T-tube vs no T-tube for biliary tract reconstruction in adult orthotopic liver transplantation:An updated systematic review and metaanalysis

BACKGROUND Whether to use a T-tube for biliary anastomosis during orthotopic liver transplantation(OLT)remains a debatable *** surgeons chose to use a T-tube because they believed that it reduces the incidence of biliary *** in surgical techniques during the last decades have significantly decreased the overall incidence of postoperative biliary *** using a T-tube during OLT is still associated with the reduced incidence of biliary strictures needs to be *** To provide an updated systematic review and meta-analysis on using a T-tube during adult *** In the electronic databases MEDLINE,PubMed,Scopus,***,the Cochrane Library,the Cochrane Hepato-Biliary Group Controlled Trails Register,and the Cochrane Central Register of Controlled Trials,we identified 17 studies(eight randomized controlled trials and nine comparative studies)from January 1995 to October *** data of the studies before and after 2010 were separately *** chose the overall biliary complications,bile leaks or fistulas,biliary strictures(anastomotic or non-anastomotic),and cholangitis as *** ratios(ORs)with 95%confidence intervals(CIs)were calculated to describe the results of the ***,the test for overall effect(Z)was used to test the difference between OR and 1,where P≤0.05 indicated a significant difference between OR value and *** A total of 1053 subjects before 2010 and 1346 subjects after 2010 were included in this *** pooled results showed that using a T-tube reduced the incidence of postoperative biliary strictures in studies before 2010(P=0.012,OR=0.62,95%CI:0.42-0.90),while the same benefit was not seen in studies after 2010(P=0.60,OR=0.76,95%CI:0.27-2.12).No significant difference in the incidence of overall biliary complications(P=0.37,OR=1.41,95%CI:0.66-2.98),bile leaks(P=0.89,OR=1.04,95%CI:0.63-1.70),and cholangitis(P=0.27,OR=2.00,95%CI:0.59-6.84)was obse

A new energy-absorbing bolt used for large deformation control of tunnel surrounding rock

In order to control the large deformation of tunnel surrounding rock,a new energy-absorbing bolt is *** bolt can be transformed into a rigid support when the deformation of the surrounding rock reaches the length of the sleeve tube,thus preventing the surrounding rock from continuing to ***,this bolt has a simple structure and is easy to manufacture and *** the static tensile test is conducted on the bolt specimen to test its working *** test results show that when the cone angle of the cone block is small,the load–displacement curve of the bolt contains three stages;when the cone angle is large,the load–displacement curve contains only two ***,both the average constant resistance and the maximum absorbed energy increase linearly with the increase of cone *** this basis,ignoring the influence of shear stress,and it is supposed that the thickness of the sleeve tube is constant,then the theoretical calculation formula of constant resistance for the new bolt is derived,and the rationality of the formula is verified using the static tensile test *** is found that the error of the calculated result is less than 15%when the cone angle does not exceed 15.At last,the numerical simulation method is used to analyze the performance of the new *** simulation results indicate that the generation of shear stress and the change of tube thickness during the movement of the cone block are two important factors that cause theoretical errors.

Transcatheter aortic valve replacement(TAVR)has been performed for nearly 20years,with reliable safety and efficacy in moderate-to high-risk patients with aortic stenosis or regurgitation,with the advantage of less trauma and better prognosis than traditional open ***,because surgeons have not been able to obtain a full view of the aortic root,3-dimensional printing has been used to reconstruct the aortic root so that they could clearly and intui-tively understand the specific anatomical *** addition,the 3D printed model has been used for the in vitro simulation of the planned procedures to predict the potential complications of TAVR,the goal being to provide guidance to reasonably plan the procedure to achieve the best *** 3D printing can be used to understand the depth,shape,and distribution of the *** 3D printing has achieved remarkable results in TAVR and has a great potential.

Background:In clinical datasets,the characteristics of an individual patient vary so much that data loss becomes a normal event,which may be a unignorable dilemma in clinical data ***,the construction of a machine learning model aimed at missing clinical datasets(MCD)is of great clinical ***:All included patients were divided into two groups according to outcome within a period of up to 36 months or *** following characteristics(variables)were collected:age,sex,Child-Pugh status,hepatitis status,cirrhosis status,treatment,tumor size,portal vein tumor thrombus,and alpha fetoprotein(μg/mL),and a missing dataset-independent support vector machine(MDI-SVM)independent of missing data was built for the ***:A MCD-independent SVM was developed based on clinical data from 1334 patients with hepatocellular carcinoma(HCC)at a single center,which had an accuracy of 84.43%in the survival analysis in the presence of 5%missing *** on the different combinations of features,our model calculated five features(tumor size,age,treatment,hepatitis status,and alpha fetoprotein)that had the greatest impact on survival in patients with HCC and extracted their weighting ***:A MCD-independent SVM was developed to achieve prognosis prediction for patients with HCC in the absence of first-visit data.

Activated carbon supported palladium catalyst was prepared by excessive impregnation *** performance of catalyst was evaluat-ed by selective dechlorination of 2.3.6-richlopridine to *** inluence of palladium *** temperature and acid-binding agent on the eatalyst activity was *** showed that the palladium load of 0.5%6(w),under the condition of the reaction temperature 140℃,the cornverion of 2.3.6 trichloropyridine was 100%and the sletivrity of 2.3-dichlropridine was more than 71%.Adding ti-ethylamine in solvent methanol can efectively improve the target product selectivity,and under the optimal conditions,the Belectivity of 23-dichlopyridine can reach to 80%.
