To the Editor:Abdominal aortic aneurysm(AAA)is a life-threatening vascular *** intervention is typically recommended when the maximum AAA diameter reaches a threshold of 5.5cm for men and 5.0cm for women,or when an AAA experiences a rapid expansion of greater than 1.0cm/year.[1]Survival after AAA repair is inuenced by various factors,and the identication of independent prognostic factors and the development of accurate prediction models are crucial for optimal treatment planning,counseling,and postoperative *** several scoring systems with varying degrees of accuracy have been developed to predict the risk of mortality after intervention for patients with ruptured AAA.

BACKGROUND With the popularization of various cerebrovascular imaging methods and increased attention to the field,more cerebrovascular diseases are being detected in asymptomatic *** cerebrovascular diseases are typically isolated but occasionally occur simultaneously,causing difficulties in diagnosis and *** changes in the collateral circulation of blood vessels in chronic cerebral artery occlusion patients are slow and dynamic,intercepting morphological development at a specific *** reliance on single imaging tests such as digital subtraction cerebral angiography(DSA)can lead to *** SUMMARY We report a 52-year-old male who was admitted to our department for treatment of an unruptured aneurysm during a follow-up examination for brain trauma after 1 *** tomography(CT)scan was negative,but CT angiography(CTA)revealed a sac-like bulge at the bifurcation of the left middle cerebral *** revealed an unruptured aneurysm with unique scapular *** stump of a middle cerebral artery occlusion was observed during exposure during aneurysm clipping surgery,and the diagnosis of chronic cerebral artery occlusion was confirmed *** case was confusing because of the peculiar morphology of the arterial stump and compensatory angiogenesis due to multiple cerebral artery stenoses observed on preoperative CTA and *** surgery did not cause secondary damage to the patient,and medical treatment for risk factors was continued *** Multiple cerebral arterial stenoses can occur in conjunction with aneurysms or arteriovenous malformations,and their unique morphology can lead to misdiagnosis.

Internal carotid artery (ICA) aneurysms are an unusual but serious cause of epistaxis. This epistaxis is massive and sometimes uncontrollable threatening the vital prognosis of patients. We report the case of a 16-year-old adolescent received in emergency with severe bilateral epistaxis, asthenia and grade-3 left exophthalmos. In his history, the subject had been the victim of an assault six months before consultation. He had received blows on the cephalic extremity with light but repeated epistaxis. The treatment consisted to blood products transfusion and local compression by sterile gazes. An ICA aneurysm in sphenoid sinus has been confirmed in a craniofacial CT scan coupled to vascular opacification. Although the ICA has a variable course in contact with the sphenoid sinus, massive epistaxis would be the consequence of a pronounced dehiscence of the ICA in the sphenoid sinus, particularly in a traumatic context. In front of this type of epistaxis in our context, general practitioners must be able to suspect a ruptured ICA aneurysm in the presence of exophthalmos and a notion of old or recent cranio-encephalic injury. Additionally, due to the high morbidity and mortality of this condition, a monitoring algorithm is necessary for patients with head trauma to facilitate early detection.

Left atrial appendage aneurysm(LAAA)was first reported in the 1960s.1 LAAA is a rare condition,with just over 100 congenital or acquired cases reported to date.2 LAAA is frequently diagnosed incidentally during echocardiography or computed tomography(CT)*** patients with LAAA are asymptomatic,while a few exhibit nonspecific symptoms,such as dyspnea,palpitation,and chest *** with LAAA frequently present with atrial arrhythmias and systemic thromboembolism,such as stroke or multiorgan infarctions,due to the formation of a left atrial appendage thrombus.3 The lesion may be cured using aneurysm *** it as the potential cause of atrial arrhythmias and thromboembolism,the lesion must be identified on time and cured using a suitable treatment approach.

The aneurysms at the initial segment of splenic artery are rare. This paper aimed to investigate the methods to treat the true aneurysm at the initial segment of splenic artery by aneurysmectomy plus vascular reconstruction. Retrospectively reviewed were 11 cases of true aneurysm at the initial segment of splenic artery who were treated in our hospital from January 2000 to June 2013. All cases were diagnosed by color ultrasonography, computer tomography（CT） and angiography. Upon resection of the aneurysm, the auto-vein transplantation was performed in situ between the hepatic artery and the distal part of the splenic artery in 1 case; the artificial vessel bypass was done between the infra-renal aorta and distal portion of the splenic artery in 7 cases; the splenectomy was done in 2 cases; the splenectomy in combination with ligation of multiple small aneurysms were performed in 1 case. All cases were cured and discharged from the hospital 10–14 days after operation. A 1–14 year follow-up showed that 9 cases survived, and 2 cases died, including 1 case who died of acute myocardial infarction 2 years after aorta-splenic artery bypass operation and 1 case who died of acute cerebral hemorrhage 5 years after aneurysm resection and the splenectomy. Among 6 cases receiving aorta-splenic artery bypass, 1 gradually developed stenosis at anatomosed site, which eventually progressed to complete occlusion 2 years to 6 years after operation, without suffering from splenic infarction because the spleen was supplied by the short gastric vessel and its collaterals. The other 5 cases receiving aorta-splenic artery bypass and 1 case undergoing autologous vascular transplantation did not develop stricture or pseudoaneurysm at the stoma. Our study showed that the aneurysmectomy plus vascular reconstruction is a better treatment for aneurysm at the initial segment of splenic artery.

