Efficacy comparison between hepatic arterial infusion chemotherapy plus systemic chemotherapy used as first-line and non-first-line treatments for the patients of colorectal cancers with unresectable hepatic metastases

Objective: The combination of hepatic arterial chemotherapy(HAIC) and systemic chemotherapy(SYC) has potential effect on colorectal cancer(CRC) patients with unresectable hepatic metastasis. The aim of this retrospective study was to investigate the efficacy and safety of this combined therapeutic regimen on chinese patients based on single institute experiences. Methods: All 54 patients of this retrospective analysis were diagnosed with CRC with unresectable liver metastasis and received combined HAIC and SYC. Among the patients, 23 of them received HAIC plus SYC when they developed liver metastases as first-line treatment(Group 1), and 31 patients received HAIC plus SYC as non-first-line treatment(Group 2). The different efficacy in two groups was analyzed by SPSS 19.0. Results: The overall response rate(ORR) were 52.2% and 25.8% respectively in Groups 1 and 2(P = 0.047), and the disease control rate(DCR) were 65.2% and 35.5% respectively in Groups 1 and 2(P = 0.031). The median progression-free survival(PFS) were 6.8 and 3.3 months(P = 0.002), the median hepatic progression-free survival(H-PFS) were 8.8 and 3.7 months(P = 0.001), and the median overall survival(OS) were 18.8 and 13.7 months(P = 0.121) in Groups 1 and 2, respectively. No fatal reaction was observed and no significant difference of adverse reaction was found in two groups. Grade 3/4 toxic effects included neutropenia(9.7% in Group 2 only), gastrointestinal reaction(8.7% in Group 1 and 6.5% in Group 2), stomatitis(6.5% in Group 2 only) and hyperbilirubinemia(4.3% in Group 1 only). Conclusion: HAIC combined with SYC showed promising efficacy and safe profiles on CRC patients with unresectable liver metastases.

血管钙化(vascular calcification,VC)在糖尿病、慢性肾脏病、及心血管疾病等患者中是一种常见的病理过程,表现为羟基磷灰石在血管壁上沉积。硫化氢(hydrogen sulfide,H_(2)S)是继NO和CO之后在哺乳动物中发现的第三种气体信号分子,它在心血管系统中具有抗炎、抗氧化应激等作用。近年来,人们已经认识到H_(2)S具有抗VC的作用,补充H_(2)S及其供体可以减轻VC。本文综述了H_(2)S对VC起保护作用的多种证据,并强调了在VC发展过程中H_(2)S代谢改变,及H_(2)S对VC病理生理改变的调控机制。

Medication Rules of traditional chinese medicine for Treating Threatened Abortion in the Early Stage of Pregnancy with Subchorionic Haematoma Based on Data Mining

Objective:Using data mining technology to explore the rules of traditional chinese medicine(TCM)in the treatment of threatened abortion in the early stage of pregnancy with sub-chorionic haematoma(SCH).Methods:Literature of TCM in the treatment of threatened abortion in the early stage of pregnancy with SCH were retrieved from CNKI,VIP,WANFANG and Pubmed,*** literature information database was established to be used for descriptive analysis,association rule analysis and cluster analysis of relevant ***:A total of 100 literatures were included,involving 114 chinese *** efficacy of chinese herbs were mainly tonic drugs,hemostatic drugs,heat-clearing drugs,dissolving blood stasis and hemostatic *** medicinal properties were mostly mild and warm,and the taste of the drug was mainly sweet,bitter and *** liver meridian,spleen meridian and kidney meridian were frequently *** commonly used drug pair combination was"Xu duan(Radix dipsaci,续断)-Tusizi(Semen Cuscutae,菟丝子)",and the core combination was"Tusizi-Xu duan-Ejiao(Donkeyhide gelatin,阿胶)-Baizhu(Atractylodes macrocephala,白术)-Dangshen(Codonopsis pilosula,党参)".Commonly used drugs for removing blood stasis and hemostasis were with Sanqi(Panax notoginseng,三七),Puhuang(cattail pollen,蒲黄),and Qiancao(Radix Rubiae,茜草).Conclusion:Data mining traditional chinese medicine for the treatment of threatened abortion in the early stage of pregnancy with SCH clinically commonly used drug efficacy,taste,meridian,commonly used drug pairs,core combination and commonly used blood stasis hemostatic drugs,has important reference significance for the treatment of threatened abortion in the early stage of pregnancy combined with SCH.

Hepatocellular carcinoma(HCC)is one of most common and deadliest ***(Cel),a natural product derived from the Tripterygium wilfordii plant,has been extensively researched for its potential effectiveness in fighting ***,its clinical application has been hindered by the unclear mechanism of ***,we used chemical proteomics to identify the direct targets of Cel and enhanced its targetability and antitumor capacity by developing a Cel-based liposomes in *** demonstrated that Cel selectively targets the voltage-dependent anion channel 2(VDAC2).Cel directly binds to the cysteine residues of VDAC2,and induces cytochrome C release via dysregulating VDAC2-mediated mitochondrial permeability transition pore(mPTP)*** further found that Cel induces ROS-mediated ferroptosis and apoptosis in HCC ***,coencapsulation of Cel into alkyl glucoside-modified liposomes(AGCL)improved its antitumor efficacy and minimized its side *** has been shown to effectively suppress the proliferation of tumor *** a xenograft nude mice experiment,AGCL significantly inhibited tumor growth and promoted *** findings reveal that Cel directly targets VDAC2 to induce mitochondria-dependent cell death,while the Cel liposomes enhance its targetability and reduces side ***,Cel shows promise as a therapeutic agent for HCC.

