Background There is much concern about declining rates of exclusive breastfeeding(EBF)all over the world including Saudi arabia which deprives infants from many immunological,nutritional,and psychosocial benefits.This study was per-formed to determine EBF prevalence and factors influencing EBF in relation to knowledge,attitude and practice(KAP)of breastfeeding mothers(BFM)in Rabigh,Western Saudi arabia.Methods Data about factors that could be associated with EBF and KAP of BFM were collected by cross-sectional ques-tionnaire-based study from 420 BFM.Results Prevalence of EBF for 6 months was 27.6%which is far below World Health Organization recommendations.Demographic,socioeconomic,biomedical as well as most studied variables of KAP of 50%to over 90%of participating mothers were adequate to account for low EBF rate.Proper knowledge about benefits of colostrum and continuation of breastfeeding for 2 years was given by only 86.2%and 36.7%of BFM,respectively.The most significant independent factors associated with EBF were perception of sufficient human milk[adjusted odds ratio(AOR)2.89;95%confidence interval(CI)1.72-4.89],absence of nipple pain(AOR 2.77;95%CI 1.72-4.45)and mothers without university education(AOR 1.86;95%CI 1.15-3.01).Early introduction of formula in hospital after birth was the main inappropriate feeding practice in 76.2%of infants of BFM.Conclusion Findings of this study should be utilized by Rabigh community and other national or worldwide communities with similar characters to promote EBF by targeted educational programs after identification of significant factors inltuenc-ing EBF and gaps in KAP of BFM.

Objective: To study epidemiological trends related to cutaneous leishmaniasis (CL) in Al Hassa, an endemic area in Saudi arabia. Methods: This retrospective study included the spatial/ temporal analysis of the reported cases of CL using the available surveillance database for the disease at the regional Vector Control Unit, from 2000 to 2010. Results: The incidence of CL was declining at a stable rate especially during the last 3 years of the study (2008-2010). An interesting finding was the percentage of expatriates affected was increasing over the last 10 years compared to that of the Saudis. Conclusions: A definite declining trend in the incidence of CL was observed in Al Hassa. Further studies are warranted to assess whether special public health measures are needed for better control of CL in expatriate populations in Saudi arabia.

Objective:To describe the profile and geographical distribution of reported cases of cutaneous leishmaniasis(CL)in the province of Hail,Saudi arabia from 2010 to 2013.Methods:Human data were collected from the dermatology service of both King Khaled and General Hail hospitals.Results:A total of 483 confirmed CL cases were investigated.Cutaneous leishmaniasis follows a seasonal distribution related to the activity of the phlebotomine sand fly fauna.Both saudi and non saudi patients were almost equally infected.The number of reported cases of CL in men was higher than that in females.The frequency of CL was asoociated with age and increased in teenager and adult group.By analyzing the geographical distribution,the majority of Hail districts were affected with this disease.However it is more prevalent in Hail city.Conclusions:The lack of data concerning the parasite,the sand fly fauna and the existing transmission cycles in Hail province make difficult to interpret the reported results and to follow up the spatio-temporal evolution of the disease.

Objective:To investigate the prevalence of different intestinal parasitic infections among male primary schoolchildren in Al-Ahsa,Saudi arabia,and to explore the possible relative socioeconomic, environmental and behavioural sanitary correlates.Methods:A cross-sectional descriptive study was performed.A total of 1 289 male students aged from 7 to 12 years were selected randomly from 20 urban and 12 rural primary schools by multistage sampling method. Data collection was carried out by self administered questionnaire form to the parents/guardians of students and included inquires about socio-demographics,environmental conditions,and behavioural sanitary habits.Stool analysis was carried out to examine the presence of intestinal parasitic infections.Results:Overall prevalence of parasitic infections was 27.2%,more among rural students.Frequently encountered infections included Entamoeba histolytica/dispar(8.2%), Giardia lamblia(6.5%),Entamoeba coli(4.0%) and Enterobius vermicularis(1.6%).Logistic regression revealed that lower maternal educational level and occupational status,low family income,big family size,poor personal hygienic practices and positive history of previous intestinal infections among family members increased the likelihood of infections.Conclusions: Although of low magnitude,intestinal parasitic infections still represent a public health concern among male schoolchildren in Al-Ahsa region.Socio-demographic and poor personal hygienic habits are the main predictors for these infections.

