Cost-effectiveness analysis of tele-retinopathy of prematurity screening in Iran

AIM:To conduct a cost-utility analysis of the teleretinopathy of prematurity(ROP)screening program against no ***:A decision tree model was developed to identify and treat the infants with threshold ROP through the tele-screening system compared with no screening program from the societal *** used the quality adjusted life years(QALY)index to measure the scenarios’effectiveness,which was discounted for the future years by *** hundred twenty-six randomly selected newborns with ROP required treatment were investigated to extract the treatment *** considered the direct medical and non-medical costs in cost calculations analysed by the bottom-up *** figures of the model’s inputs were calculated using the Monte Carlo simulation that generated 1000 random iteration of the data,and a one-way sensitivity analysis was performed on the model to cope with the potential ***:The total and per capita needed the budget to establish a tele-ROP screening system were estimated at over 1.5 million and 35.13 USD,*** total cost of identifying and treating an ROP case in tele-screening and no screening strategies were obtained as 108.72 and 63.52 USD,respectively,and their lifetime discounted QALY gained were calculated as 15.39 and 15.11,***,incremental cost-effectiveness ratio(ICER)of tele-screening strategy against the competitive strategy was achieved as 161.43 ***:Tele-ROP screening program is one of the most cost-effective interventions in the Iranian health system and has a high priority to receive a budget for implementation.

Objective This study aimed to investigate the effects of Montanide ISA-720 and Naloxone(NLX)in Hepatitis B surface antigen(HBsAg)vaccine formulation on cytokine and long-lasting antibody *** First,the HBsAg was formulated in Montanide ISA-720 adjuvant and Naloxone at 5 and 10mg/*** experimental mice were immunized three times at a 2-week interval,and then IL-4,IL-2,TNF-α,and IFN-γcytokines;long-lasting IgG antibody responses 220 days after the last shot;and IgG1/IgG2a isotypes were assessed by *** The HBsAg-Alum group exhibited the highest IL-4 cytokine response among the experimental groups,whereas NLX in HBsAg-MON720 vaccine formulation did not affect cytokine *** addition,NLX in Alum-based vaccine suppressed IL-4 cytokine response and increased the IL-2/IL-4 cytokine ***,HBsAg-MON720 was more potent than HBsAg-Alum in the induction of antibody responses,and NLX in Alum-and MON720-based vaccines induced long-lasting antibody *** NLX in Alum-based vaccine decreased IL-4 cytokine response,increased IL-2/IL-4 cytokine ratio,and improved long-lasting humoral immune responses in both vaccine ***,the adjuvant activity of NLX in the vaccine formulation depends on the type of adjuvant and the nature of the antigen in the vaccine formulation.

Spatial outline of malaria transmission in Iran

Objective:To conduct for modeling spatial distribution of malaria transmission in Iran. Methods:Records of all malaria cases from the period 2008-2010 in Iran were retrieved for malaria control ***&*** data including annual rainfall,maximum and minimum temperature,relative humidity,altitude,demographic,districts border shapefiles. and NDVI images received from Iranian Climalologic Research *** arranged in ArcCIS. Results:Results:99.65%of malaria transmission cases were focused in southeast part of Iran. These transmissions had statistically correlation with altitude(650 m),maximum(30℃),minimum (20℃) and average temperature(25.3℃).Statistical correlation and overall relationship between NDVI(118.81),relative humidity(≥45%) and rainfall in southeast area was defined and explained in this ***:According to ecological condition and mentioned cut-off points, predictive map was generated using rokriging method.

Age-specific reference ranges of prostatespecific antigen in the elderly of Amirkola: A population-based study

Objective:To determine the age-specific reference ranges of prostate-specific antigen(PSA)in the older men in the city of ***:This cross-sectional study is a part of Amirkola health and Ageing Project(AHAP)which has been conducted as a cohort study since 2011 in Amirkola,a city in northern *** demographic information of all men aged 60 and older were collected through questionnaires and interviews and the PSA measurements were performed using ELISA and Diametra *** acquired data were analyzed ***:A number of 837 elderly men with a mean age of 69.99±7.72 years participated in this *** serum PSA level(95th percentile)was determined to be 0.9(0-4.89)ng/mL in the age group of 60-64 years,1.1(0-4.88)ng/mL in the age group of 65-69 years,0.93(0-9.01)ng/mL in the age group of 70-74 years,1.3(0-7.95)ng/mL in the age group of 75-79 years,1.9(0-11.98 ng/mL)in the age group of 80-84 years,and 1.45(0-33.17)ng/mL in the 85 and older *** serum PSA level was significantly correlated with age(p=0.000).Conclusion:This study indicated that there is a direct correlation between the age and serum PSA *** use of age-specific reference range could guide clinicians on the incidence of prostate cancer in this population and perhaps reduce the number of unnecessary tests in this population group.

Provincial clustering of malaria in Iran between 2005 and 2014

Objective: To reveal the provincial clustering of malaria in Iran between 2005 and 2014 based on the epidemiologic factors and the climatic indicators affecting the disease. Methods: This was a descriptive-analytical study using malaria and meteorological data from the Malaria Elimination Programme of the Ministry of health and Medical Education and National Meteorological Organization. After standardization, the aggregate data was used to produce 10-year means for each province. The data analysis included grouping the provinces with respect to factors using hierarchical clustering method and Kruskal-Wallis test to examine the difference between clusters using SPSS ver.23. Results: The hierarchical clustering stratified the provinces' in 5 clusters. Kruskal-Wallis H test revealed a significant difference in the incidence rate per 100000 population (P=0.001), male gender (P=0.001), Iranian nationality (P=0.001), Afghan nationality (P=0.003), Pakistani nationality (P=0.001), urban residence (P=0.006), rural residence (P=0.004), autochthonous cases (P=0.007), average minimum temperature (P=0.001), average maximum temperature (P=0.007), average relative humidity (P=0.011), average pressure level (P=0.038), prevailing wind direction (P=0.023), average wind speed (P=0.031) and average precipitation sum (P=0.002) among the clusters. Conclusions: The results of this study and stratification of the provinces could help health policy makers to better manage malaria by allocating resources accordingly.

Strengthening Injury Surveillance System in Iran

Objective： To strengthen the current Injury Surveillance System （IS System） in order to better monitor injury conditions, improve protection ways and promote safety. Methods： At first we carried out a study to evaluate the frameworks of IS System in the developed countries. Then all the available documents from World health Organization, Eastern Mediterranean Regional Organization, as well as Minister of health and Medical Education concerning Iran were reviewed. Later a national stakeholder＇s consultation was held to collect opinions and views. A national workshop was also intended for provincial representatives from 41 universities to identify the barriers and limitations of the existing program and further to strengthen injury surveillance. Results： The evaluation of the current IS System revealed many problems, mainly presented as lack of accurate pre- and post-hospital death registry, need of precise injury data registry in outpatient medical centers, incomplete injury data registry in hospitals and lack of accuracy in definition of variables in injury registry. The five main characteristics of current IS System including flexibility, acceptability, simplicity, usefulness and timeliness were evaluated as moderate by experts.Conclusions： Major revisions must be considered in the current IS System in Iran. The following elements should be added to the questionnaire： identifier, manner of arrival to the hospital, situation of the injured patient, consumption of alcohol and opioids, other involved participants in the accident, intention, severity and site of injury, side effects of surgery and medication, as well as one month follow-up results. Data should be collected from 10% of all hospitals in Iran and analyzed every 3 months. Simultaneously data should be online to be retrieved by researches.
