Background:In China, the prevalence of allergic rhinitis (AR) and asthma has increased in the past decade. As these two diseases frequently coexist, the asia-pacific Survey of Physicians on Asthma and Allergic Rhinitis (ASPAIR) study aimed to assess physicians5 beliefs and treatment patterns of coexistent asthma-AR across six asian countries. This analysis presents the results from China. Methods: The 200 hospital-based general physicians and pediatricians were interviewed from five cities in China. Physicians were questioned in-person about their knowledge, beliefs and management practices for patients with coexistent asthma-AR. Results: Approximately 70% of the physicians interviewed routinely evaluated their patients with asthma or AR for signs of coexistent disease. While the majority of physicians (>90% of physicians) recognized the increased burden of coexistent asthma-AR vs. one condition alone and that coexistent disease requires additional treatment, most physicians (96%) also believed that patients with coexistent asthma-AR were well managed if either condition alone improved. Similarly, although 71% of physicians selected a combination of intranasal and inhaled corticosteroids as their preferred treatment for coexistent asthma-AR, in line with treatment guidelines, two fifths of physicians indicated that treatment for coexistent disease requires too much medication and that their patients prefer oral medications and a third of physicians believed that corticosteroids should be delayed in children. Conclusions: This survey demonstrates that physicians interviewed in China have a broad understanding of coexistent asthma-AR and its impact on patients. A holistic approach to patient management with informed decisions regarding patients' overall treatment will benefit patients who suffer from coexistent disease.

BACKGROUND Major societies provide differing guidance on management of Barrett’s esophagus(BE),making standardization challenging.AIM To evaluate the preferred diagnosis and management practices of BE among asian endoscopists.METHODS Endoscopists from across asia were invited to participate in an online questionnaire comprising eleven questions regarding diagnosis,surveillance and management of BE.RESULTS Five hundred sixty-nine of 1016(56.0%)respondents completed the survey,with most respondents from Japan(n=310,54.5%)and China(n=129,22.7%).Overall,the preferred endoscopic landmark of the esophagogastric junction was squamocolumnar junction(42.0%).Distal palisade vessels was preferred in Japan(59.0%vs 10.0%,Pasian endoscopists(70.8%)would survey long-segment BE without dysplasia every two years.Adherence to Seattle protocol was poor with only 6.3%always performing it.73.2%of Japanese never did it,compared to 19.3%outside Japan(Pasia,6-monthly surveillance was preferred in 61.9%within Japan vs 47.9%outside Japan(Pasia,with stark contrast between Japan and outside Japan.Most asian endoscopists chose squamo-columnar junction to be the landmark for esophagogastric junction,which is incorrect.Most also did not consistently use Prague criteria,and Seattle protocol.Lack of standardization,education and research are possible reasons.

Adaptation of asia-pacific forests to climate change

Climate change is a threat to the stability and productivity of forest ecosystems throughout the asiapacific region. The loss of forests due to climate-induced stress will have extensive adverse impacts on biodiversity and an array of ecosystem services that are essential for the maintenance of local economies and public health. Despite their importance, there is a lack of decision-support tools required to evaluate the potential effects of climate change on asia-pacific ecosystems and economies and to aid in the development of regionally appropriate adaptation and mitigation strategies. The project Adaptation of asiapacific Forests to Climate Change, summarized herein,aims to address this lack of knowledge and tools and to provide support for regional managers to develop effective policy to increase the adaptive capacity of asia-pacific forest ecosystems. This objective has been achieved through the following activities：（1） development of a highresolution climate downscaling model, Climate AP, applicable to any location in the region;（2） development of climate niche models to evaluate how climate change might affect the distribution of suitable climatic conditions for regionally important tree species;（3） development and application of forest models to assess alternative management strategies in the context of management objectives and the projected impacts of climate change;（4） evaluation of models to assess forest fire risk and the relationship between forest fire and climate change;（5） development of a technique to assess ecosystem carbon storage using Li DAR; and（6） evaluation of how vegetation dynamics respond to climate change using remote sensing technology. All project outputs were developed with a focus on communication and extension to facilitate the dissemination of results to regional forest resource managers to support the development of effective mitigation and adaptation policy.

