The outbreak of the 2019-ncov infection began in December 2019 in Wuhan,Hubei province,and rapidly spread to many provinces in China as well as other *** we report the epidemiological,clinical,laboratory,and radiological characteristics,as well as potential biomarkers for predicting disease severity in 2019-ncov-infected patients in Shenzhen,*** 12 cases of the 2019-ncov-infected patients developed pneumonia and half of them developed acute respiratory distress syndrome (ARDS).The most common laboratory abnormalities were hypoalbuminemia,lymphopenia,decreased percentage of lymphocytes (LYM) and neutrophils (NEU),elevated C-reactive protein (CRP) and lactate dehydrogenase (LDH),and decreased CD8 *** viral load of 2019-ncov detected from patient respiratory tracts was positively linked to lung disease ***,LYM,LYM (%),LDH,NEU (%),and CRP were highly correlated to the acute lung ***,viral load,lung injury score,and blood biochemistry indexes,albumin (ALB),CRP,LDH,LYM (%),LYM,and NEU (%),may be predictors of disease ***,the Angiotensin Ⅱlevel in the plasma sample from 2019-ncov infected patients was markedly elevated and linearly associated to viral load and lung *** results suggest a number of potential diagnosis biomarkers and angiotensin receptor blocker (ARB) drugs for potential repurposing treatment of 2019-ncov infection.

It has been known that,the novel coronavirus,2019-ncov,which is considered similar to SARS-CoV,invades human cells via the receptor angiotensin converting enzyme II(ACE2).Moreover,lung cells that have ACE2 expression may be the main target cells during 2019-ncov ***,some patients also exhibit non-respiratory symptoms,such as kidney failure,implying that 2019-ncov could also invade other *** construct a risk map of different human organs,we analyzed the single-cell RNA sequencing(scRNA-seq)datasets derived from major human physiological systems,including the respiratory,cardiovascular,digestive,and urinary *** scRNA-seq data analyses,we identified the organs at risk,such as lung,heart,esophagus,kidney,bladder,and ileum,and located specific cell types(i.e.,type II alveolar cells(AT2),myocardial cells,proximal tubule cells of the kidney,ileum and esophagus epithelial cells,and bladder urothelial cells),which are vulnerable to 2019-ncov *** on the findings,we constructed a risk map indicating the vulnerability of different organs to 2019-ncov *** study may provide potential clues for further investigation of the pathogenesis and route of 2019-ncov infection.

核衣壳(nucleocapsid,N)蛋白是新型冠状病毒的结构蛋白,与病毒RNA的转录和复制有关。目前,N蛋白已被广泛应用于新型冠状病毒肺炎的检测诊断及治疗。本研究克隆的2019-ncov-n基因CDS由1257个碱基构成,编码419个氨基酸。将该基因连接到昆虫表达载体pFastBacTMHTB构建重组Bacmid,转染至BmN细胞中获得重组病毒,将重组病毒注射家蚕蛹体内,收集发病家蚕蛹血淋巴,通过镍柱亲和纯化,SDS-PAGE和Western blot检测表明获得一定纯度具有活性的重组2019-ncov-N蛋白,为后期进一步开发抗体检测试剂盒鉴定了基础。

In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named "2019 novel coronavirus(2019-ncov)" by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-ncov infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-ncov. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-ncov infected pneumonia and experience and lessons of hospital rescue for 2019-ncov infections. This rapid advice guideline is suitable for the first frontline doctors and nurses, managers of hospitals and healthcare sections, community residents, public health persons, relevant researchers, and all person who are interested in the 2019-ncov.

目的评估血清新型冠状病毒(2019-ncov)IgM/IgG检测在COVID-19快速诊断中的应用价值,以期进一步完善COVID-19的诊断/筛查体系。方法收集32例COVID-19患者(RT-PCR法检测2019-ncov核酸阳性,且有临床症状者)和34例非COVID-19患者(RT-PCR法检测2019-ncov核酸阴性,并经临床诊断排除COVID-19者)的血液样本,采用胶体金免疫层析法对样本进行2019-ncov IgM/IgG快速检测,并分析检测的灵敏性、特异性及其与病程变化的相关性。结果32份COVID-19患者样本中,9份样本2019-ncov IgM阳性,检出率为28.1%(9/32);2019-ncov IgG阳性样本25份,检出率为78.1%(25/32);IgM/IgG总检出率为84.4%(27/32)。34份非COVID-19患者样本中,2019-ncov IgG阳性样本2份,检出率为5.9%(2/34);未检测到2019-ncov IgM阳性样本。COVID-19患者出现症状后的10~20 d,血清IgM检出率为42.9%(3/7),21~30 d为30.8%(4/13),31~40 d为16.7%(2/12),随病程延长呈下降趋势;COVID-19患者出现症状后的10~20 d,血清IgG检出率为57.1%(4/7),21~30 d为84.6%(11/13),31~40 d为83.3%(10/12),随病程延长呈上升趋势,在21~30 d左右达高峰。结论血清2019-ncov IgM/IgG检测(胶体金法)在诊断2019-ncov感染中具有较高的灵敏性(84.4%)和较强的特异性(94.1%),因该检测方法具有灵活、快速、简便等特点,在COVID-19的诊断/筛查中具有较强的应用价值,可作为COVID-19诊断/筛查体系的有益补充。

Preliminary Exploration of the Initial Diagnostic Prediction Model of Moderate Coronavirus Disease 2019 (2019-ncov) Based on Clinical Data

Objective: To explore those differences and relationships of the initial diagnostic clinical data between confirmed cases of 2019-ncov and suspected cases of COVID-19, and then to establish prediction models for predicting the probability of the first diagnosis of 2019-ncov. Methods: A total of 81 suspected cases and 87 confirmed cases of moderate 2019-ncov diagnosed initially in the isolation wards of the First People’s Hospital of Wuhu and the People’s Hospital of Wuwei and Wuhan Caidian Module Hospital with the help of our hospital doctors were gathered, and retrospectively analyzed. Results: The most common symptoms were fever (76.79%) and cough (64.29%) in the total of 168 cases. The median age was 45 (35 - 56) years old in the 87 confirmed cases of moderate 2019-ncov, older than the median age 36 (29 - 50) in the 81 suspected cases. There were significant more in the former than in the latter in the incidence of myalgia, ground-glass opacity (GGO), invasions of lesion in the peripheral lobes, vascular thickening and bronchial wall thickening, interlobular septal thicking, and small pulmonary nodules. On the contrary, there were less in the former than in the latter in the total number of leukocytes and neutrophils in blood routine examination and the levels of procalcitonin (PCT). Two groups were statistically significantly different (P 2019-ncov, and the total number of leukocytes, cough, pharyngalgia and headache were negatively related. In December of 2019,27 patients with viral pneumonia including 7 severe cases were reported inWuhan,the capital city of Hubei province and one of the largest cities of *** the morning of January 23,2020,the infected cases have increased to 571 including 17 deaths,and spread to 25 of 31 mainland provinces and municipals and Hong Kong,Macao and Taiwan as well as to 7 other countries,including South Korea,Japan,Thailand,Singapore,Philippines,Mexico and the United States of America.
