BACKGROUND Anaplastic thyroid cancer(ATC)is a rare but aggressive type of thyroid carcinoma.braf V600E-mutation,which is found in 10%-50%of ATCs,is associated with poor prognosis.A recent clinical trial reported a substantial clinical benefit of concomitant treatment of dabrafenib(braf inhibitor)and trametinib(MEK inhibitor)for treating braf V600E-mutant ATC.However,reports on patients with ATC treated with this regimen following surgery are lacking.CASE SUMMARY We report the case of a 63-year-old female patient diagnosed with braf V600Emutant ATC.Following three surgeries—total thyroidectomy,total laryngectomy,and neck dissection—she was diagnosed with lung metastasis during follow-up.The metastatic ATC was successfully treated with dabrafenib and trametinib.The patient achieved a complete response at the 32-mo follow-up.CONCLUSION Adjuvant chemotherapy with dabrafenib plus trametinib is efficacious for treatment and prevention of recurrent ATC with braf mutation following surgery.

The 2004 discovery of EGFR mutations,followed by ALK rearrangements,ushered in a targeted therapy era for advanced non-small cell lung cancer(NSCLC).Tyrosine kinase inhibitors targeting gene alterations have substantially improved survival and quality of life for patients with NSCLC.In the last decade,rearrangements of the ROS1 oncogene have been incorporated into healthcare practice that are applicable to another small subgroup of patients who benefit from similar targeted strategies.Recent genome studies of lung adenocarcinoma have identified other possible therapeutic targets,including RET,NTRK fusions,c-MET alterations,and activating mutations in KRAS,braf,and HER2,all with frequencies greater than 1%.Lung cancers harbouring these genome changes can potentially be treated with agents approved for other indications or under clinical development.This review updates the therapeutic arsenal that especially targets those genes.

近日,壁辰医药(ABM Therapeutics)自主研发的小分子braf抑制剂ABM-1310已经获得美国食品药品监督管理局(FDA)孤儿药资格认证,用于治疗brafV600突变的脑胶质母细胞瘤(CBM)。GBM是一种治疗困难的高侵袭性脑肿瘤,由于其具有复杂性和抗药性,传统的治疗方法如手术切除、放射疗法和化学治疗仍无法完全控制疾病的进展,且药物选择也非常有限。braf突变是一个主要的肿瘤驱动基因,占所有肿瘤的8%左右,在黑色素瘤、结肠癌、肺癌、脑胶质瘤等癌种中均有分布。

Objective:Signet ring cell carcinoma is a rare subtype of colorectal carcinoma(CRC)with an associated brafV600E mutation.We investigated frequencies of braf mutation in 28 CRCs containing variable signet ring cell component and their relation with clinicopathologic parameters.Methods:According to the presence of signet ring cell component,tumors were categorized into groups as follows:0%–9%,10%–24%,25%–49%,and>50%.Genomic DNA was isolated and analyzed for braf V600E gene mutation by polymerase chain reaction-restriction fragment length polymorphism.Eleven of 28 cases(39.3%)showed brafV600E mutation,which was also confirmed by Sanger sequencing.To elucidate the importance of existence of signet ring cell component at the molecular level,we separated cases into two groups with cut-off levels of 10%and 50%,which pertain to percentages of signet ring cells.Results:Seven of 19 cases(36.8%)under the threshold of 50%and four of nine cases(44.4%)over this threshold value demonstrated braf mutation.Three of 7 cases(42.8%)featuring10%were braf mutated.Conclusions:braf mutation must be closely associated with the presence of malignant signet ring cells regardless of their percentages.

肺癌是中国发病率和死亡率均排第1位的恶性肿瘤,其中非小细胞肺癌(NSCLC)约占肺癌的85%。braf突变在NSCLC患者中的发生率约为1.5%~5.5%,而braf V600突变约占所有braf突变的30%~50%,braf突变患者整体预后较差。目前,关于braf突变NSCLC的临床研究探索越来越多,新药研发层出不穷,但国内尚无braf突变NSCLC诊疗相关的规范化共识。中国抗癌协会肺癌专业委员会专家组整合国内外braf突变相关的指南共识和现有的临床研究结果,结合我国braf突变的NSCLC诊疗经验,制定了中国晚期非小细胞肺癌braf突变诊疗专家共识,针对braf突变NSCLC的临床评估、诊疗流程、合理用药、不良反应管理等给出了详细建议,旨在为braf突变NSCLC的规范化管理和诊疗提供参考意见。

Wild-Type KRAS and braf Could Predict Response to Cetuximab in Chinese Colorectal Cancer Patients

Objective: To analyze the relationship between KRAS, braf mutations and the response to Cetuximab in Chinese colorectal cancer patients. Methods: A total of 273 Chinese colorectal cancer patients were evaluated for KRAS and braf mutations by Sanger sequencing. Among them, 59 patients with metastatic colorectal cancer (mCRC) were treated with Cetuximab in combination with chemotherapy from August 2005 to July 2009. Statistical analysis was conducted to assess the relationship between KRAS, braf mutations and the response or survival of 59 mCRC patients. Results: KRAS and braf mutation rates were 38.5% (105/273) and 5.1% (14/273), respectively, and KRAS/braf mutations were mutually exclusive. Among 59 patients treated with Cetuximab plus chemotherapy, KRAS and braf mutations were identified in 11 and 5 patients, respectively. The response rates and median progression-free survivals (PFS) in KRAS wild-type and mutant patients were 35.4% (17/48) vs. 9.1% (1/11) (P=0.054) and 153 days vs. 99 days (P=0.01), respectively. Also, the response rates and median PFS in braf wild-type and mutant patients were 37.2% (16/43) vs. 20% (1/5) (P=0.016) and 138 days vs. 90 days (P=0.036), respectively. Conclusion: Besides KRAS, assessing braf mutation should also be required to select patients eligible for Cetuximab. Further prospective evaluation in large samples should be performed to confirm these preliminary findings.