Dengue is still a substantial vector-borne viral disease threat and burden of public health importance worldwide.This situation is complicated by dengue virus unprecedented resurgence and persistence of varied serotypes in endemic-prone areas,and man-made and natural activities consequences that promote vector emergence,transmission dynamics and spread across the asia-pacific region.There is an urgent need to strengthen operational and contextual surveillance-response research in improving early detection of active reservoir detection,novel drug in case management and quality evidence-based response including the deployment of dengue mass vaccination.Moreover,sustained mapping and watching of dengue risk factors or determinants,performance and outcome indicators of control or elimination programs effectiveness in defining minimum effective data towards community knowledge-based decision-making policy and effective response packages is imperative.Moreover,implementation of a robust,integrated dengue early warning surveillance,monitoring and response systems metrics is required for evidence-based,timely and cost-effective contextual mitigation strategies,and innovative interventions.

On the Major Transformation of the asia-pacific Regional Structure

Recent years have witnessedsignificant changes in the asia-pacific regionalstructure featuring the shift in power balance among related countries as well as adjust-ments in their respective regional strategies. So what is the prevailing trend in thistransformation and what kind of regional order will likely emerge in the future？ To an-swer these questions, the paper tries to explore the following dimensions： the evolvingregional context, shift in power balance and power conversion, geo-political and geo-e-conomic developments and interactions between them, and evolving regional order. Asthe study shows, the broadening of regional geographical scope, the rise in the numberof actors involved, as well as changesin the relations among them, have combined tocast a pluralistic and complex flavor to the regional structure. While the power balanceamong China, the United States and Japan shifts, and power conversion is also occur-ring. China and Japan are paying increasing attention to their security roles, whereasthe United States under the Obama administration laid more stress on expanding itsgeo-economic clout. In spite of the intensifying geopolitical competition among China,the United States and Japan in recent years, a Sino-US strategic compromise-whichholds the key to regional geopolitics-isother front, some kind of geo-economicmore likely to take place in the future. On thecooperation will probably emerge. In the longerterm, it will be the geo-economic trend thatwill prevail over the geopolitical trend inthe region. Finally, the future evolution of regional order wiI1 mainly follow the eco-nomic logic, accompanied by the dilutionof its hegemonic and hierarchical nature.Hence, a pluralistic and complex regional community will emerge.

Utility of the asia-pacific colorectal screening scoring system and the presence of metabolic syndrome components in screening for sporadic colorectal cancer

AIM: To determine the utility of the asia-pacific colorectal screening(APCS) scoring system and metabolic syndrome components in individual screening for sporadic colorectal cancer.METHODS: The subjects were patients admitted to the Peking University People's Hospital for colonoscopy between October 2012 and July 2013. Clinical information, including patient willingness to undergo colonos-copy, medical history, endoscopic findings, histology, and other information, was collected, and the patients were grouped according to APCS scores and the presence of metabolic syndrome components. Colorectal tumor detection rates were compared between the groups.RESULTS: A total of 219 patients were included in the study, 108 were male and 111 were female, resulting in a male-to-female ratio of 1:1.03. The average age of the patients was 56.8 ± 13.7 years. According to APCS scores, 88(40.2%) patients were included in the average-risk(AR) group, 113(51.6%) patients were included in the moderate-risk(MR) group, and 18(8.2%) patients were included in the high-risk(HR) group. Colorectal tumors were detected in 69(31.5%) subjects, and the detection rates in the AR, MR, and HR groups were 15.9%, 36.3%, and 77.8%, respectively. The difference in the detection rates between the three groups was statistically significant(P < 0.01). The combined detection rate of colorectal tumors in the APCS MR and HR groups was 42.0%. However, patients in the MR and HR groups who presented with metabolic syndrome components, in particular obesity, exhibited a significantly higher colorectal tumor detection rate(59.5%) than did those without these components(19.2%, P < 0.01) and those who underwent colonoscopy because of doctor's recommendation(36.5%, P < 0.01). CONCLUSION: The APCS scoring system can be used in individual screening for sporadic colorectal cancer. The combined use of APCS scores and the metabolic syndrome components, in particular obesity, will significantly improve the efficacy of individual colorectal cancer screening.