Objective:To detect Coxiella burnetii(C.burnetii)DNA in clinical specimens from camel,goats,cattle and sheep in the Kingdom of Saudi arabia.Methods:A total of 367 clinical samples including blood,milk,faeces and urine were collected from different livestock and subjected to PCR amplification using primers which amplify transposon-like region and transposase gene.Results:Positive amplification from both regions was obtained from camel,goats and cattle but not from sheep.A percentage of 10.8%samples yielded positive PCR amplification from both blood and milk,where 15 of 139 blood and 16 of 148 milk samples were positive.Faeces and urine showed higher percentages of positive samples reaching 40.8%and 23.8%respectively.Conclusions:The preferred route of shedding in camel appeared to be the faeces followed by urine,while that of goats appeared to be the faeces and that of the cattle appeared to be the milk.

Objective: To present the histopathological and clinical correlation of mycetoma among patients attending King Abdulaziz University Hospital between 1998-2013.Methods: The data of all histopathologically diagnosed mycetomas in the period between January 1998 and January 2013 were collected through a computerized database search of the anatomic pathology archives at King Abdulaziz University Hospital. The collected data were analysed. Identification of species were performed for five patients using 16 S ribosomal DNA and internal transcribed spacer 2.Results: There were 19 patients with mycetoma with an average age of 44.26 years and male: female ratio of 4:1. Actinomycetoma were 63.15% and eumycetoma were 36.84%. All patients presented with the classic lesions; presenting as painless subcutaneous mass, sinuses and discharge containing grains. The swellings were of slow evolution, with preferential foot localization. Species specification performed for samples from five patients with active lesions revealed species of Actinomyces israelii and Madurella mycetomatis in respective cases.Conclusions: Actinomycetoma is more common than eumycetoma in this region. The fact that one of the patients with eumycetoma was a Saudi national raises the possibility of an indigenous species similar to Maduraella mycetomatis to be further explored for characteristics and pathogenesis. The disease has to be prioritized again and more robust and quick molecular diagnostic tools should be made available in order to save patients form disfiguring amputations.

Double diabetes in Saudi arabia: A new entity or an underestimated condition

AIM To determine the clinical and biological characteristics of double diabetes(DD) among young people in Saudi arabia.METHODS This was a retrospective descriptive chart review study including 312 young newly diagnosed diabetic patients(aged 12-20 years), whom were admitted over a five year period(January 2009 to December 2013). Family history of diabetes mellitus(DM)(first degree), physical body mass index(BMI), acanthosis nigricans, history of auto-immune disease and laboratory information for glycosylated hemoglobin, basal C peptide level and diabetes autoantibody response(anti-GAD, anti-IA2 and anti-ICA) were collected from medical report. A mean follow-up of 3 years for these patients was performed.RESULTS Patients were categorized into 4 groups, based on the autoantibody response(Ab+ or Ab-) and C-peptide secretion(β+ for fasting level 0.4-2.1 ng/mL and β- if < 0.4 ng/mL). Group1(type 1a): Ab+ β-(21%), group 2(type 1b): Ab- β-(9%), group 3(DD): Ab+ β+(31%)and group 4(classic type 2 DM): Ab- β+(39%). The mean age of the DD patients in our study was 15.1 ± 6.4 years. A total of 41% of the study population presented with diabetic ketoacidosis and 61% of the study population presented with positive family history of DM. The mean BMI was 26.8 kg/m2 with 64% of overweight or obese patients. Ninety two percent of the patients were started on insulin at the time of diagnosis. During a mean follow-up of 3 years, only 32% of the patients with DD required insulin and 78% were on metformin alone or with insulin. CONCLUSION Our findings enable us to arrive at the conclusion that almost one-third of the young Saudi diabetic patients reveal atypical forms of DM(double diabetes) expressing features resulting from both T1D and T2D.
