动脉粥样硬化仍然是全球发病率和死亡率的主要原因,斑块内新生血管形成显著促进了动脉粥样硬化的进展和不稳定性.血管内皮生长因子A (VEGFA)通过诱导内皮细胞增殖和迁移,刺激斑块中异常微血管的生长.色素上皮衍生因子(PEDF)能够高效抑制VEGFA依赖的新生血管形成.本研究介绍了一种由泊洛沙姆407(F127)和海藻酸钠(SA)制备的温敏水凝胶(PFSgel),将PEDF局部递送至动脉粥样硬化斑块周围. PFSgel在体温(37°C)下表现出优异的液固转变,随后在SA与生理环境中的Ca2+凝胶化后形成稳定的三维网络结构,实现控制释放的效果.流变学分析证实其相变温度为28.7°C,并具有显著的自愈特性.体外实验显示, PFSgel可通过调节CD31和MMP-2/MMP-9信号通路,抑制VEGFA诱导的内皮细胞增殖和迁移.体内降解测试验证了PFSgel的控制释放性能.在Apoe缺陷的动脉粥样硬化小鼠中,超声引导的PFSgel注射到腹主动脉周围,可逐步在原位释放包封的PEDF,有效减少了斑块负担、新生血管形成和腔狭窄.总之,这一原位注射的PFSgel凝胶通过CD31和MMP-2/MMP-9通路,有效抑制病理性新生血管形成,通过干预VEGFA依赖的新生血管形成来稳定动脉粥样硬化中的高风险斑块.

The Mechanism of Cortico-Striato-Thalamo-Cortical Neurocircuitry in Response Inhibition and Emotional Responding in Attention Deficit Hyperactivity Disorder with Comorbid Disruptive Behavior Disorder

The neurocircuitries that constitute the corticostriato-thalamo-cortical（CSTC） circuit provide a framework for bridging gaps between neuroscience and executive function in attention deficit hyperactivity disorder（ADHD）, but it has been difficult to identify the mechanisms for regulating emotional problems from the understanding of ADHD comorbidity with disruptive behavior disorders（DBD）. Research based on ＂cool＇＇ and ＂hot＇＇executive functional theory and the dual pathway models,which are thought of as applied response inhibition and delay aversion, respectively, within the neuropsychological view of ADHD, has shed light on emotional responding before and after decontextualized stimuli, while CSTC circuit-related domains have been suggested to explain the different emotional symptoms of ADHD with or without comorbid DBD. This review discusses the role of abnormal connections in each CSTC circuit, especially in the emotion circuit, which may be responsible for targeted executive dysfunction at the neuroscience level. Thus, the two major domains – abstract thinking（cool） and emotional trait（hot） – trigger the mechanism of onset of ADHD.

Short-term effects of air pollution on acute myocardial infarctions in shanghai, China, 2013-2014

Background Although particulate matter, with diameters 〈 2.5 μm （PM2.5） and 〈 10 μm （PM10）, and other pollutants have been associated with cardiovascular morbidity and mortality, the effect of pollutants on acute myocardial infarctions （AMIs） has rarely been investigated in Asia, especially in shanghai, China. Methods Between 1 November 2013 and 27 April 2014, 972 patients from the Pudong District, shanghai City, were assessed by the Emergency Medical Service. A case-crossover design was used to analyze exposure to air pollution and the AMI risk. Exposures to PM2.5, PM10, nitrogen dioxide （NO2）, sulphurdioxide （SO2）, and carbon monoxide （CO） were based on the mean urban background levels. The associations among AMI admissions, the included pollutants, temperature, and relative humidity were analyzed using correlation and logistic regression. Results The urban background levels of PM2.5, PM10 and CO were associated with an increased risk of AMI, unlike NO2 and SO2 levels. The OR （95% CI） for AMI were 1.16 （1.03-1.29）, 1.05 （1.01-1.16）, 0.82 （0.75-1.02）, 0.87 （0.63-1.95）, and 1.08 （1.02-1.21） for PM2.5, PM10, NO2, SO2, and CO, respectively. Increases in the air quality index （AQI） were associated with more AMI occurrences. There was no correlation between fluctuations in temperature and relative humidity with AMI hospital admissions. Conclusions Short-term exposure to moderate-serious pollution levels is associated with increased risk of AMI. Increased PM2.5, PM10 and CO levels are related to increased AMI admissions.

