BACKGROUND Fracture nonunion represents a challenging complication during fracture repair,often necessitating surgical ***,a recombinant human parathyroid hormone,has demonstrated promise in enhancing fracture healing,although its efficacy in treating established nonunion remains under *** SUMMARY We report a case of a 27-year-old male who presented with a right humerus fracture following a traffic *** undergoing open reduction and internal fixation,the fracture resulted in a delayed union and subsequent *** 4 years of conservative management,teriparatide treatment was initiated due to persistent *** injections were administered daily for 6 months,resulting in complete fracture healing and resolution of *** Our case demonstrates the successful use of teriparatide in treating a prolonged nonunion of a humerus *** may provide a valuable therapeutic option for established bone nonunion,even in cases that have not responded to conservative treatments.

本文综述了不同类型的T淋巴细胞亚群在心脏纤维化重构中的作用。辅助性T细胞17 (T helper cell 17, Th17)参与促进心脏纤维化重构的发展，而调节性T细胞(regulatory T cell, Treg)作为负调节因子发挥免疫抑制功能，这归因于其分泌白介素-10 (interleukin-10, IL-10)和功能表型。Th1和Th2细胞参与病理性心脏纤维化重构中炎症反应的不同阶段，这两类细胞的作用因不同心脏疾病的病理机制而异。此外，CD8+T细胞调节巨噬细胞的活化和极化，促进颗粒酶B的分泌，诱导心肌细胞凋亡，加重心肌梗死后的心脏纤维化。考虑到细胞因子调节在心力衰竭临床治疗中的局限性，靶向T细胞共刺激分子成为治疗病理性心脏重塑的一种有前景的策略。未来的研究将探索嵌合抗原受体修饰T细胞(chimeric antigen receptor modified T cells, CAR-T cells)技术和靶向调节Treg细胞数量与表型，二者具有成为心脏病有效疗法的潜力。

BACKGROUND Bone is a major site of metastasis in nasopharyngeal carcinoma(NPC).Recently,nuclear factor kappa-beta ligand(RANKL)inhibitors have garnered attention for their ability to inhibit osteoclast formation and bone resorption,as well as their potential to modulate immune functions and thereby enhance the efficacy of programmed cell death protein 1(PD-1)inhibitor *** SUMMARY We present a case of a patient with NPC who developed sternal stalk metastasis and multiple bone metastases with soft tissue invasion following radical chemoradiotherapy and targeted *** to chemotherapy,the patient experienced severe bone marrow suppression and opted out of further chemotherapy ***,the patient received combination therapy,including RANKL inhibitors(denosumab)alongside PD-1,radiotherapy,and granulocyte-macrophage colonystimulating factor(PRaG)therapy(NCT05435768),and achieved 16 months of progression-free survival and more than 35 months of overall survival,without encountering any grade 2 or higher treatment-related adverse *** Denosumab combined with PRaG therapy could be a new therapeutic approach for the second-line treatment in patients with bone metastases.

A new method for parameter optimization of pharmacokinetics based on an artificial immune network named PKAIN is proposed. To improve local searching ability of the artificial immune network, a partition-based concurrent simplex mutation is developed. By means of evolution of network cells in the PKAIN artificial immune network, an optimal set of parameters of a given pharmacokinetic model is obtained. The Laplace transform is applied to the pharmacokinetic differential equations of remifentanil and its major metabolite, remifentanil acid. The PKAIN method is used to optimize parameters of the derived compartment models. Experimental results show that the twocompartment model is sufficient for the pharmacokinetic study of remifentanil acid for patients with mild degree of renal impairment.

Background Available evidence suggests that our country bear great burden of severe ***,the causes have not been explored recently in different regions of China to guide necessary clinical and public health *** This was a prospective,observational study conducted from March 1,2018,to February 28,*** hospitals in three regions of China participated in the *** from infants with a gestational age≥35 weeks,birth weight>2000 g,and total serum bilirubin(TSB)level≥17 mg/dL(342μmol/L)were prospectively *** A total of 783 cases were *** were identified in 259 *** major causes were ABO incompatibility(n=101),glucose-6-phosphate dehydrogenase deficiency(n=76),and intracranial hemorrhage(n=70).All infants with glucose-6-phosphate dehydrogenase deficiency were from the central south *** from the central south region had much higher peak total bilirubin levels[mean,404μmol/L;standard deviation(SD),75μmol/L]than those from the other regions(mean,373μmol/L;SD,35μmol/L)(P<0.001).Conclusions ABO incompatibility was the leading cause in the east and northwest regions,but cases in the central south region were mainly caused by both ABO incompatibility and glucose-6-phosphate dehydrogenase deficiency,and infants in this region had a much higher peak total bilirubin *** hemorrhage may be another common *** thorough assessments and rigorous bilirubin follow-up strategies are needed in the central south region.
