by pul-monary administration,along with a favorable safety ***,this study emphasizes the potential of inhaled TA-PM as a promising treatment for IPF,providing a new opportunity for their clinical appli-cation.

The cell cycle inhibitor P21 has been implicated in cell senescence and plays an important role in the injury-repair process following lung *** fibrosis(PF)is a fibrotic lung disorder characterized by cell senescence in lung alveolar epithelial *** this study,we report that P21 expression was increased in alveolar epithelial type 2 cells(AEC2 s)in a time-dependent manner following multiple bleomycin-induced *** injury of AEC2 s resulted in telomere shortening and triggered P21-dependent cell ***2 s with elevated expression of P21 lost their self-renewal and differentiation *** particular,elevated P21 not only induced cell cycle arrest in AEC2 s but also bound to P300 andβ-catenin and inhibited AEC2 differentiation by disturbing the P300-β-catenin ***,senescent AEC2 s triggered myofibroblast activation by releasing profibrotic *** of P21 restored AEC2-mediated lung alveolar regeneration in mice with chronic *** results of our study reveal a mechanism of P21-mediated lung regeneration failure during PF development,which suggests a potential strategy for the treatment of fibrotic lung diseases.

Idiopathic pulmonary fibrosis(IPF),characterized by aggravated alveolar destruc-tion and fibrotic matrix deposition,tendentiously experiences the stage called acute exacerbation IPF(AE-IPF)and progresses to multiple organ damage,especially liver *** studies have found a variety of immune microenvironment disorders associated with elevated IPF risk and secondary organ injury,whereas current animal models induced with bleomycin(BLM)could not completely reflect the pathologi-cal manifestations of AE-IPF patients in clinic,and the exact underlying mechanisms are not yet fully *** the current study,we established an AE-IPF model by tracheal administration of a single dose of BLM and then repeated administrations of lipopolysaccharide in *** mouse model successfully recapitulated the clinical features of AE-IPF,including excessive intrapulmonary inflammation and fibrosis and extrapulmonary manifestations,as indicated by significant upregulation of Il6,Tnfa,Il1b,Tgfb,fibronectin,and Col1a1 in both lungs and liver and elevated serum aspartate transaminase and alanine transaminase *** effects might be attributed to the regulation of Th17 *** sharing this novel murine model,we expect to pro-vide an appropriate experimental platform to investigate the pathogenesis of AE-IPF coupled with liver injury and contribute to the discovery and development of targeted interventions.