Serum from asthmatic rat treated with acupuncture inhibits acetyl-choline-induced contractile responses of airway smooth muscle cells

OBJECTIVE: To investigate the influence of serum from asthmatic rat treated with acupuncture(acupuncture serum) on the contractile responses of airway smooth muscle cells(ASMCs).METHODS: OVA-induced asthmatic rats were subject to acupuncture treatment at the points of Dazhui(GV 14), Fengmen(BL 12), and Feishu(BL 13).The resulting serum was collected, as well as serum from normal animals. Contraction of ASMCs was induced with 10 μM acetylcholine(Ach), and acupuncture serum or normal serum was supplemented 15 min later. The effects of the sera on the changes in cell length, width, and surface areawere monitored in real time via a live cell imaging ***: The contraction rates of ASMCs 15 min and 30 min after Ach stimulation reached 38.2% ±3.8% and 49.7% ± 4.6%, both of which were significantly higher than unstimulated control cells(P <0.01). Acupuncture serum at the 1∶100 dosage reduced the contraction rate of ASMCs from 40.1% ±5.1% to 17.6% ± 6.5%(P < 0.01). Thus, the treatment significantly inhibited the Ach-induced contractile responses of ASMCs and yielded a relaxation efficiency of 58%, which was greater than the effects obtained with a 1∶250 dosage. Treatment with acupuncture serum at 1∶500 dosage or normal serum displayed no clear effect of suppressing Ach-induced contraction of ***: Our data suggest that acupuncture serum has the effect of inhibiting Ach-induced contraction of ASMCs, thereby promoting relaxation of the cells.

Wall shear stress can improve prediction accuracy for transient ischemic attack

BACKGROUND Early prediction of transient ischemic attack (TIA) has important clinical value. To date, systematic studies on clinical, biochemical, and imaging indicators related to carotid atherosclerosis have been carried out to predict the occurrence of TIA. However, their prediction accuracy is limited. AIM To explore the role of combining wall shear stress (WSS) with conventional predictive indicators in improving the accuracy of TIA prediction. METHODS A total of 250 patients with atherosclerosis who underwent carotid ultrasonography at Naval Military Medical University Affiliated Gongli Hospital were recruited. Plaque location, plaque properties, stenosis rate, peak systolic velocity, and end diastolic velocity were measured and recorded. The WSS distribution map of the proximal and distal ends of the plaque shoulder was drawn using the shear stress quantitative analysis software, and the average values of WSS were recorded. The laboratory indicators of the subjects were recorded. The patients were followed for 4 years. Patients with TIA were included in a TIA group and the remaining patients were included in a control group. The clinical data, laboratory indicators, and ultrasound characteristics of the two groups were analyzed. Survival curves were plotted by the Kaplan-Meier method. Receiver operating characteristic curves were established to evaluate the accuracy of potential indicators in predicting TIA. Logistic regression model was used to establish combined prediction, and the accuracy of combined predictive indicators for TIA was *** The intraclass correlation coefficients of the WSS between the proximal and distal ends of the plaque shoulder were 0.976 and 0.993, respectively, which indicated an excellent agreement. Long-term prognosis in patients continuing taking antithrombotics after peptic ulcer bleeding

