Dear Editor:Brain metastasis(BM)has long been recognized as a prognostic factor associated with poor prognosis for non-small cell lung cancer(NSCLC)in the era of con-ventional chemotherapy and targeted therapy[1].In the era of immunotherapy,controversial findings have been reported regarding the prognostic significance of BM in patients with NSCLC treated with programmed death-1/programmed death-ligand 1(PD-1/PD-L1)inhibitors.

OBJECTIVE:To investigate the impact of Yemazhui(Herba Eupatorii Lindleyani,HEL)against lipopolysaccharide(LPS)-induced acute lung injury(ALI)and explore its underlying mechanism in ***:The chemical constituents of HEL were analyzed by ultra-high performance liquid chromatographyquadrupole time-of-flight mass spectrometry ***,HEL was found to suppress LPS-induced ALI in ***-week-old male Sprague-Dawley rats were randomly divided into 6 groups:control,LPS,Dexamethasone(Dex),HEL low dose 6 g/kg(HEL-L),HEL medium dose 18 g/kg(HEL-M)and HEL high dose 54 g/kg(HEL-H)*** model rats were intratracheally injected with 3 mg/kg LPS to establish an ALI *** counts,lung wet/dry weight ratio,as well as myeloperoxidase(MPO)activity were determined followed by the detection with hematoxylin and eosin staining,enzyme linked immunosorbent assay,quantitative real time polymerase chain reaction,western blotting,immunohistochemistry,and ***,to explore the effect of HEL on ALI-mediated intestinal flora,we performed 16s rRNA sequencing analysis of intestinal ***:HEL attenuated LPS-induced inflammation in lung tissue and intestinal flora *** study indicated that HEL suppressed the lung coefficient and wet/dry weight ratio of LPS-induced ALI in rats,inhibited leukocytes exudation and MPO activity,and improved the pathological injury of lung *** addition,HEL reduced the expression of tumor necrosis factoralpha,interleukin-1beta(IL-1β)and interleukin-6(IL-6)in bronchoalveolar lavage fluid and serum,and inhibited nuclear displacement of nuclear factor kappa-B p65(NF-κBp65).And 18 g/kg HEL also reduced the expression levels of toll-like receptor 4(TLR4),myeloid differentiation factor 88,NF-κBp65,phosphorylated inhibitor kappa B alpha(phospho-IκBα),nod-like receptor family pyrin domain-containing 3 protein(NLRP3),IL-1β,and interleukin-18(IL-18)in lung tissue,and regulated intestinal flora disturban

Gene fusions and MET alterations are rare and difficult to detect in plasma *** clinical detection efficacy of molecular residual disease(MRD)based on circulating tumor DNA(ctDNA)in patients with non-small cell lung cancer(NSCLC)with these mutations remains *** prospective,non-intervention study recruited 49 patients with operable NSCLC with actionable gene fusions(ALK,ROS1,RET,and FGFR1),MET exon 14 skipping or de novo MET *** analyzed 43 tumor tissues and 111 serial perioperative plasma samples using 1021-and 338-gene panels,*** MRD correlated with a significantly higher recurrence rate(P<0.001),yielding positive predictive values of 100%and 90.9%,and negative predictive values of 82.4%and 86.4%at landmark and longitudinal time points,*** with detectable MRD showed reduced disease-free survival(DFS)compared to those with undetectable MRD(P<0.001).Patients who harbored tissue-derived fusion/MET alterations in their MRD had reduced DFS compared to those who did not(P=0.05).To our knowledge,this is the first comprehensive study on ctDNA-MRD clinical detection efficacy in operable NSCLC patients with gene fusions and MET *** with detectable tissue-derived fusion/MET alterations in postoperative MRD had worse clinical outcomes.

OBJECTIVE:To evaluate the efficacy of Qidong Huoxue decoction(芪冬活血饮,QDHX)in treating acute lung injury and acute respiratory distress syndrome(ALI/ARDS)when used as an adjunctive ***:ALI/ARDS patients admitted to our medical intensive care unit were randomly allocated to the control group or the QDHX group and received standard *** QDHX group received QDHX(50 mL per day for 14 d)orally or via a gastric *** primary outcome was measured according to Traditional Chinese Medicine(TCM)syndrome scores,with partial pressure of oxygen/fraction of inspired oxygen(PaO_(2)/FiO_(2))levels as the secondary ***:A total of 73 patients completed the study(36 in the TCM and 37 in the conventional group),and their records were *** 14-d treatment,the TCM group showed a significant decrease in TCM syndrome scores(P<0.05)and increased PaO_(2)/FiO_(2) levels(P<0.05).The therapeutic effect of integrated Chinese and western medicine was more significant than that of Western Medicine *** serious side effects were ***:Our study results show that QDHX in combination with conventional drug therapy can significantly reduce some clinical symptoms in patients with ALI/ARDS.

OBJECTIVE: To assess clinical value of modified Shenling Baizhu powder( 加味参苓白术散, SBP) in intervening targeted therapy-induced diarrhea. METHODS: This study was a prospective randomized controlled study. Eighty-five non-small cell lung cancer(NSCLC) patients with diarrhea who took targeted drugs were randomly divided into two groups. The experimental group received modified SBP, while the control group received imodium. During 2 courses of treatment(1 week/course) and 2 weeks of follow-up, we observed remission and recurrence of diarrhea, as well as the improvement of Karnofsky score(KPS) in the two groups and drug safety. RESULTS: Eighty cases were completed, with 40 cases in the experimental group and 40 cases in the control group. The control group's diarrhea remission rate was significantly lower than the experimental group's(P 0.05). No unique adverse events occurred in experimental group or control group. CONCLUSION: The modified SBP could improve targeted therapy-induced diarrhea in NSCLC, and is superior to imodium in relieving diarrhea, improving related symptom scores and symptoms, with no obvious drug-related adverse events.

Objective:This study aims to identify the factors influencing moderate to severe malnutrition in elderly lung cancer patients undergoing chemotherapy and to provide a basis for developing clinical prevention and intervention ***:The clinical data of 150 elderly lung cancer patients who received chemotherapy were retrospectively *** data,clinical characteristics,laboratory indicators,treatment plans,and other relevant information were *** risk factors for moderate to severe malnutrition during chemotherapy were ***:During chemotherapy,50 patients(33.33%)developed moderate to severe *** BMI of patients with moderate to severe malnutrition was significantly lower than that of patients with no or mild malnutrition(21.20±1.60 vs.26.14±2.31,P lung cancer patients during *** clinical attention should be given to these patients,with early intervention measures such as nutritional support to reduce the incidence of malnutrition and improve patients’quality of life and prognosis.

Idiopathic pulmonary fibrosis(IPF)is characterized by accumulation of myofibroblasts(MYFs)and extracellular matrix components,which leads to severe distortion and scarring of the gas exchange units of the lung,the alveoli,and ultimately respiratory ***-associated MYFs are therefore widely regarded as the culprits that compromise the architectural makeup of the lung in fibrotic *** the past decade,the cellular source of MYFs has been intensely *** rationale for such studies is that identifying the origin of these cells might help identify novel therapeutic targets and candidates to treat IPF *** advances in basic and translational research employing lineage tracing and multi-omics approaches have helped address the identity of MYF precursors,highlight the underlying heterogeneity,and to a less extent investigate MYF fate during fibrosis *** this review,we discuss the current understanding of such important aspects of MYF biology as well as recent developments in the treatment of IPF.