Accurate and dynamic visualization of vascular diseases can contribute to restraining further deterioration from diseases in a timely ***,visualization is still unable to precisely determine whether and to what extent blood vessels or brain tissues are ***,we report novel benzobis(1,2,5-thiadiazole)-based second near-infrared region(NIR-II)fluorophores HY1-HY4 with highly twisted structures(55°at the S_(0) state),extremely strong aggregation-induced emission(AIE)characteristics(I/I_(0)>13),and remarkably high fluorescence quantum yields(QYs)(up to 14.45%)in the NIR-II region(>1000 nm)and∼0.27%in the nearinfrared IIb window(NIR-IIb,>1500 nm)in aqueous *** NIR-IIb AIE HY4 dots,high-resolution NIR-IIb fluorescence imaging of revascularization and thrombolysis,and real-time feedback of the therapeutic efficacy of chinese medicine Dengzhan Xixin injection(DXI)on ischemic stroke,were achieved for the first *** addition,results showed that DXI conferred neuroprotection against cerebral ischemia injury mediated via the angiogenesis *** attractive results provide a new perspective for designing ultrabright NIR-IIb probes for vascular-related phenomena,disease assessment,and precise intraoperative imageguided therapy with a deeper tissue penetration depth and higher resolution.

Therapeutic Effects of Acupuncture on Tendinopathy: Systematic Review and Meta-Analysis

Objective:To compare the effects of acupuncture and other treatments on tendinopathy through ***:Computerized databases,including PubMed,EBSCO,EMBASE,WanFang database,Web of Science,OVID and Cochrane Library were searched for trials concerning acupuncture for tendinopathy from establishing time to October *** controlled trials,quasi-randomized controlled trials,and non-randomized controlled trials related to acupuncture for tendinopathy were ***,we used Review Manager 5.3 to perform a Meta-analysis of included ***:Meta-analysis of ten studies showed that the reductions in visual analog scale were signiﬁcantly different between two groups whether in short-time follow-up(MD=–0.61,95%CI[–1.07 to–0.15],P=0.009)or in long-time follow-up(MD=–1.92,95%CI[–2.08 to–1.77],P<0.00001).The cure rates were signiﬁcantly different(OR=8.97,95%CI[1.57–51.09],P=0.01),as well as the adverse events(OR=0.91,95%CI[0.30–2.74],P=0.87).Conclusion:Acupuncture provides superior pain relief,improvement in function,and safety than other control conditions/active non-acupuncture interventions in tendinopathy patients.

目的研究黄芪的主要有效成分黄芪甲苷和三七的主要有效成分人参皂苷Rg1、人参皂苷Rb1、三七皂苷R1配伍对小鼠脑缺血再灌注后脑组织能量代谢的影响。方法 :C57BL/6小鼠随机分组,连续给药3天,末次给药1小时后,结扎双侧颈总动脉造成脑缺血20 min,再灌注24 h。用高效液相色谱法(high performance liquid chromatography,HPLC)测定脑组织中ATP、ADP、AMP含量,计算总腺嘌呤核苷酸(total adenine nucleotide,TAN)、能荷(energy charge,EC)值;逆转录聚合酶链式反应(Reverse Transcription-Polymerase Chain Reaction,RT-PCR)法测定GLUT3mRNA表达,western-blotting法测定脑组织磷酸化的单磷酸腺苷激活的蛋白激酶α1/2(AMP-activatedprotein kinase,p-AMPKα1/2)、葡萄糖转运蛋白3(glucose transporter 3,GLUT3)蛋白表达。结果 1、脑缺血再灌注后,脑组织ATP ADP、AMP含量及TAN、EC值显著降低,黄芪甲苷,人参皂苷Rg1、人参皂苷Rb1、三七皂苷R1可显著增加脑组织ATP、ADP、AMP含量,提高TAN值;黄芪甲苷+人参皂苷Rg1、黄芪甲苷+人参皂苷Rb1,黄芪甲苷+三七皂苷R1及四种有效成分配伍均可显著提高脑组织ATP、ADP、AMP含量及TAN、EC值,且两种有效成分配伍的效应强于各有效成分单用,四种有效成分配伍的效应强于各有效成分单用及大部分两有效成分配伍的效应。2、脑缺血再灌注后,脑组织p-AMPKα1/2蛋白表达显著升高,黄芪甲苷,三七皂苷R1,四种有效成分配伍组,黄芪甲苷+人参皂苷Rg1及黄芪甲苷+三七皂苷R1能显著增加p-AMPKα1/2蛋白表达;且四种有效成分配伍,黄芪甲苷与人参皂苷Rg1、三七皂苷R1配伍升高p-AMPKα1/2的效应分别高于各有效成分单用,四种有效成分配伍组的效应高于黄芪甲苷+人参皂苷Rb1组。3、脑缺血再灌注后,各药物均能提高脑组织GLUT3基因和蛋白表达,且黄芪甲苷与人参皂苷Rg1和三七皂苷R1两药或四种有效成分配伍增强GLUT3表达的效应大于各有效成分单用;四种有效成分配伍上调GLUT3 mRNA表达的效应大于黄芪甲苷+人参皂苷Rb1配伍,上调GLUT3蛋白表达的效应大于黄芪甲苷+人参皂苷Rb1及黄芪甲苷+三七皂苷R1。结论黄芪和三七的四种主要有效成分配伍或者黄芪甲苷与人参皂苷Rg1、三七皂苷R1合用对脑缺血再灌注后脑组织能量代谢的改善具有促进作用,其机制可能是黄芪和三七的主要有效成分配伍可上调脑组织AMPK磷酸化水平,增强脑组织GLUT3的表达,从而介导葡萄糖进入神经细胞,增加神经细胞的葡萄糖供应和摄取,改善了缺血后脑组织的能量代谢。
