Co-existence of blaOXA-23 and blaVIM in carbapenem-resistant acinetobacter baumannii isolates belonging to global complex 2 in a Chinese teaching hospital

Background: Carbapenem-resistant acinetobacter baumannii (CRAB) have been a challenging concern of health-care associated infections. The aim of the current study was to investigate the molecular epidemiology and clonal dissemination of CRAB isolates in a Chinese teaching hospital. Methods: Non-duplicate clinical A. baumannii isolates were collected from inpatients, and we measured the minimal inhibitory concentrations to determine antimicrobial susceptibility. Polymerase chain reaction (PCR) and sequencing were performed to detect carbapenem-resistance genes and occurrence of transposons among CRAB isolates. Moreover, the genetic diversity among isolates and clonal dissemination were determined by repetitive element PCR-mediated DNA fingerprinting (rep-PCR) and multilocus sequence typing (MLST). Results: A total of 67 CRAB isolates displayed resistance to most of the antibiotics tested in this study, except tigecycline. We detected bldOXA-23,bldOXA-51, bldOXAA-58, and bldVIM genes in 94.0%, 100.0%, 1.5%, and 80.6% of the CRAB isolates, respectively. Nevertheless, 74.6% of the CRAB isolates co-harbored the bldOXA-23 and bldVIM. Only one type of transposons was detected: Tn2008 (79.1%, 53/67). Although 12 distinctive types (A-L) were determined (primarily A type) STI95 was the most prevalent sequence type (ST). ST368, ST210, ST90, ST829, and ST136 were also detected, and all belonged to clonal complex 208 (CC208) and global complex 2 (GC2). Conclusion: The bldOXA-23 and bldVIM genes contributed to the resistance among CRAB isolates collected in our study. Notably, most of the CRAB strains co-harbored bldOXA-23 and vim genes, as well as Tn2008, which could contribute to clonal dissemination. The prevalence of such organisms may underlie hospital acquired infections.

BACKGROUND:acinetobacter baumannii(AB)bacteremia is an increasingly common and often fatal nosocomial infection.Identification of morbidity and mortality risk factors for AB bacteremia in emergency department(ED)patients may provide ways to improve the clinical outcomes of these patients.METHODS:The records for 51 patients with AB bacteremia and 51 patients without AB infection were collected and matched in a retrospective case-control study between 2013 and 2015 in a singlecenter ED.Risk factors were analyzed by Chi-square and multivariate logistic regression statistical models.RESULTS:A significant risk factor for morbidity was the presence of a central venous catheter(CVC)(P<0.001).The mortality rate for the 51 patients with AB bacteremia was 68.6%.Risk factors for mortality were the presence of a CVC(P=0.021)and an ED stay longer than two weeks(P=0.015).CONCLUSION:AB infections lead to high morbidity and mortality.The presence of a CVC was associated with higher morbidity and mortality in patients with AB bacteremia.Avoiding CVC insertions may improve outcomes in ED patients with AB bacteremia.

Objective:To investigate the efficacy of 17 ethnomedicinal plants belonging to Apocynaceae family used in combination with 16 conventional antibiotics against non-multidrug resistant-,multidrug resistant(MDR)-.and extensive drug resistant(XDR) acinetobacter baumannii(A.haumannii).Methods:Antibacterial activity and resistance modifying ability of 272combinations were determined by growth inhibition assays and further confirmed by time-kill assay.Results:Among the combinations of the antibiotics with Apocynaceae ethanol extracts on this pathogen,15(5%) had synergistic effects,23(8%) had partial synergistic effects and 234(86%) had no effects.Synergistic activity was observed mostly when the Apocynaceae extracts were combined with rifampicin or cefazolin.Interestingly.10 out of 17 combinations between the extracts and rifampicin displayed synergistic or partial synergistic behaviors.Holarrhena antidysenterica extract was additionally tested to restore rifampicin activity against clinical isolates of MDR and XDR A.haumannii.With respect to total or partial synergy,70%was XDR A.haumannii isolates and 66%was MDR A.haumannii isolates.Conclusions:Holarrhena antidysenterica extract clearly demonstrated the ability to restore rifampicin activity against both A.haumannii ATCC19606 and clinically isolated A.haumannii.Additional studies examining its active principles as well as nieclianisms of actions such as the effects on efflux pumps and outer membrane permeability alterations are recommended.

