The total number of cancer patients who are eligible for and will benefit from immune checkpoint inhibitors(ICIs)in China has not been quantified.This cross-sectional study was conducted to estimate the number of Chinese cancer patients with eligibility and response to ICIs based on the 2015 Chinese cancer statistics and the immune checkpoint inhibitor clinical practice guideline of the Chinese Society of Clinical Oncology.A total of 11 ICIs were recommended for 17 cancer types.The estimated number of eligible patients annually was 1290156(55.18%),which included 888738 males(60.05%)and 400468 females(46.67%).The estimated number of responders annually was 448972(19.20%),which included 309023 males(20.88%)and 139764 females(16.29%).Gastric cancer(n=291000,12.45%),non-small-cell lung cancer(n=289629,12.39%),and hepatocellular carcinoma(n=277100,11.85%)were the top three cancer types with the highest number of eligible patients.Non-small-cell lung cancer(n=180022,7.70%),hepatocellular carcinoma(n=75648,3.24%),and small-cell lung cancer(n=64362,2.75%)were the top three cancer types with the highest number of responders.In conclusion,ICIs provide considerable benefit in Chinese cancer patients under optimal estimation.

Objective To explore the benefits and risks of PD-1/PD-L1 inhibitors Atezolizumab and Nivolumab in the treatment of non-squamous non-small cell lung cancer and provide some references for clinicians.Methods Based on the data results of relevant studies published by ClinicalTrical.gov in the US clinical trial database and foreign peer-reviewed journals,the internationally recognized multi-criteria decision analysis(MCDA)model was used to assess the benefit and risk of PD-1/PD-L1 inhibitors for non-squamous non-small lung cancer comprehensively.Finally,a sensitivity analysis was performed to test the sensitivity of the weight to the evaluation.Results and Conclusion The benefit-risk evaluation result of Atezolizumab for the treatment of non-squamous non-small cell lung cancer is better than that of Nivolumab.Specifically,Atezolizumab has more benefits than Nivolumab with a lower risk.The results of MCDA model in drug benefit and risk evaluation are easy to understand.However,the selection of indicators in the model and the degree of data acquisition are limited.The evaluation results of the MCDA model should be comprehensively viewed with other evaluations to make decisions objectively.

Is there a survival benefit from adjuvant chemotherapy for patients with liver oligometastases from colorectal cancer after curative resection?

Background:Although colorectal oligometastases to the liver can potentially be cured with aggressive local abla-tion,the efficacy of adjuvant chemotherapy(ACT)for such metastasis remains unclear.The present study explored the effects of ACT on patients with colorectal liver oligometastases(CLO)after curative resections and aimed to iden-tify patients who could benefit from ACT.Methods:We retrospectively analyzed 264 eligible patients with CLO who underwent curative resection between September 1999 and June 2015.Recurrence-free survival(RFS)and overall survival(OS)were analyzed using the Kaplan-Meier method and log-rank test;prognostic factors were a by Cox regression modeling.Results:Among 264 patients,200(75.8%)patients received ACT and 64(24.2%)did not receive ACT.These two groups did not significantly differ in clinicopathologic characteristics,and had comparable 3-year OS and RFS rates(RFS:42.1%vs.45.7%,P=0.588;OS:69.7%vs.62.7%,P=0.446)over a median follow-up duration of 35.5 months,irrespective of preoperative chemotherapy.ACT markedly improved 3-year OS in high-risk patients with Memorial Sloan-Kettering Cancer Center clinical risk scores(MSKCC-CRS)of 3-5(68.2%vs.33.8%,P=0.015),but presented no additional benefit in patients with MSKCC-CRS of 0-2(72.2%vs.78.6%,P=0.834).In multivariate analysis,ACT was independently associated with improved OS in patients with MSKCC-CRS of 3-5.Conclusions:ACT might offer a prognostic benefit in high-risk patients with CLOs after curative liver resection,but not in low-risk patients.Therefore,patients’risk status should be determined before ACT administration to optimize postoperative therapeutic strategies.

Predictors of functional benefit of hepatitis C therapy in a‘real-life’cohort

AIM To define predictors of functional benefit of direct-acting antivirals(DAAs) in patients with chronic hepatitis C virus(HCV) infection and liver cirrhosis.METHODS We analysed a cohort of 199 patients with chronic HCV genotype 1, 2, 3 and 4 infection involving previously treated and untreated patients with compensated(76%) and decompensated(24%) liver cirrhosis at two tertiary centres in Germany. Patients were included withtreatment initiation between February 2014 and August 2016. All patients received a combination regimen of one or more DAAs for either 12 or 24 wk. Predictors of functional benefit were assessed in a univariable as well as multivariable model by binary logistic regression analysis.RESULTS Viral clearance was achieved in 88%(175/199) of patients. Sustained virological response(SVR) 12 rates were as follows: among 156 patients with genotype 1 infection the SVR 12 rate was 90%(n = 141); among 7 patients with genotype 2 infection the SVR 12 rate was 57%(n = 4); among 30 patients with genotype 3 infection the SVR 12 rate was 87%(n = 26); and among 6 patients with genotype 4 infection the SVR 12 rate was 67%(n = 4). Follow-up MELD scores were available for 179 patients. A MELD score improvement was observed in 37%(65/179) of patients, no change of MELD score in 41%(74/179) of patients, and an aggravation was observed in 22%(40/179) of patients. We analysed predictors of functional benefit from antiviral therapy in our patients beyond viral eradication. We identified the Child-Pugh score, the MELD score, the number of platelets and the levels of albumin and bilirubin as significant factors for functional benefit.CONCLUSION Our data may contribute to the discussion of potential risks and benefits of antiviral therapy with individual patients infected with HCV and with advanced liver disease.

