Debulking of Keloid Combined with Intralesional Injection of 5-flurouracil and Triamcinolone versus Intralesional Injection of 5-flurouracil and Triamcinolone

Background: A wide range of therapies exist for keloids. But despite the multiple treatment modalities available, keloids still remain a significant challenge for both the clinician and the patient. Objective: To assess the effectiveness and safety of debulking of keloid with intralesional injection of 5-flurouracil and triamcinolone in comparison to intralesional injection of 5-flurouracil and triamcinolone alone. Patient and Methods: This single blinded comparative therapeutic outpatient based study was done at the Department of Dermatology, Baghdad Teaching Hospital during the period from February 2011 to February 2013. Twenty-three patients with 52 lesions were enrolled in the study and consisted of two groups: Group A (26 lesions): Treated by intralesional injection of combination of 5-flurouracil and triamcinolone acetonide and repeated every month;Group B (26 lesions): Debulking was carried out first and then injection of combination of 5-flurouracil and triamcinolone acetonide at time of operation and then repeated injection monthly. Evaluation was performed by scoring system with 5 criteria (redness, elevation, hardness, itching and tenderness). The response was graded as no response, minimal, moderate or complete response. Results: Both modalities of treatments showed significant results in treating keloid, but the intralesional therapy alone was slightly better than debulking plus intralesional and no systemic adverse effects were noticed in both groups. Conclusion: Both modalities of therapy were effective in treatment of keloid, and there was no much difference between them.

Synthesis,characterization and antitumor activity of rare earth(Y,La) substituted phosphotungstates containing 5-fluorouracil

Two rare earth substituted Keggin-type phosphomngstates containing 5-fluorouracil, K10C4H4FN2O2Y（PW11O39）2· 10H2O （FYPW） and Kg（C4H4FN2O2）2La（PWl1O39）2· 18H2O （FLaPW） were synthesized in aqueous solution, and the structures were characterized by Fourier transform infrared spectroscopy （FT-IR）, element analysis, inductively coupled plasma spectrometry （ICP）, X-ray powder diffraction （XRD） and 1H nuclear magnetic resonance （NMR）. Thermal analysis showed that the decomposition processes of the two compounds could be di- vided into three steps, the loss of crystal water, the decomposition of 5-fluorouracil unit and the decomposition of Keggin-type polyanions. The in vitro cytotoxicities against HEK 293, HeLa and HepG-2 cells were studied by methyl thiazolyl tetrazolium （MTT） assay. The results showed that FYPW and FLaPW had higher antitumor activities than 5-fluorouracil, C4H4FN2O2H2PW12O40.8H2O and rare earth substituted polyoxometalates KllLn（PW11O39）2@nH2O （Ln-Y, La）. It was concluded that introducing 5-fluorouracil and rare earth （Y or La） into its structure was a considerable way to enhance antitumor activities ofpolyoxometalates.

Babao Dan Alleviates 5-Fluorouracil-Induced Intestinal Damage via Wnt/β-Catenin Pathway

Evidence-based medical oncology and interventional radiology paradigms for liver-dominant colorectal cancer metastases

Colorectal cancer metastasizes predictably, with liver predominance in most cases. Because liver involvement has been shown to be a major determinant of survival in this population, liver-directed therapies are increasingly considered even in cases where there is(limited) extrahepatic disease. Unfortunately, these patients carry a known risk of recurrence in the liver regardless of initial therapy choice. Therefore, there is a demand for minimally invasive, non-surgical, personalized cancer treatments to preserve quality of life in the induction, consolidation, and maintenance phases of cancer therapy. This report aims to review evidence-based conceptual, pharmacological, and technological paradigm shifts in parenteral and percutaneous treatment strategies as well as forthcoming evidence regarding next-generation systemic, locoregional, and local treatment approaches for this patient population.