Objective:To identify and to determine the antimicrobial susceptibility of acinetobacter baumannii(A.baumannii) clinical isolates from ICU at Aseer Central Hospital.Methods:The study was conducted in the Intensive Care Unit,Aseer Central Hospital,Saudi Arabia over 13 months period(2014-2015).acinetobacter species(n= 105) were isolated from various clinical samples.Isolates were identified using selected phenotypic criteria and confirmed using the Vitek 2 automated system.This system was used to determine the susceptibilities of 21 antimicrobial agents.Patients,isolates and drug data were analyzed using the SPSS statistical software package to determine some epidemiological and microbiological patterns.Results:Of the 105 stains,A.baumannii accounted for 49(46.67%),A.baumannii complex,19(18.09%),A.baumannii/haemolyticus 32(30.47),acinetobacter haemolyticus 4(3.81%),Acinetobater lwoffii 1(0.95%) and unidentified Acinetobater species 2(1.3%).Of the 105 acinetobacter strains,103(98.1%) were found multidrug resistant(MDR).A.baumannii strain were 100% sensitive to colistin and 74.5% to trimethoprim + sulfamethoxazole.The remaining 19 antimicrobial agents revealed low or no sensitivities:amikacin 16.3%; ampicillin 7.7%; ceftazidime 7.3%.Distribution of similar sensitivities was shown by other acinetobacter species.Mean number of isolates from males and females indicates no statistical variation(P=0.867) whereas age groups showed significant differences(P= 0.008) as it is clear from the high percentage of infected individuals more than 60 years followed by those aged 20 to 29 years old(19.05%).Upper respiratory tract(30.48%),lower respiratory tract(47.65%) and subcutaneous tissue(9.5%) were the main sources of acinetobacter spp.but mean numbers of isolates from these specimens indicate no discrepancy between specimens(P=0.731).Conclusions:acinetobacterspecies including A.baumannii were found MDR(98.1%) according to the current acinetobacter spp.antimicrobial categorization.Approximately half of these strains were A.bauman

BACKGROUND Nosocomial infections with carbapenem-resistant acinetobacter baumanniicalcoaceticus complex(ABC)strains are great problem for intensive care units.ABC strains can develop resistance to all the antibiotics available.Carbapenem resistance is common and colistin resistance is rare in our country.Knowing the risk factors for colistin resistance is important since colistin seems to be the only remaining therapeutic option for the patients with pneumonia due to extensively drug resistant ABC for our country.AIM To investigate the comparison of clinical responses and outcomes between pneumonia patients with colistin-susceptible and-resistant acinetobacter sp.Strains.METHODS During the study period,108 patients with pneumonia due to colistin-susceptible strains and 16 patients with colistin-resistant strains were included retrospectively.Continuous variables were compared with the Mann-Whitney U test,and categorical variables were compared using Pearson’s chi-square test or Fisher’s Exact chi-square test for two groups.A binary logistic regression model was developed to identify the potential independent factors associated with colistin resistance in patients with colistin-resistant strains.RESULTS High Acute Physiology and Chronic Health Evaluation II scores(OR=1.9,95%CI:1.4-2.7;Pacinetobacter sp.Different combinations of antibiotics including colistin,meropenem,ampicillin/sulbactam,amikacin and trimethoprim/sulfamethoxazole were used for the treatment of patients with colistin-resistant strains.Although the median duration of microbiological cure(P<0.001)was longer in the colistin-resistant group,clinical(P=0.703),laboratory(P=0.277),radiological(P=0.551),microbiological response(P=1.000)and infection related mortality rates(P=0.603)did not differ between the two groups.Among the patients with infections due to colistin-resistant strains,seven were treated wit