A new Willis aneurysm system is proposed, which contains the Atangana-Baleanu(AB) fractional *** obtain the numerical solution of the Atangana–Baleanu fractional Willis aneurysm system(ABWAS) with the AB fractional integral and the predictor–corrector ***, we research the chaotic properties of ABWAS with phase diagrams and Poincare *** different values of pulse pressure and system order are used to evaluate and compare their effects on *** simulations verify that the changes of pulse pressure and system order are the significant reason for ABWAS'states varying from chaotic to *** addition, compared with Caputo fractional WAS(FWAS),ABWAS shows less state that is ***, the results of bifurcation diagrams of blood flow damping coefficient and reciprocal heart rate show that the blood flow velocity tends to stabilize with the increase of blood flow damping coefficient or reciprocal heart rate, which is consistent with embolization therapy and drug therapy for clinical treatment of cerebral ***, in view of the fact that ABWAS in chaotic state increases the possibility of rupture of cerebral aneurysms, a reasonable controller is designed to control ABWAS based on the stability *** with the control results of FWAS by the same method, the results show that the blood flow velocity in the ABWAS system varies in a smaller ***, the control effect of ABWAS is better and more *** new Willis aneurysm system with Atangana–Baleanu fractional derivative provides new information for the further study on treatment and control of brain aneurysms.

Intracranial aneurysms are pathological dilatations which endanger people＇s health. Hemodynamics is thought to be an important factor in the pathogenesis and treatment of aneurysms. To date, the bulk of investigations into hemodynamics have been conducted by making use of mathematically idealized models for rigid aneurysms and associated arteries. However the walls of aneurysms and associated arteries are elastic in vivo. This study shows the differences of the simulation between elastic and rigid wall models. The numerical simulation of elastic aneurysm model is made fi＇om a representative Digital Subtraction Angiography （DSA） image and calculated with CFD software to get the wall deformation and the velocity field. Then the results are analyzed. By comparing the simulation results of the two models from their velocity vectors and shear stress distribution, many differences can be noted. The main difference exists in the distribution of velocity magnitude at some sections, with one outlet having obviously off-center distribution for the elastic wall model. The currents of the distribution of wall shear stress along the wall of aneurysm simulated in rigid and elastic wall models were similar. But there were apparent differences between the two models on the values of wall shear stress especially at the neck of aneurysm. The off-center distribution of velocity magnitude affects the distribution of wall shear stress and the exchange of substance through the wall. The analysis demonstrated clearly that the results of 2-D elastic numerical simulation were in good agreement with the clinical and pathological practice. The results of this study play an important role in the formation, growth, rupture and prognosis of an aneurysm on clinic application.

Abdominal aortic aneurysm(AAA) is one of the most common and catastrophic manifestations of the acute aortic syndrome that can be treated with endovascular aneurysm repair(EVAR) which requires a specially designed stent-graft *** this work, a self-expanding nickel–titanium(nitinol) stent-graft system is aiming at AAA using finite element analysis(FEA) methods to analyze both fatigue behaviors and radial *** on the systematic analysis of the parametric variations, a final stent-graft system was developed by the selection and arrangement of the individual stent components, targeting an optimal performance for the treatment of *** tests, animal tests and clinical trials were carried out to confirm the *** animal trials and clinical trials showed comparable curative effects with Medtronic Endurant stent-graft(SG) systems.

Experimental and numerical study of the effect of pulsatile flow on wall displacement oscillation in a flexible lateral aneurysm model

This study experimentally and numerically investigated the effect of pulsatile flow of different frequencies and outflow resistance on wall deformation in a lateral aneurysm.A method for constructing a flexible aneurysm model was developed,and a self-designed piston pump was used to provide the pulsatile flow conditions.A fluid-structure interaction simulation was applied for comparison with and analysis of experimental *** maximum wall displacement oscillation increased as the pulsation frequency and outflow resistance increased,especially at the aneurysm *** is an obvious circular motion of the vortex center accompanying the periodic inflow fluctuation,and the pressure at the aneurysm dome at peak flow increased as the pulsatile flow frequency and terminal flow resistance *** results could explain why abnormal blood flow with high frequency and high outflow resistance is one of the risk factors for aneurysm rupture.