asia-pacific WINTER LFO STRUCTURE AND LOWFREQUENCY VORTEX ACTIVITY FEATURES

Based on the winter 1984/ 1985 ECMWF grid point data subjected to the 30-60 day band-pass filtering and composite analysis,a study is undertaken of the LFO (low-frequency oscillation) structure in the eastern asian westerly jet entrance and exit regions and the asia-pacific low-frequency vortex activity characteristics.Results show that zonal wind oscillations on both sides of the jet core are in anti-phase,in close relation to the E-W displacement of the core. Ranging in NW-SE direction is a low-frequency vortex train (LFVT) emanating from Ural via central asia to East asia.A low-frequency vortex of Ural origin,when reaching around 50,80,is split into two parts,one travelling eastward and the other southward,and finally they arc connected cyclonically or anticyclonically at low latitudes,form- ing a vigorous low frequency cyclone or anticyclone in the eastern part of China mainland,completing a full cycle of the LFVT.Further,observed in the central pacific are a meridional LFVT and a cyclone/anticyclone couplet looping in a counterclockwise sense,giving rise to the LFVT phase shift over this region.

Sino-US Strategic Dilemma–From the Perspective of Power Transition in asia-pacific and Middle East

The order in international system is defined as“a pattern or activity that sustains the elementary or primary goals of the society of states”.The objective including the maintenance of the international system and national survival of society itself is to safeguard national independence and sovereignty.The nature and practice of international hierarchy helps explain patterns of the international order after the Cold War and understand its transition,especially the role of China’s rise in international politics.The asia-pacific region has showed a stable political order generally,but events in the region still may lead to uncertainties of military conflict,such as the North Korean nuclear issue,the Taiwan question,terrorist attacks in the Middle East and Central asia and so on.China’s rise will inevitably have an impact on US hegemony in the asia-pacific region.But China and the United States can work together to build an effective mechanism to address the challenges from regional security issues,to set up the sustainability of asia-pacific and the Middle East security decisions.This paper analyzes the concept of Sino-US asia-pacific and the Middle East strategic alternatives from maintaining the status quo;transition from the status quo must go through negotiation and dialogue to the transition of asia-pacific and the Middle East power structure.

China’s Involvement with asia-pacific Economic Cooperation

asia-pacific Economic Cooperation(APEC) is the most influential economic cooperation mechanism in the asia-pacific region. China is not only a participant but also a staunch supporter of and a major contributor to economic cooperation in the asia-pacific region. China has always upheld the spirit of asia-pacific partnership, leaving its footprints deeply engraved in the historical development of APEC. Through its involvement with APEC, China has not only furthered its own development, but also made important contributions to regional and even global economic development. Over the years, the practice of APEC, centering on the cooperative vision represented by the Bogor Goals, has greatly improved the liberalization and facilitation of trade and investment and has raised the level of economic cooperation in the asia-pacific region. With the deadline for achieving the Bogor Goals approaching, the direction that such cooperation will take after 2020 has become a focus of popular concern. The international structure is now undergoing profound changes unseen in the past century. As unilateralism and protectionism affect the globalization process, APEC faces both opportunities and challenges in the near future. To draw up a future cooperation blueprint together with the other economies involved, China has to forge ahead bravely through all kinds of weather.
