Aim:Although vascularized lymph node transplantation(VLNT)has gained recognition as an effective treatment option for lymphedema,no consensus on the timing of transplant with other lymphatic procedures has been *** aim of this study is to describe our institutional experience with VLNT,including our staged approach and report postoperative ***:A retrospective review of patients who underwent VLNT for upper extremity lymphedema from May 2017 to April 2022 was *** were divided into fat-or fluid-dominant phenotypes based on preoperative *** with a minimum of 12-month follow-up were *** were reviewed for demographic,intraoperative,and surveillance ***:Twenty-three patients underwent VLNT of the upper extremity during the study period,of which eighteen met the study *** patients had fluid-dominant disease and nine patients had fat-dominant disease and had undergone prior debulking at our ***-dominant patients demonstrated slight reductions in limb volume and hours in compression,and improvement in quality-of-life scores at twelve ***-dominant patients who underwent prior debulking had a slight increase in limb volume without a change in hours of compression,and demonstrated improvements in quality-of-life scores in nearly all ***,17% of patients discontinued compression therapy *** in extremity edema was present in 83% of postoperative ***:VLNT had varying effects on limb measurements while reliably improving quality-of-life and allowing for the potential of discontinuing *** a staged approach wherein debulking is performed upfront may be particularly beneficial for patients with fat-dominant disease.

Engineering complex systems applied to risk management in the mining industry

Related to complexity, there is a wide diversity of concepts, ranging from ‘‘systemic" to ‘‘complex", implying a need for a unified terminology. Per different authors, the main drivers of complexity can be found in human behaviour and uncertainty. This complexity, structural or dynamic can be organizational, technological, or nested in their relationship. ISO international standard 31000:2009 definition of risk management ‘‘coordinated activities to direct and control an organization with regard to risk", when applied to economic sectors, industry, services, project, or activity, it requires the use of models or theories as guidelines. Therefore, as its basic elements comprehend human behaviour and/or uncertainty, risk management to be effective and adapted as much as possible to reality, must be operational within complex systems, as already demonstrated in different R&D environments. Risk management faces demanding challenges when approaching specific and endogenous needs, such as the mining sector. This paper presents a multivariable function analysis methodology approach based on complex system modelling and through real data corresponding to a risk management tool in the mining sector.

Sustainable development and continued prosperity of humanity hinge on the availability of renewable energy sources on a terawatts scale. In the long run, solar energy is the only source that can meet this daunting demand. Widespread utilization of solar energy faces challenges as a result of its diffusive （hence low energy density） and intermittent nature. How to effectively harvest, concentrate, store and redistribute solar energy constitutes a fundamental challenge that the scientific community needs to address. Photoelectrochemical （PEC） water splitting is a process that can directly convert solar energy into chemical energy and store it in chemical bonds, by producing hydrogen as a clean fuel source. It has received significant research attention lately. Here we provide a concise review of the key issues encountered in carrying out PEC water splitting. Our focus is on the balance of considerations such as stability, earth abundance, and efficiency. Particular attention is paid to the combination of photoelectrodes with electrocatalysts, especially on the interfaces between different components.

Alarm cue induces an antipredator morphological defense in juvenile Nicaragua cichlids Hypsophrys nicaraguensis

Olfactory cues that indicate predation risk elicit a number of defensive behaviors in fishes, but whether they are sufficient to also induce morphological defenses has received little attention. Cichlids are characterized by a high level of morphological plasticity during development, and the few species that have been tested do exhibit defensive behaviors when exposed to alarm cues released from the damaged skin of conspecifics. We utilized young juvenile Nicaragua cichlids Hypsophrys nicaraguensis to test if the perception of predation risk from alarm cue （conspecific skin extract） alone induces an increased relative body depth which is a defense against gape-limited predators. After two weeks of exposure, siblings that were exposed to conspecific alarm cue increased their relative body depth nearly double the amount of those exposed to distilled water （control） and zebrafish Danio rerio alarm cue. We repeated our measurements over the last two weeks （12 and 14） of cue exposure when the fish were late-stage juveniles to test if the rate of increase was sustained; there were no differences in final dimensions between the three treatments. Our results show that 1） the Nicaragua cichlid has an innate response to conspecific alarm cue which is not a generalized response to an injured fish, and 2） this innate recognition ultimately results in developing a deeper body at a stage of the life history where predation risk is high [Current Zoology 56 （1）： 36-42, 2010].

