The paper reports research findings from a field study on accidental ICT help-seeking behaviors in Huifeng West Village and Tuhe Village of Jinghai County,Tianjin.By employing field observation and in-depth interviews,the relations between local residents’ access to and usage of ICT and their social capital are described:seniors tend to rely on bonding social capital when confronting with difficulties in using mobile devices and computers;the middle-aged always make use of bridging and bonding social capital to shoot ICT troubles and prefer to bridging;children and young adults generally ask for help from friends,or after-sales services in their bridging social capital.

Introduction: Acute poisoning in children is cause of hospitalization and death worldwide. It is a Paediatrics emergency with significant economic cost. There are epidemiological differences in acute poisonings in children evaluated in Emergency departments across different countries and regions of the world. The aim of this study was to determine the causes of poisoning in children in our health facility during the period 2000-2019. Methodology: Case notes of children 6 months to 18 years managed for acute poisoning from January 2000 to December 2019 at Paediatrics unit, Federal Teaching Hospital Gombe (FTHG), were analysed. The International classification of Disease ICD 10 was used. Ethical Clearance was obtained. Results: 26,716 children were admitted with 76 cases of Acute poisoning giving a prevalence rate of 2.8/1000 children. 56.6% (43) were males. 63.2% (48) of children were years of age. 69.7% (53) were urban dwellers and 59.2% (45) from low socioeconomic class. 52.6% (40) of poisoning occurred in rainy season. 97.3% (74) of children had unintentional poisoning. Organophosphates 22.3% (17) and kerosine 22.3% (17) were the most common substances ingested. Snake-bite and corrosive ingestion occurred in 13.1% and 11.8% of children respectively. Ingestion was the route in 84.2% of cases. 75.0% (57) of poisoning occurred at home. ACP occurred in children aged 1 - 5 years (p-value = 0.025);More in Children from low socioeconomic status (p-value = 0.001) and urban settings (p-value = 0.001). There was no statistically significant relation between type of poisoning and gender p = 0.839, season p = 0.608, religion p = 0.504 and ethnic background p = 0.342. 4.2% (3) children died each from organophosphate, snake bite and tramadol poisoning. Conclusion: Kerosine and organophosphate were the leading cause of accidental childhood poisoning.

Background:To determine the clinical and endoscopic demographic characteristics of the ingestion of acidic products in the digestive endoscopy unit of the Cocody University Hospital.Methods:This was a retrospective,descriptive and analytical study on endoscopic reports,which took place for five years,from January 1,2014,to December 31,2018.All patients were admitted to the digestive endoscopy unit with the indication of ingestion of caustic products.The parameters studied:are demographic,clinical,and endoscopic.The statistical tests used were Pearson’s CHI 2 and Fisher’s exact tests.The significance threshold was set at 5%.Results:83 patients were included for an endoscopic prevalence of 1.33%.The mean age was 20.77±16.58 years,with extremes ranging from 1 to 63 years.They were divided into 34 men and 49 women for a sex ratio of 0.69.accidental circumstances were observed in 73.49%of cases,followed by attempted autolysis(26.51%).Bleach was the most offending caustic(75.90%),followed by hydrochloric acid(14.46%).FOGD was normal in 53.01%and revealed stage I(41.54%)digestive lesions,followed by stage II(35.39%)and stage III(21.53%).The lesions were preferentially gastric(36.11%),esogastroduodenal(27.78%),and esogastric(22.23%).Based on severity,54.55%of esophageal lesions were stage IIA,53.12%were stage I gastric lesions,and 54.55%were stage IIA duodenal lesions.One case of gastric perforation was noted(1.54%).In univariate analysis,we found a statistically significant link between the age group of 1–14 years and the accidental intake of caustics(P<0.001)on the one hand and the other hand between the age group of 26 years and over and taking for autolysis(P=0.02).Likewise,bleaching was responsible for less severe lesions(P=0.006).Conclusions:The seriousness of the ingestion of caustic products requires early,specialized and multidisciplinary management to improve the short,medium,and long-term prognosis of patients.

Nonnatural Deaths of Children under the Age of 5 Years in Ontario, Canada: A Retrospective Autopsy Analysis of 10 Years (2006–2015)

The goal of this study is to unravel the common patterns observed among nonnatural deaths of children under 5 years and suggest ways to prevent such unnecessary deaths.The study was confined to the province of Ontario to allow the identification of regional specific patterns.The database at the Office of the Chief Coroner of Ontario was searched for all pediatric deaths from the years 2006–2015.All nonnatural deaths for children under the age of 5 years,including sudden infant death syndrome,were selected for the study.Manner and cause of death along with the corresponding year were recorded in tables.In the 10‑year period,940 nonnatural deaths were identified in Ontario through autopsy records which are equal to approximately 36.7%of the total childhood deaths.There were 295 accidental cases(11.6%)and 71 homicides(2.8%).The leading causes of nonnatural childhood deaths in Ontario were drowning,traffic accidents,blunt trauma,and asphyxia.Nonnatural deaths are preventable through education and training of parents.Implementation of educational interventions can significantly reduce nonnatural deaths of children in Ontario.