The first automobile engine remanufacture company in China, Jinan Fuqiang Power Co, Ltd, was introduced. The engine remanufacturing technological process of this company was described. The benefit statistic of remanufacturing 10000 Styer engines were analyzed, and the contribution of engine remanufacturing to cycle economy was predicted. The results show that remanufacturing engineering could use the maximal additional values of obsolete engines, and make contributions to materials conservation, capital saving, energy conservation and environment protection. 10000 engines are supposed to be remanufactured per year, the following benefits would be obtained: reclaiming additional values of RMB3.23 hundred million, saving metallic materials about 7.65 thousand tons, saving energy of 16 million kilowatt-hours, reducing emission of CO2 about 11.3[CD*2]15.3 thousand tons, and providing employment for 500 persons. According to the survey and analysis, tremendous benefits will be gained by the year of 2020. For example, reclaiming additional values per year of 1424[CD*2]2236 hundred million RMB, saving energy per year of 60[CD*2]90 hundred million kilowatt-hours, reducing emission of CO2 about 6.67[CD*2]9.69 million tons. It can be deduced that developing remanufacturing will play an important role in enriching the cycle economy and accelerating the development of national economy.

Taking the flood resources utilization in Baicheng, Jilin during 2002–2007 as the research background, and based on the entropy weight and multi-level & multi-objective fuzzy optimization theory, this research established a multi-level & semi-constructive index system and dynamic successive evaluation model for comprehensive benefit evaluation of regional flood resources utilization. With the year 2002 as the base year, the analyzing results showed that there existed a close positive correlation between flood utilization volume and its benefits, comprehensive evaluation value and its comparison increment. Within the six successive evaluation years, the comprehensive benefit of 2003 was the best, in which the benefit evaluation increment reached 82.8% whereas the year of 2004 was the worst, in which the increment was only 18.2%. Thus the sustainability and correctness of the evaluation were verified by six years successive evaluation and increment comparison. The analyzing results showed that the economic benefits, ecological benefits and social benefits of flood utilization were remarkable, and that the comprehensive benefit could be improved by increasing flood utilization capacity, which would promote the regional sustainable development as well. The established dynamic successive evaluation provides a stable theoretical basis and technical support for further flood utilization.

This paper proposes a multi-objective benefit function for operation of active distribution systems considering demand response program(DRP)and energy storage system(ESS).In the active distribution system,active network management(ANM)is applied so that the distribution system equipment is controlled in real-time status based on the real-time measurements of system parameters(voltages and currents).The multi-objective optimization problem is solved using e-constraint method,and a fuzzy satisfying approach has been employed to select the best compromise solution.Two different objective functions are considered as follows:benefit maximization of distribution company(DisCo);benefit maximization of distributed generation owner(DGO).To increase the benefits and efficient implementation of distributed generation(DG),DGO has installed battery as energy storage system(ESS)in parallel with DG unit.Consequently,DGO decides for the battery charging/discharging.DisCo has the ability to exchange energy with the upstream network and DGO.Also,DisCo focuses to study the effect of demand response program(DRP)on total benefit function and consequently its influence on the load profile has been discussed.This model is successfully applied to a 33-bus radial distribution network.

Surveillance with abdominal ultrasound(US)of patients with chronic liver disease who are at risk of developing a hepatocellular carcinoma(HCC),has progressively emerged as a consolidated practice for achieving early diagnosis and improving treatment of liver cancer,despite the lack of robust evidence-based data.Surveillance is recommended by the international societies from both hemispheres(1-3),where however recommendations from Far East voice some nuances with respect to the need for adding serum alfa fetoprotein(AFP)to US and of adopting accelerated intervals of screening for patients at very high risk of HCC,like those with viral hepatitis and multiple co-morbidities.A recent meta-analysis form the US confirms indeed a benefit in adding serum AFP to US(4).In the western world,a 6-month interval is considered more than adequate for screening patients with compensated cirrhosis of any aetiology,as several meta-analyses have reported appreciable survival benefits in those patients who had a small tumor detected with US surveillance that ultimately could access curative treatment with liver transplantation,hepatic resection or local ablation(5,6).
