Between evidence and new perspectives on the current state of the multimodal approach to gastric cancer:Is there still a role for radiation therapy?

In patients affected by gastric cancer(GC), especially those in advanced stage, the multidisciplinary app-roach of treatment is fundamental to obtain a good disease control and quality of life. Although many chemotherapeutics in combination to radiotherapy are adopted in the peri- or postoperative setting, the most optimal timing, regimens and doses remains con-troversial. In the era of radical surgery performed with D2-lymphadenectomy, the role of radiation therapy remains to be better defined. Categories of patients, who could benefit more from an intensified local trea-tment rather than more toxic systemic therapy, are still under investigation. Evidence and recent updates of the randomized trials, meta-analysis and prospective trials show that the postoperative radiotherapy plays a fundamental role in reducing the loco-regional recurrence and in turn the disease-free survival in operable advanced GC patients, also after a well performed D2 surgery. Therapeutic decisions should be taken considering the individual patients, but the multimodal approach is necessary to guarantee a longer survival and a good quality of life. Ongoing randomized trials could better define the timing and the combination of radiotherapy and systemic therapy.

Sarcomatoid Carcinoma of the Mandible: A Case Report and Literature Review

Background: Sarcomatoid carcinoma is a rare biphasic malignancy with an epithelial and mesenchymal component. Given its histological particularity, the diagnosis of this tumor still represents a challenge for pathologists and surgeons. Sarcomatoid carcinoma of the mandible is a very rare and aggressive entity. To date, only sporadic cases have been reported in the literature. Aim: To report a rare case of aggressive mandibular sarcomatoid carcinoma treated with radical surgery and adjuvant chemoradiation, and discuss the related literature. Case Report: We introduce a rare case of mandibular sarcomatoid carcinoma occurring in a 34-year-old woman. Treatment consisted of radical surgery and re-excision for positive margins, adjuvant radiation therapy (66 Gy in 33 fractions in simultaneous integrated boost delivered using Volumetric Modulated Arc Therapy) and Cisplatin-based concurrent chemotherapy. The tumor displayed aggressive behavior and high metastatic potential, causing tumour recurrence and extensive lung metastasis. Conclusion: Sarcomatoid carcinoma of the mandible is an extremely rare malignant entity, with a high tendency for local recurrence, distant metastasis. Treatment involves radical surgery and adjuvant chemoradiation, although the outcome and prognosis might be poor.

Peripheral primitive neuroectodermal tumors:a rare case report

We aimed to explore the diagnosis and treatment of peripheral primitive neuroectodermal tumors （pPNETs）. We retrospectively analyzed the diagnosis and treatment process of a patient who was diagnosed with pPNETs by pathology. This case was a man with soft masses arising from the left chest wall near the armpit and left supraclavicular of a 47-year-old man. The patient mainly presented with the masses which increasing gradually with obvious pain. Needle biopsy showed that they were both metastatic adenocaroinoma. Ultrasonography B revealed blood flow of these two low density placeholders can be seen in the signal, not oppression axillary and vein. Radical resection of the masses were performed. Histopathologic study and immunohistochemistry （IHC） confirmed the masses to be peripheral primitive neuroectodermal tumors, pPNETs is a rare malignant small round cell tumor. CT and MRI examination can estimate the resectability of the tumor; Ultrasound B can make sure its inside blood supply and the positional relationship between the mass and the surrounding vasculature. The diagnosis of pPNETs is based primarily on histopathologic study and IHC, especially those with the characteristics of the Homer-Wright and neuroendocrine markers. Radical resection of the tumor is the most effective therapeutic method. The effect of adjuvant chemo-radiation is worth affirmation. Autologous stem cell rescue besides adjuvant chemotherapy has been associated with prolonged survival.