The location of an ingested foreign object is often difficult to determine by X-ray if gastric air bubbles are not clear in the image.Methods that provide negative contrast can facilitate precise object localization,which is important for object retrieval and treatment of the patient.This case report describes a male child,2 years and 2 mo of age,who accidentally swallowed a lithium battery while playing at home.A plain X-ray showed that the battery was in the abdomen,but it was unclear whether the object was still inside the stomach.A second X-ray examination performed after oral administration of a bloating agent to produce expansion of the stomach and provide negative contrast confirmed that the ingested battery was still in the stomach.The battery was then carefully removed using magnetic and balloon catheters under fluoroscopic guidance.This case report describes the successful use of an orally administered bloating agent without pain to the child in orderto determine the precise location of a foreign object in the abdomen.

Physical restraint use rate and total fall and injurious fall rates: An exploratory study in two US acute care hospitals

This exploratory study used archived hospital data to investigate the relationships between the percentage of patients with physical restraints and the total fall rate as well as the injurious fall rate per 1000 patient-days. The two tested research questions were 1) What is the relationship between the restraint use rate and the total fall rate? 2) What is the relationship between the restraint use rate and the injurious fall rate? The results showed that a higher restraint use rate was associated with a higher total fall rate, yet a lower injurious fall rate in adult inpatient acute care settings. In efforts for fall and injurious fall prevention, front-line managers need to balance the frequency and appropriateness of physical restraint use with optimizing patients’ physical activity. Future research should explore the cause–effect relationship between physical restraint use and consequent injurious fall incidents.

Introduction: In elderly, the progressive degenerations in the nervous system and vestibular system compromise the human body balance leading to a greater risk of falls. The Dynamic Gait Index is a protocol that predicts risks to falls in seniors. Objective: To analyze the results of the application of the Dynamic Gait Index (DGI)-Brazilian brief proposal and establish correlation with the original version. Methods: Study approved by the Ethics and Research Committee of Federal University of Sergipe (number 0197.0.107000.09). We analyzed 223 files of socially active elderly, female gender, aged ranged 60 to 85 years, mean 68.54 (±7.61) years since 2014. The volunteers were evaluated by the DGI-Brazil. For the statistical analysis, the Pearson Correlation Momentum Correlation Test with Spearman Correlation was used, with p ≤ 0.05 and r = 1.0. Results: Sixty-three (27.8%) of the volunteers presented an altered DGI scores. The statistical analysis indicated that Task 1 and Task 2 did not contribute to decrease the total score in the test. The worst performances occurred in Tasks 3, 5 and 6. There was founded a positive correlation between Tasks 1 and 2;1 and 8;3 and 4;3 and 5;4 and 5;2 and 4;2 and 7. Negative correlation occurred between Tasks 4 and 8, and 6 was not correlated with another Task. Conclusion: The DGI-Brazilian brief version presented an excellent correlation with the original proposal and may be useful in clinical practice as a tool to evidence future risks of falls in the elderly people.

Influence of Proxies on the Reported Frequency of Falls, Fear of Falling and Activity Restriction in Older People

Introduction: In older populations, the help of a third person may be necessary for a certain number of individuals in order to fill in questionnaires. The influence of this assistance on the collected information can raise questions, among others concerning the concordance between the information provided by the persons themselves and by their proxies, or the introduction of a potential bias. Our study’s objective is to examine, among older people, the differences in the reported frequency of falls, fear of falling and activity restriction due to fear of falling, according to the fact that people had filled in a questionnaire with or without the help of a third person. Methods: The data used come from the secondary analysis of a cross-sectional survey using a self-administered questionnaire and conducted in 2006 in a Belgian semi-rural town among non-institutionalised persons aged 65 years and over. Results: Among the 501 questionnaires, 16.4% were filled in with the help of another person. In bivariate analysis, the persons who needed help reported fall history, fear of falling and activity restriction significantly more often. In multivariable analysis, when considering concurrently the covariates (sex, age, living alone and fall history for fear of falling and for activity restriction), the influence of having received help to fill in the survey was only significant for activity restriction. Conclusion: Our study shows that it is important to know whether or not a person has received help to fill in a questionnaire. This can actually influence the reported frequency of certain outcomes, even, for activity restriction, when considering simultaneously the effect of other covariates. Given the current and future ageing of our populations, it seems that more extensive exploration is needed of the influence of proxies on the collected information about falls, fear of falling and activity restriction among older people.

Analysis of Functional Capacity and Postural Balance in Former Cutters Cane

Background: Large-scale cane cultivation can promote health impacts of workers, resulting from weight bearing and repetitive movements. Musculoskeletal conditions can alter balance and increase risk of falls. Aim of Work: The objective of this study is to identify clinical characteristics and to correlate balance with functional capacity in ex-cane cutters. Methods: This was a descriptive exploratory study, 42 former workers of the cane cut, with mean age of 49 (±13) years, who were submitted to balance evaluation through the Berg Balance Scale (BSE) and the functional capacity was evaluated by the Incremental Shuttle Walk Test (ISWT). To investigate respiratory symptoms, the Medical Research Council questionnaire was used. Results: The individuals reached a mean of 52 points in BSE, considered low to moderate risk for falls and the average distance traveled of 446.2 (±188.5) meters, corresponding to 52.9% (±23.1) of the normal distance. Weak and negative correlations were found between changes in balance and exposure time (r2-0.16). Conclusion: It is concluded that excessive physical effort contributes to changes in the balance and, consequently, diminish functional capacity.