Objective:To assess and characterize antibiotic resistance in acinetobacter baumannii strains recovered from 5 health-care facilities in Algiers.Methods:Antibiotic susceptibility testing was performed by agar diffusion and agar dilution methods,resistance genes were identified by PCR and sequencing,and molecular typing of isolates was carried out by enterobacterial repetitive intergenic consensus-PCR(ERIC-PCR).Results:Among 125 tested isolates,117(93.6% ) were multidrug-resistant.of which 94(75.2% ) were imipenem resistant.The bla_(ADC)and bla_(OXA-51-like) genes were detected in all isolates,in association with ISAba I sequence in 84% and 8% (imipenem resistant) of isolates,respectively.The bla_(OXA-23-like) and bla_(OXA-24-like)carbapenemase genes were delected in 67.02% and 20.21% of imipenem-resistant isolates,respectively.The bla_(OXA-23-like) gene is linked to ISAba1 or ISAba4 elements.The metallo-β-lactamase NDM-1 gene was found in 10(10.6% ) imipenem-resisianl strains from three hospitals,it is linked to ISAba125 clement in nine strains.Extended spectrum β-lactamases production was not detected.Imipenem and cefotaxime resistance phenolypes could not be transferred to Escherichia coli by conjugation.Outer membrane protein CarO gene was not delected in four imipenem-resisianl isolates.The aac(6')-1b.sul1,sul2,tetA and tetB genes were present in 5.31% .36.17% .77.65% .1.06% and 65.92% of strains,respectively.Class 1 integrons were detected in 23.4% strains.KRIC-PCR typing showed a genetic diversity among bla_(OXA-23-like) and bla_(OXA-24-like) positive strains,while clonality was observed among bla_(NDM-1)positives.Conclusions:This study highlighted the high prevalence of imipenem resistance in acinetobacter baumannii in Algiers hospitals mediated mainly by bla_(OXA-23-like),bla_(OXA-24-like),and bla_(NDM-1) genes.

Thermostable ethanol tolerant xylanase from a cold-adapted marine species acinetobacter johnsonii

A xylanase-producing bacterium, isolated from deep sea sediments, was identified as the cold-adapted marine species acinetobacter Johnsonii. A cold-adapted marine species acinetobacter Johnsonii could grow at 4 ℃. The optimum temperature and pH of xylanase from a cold-adapted marine species acinetobacter Johnsonii were 55 ℃ and pH 6.0. Xylanase from a cold-adapted marine species acinetobacter Johnsonii remained at 80% activity after incubation for 1 h at 65 ℃. The xylanase activity was 1.2-fold higher in 4% ethanol solution than in ethanol free solution. Gibbs free energy of denaturation, ΔG, was higher in 4% ethanol solution than in ethanol free solution. Thermostable ethanol tolerant xylanase was valuable for bioethanol production by simultaneous saccharification and fermentation process with xylan as a carbon source.

acinetobacter harbinensis HITLi 7^T, a novel species in the genus acinetobacter, was isolated from Songhua River in Harbin, China. It is a psychrotolerant heterotrophic nitrification bacterium that grows in temperatures from 2 ℃ to 35 ℃ with an optimal growth temperature of 8 ℃- 20 ℃[1]. The strain plays an important role in ammonium removal from source water and water sanitation improvement in cold areas. Biologically enhanced activated carbon (BEAC) filters immobilized with HITLi 7^T are used to treat source water in winter, exhibiting stronger ammonium removal capability and higher microbial stability during BEAC treatment at low temperatures[2]. However, the low temperature adaptation mechanism of HITLi 7^T has not been clearly elucidated yet.

We report an atypical case of anti-N-methyl-D-aspartate receptor encephalitis(ANMDARE). A 27-year-old man diagnosed with ANMDARE received immunotherapy and had a good recovery. However, within one month, he developed severe status epilepticus and decreased level of conscience with new hyperpyrexia and dyspnea, and was admitted to the emergency intensive care unit. acinetobacter baumanii were found in the sputum culture; and anti-NMDAR antibodies were positive(titer: 1/80) in the cerebrospinal fluid. Repeated immunotherapy was administered with antibacterial agents, and the patient recovered except for mild psychiatric sequelae. This is the first report of ANMDARE that aggravates after acinetobacter baumannii pneumonia. Awareness and knowledge of this disorder should be extended, especially in the emergency medicine community.
