To the Editor:Ischemic stroke remains a major cause of death and disability worldwide and contributes to the rising costs of health care.Thrombolytic therapy with tissue plasminogen activator (tPA,alteplase)is beneficial for the treatment of acute ischemic stroke.However,only 40%to 50%of stroke patients show a significant improvement after treatment.In addition,the use of alteplase is restricted because symptomatic intracranial hemorrhase (slCH)occurs in 1.7%to 6.4%of treated patients.[1] 25-hydroxyvitamin D (25(OH)D),which is the major circulating metabolite of vitamin D,has been confirmed to be closely related to cerebrovascular disease (CVD).A series of studies indicated that vitamin D deficiency was associated with an increased risk of ischemic stroke,high stroke severity and poor functional outcomes.

The effect of ageing was evaluated on quality parameters(pH,electrical conductivity(EC),osmolality,drip loss(DL)and cooking loss(CL)),proteins,fat and 25-hydroxyvitamin D(25-OH-D)total levels and antioxidant status(malondialdehyde(MDA)and catalase(CAT)activity)in meat of camels during storage at 4±1°C.Samples were taken from the brachial triceps muscle(triceps barchii)and were stored at 4±1°C for 10 d.Quality parameters,chemical composition and antioxidant status were assessed at 3 h and 24 h postslaughter and 5,7 and 10 d postmortem during cold storage.CAT activity significantly decreased while osmolality,EC,DL,CL and MDA contents significantly increased,from the 5th or 7th postmortem day of cold storage of the camel meat.However,proteins,lipids and 25-OH-D total contents showed no significant differences during all period of ageing.In conclusion,in the dromedary camel,ageing time of triceps muscle influenced significantly its quality characteristics and antioxidant status from the 5th or 7th postmortem day of refrigerated storage,without any variation of proteins,fat and 25-OH-D contents.

Vitamin D levels do not predict the stage of hepatic fibrosis in patients with non-alcoholic fatty liver disease: A PRISMA compliant systematic review and meta-analysis of pooled data

Evidence for the association between vitamin D and risk of recurrent stroke remains sparse and limited. We aimed to assess the relationship between serum circulating 25-hydroxyvitamin D(25(OH)D) level and risk of recurrent stroke in patients with a stroke history, and to identify the optimal 25(OH)D level in relation to lowest recurrent stroke risk. Methods Data from the nationwide prospective United Kingdom Biobank were used for analyses. Primary outcome was time to first stroke recurrence requiring a hospital visit during follow-up. We used Cox proportional hazards regression model with restricted cubic splines to explore 25(OH)D level in relation to recurrent stroke. The dose-response relationship between 25(OH)D and recurrent stroke risk was also estimated, taking the level of 10 nmol/L as reference. Results A total of 6,824 participants(mean age: 60.6 years, 40.8% females) with a baseline stroke were included for analyses. There were 388(5.7%) recurrent stroke events documented during a mean follow-up of 7.6 years. Using Cox proportional hazards regression model with restricted cubic splines, a quasi J-shaped relationship between 25(OH)D and risk of recurrent stroke was found, where the lowest recurrent stroke risk lay at the 25(OH)D level of approximate 60 nmol/L. When compared with 10 nmol/L, a 25(OH)D level of 60 nmol/L was related with a 48% reduction in the recurrent stroke risk(hazard ratio = 0.52, 95% confidence interval: 0.33-0.83). Conclusion Based on data from a large-scale prospective cohort, we found a quasi Jshaped relationship between 25(OH)D and risk of recurrent stroke with the lowest risk at around 60 nmol/L. These results may provide some insights into the optimal vitamin D status recommendation for recurrent stroke prevention in patients with a prior stroke history.

AIM To investigate the relationship between 25-hydroxyvitamin D [25(OH)D] levels and fibrosis stage in patients with non-alcoholic fatty liver disease(NAFLD).METHODS Two individual reviewers identified relevant studies using the Pub Med, EMBASE, Cochrane, and Scopus databases. Inclusion criteria were as follows:(1) Studies that evaluated adults with NAFLD and serum or plasma 25(OH)D levels; and(2) assessed fibrosis stage using liver biopsy. A rigorous analysis yielded six articles as having sufficient data to employ in evaluating the association of serum vitamin D levels in patients with NAFLD based on their liver fibrosis stage by histopathological analysis. The lead investigators of each of the six studies were contacted and the data were collected. To meta-analyze vitamin D levels in F0-F2 vs F3-F4 fibrosis, a random-effects meta-analysis fit using restricted maximum likelihood was applied. To examine trends across each stage of fibrosis with respect to vitamin D levels, a meta-regression was performed. P 25(OH)D levels across the individual stages(F0-F4) of fibrosis did not show an association for the six included studies.CONCLUSION Low vitamin D status is not associated with higher stages of liver fibrosis in patients with NAFLD.

