Accuracy of corneal astigmatism correction with two barrett Toric calculation methods

AIM: To compare the prediction error between barrett Toric calculator and the new online AcrySof Toric calculator which incorporated barrett astigmatism algorithm in Chinese cataract eyes with normal axial length and anterior chamber depth(ACD). METHODS: Prospective case-control study. All the cases had axial length(21-26 mm) with ACD no less than 2.4 mm.Keratometric values were measured by LenSTAR 900. The barrett Toric calculator was used in group 1. In group 2, SRK-T formula was used to determine the spherical power of the Toric lens, and subsequent calculation of the cylinder type was performed using the new online Alcon Toric calculator. At 1 and 3 mo after surgery, a comprehensive subjective optometry was performed. The predicted residual astigmatism calculated by the two calculators was compared with that obtained by postoperative refraction,and the difference was defined as the astigmatism correction error [error of refractive astigmatism(ERA)].The error magnitude(EM) refers to the algebraic deviation of ERA, and the error vector(EV) indicates the vector deviation of ERA. The influence of the two calculation methods on the correction accuracy of toric IOL was quantitatively analyzed. RESULTS: The |EM| obtained at 1 mo after surgery were0.21±0.12 D, 0.22±0.18 D in group 1 and group 2 respectively,and correspondingly turned to be 0.19±0.13 D, 0.20±0.19 D at3 mo after surgery, with no statistical difference(P=0.633,P=0.877). The vector analysis showed that |EV| values in two groups at 1 mo after surgery were 0.29±0.14@105(D@angle) and 0.35±0.20@113(D@angle), respectively,whereas |EV| values 3 mo after surgery were 0.27±0.16@86(D@angle) and 0.32±0.23@102(D@angle), respectively. The differences between the groups were not statistically significant(P=0.119, P=0.261). CONCLUSION: The clinical effect of barrett Toric calculator has a much more accurate tendency than that of new online AcrySof Toric calculator, but is not evident in cases with normal axial length and normal anterior posterior ratio.

Systemic sclerosis(SSc)is a multisystem connective-tissue disorder of unknown etiology affecting various internal organs and skin.Esophageal involvement is present in≤70%–90%of the patients and is manifested by severe gastroesophageal-reflux disease and dysmotility.Although barrett’s esophagus(BE)has been reported in patients with SSc[1,2],the risk of neoplastic progression has not been reported before in the USA.Therefore,we aimed to study the prevalence of BE in SSc and incidence of neoplastic progression.

AIM:To examine how the expression of caudal type homebox transcription factor 2(Cdx2) is regulated in the development of malignancy in barrett's esophagus.METHODS:Cdx2,mucin(MUC) series(MUC2,MUC5AC and MUC6),p53 and E-cadherin expression in barrett's esophagus and adenocarcinoma specimens were examined by immunostaining.Isolated clusters of cells from(1) MUC2 and Cdx2-positive intestinal metaplastic mucosa;(2) MUC5AC and MUC6-positive,and MUC2 and Cdx2-negative high-grade dysplasia(HD),or intramucosal adenocarcinoma(IMC);and(3) MUC5AC,MUC6 and Cdx2-positive poorly-differentiated invasive adenocarcinoma(PDA) were analyzed by methylationspecific polymerase chain reaction using sets of primers for detecting methylation status of the Cdx2 gene.RESULTS:Most of the non-neoplastic barrett's esophageal mucosa showing intestinal-type metaplasia with or without low-grade dysplasia was positive for E-cadherin,MUC series and Cdx2,but negative for p53.A portion of the low-grade to HD was positive for E-cadherin,MUC5AC,MUC6 and p53,but negative for MUC2 and Cdx2.The definite IMC area was strongly positive for MUC5AC,MUC6 and p53,but negative for MUC2 and Cdx2.Methylation of the Cdx2 promoter was not observed in intestinal metaplasia,while hypermethylation of part of its promoter was observed in hot dipped and IMC.Hypermethylation of a large fraction of the Cdx2 promoter was observed in PDA.CONCLUSION:Cdx2 expression is restored irrespective of the methylation status of its promoter.Apparent positive immunohistochemical results can be a molecular mark for gene silencing memory.

AIM:To investigate the lifetime risk of development of esophageal adenocarcinoma and/or high-grade dysplasia in patients diagnosed with barrett’s esophagus.METHODS:Data were extracted from the United Kingdom National barrett’s Oesophagus Registry on date of diagnosis,patient age and gender of 7877 patients from who had been registered from 35 United Kingdom centers.Life expectancy was evaluated from United Kingdom National Statistics data based upon gender and age at year at diagnosis.These data were then used with published estimates of annual adenocarcinoma and high-grade dysplasia incidences from metaanalyses and large population-based studies to estimate overall lifetime risk of development of these study endpoints.RESULTS:The mean age at diagnosis of barrett’s esophagus was 61.6 years in males and 67.3 years in females.The mean life expectancy at diagnosis was23.1 years in males,20.7 years in females and 22.2years overall.Using data from published meta-analyses,the lifetime risk of development of adenocarcinoma was between 1 in 8 and 1 in 14 and the lifetime risk of high-grade dysplasia or adenocarcinoma was 1 in 5 to1 in 6.Using data from 3 large recent population-based cohort studies the lifetime risk of adenocarcinoma was between 1 in 10 and 1 in 37 and of the combined endpoint of high-grade dysplasia and adenocarcinoma was between 1 in 8 and 1 in 20.Age at barrett’s esophagus diagnosis is reducing and life expectancy is increasing,which will partially counter-balance lower annual cancer incidence.CONCLUSION:There is a significant lifetime risk of development of high-grade dysplasia and adenocarcinoma in barrett’s esophagus.

AIM:To evaluate the Z-line visualization by the PillCamTM SB2 using three different ingestion protocols. METHODS:Ninety consecutive patients undergoing small bowel capsule endoscopy(SBCE)between January and May 2008 were included in the study. They swallowed the capsule in the standing(Group A= 30),supine(Group B=30)and right supine positions (Group C=30).Baseline patient characteristics, difficulties in capsule ingestion,esophageal transit times(ETT)and Z-line visualization were noted. RESULTS:No significant differences were found between the groups with regard to baseline patient characteristics,ingestion difficulties and complete SB examinations(P>0.05).At least 1 frame of the Z-line was detected in 15.8%,46.7%and 90%of patients in groups A,B and C,respectively(P<0.001).The average number of Z-line images was 0.21±0.53,3.23 ±6.59 and 5.53±7.55 and the mean%of the Z-linedetected was 71.3,25.1 and 8.3,in groups A,B and C, respectively(both P<0.001).ETT times were longer in the supine group followed by the right supine and the standing groups(median of 237 s vs 64 s and 39 s, respectively;P<0.001). CONCLUSION:Z-line visualization in patients undergoing SBCE can be accurately achieved in most cases when the capsule is swallowed in the right supine position.

Dear Editor,Genetic engineering of T cells to express chimeric antigen receptors(CARs)is an efficient approach for clinical therapy of hematological malignancies(Kuwana et al.,1987;Eshhar et al.,1993;barrett et al.,2014).The CARs endow T cells with the ability to recognize specific antigens and bind them in an MHC-independent mariner,thereby overcoming some of the mechanisms that mediate tumor immune escape.In addition,by providing co-stimulatory signals,CARs endow T cells with enhanced cytotoxicity and persistence compared with primary T cells.A typical CAR comprises a single-chain variable frag me nt(scFv)derived from a monoclonal an tibody(mAb)for an tige n recog nition and signaling domains for co?activation(Eshhar et al.,1993;Sadelain et al.,2013).