Objective To investigate the correlation between braf V600 E mutation and anti-epidermal growth factor receptor(EGFR) monoclonal antibodies(Mo Abs) therapeutic effects in metastatic colorectal cancer. Methods Studies were included into meta-analysis to investigate the association between braf V600 E mutation and clinical outcome in metastatic colorectal cancer patients treated with anti-EGFR Mo Abs. Results A total of 7 studies were included in this meta-analysis. The 7 studies included 1352 patients in total, sample sizes ranged from 67 to 493. Objective response rate(ORR), progression-free survival(PFS) and overall survival(OS) were collected from included studies and were used to assess the strength of the relation. In patients with wild-type KRAS, the pooled odds ratio for ORR of mutant braf over wild-type braf was 0.27(95% CI=0.10-0.70). braf mutation predicted a deterioration in PFS and OS in wild-type KRAS patients treated with anti-EGFR Mo Abs(hazard ratio=2.78, 95% CI=1.62-4.76; hazard ratio=2.54, 95% CI=1.93-3.32). Conclusion braf V600 E mutation is related to lack of response and worse survival in wild-type KRAS metastatic colorectal cancer patients treated with anti-EGFR Mo Abs.

目的探讨前列腺多形性巨细胞型腺癌(PGCC)的临床病理及分子特征。方法收集2例前列腺PGCC,复查切片并进行免疫组织化学(IHC)及425基因高通量测序(NGS)检测,同时复习文献,总结其临床病理、IHC及分子特征。结果2例前列腺PGCC患者年龄为55岁和70岁,组织形态上均可见数量不等多形性巨细胞伴有奇异细胞核(均>50%),伴有高级别经典型腺泡腺癌;IHC只有部分肿瘤性巨细胞表达雄激素受体、NKX3.1、P504s、前列腺特异性抗原(PSA)等前列腺上皮分化的标志物,同时部分细胞表达GATA3。NGS检测到多个基因的致病性/可能致病性变异及融合(TP53变异、braf变异、ZSCAN25-braf融合),导致TP53及braf功能异常,同时检测到SDHA及DNMT3A基因变异,形成截短蛋白,可能参与肿瘤发生发展;例2内分泌治疗后出现雄激素受体拷贝数扩增;微卫星状态分析提示2个病例均为微卫星稳定型,肿瘤突变负荷为低水平。结论PGCC多见于老年男性,形态学表现为在高级别经典型腺泡腺癌中出现多形性奇异核分化,部分前列腺分化标志物表达减少或缺失,具有不同于经典型前列腺腺癌的基因变异,可能影响肿瘤的进展和预后,并为靶向治疗提供了依据。

Clinicopathologic features and treatment efficacy of Chinese patients with braf-mutated metastatic colorectal cancer: a retrospective observational study

Background: The prognostic role of the V600 E mutation of v-raf murine sarcoma viral oncogene homolog B1(braf)in metastatic colorectal cancer(mCRC) is well established, but the therapeutic regimen targeting this disease is lacking. This study aimed to analyze the clinicopathologic features of and treatment efficacy of commonly used regimens on braf-mutated mCRCs.Methods: We collected and reviewed the medical records of mCRC patients treated at Peking University Cancer Hospital & Institute(Beijing, China) between July 2011 and July 2016. Kirsten rat sarcoma viral oncogene homolog(KRAS),neuroblastoma RAS viral oncogene homolog(NRAS), and braf mutational status was assayed using direct sequencing. The details of clinicopathologic characteristics of patients and their responses to FOLFOXIRI regimen or standard therapy were obtained by reviewing the medical records. The progression-free survival(PFS) and overall survival(OS)were assessed using Kaplan-Meier analysis and compared using the log-rank test.Results: Of 1694 patients studied, 75 had braf exon 15 mutations. Of these 75 patients, 71 had V600 E mutation, 1 had D594 G mutation, 2 had K601 E mutation, and 1 had a novel T599_V600 insAGA alteration. No patients had KRAS or NRAS mutations. Of 63 patients with brafV600 E-mutated mCRC and sufficient clinical data, 27(42.9%) had right-sided colon tumors, 19(30.2%) had left-sided colon tumors, and 17(26.9%) had rectal tumors; 26(41.3%) had peritoneal metastases, and 50(79.4%) had distant lymph node metastases.The patients with braf K601 E-and T599_V600 insAGA-mutated tumors had similar clinicopathologic features to those with brafV600 E-mutated tumors. Patients with the brafV600 E mutation benefited more from FOLFOXIRI regimen compared with patients who underwent standard therapy(overall response rate 83.3% vs. 14.0%; median PFS 6.4 months vs. 2.8 months, P = 0.220; median OS11.0 months vs. 6.9 months, P = 0.048).Conclusions: brafV600 E mutations were commonly identified in right-sided tumors and showed a high incidence