AIM: To investigate the relationship between serum vitamin D3 levels and liver fibrosis or inflammation in treatment-naive Chinese patients with chronic hepatitis C(CHC).METHODS: From July 2010 to June 2011, we enrolled 122 CHC patients and 11 healthy controls from Dingxicity, Gansu Province, China. The patients were infected with Hepatitis C virus(HCV) during blood cell retransfusion following plasma donation in 1992-1995, and had never received antiviral treatment. At present, all the patients except two underwent liver biopsy with ultrasound guidance. The Scheuer Scoring System was used to evaluate hepatic inflammation and the Metavir Scoring System was used to evaluate hepatic fibrosis. Twelve-hour overnight fasting blood samples were collected in the morning of the day of biopsy. Serum levels of alanine aminotransferase, aspartate aminotransferase, total bilirubin, direct bilirubin, cholineste rase, prothrom binactivity, albumin, γ-glutamyl transpeptidase, hemoglobin, calcium and phosphorus were determined. Serum HCV RNA levels were measured by real-time PCR. Serum levels of 25-hydroxyvitamin D3 [25(OH)D3] and 24,25-dihydroxyvitamin D3 [24,25(OH)2D3] were measured by high-performance liquid chromatography tandem mass spectrometry.R E S U LT S : Serumlevels of 2 5(OH) D3 but not 24,25(OH)2D3 were significantly lower in CHC patients than in control subjects. Serum 25(OH)D3 levels did not correlate with liver fibrosis, inflammation, patient age, or levels of alanine aminotransferase, aspartate aminotransferase, total bilirubin, direct bilirubin, prothrombin activity, cholinesterase or HCV RNA. However, serum 25(OH)D3 levels did correlate with serum 24,25(OH)2D3 levels. Serum 25(OH)D3 and 24,25(OH)2D3 levels, and the 25(OH)D3/24,25(OH)2D3 ratio, have no difference among the fibrosis stages or inflammation grades.CONCLUSION: We found that serum levels of 25(OH)D3 and its degradation metabolite 24,25(OH)2D3 did not correlate with liver fibrosis in treatment-naive Chinese patient with CHC.

Circulating levels of vitamin D and colorectal adenoma: A case-control study and a meta-analysis

AIM: To examine the association between circulating 25-hydroxyvitamin D [25(OH)D] levels and colorectal adenoma in a case-control study and a meta-analysis. METHODS: We conducted a matched case-control study(112 cases and 112 matched controls) and combined 15 studies, including our study, in a metaanalysis. The study-specific odds ratios(ORs) and 95% confidence intervals(CIs) were pooled using a randomeffects model. In total, 5454 colorectal adenomas and 6656 controls were included in the meta-analysis. RESULTS: In a meta-analysis including 14 previous studies and our study, we observed a significant inverse association between circulating 25(OH)D levels and colorectal adenoma(OR = 0.68; 95%CI: 0.54-0.82) when comparing the highest category with the lowest category. Stratification by adenoma location(proximal or distal adenoma) showed similar estimates. When we stratified by study region, the ORs(95%CIs) were 0.70(0.52-0.88) in the US and 0.66(0.34-0.97) in Asia. CONCLUSION: These data suggest an inverse association between circulating 25(OH) Dlevels and colorectal adenoma in both Western and Asian populations.

目的调查分析极低出生体重早产儿血清磷（P）、骨碱性磷酸酶（BALP）、25-羟基维生素D[25-（OH）D3]水平动态变化,为临床早产儿代谢性骨病的早期预防提供指导.方法选择入住烟台山医院新生儿科病房且符合入选标准的110例新生儿为研究对象,其中极低出生体重早产儿60例（病例组）,足月产儿50例（对照组）,分别于出生后1、4、12周空腹采取静脉血2 ml,测定血清P、BALP、25-（OH）D3水平,分析骨代谢指标的变化情况.结果病例组25-（OH）D3水平增长速度低于对照组;两组血清P水平无明显变化.与对照组比较,病例组出生后1、4、12周血清BALP水平均显著性偏高;出生后第12周25-（OH）D3水平的差异有统计学意义,其中病例组异常率达26.7％,对照组异常率为0.结论极低出生体重早产儿（病例组）出生后追赶性生长过程中血清BALP、25-（OH）D3水平与足月儿（对照组）有明显差异,提示该两项指标检测有助于极低出生体重儿代谢性骨病的早期发现及干预,其中以血清BALP指标敏感性较好.