AIM To investigate the long-term prognosis in peptic ulcer patients continuing taking antithrombotics after ulcer bleeding, and to determine the risk factors that influence the prognosis. METHODS All clinical data of peptic ulcer patients treated from January 1, 2009 to January 1, 2014 were retrospectively collected and analyzed. Patients were divided into either a continuing group to continue taking antithrombotic drugs after ulcer bleeding or a discontinuing group to discontinue antithrombotic drugs. The primary outcome of follow-up in peptic ulcer bleeding patients was recurrent bleeding, and secondary outcome was death or acute cardiovascular disease occurrence. The final date of follow-up was December 31, 2014. Basic demographic data, complications, and disease classifications were analyzed and compared by t- or χ2-test. The number of patients that achieved various outcomes was counted and analyzed statistically. A survival curve was drawn using the Kaplan-Meier method, and the differencewas compared using the log-rank test. COX regression multivariate analysis was applied to analyze risk factors for the prognosis of peptic ulcer patients. RESULTS A total of 167 patients were enrolled into this study. As for the baseline information, differences in age, smoking, alcohol abuse, and acute cardiovascular diseases were statistically significant between the continuing and discontinuing groups(70.8 ± 11.4 vs 62.4 ± 12.0, P < 0.001; 8(8.2%) vs 15(21.7%), P < 0.05; 65(66.3%) vs 13(18.8%), P < 0.001). At the end of the study, 18 patients had recurrent bleeding and three patients died or had acute cardiovascular disease in the continuing group, while four patients had recurrent bleeding and 15 patients died or had acute cardiovascular disease in the discontinuing group. The differences in these results were statistically significant(P = 0.022, P = 0.000). Clinical Presentation and Treatment Strategies for Ulcerative Colitis： A Retrospective Study of 247 Inpatients

Objective: Complementary and alternative medicine, particularly herbal therapy, is widely used by patients with ulcerative colitis(UC), but controlled data are limited. To describe the clinical presentation and treatment strategies for UC in inpatients from shanghai, China and to improve the therapeutic outcomes for patients with UC. Methods: Medical records from 247 patients with UC who were admitted to Longhua Hospital Affiliated to shanghai University of Traditional Chinese Medicine between January 2008 and June 2013 were analyzed for gender, age, course of the disease, clinical type, extent and severity of the disease, treatment strategies, and therapeutic outcomes. Results: Gender ratios and disease onset of inpatients with UC in the shanghai area were consistent with other reports in the literature. In contrast to previous studies, most patients exhibited disease of the left colon, over half of the patients had problems of the rectum or sigmoid colon, and most patients had either mild or moderate UC. Comparison of Sutherland Disease Actirity Index scores for patients treated with Chinese medicine(CM) and those treated with integrated CM and Western medicine revealed significant reductions in scores for both groups after treatment(P<0.01), with no significant difference in therapeutic effects between groups(P=0.938). Conclusions: Herbal medicine has been widely used in patients with mild to moderate disease and as adjunct therapy in patients with moderate to severe disease. Therefore, the strategy was proposed for the treatment of UC with CM therapy based on 2 steps according to the stage of the disease, even in the clinical setting.

Population-based survey of prevalence,causes,and risk factors for blindness and visual impairment in an aging Chinese metropolitan population

AIM： To assess the prevalence, causes, and risk factors for blindness and visual impairment among elderly （〉60 years of age） Chinese people in a metropolitan area of shanghai, China. METHODS： Random cluster sampling was conducted to identify participants among residents ≥60 years of age living in the Xietu Block, Xuhui District, shanghai, China. Presenting visual acuity （PVA） and best-corrected visual acuity （BCVA） were checked by the Early Treatment Diabetic Retinopathy Study （ETDRS） visual chart. All eligible participants underwent a comprehensive eye examination. Blindness and visual impairment were defined according to World health Organization （WHO） criteria. RESULTS： A total of 4190 persons （1688 men and 2502 women） participated in the study, and the response rate was 91.1%. Based on PVA, the prevalence of blindness was 1.1% and that of visual impairment was 7.6%. Based on BCVA, the prevalence of blindness and visual impairment decreased to 0.9% and 3.9%, respectively. Older （〉80 years of age） women, with low educational levels and smoking habits, exhibited a significantly greater chance for blindness and visual impairment than did those with high educational levels and no smoking habits （P〈0.05）. Based on PVA and BCVA, the main causes of blindness were cataract, myopic maculopathy, and age-related macular degeneration （AMD）. CONCLUSION： Our findings help to identify the population in need of intervention, to highlight the need for additional eye healthcare services in urban China.
