Clinical evaluation of prone position ventilation in the treatment of acute respiratory distress syndrome induced by sepsis

BACKGROUND Acute respiratory distress syndrome(ARDS)is an acute,diffuse,inflammatory lung *** studies have shown prone position ventilation(PPV)to be associated with improvement in ***,its role in patients with ARDS caused by sepsis remains *** To analyze the clinical effects of PPV in patients with ARDS caused by *** One hundred and two patients with ARDS were identified and divided into a control group(n=55)and a PPV treatment group(n=47).Outcomes included oxygenation index,lung compliance(Cst)and platform pressure(Pplat),which were compared between the two groups after *** outcomes included heart rate(HR),mean arterial pressure(MAP),central venous pressure(CVP),left ventricular ejection fraction(LVEF),the length of mechanical ventilation time and intensive care unit(ICU)stay,and levels of C-reactive protein(CRP),procalcitonin(PCT),and interleukin-6(IL-6)after ***,mortality rate was also compared between the two *** On the first day after ventilation,the oxygenation index and Cst were higher and Pplat level was lower in the PPV group than in the conventional treatment group(P0.05).There were no significant differences in HR,MAP,CVP,LVEF,duration of mechanical ventilation and ICU stay,and the levels of CRP,PCT,and IL-6 between the two groups on the first day after ventilation(all P>0.05).The mortality rates on days 28 and 90 in the PPV and control groups were 12.77% and 29.09%,and 25.53% and 45.45%,respectively(P<0.05).CONCLUSION PPV may improve respiratory mechanics indices and may also have mortality benefit in patients with ARDS caused by ***,PPV was not shown to cause any adverse effects on hemodynamics and inflammation indices.

Quantitative comparisons between relaxation enhanced compressed sensing 3D MERGE and conventional 3D MERGE for vessel wall imaging in equal scan time: preliminary studies

In this study, we quantitatively compared relaxation enhanced compressed sensing(RECS-3D MERGE) with conventional 3D MERGE techniques on blood suppression efficiency, wall-lumen contrast and plaque burden measurement for carotid atherosclerotic imaging in equal scan time. Twelve patients were recruited in the study. RECS-3D MERGE and conventional 3D MERGE were implemented. 2D DIR-FSE was carried out as a reference standard. The lumen signal-to-tissue ratio(STR) was used as the quantitative measure of blood suppression efficiency. The contrast-to-tissue ratio(CTR) was used as the quantitative measure of wall-lumen contrast. Vessel lumen area(LA) and wall area(WA) were measured for morphological comparisons. The lumen STR of RECS-3D MERGE was significantly lower than that of 3D MERGE while the wall-lumen CTR of RECS-3D MERGE was significantly higher. There were no significant differences in plaque burden measurements between RECS-3D MERGE and 2D DIR-FSE. For comparison between conventional 3D MERGE and 2D DIR-FSE, there were no significant differences in LA measurement. However, the WA of 3D MERGE was significantly larger. The RECS-3D MERGE sequence achieved more sufficient blood suppression and higher image contrast without prolonging the scan time. These improvements lead to more accurate morphological measurements of carotid atherosclerotic imaging.

BACKGROUND Colorectal cancer is a common malignant tumor in China,and its incidence in the elderly is increasing *** bowel disease is a group of chronic non-specific intestinal inflammatory diseases,including ulcerative colitis and Crohn’s *** included the clinicopathological and follow-up data of patients with colorectal cancer who underwent laparoscopic colectomy or open colectomy at our Gastrointestinal Department between January 2019 and December *** indicators,oncological indicators,and survival rates were compared between the *** results of 104 patients who met the above criteria were extracted from the database(laparoscopic colectomy group=63,open colectomy group=41),and there were no statistically significant differences in the baseline data or follow-up time between the two *** Intraoperative blood loss,time to first ambulation,and time to first fluid intake were significantly lower in the laparoscopic colectomy group than in the open colectomy *** differences in overall mortality,tumor-related mortality,and recurrence rates between the two groups were not statistically significant,and survival analysis showed that the differences in the cumulative overall survival,tumor-related survival,and cumulative recurrence-free rates between the two groups were not statistically *** In elderly patients with colorectal cancer,laparoscopic colectomy has better short-term outcomes than open colectomy,and laparoscopic colectomy has superior long-term survival outcomes compared with open colectomy.