Endoscopic ultrasound-fine needle biopsies of pancreatic lesions: Prospective study of histology quality using Franseen needle

BACKGROUND The introduction of fine needle biopsies(FNB)to clinical practice presents a changing trend towards histology in the endoscopic ultrasound-guided tissue acquisition(EUS-TA).AIM To evaluate the clinical performance of a new FNB needle,the 22-gauge(22G)Franseen needle,when sampling pancreatic solid *** Consecutive patients with an indication for EUS-TA for the assessment of pancreatic solid lesions were included in this prospective,single-center,singlearm *** patient underwent a puncture of the lesion two times using the 22G Franseen needle and the obtained samples were directly placed into formalin for histological *** primary study endpoint was the rate of high-quality obtained *** endpoints included the length and diameter of the core specimen,the diagnostic accuracy and the complication *** From June 2017 to December 2018,forty patients with pancreatic solid lesions(22 females;mean age 67.2 years)were *** acquisition was achieved in all ***-quality histology,rated with Payne score 3,was obtained in 37/40 cases(92.5%)after two needle *** mean size of the acquired histological core tissue was 1.54 mm×0.39 *** diagnostic accuracy for the correct diagnosis was 85%(34/40).Only one adverse event was occurred,consisting of a self-limiting bleeding in the puncture *** The 22G Franseen needle achieved according to our standardized protocol a high rate of histological core procurement,and a high diagnostic accuracy,with one minor adverse event reported.

Systematic review and meta-analysis of colon cleansing preparations in patients with inflammatory bowel disease

AIM To performed a systematic review and meta-analysis to determine any possible differences in terms of effectiveness, safety and tolerability between existing colon-cleansing products in patients with inflammatory bowel *** Systematic searches were performed( January 1980-September 2016) using MEDLINE, EMBASE, Scopus, CENTRAL and ISI Web of knowledge for randomized trials assessing preparations with or without adjuvants, given in split and non-split dosing, and in high(> 3 L) or low-volume(2 L or less) regimens. Bowel cleansing quality was the primary outcome. Secondary outcomes included patient willingness-torepeat the procedure and side effects/*** Out of 439 citations, 4 trials fulfilled our inclusion criteria(n = 449 patients). One trial assessed the impact of adding simethicone to polyethylene glycol(PEG) 4 L with no effect on bowel cleansing quality, but a better tolerance. Another trial compared senna to castor oil, again without any differences in term of bowel cleansing. Two trials compared the efficacy of PEG high-volume vs PEG low-volume associated to an adjuvant in split-dose regimens: PEG low-dose efficacy was not different to PEG high-dose; OR = 0.84(0.37-1.92). A higher proportion of patients were willing to repeat low-volume preparations vs high-volume; OR = 5.11(1.31-20.0). CONCLUSION In inflammatory bowel disease population, PEG lowvolume regimen seems not inferior to PEG high-volume to clean the colon, and yields improved willingness-torepeat. Further additional research is urgently required to compare contemporary products in this population.

New horizons in the endoscopic ultrasonography-based diagnosis of pancreatic cystic lesions

Pancreatic cystic lesions(PCLs) are increasingly being identified because of the widespread use of highresolution abdominal imaging. These cysts encompass a spectrum from malignant disease to benign lesions, and therefore, accurate diagnosis is crucial to determine the best management strategy, either surgical resection or surveillance. However, the current standard of diagnosis is not accurate enough due to limitations of imaging and tissue sampling techniques, which entail the risk of unnecessary burdensome surgery for benign lesions or missed opportunities of prophylactic surgery for potentially malignant PCLs. In the last decade, endoscopic innovations based on endoscopic ultrasonography(EUS) imaging have emerged, aiming to overcome the present limitations. These new EUS-based technologies are contrast harmonic EUS, needle-based confocal endomicroscopy, through-the-needle cystoscopy and through-the needle intracystic biopsy. Here, we present a comprehensive and critical review of these emerging endoscopic tools for the diagnosis of PCLs, with a special emphasis on feasibility, safety and diagnostic performance.

Enhanced thermal transport across monolayer MoS2

Characterizing the thermal properties of MoS2 is important for the design of electronic devices based on this material. We used frequency domain thermoreflectance to study the cross-plane thermal transport in mechanically exfoliated MoS2 samples supported on SiO2 and muscovite mica substrates. The thickness of MoS2 ranged between one and five layers, and the MoS2 layers were sandwiched between a metal layer and the substrate. In the case of mica, heat transport into the substrate remained the same whether or not a monolayer of MoS2 was present, whereas, for SiO2, heat transport was reduced by surface roughness. We observed a significant improvement in heat transport across monolayer MoS2 as compared to few layer MoS2. For MoS2 on SiO2, the effective thermal interface conductance was improved by more than three times if a monolayer was used. For MoS2 on mica, the thermal interface conductance was approximately two times better for monolayer MoS2. This implies that monolayer MoS2 has superior thermal properties and can be used in electronic devices as an intermediate layer between two materials. Additionally, we also report on the measurement of anisotropic thermal conductivity in bulk MoS2 and mica.