Vitamin D3 Supplementation Improved Physical Performance in Healthy Older Adults in Japan: A Pilot Study

Japan has the highest proportion of older adults and so-called “Super-aged society” in the world. These results suggested that the prevalence of both cognitive and physical functional impairment increases with age. Therefore, the present study was designed to investigate the effect of vitamin D3 supplementation in cognitive and physical functional impairment in healthy older adults in Japan. We enrolled 7 Japanese male (age: 76.0 ± 8.7) and 5 female (age: 78.3 ± 9.3) in this study. The physical function of even a person getting on a wheelchair could be tested in all subjects. Treatment group (n = 7) consisted of a 500 IU/day vitamin D3 corresponding to twice of daily requirement for 6 months. Blood was collected by venipuncture and the serum 25-hydroxy vitamin D (25 OHD), 1,25-hydroxy vitamin D (1,25 OHD), Ca and PTH concentration were measured. The Mini-Mental State Examination and the Montreal Cognitive Assessment-Japanese version were used for the cognitive function test. Physical function was measured objectively using the Timed Up and Go (TUG), 4-mgait speed test (4 MGS), 5-repetition sit-to-stand (5 STS), 30-second chair stand test (CS-30) and measurement of upper grip strength. Our results show that a 6-month intake of vitamin D3 increased serum 25 OHD within the insufficiency and sufficiency levels without changing 1,25 OHD, Ca and PTH concentration. And it significantly decreased 4 MGS without changing TUG, 5 STS, CS-30, upper grip strength and cognitive function. Serum 25 OHD cut-off values for skeletal muscle index, grip strength and 4 MGS in diagnosis of sarcopenia are 18.6 - 23.4 ng/mL. These findings suggest that serum 25 OHD levels (23.4 ng/mL) might improve moving capacity, lower limb muscle strength, and physical balance functional impairment.

Background: A relationship among vitamin D and pulmonary tuberculosis. Objective: To conclude the occurrence of Vitamin D insufficiency in subjects suffering from pulmonary TB. Design: This was a cross sectional study. Setting: Current study was performed at department of medicine, Peoples Medical College Hospital Nawabshah starting from January 2017 to December 2017. Samples: 385 AFB positive subjects of pulmonary tuberculosis after achieving the criteria of selection were recruited. Material and Methods: After a brief discussion, the patients were characterized for variable evaluation such as age, gender, sputum AFB and duration of disease and presence of low vitamin D levels. Sputum AFB criterion was used for Data collection. Fasting blood samples were collected for analysis of vitamin D levels in all subjects of AFB positive pulmonary tuberculosis. Results: In 385 diagnosed patients with smear positive PTB, 273 (70.9%) males and 112 (29.1%) were female. Vitamin D levels were within normal or sufficient range in 134 (34.8%), below normal range in 251 (65.2%) cases (insufficient in 09.56% (24/251) and deficient in 90.43% (227/251) cases). Conclusion: Decreased levels of vitamin D were common in subjects with AFB smear Positive PTB cases.

Background: Pediatric patients with type 1 diabetes (T1D) have increased risk for low bone mineral density, which may be due in part to low 25-hydroxyvitamin D levels. Vitamin D levels are influenced by sunlight exposure and thus display geographical variation. We hypothesize that the prevalence of 25-hydroxyvitamin D deficiency (25-hydroxyvitamin D levels. Methods: Medical records of patients with T1D being followed in Tennessee were reviewed for demographics, medical information, and 25-hydroxyvitamin D levels during the previous 2 years. Control subjects were obtained from a de-identified database of healthy pediatric subjects living in a similar geographical area. Chi squared tests and multivariable linear regression were performed. Results: Children and adolescents with T1D (n = 276;median age 14 years) have a significantly higher percentage of vitamin D deficiency and insufficiency compared with healthy pediatric controls (n = 100;median age 11.2 years) (68% versus 44%;p 25-hydroxyvitamin D level is 24 ng/mL versus 31 ng/mL, respectively. After adjusting for age, race, gender, UV light exposure, BMI, and multivitamin supplementation, children and adolescents with T1D have a serum 25-hydroxyvitamin D level 6.7 ng/mL lower than the control population [CI(4.11, 9.21), p 25-hydroxyvitamin D levels. Conclusions: There is an increased prevalence of 25-hydroxyvitamin D deficiency and insufficiency in US. children with T1D compared with geographically similar children without diabetes. Further research is needed to determine whether decreased serum 25-hydroxyvitamin D can be alleviated via dietary or behavioral modifications in this population.