Dear editor,Diabetic ketoacidosis(DKA)is a common clinically severe disease,with an incidence of 24.9/1,000 among diabetes patients,and approximately 180,000 people in the United States suffer from DKA every year.[1]The reported mortality rate associated with DKA ranges from 0.65%to 3.30%,and is commonly caused by electrolyte imbalances,especially hypokalemia and hyperkalemia.[2]Reports indicated that hypokalemia was observed in 5.6%of patients with DKA before therapy,[3]and the overall prevalence of hypokalemia during treatment for DKA was 38%.[4]Several studies have shown that hypokalemia can lead to fatal dysrhythmia and even cardiac arrest,which are positively associated with increased mortality.

Efficacy evaluation of intravitreal ranibizumab therapy for three types of retinopathy of prematurity

AIM:To evaluate efficacy of intravitreal ranibizumab(IVR)therapy for aggressive posterior retinopathy of prematurity(ROP),threshold ROP disease and type 1 pre-threshold ***:A retrospective analysis was performed on 40 patients(76 eyes)who had IVR as the primary treatment for ROP from April 2017 to January *** to disease pathogenic features,the 76 eyes were divided into three groups:aggressive posterior ROP(AP-ROP)group(16 eyes),threshold ROP group(28 eyes)and type 1 pre-threshold ROP group(32 eyes).The characteristics of patients and lesions situation before the first intravitreal injection,and posttreatment fundus outcomes determined by wide-angle Ret Cam fundus imaging were ***:The birth weight and postmenstrual age of first IVR treatment in AP-ROP,threshold ROP,and type 1 pre-threshold ROP groups were significant difference(1087.50±246.78,1103.75±168.30,1257.03±210.82 g,P=0.005;34.50±1.46,36.89±2.97,36.50±2.36 wk,P=0.008),while the gestational age was not difference(28.00±2.00,28.54±1.90,28.59±1.43 wk,P=0.510).The retina hemorrhage ratio(with/without:14/2,8/20,5/27),iris neovascularization or vascular engorgement ratio(with/without:12/4,11/17,6/26),and the zone I(inside/outside:16/0,2/26,5/27)in AP-ROP,threshold ROP,and type 1 pre-threshold ROP group were difference significantly(all P<0.05).The regression rates were 37.5%,92.86%,and 100%,and the recurrence rates were 62.5%,7.14%,and 0 in AP-ROP,threshold ROP,and type 1 pre-threshold ROP group,respectively(both P<0.05).The recurrence eyes were cured by secondary IVR or retinal laser ***:IVR is an effective treatment for all types of *** regression of AP-ROP is significantly lower than type 1 pre-threshold and threshold *** weight,retinal hemorrhage,iris neovascularization or vascular engorgement and lesions located in zone I may be associated with AP-ROP recurrence and retreatment,which should be noted in follow-up.

AIM:To study retinal microvascular and microstructural alterations in meibomian gland dysfunction(MGD)in severely obese population using optical coherence tomography angiography(OCTA).METHODS:Twelve MGD patients with severely obese population(PAT group;24 eyes)and 12 healthy controls(HC group;24 eyes)were *** images were segmented into five[superior(S),nasal(N),inferior(I),temporal(T),and central foveal(C)]or nine[inner superior(IS),outer superior(OS),inner nasal(IN),outer nasal(ON),inner inferior(II),outer inferior(OI),inner temporal(IT),outer temporal(OT),and C]*** superficial vessel density(SVD),retinal thickness(RT),foveal avascular zone(FAZ)parameters,and retinal volume were ***:Visual acuity was significantly different between two groups(0.8±0.17 in PAT group vs 0.2±0.06 in HC group).SVD was significantly lower in PATs in N,T,OS,IN,OT,and *** area under the receiver operating characteristic curve(AUC)for T was 0.961[95% confidence interval(CI):0.908 to 1.000],for OS was 0.962(95%CI:0.915 to 1.000).RT was significantly lower in PATs in IS,OS,OI,OT,ON,IT,IN,and *** for OT was 0.935(95%CI:0.870 to 0.999),for IS was 0.915(95%CI:0.838 to 0.992).Angiography results showed significantly lower area and perimeter of FAZ,SVD of the inner retina and both retinal volume and the average volume thickness in the PAT ***:Vision may be affected in patients with MGD due to changes in retinal microvessels and *** changes detected by OCTA may be a potential marker for diagnosing MGD in severe obesity.
