BACKGROUND Hemorrhoidal disease(HD)is considered a low-severity pathology by both general population and physicians,but the lengthy conservative therapy and postoperative complications suggest *** To assess the effectiveness of different treatment options,both conservative and surgical,in contrast with some preexisting *** We conducted a retrospective,10-yearlong study between January 2011 and December 2021 in two surgical centers,a private and a state-owned *** compared the efficacy and safety of several treatment options,such as open hemorrhoidectomy,stapled hemorrhoidopexy,rubber band ligation and infrared coagulation in terms of complication rates and types and their correlation with different preexisting comorbidities such as inflammatory bowel disease(IBD),use of anticoagulant medication(AM)and liver *** also conducted a 20-years long PubMed research(1.263 articles)for relevant *** Our study recorded 10940 patients with HD,10241 with conservative and 699 with surgical *** of these,the male-to-female ratio of 1.3,and a peak in age distribution between 59 and 68 years old(32%of patients).For the entire study,we recorded a 90%incidence of immediate pain,immediate bleeding in 1.5%(11 cases),delayed bleeding in 1.0%(7 cases),and 0.6%surgical site *** retention was also present,with 0.2%of patients,anal stricture in 1%and fecal incontinence for 0.5%of patients(4 cases).We recorded no severe complications such as Fournier`s gangrene or rectovaginal *** accounted for 6%of the patients,with ulcerative colitis in 12%and Chron`s disease in 10.5%.6.6%of the patients had AM,determining 4%immediate and 2%delayed bleeding,in surgically treated *** Our study determined that most common complications(pain,urinary retention,bleeding,and stricture)are correlated with each surgical technique and pre-existing comorbidities.

Influence of heat flux and Reynolds number on the entropy generation for different types of nanofluids in a hexagon microchannel heat sink

Based on the Second Law of Thermodynamics, the entropy generation is studied for laminar forced convection flow of different nanoparticles（Al2O3, CuO and SiO2） mixed with water through a hexagon microchannel heat sink（HMCHS）. The effects of different heat fluxes and Reynolds numbers on the entropy generation for different nanofluids, volume fractions and nanoparticles diameter are investigated. The heat flux is in the range of 125 to 500 kW·m-2and the Reynolds numbers vary between 200 and 1500. The thermal, frictional and total entropy generations are calculated by integrating the volumetric rate components over the entire HMCHS. The results clearly show that the rise in the heat flux leads to an increase in the thermal entropy generation for nanofluids and pure water but they don't have any influence on the frictional entropy generation. Moreover, when the Reynolds number increases, the frictional entropy generation increases while the thermal entropy generation decreases. The results revealed that at low heat fluxes and high Reynolds numbers, pure water gives the lowest entropy generation, while at high heat flux the nanofluid has to be used in order to lower the overall irreversibility.

Background:Although guidelines recommend systemic therapy even in patients with limited extrahepatic metastases from hepatocellular carcinoma(HCC),a few recent studies suggested a potential benefit for resection of extrahepatic ***,the benefit of adrenal resection(AR)for adrenal-only metastases(AOM)from HCC was not proved *** is the first study to compare long-term outcomes of AR to those of sorafenib in patients with AOM from ***:The patients with adrenal metastases(AM)from HCC were identified from the electronic records of the institution between January 2002 and December *** who presented AM and other sites of extrahepatic disease were ***,the patients with AOM who received other therapies than AR or sorafenib were ***:A total of 34 patients with AM from HCC were *** of these,22 patients had AOM,6 receiving other treatment than AR or ***,8 patients with AOM underwent AR(AR group),while 8 patients were treated with sorafenib(SOR group).The baseline characteristics of the two groups were not significantly different in terms of age,sex,number and size of the primary tumor,timing of AM diagnosis,Child-Pugh and ECOG *** a median follow-up of 15.5 months,in the AR group,the 1-,3-,and 5-year overall survival rates(85.7%,42.9%,and 0%,respectively)were significantly higher than those achieved in the SOR group(62.5%,0%and 0%at 1-,3-and 5-year,respectively)(P=0.009).The median progression-free survival after AR(14 months)was significantly longer than that after sorafenib therapy(6 months,P=0.002).Conclusions:In patients with AOM from HCC,AR was associated with significantly higher overall and progression-free survival rates than systemic therapy with *** results could represent a starting-point for future phase II/III clinical trials.

Physiological cellular responses and adaptations of Rhodococcus erythropolis IBB_(Po1) to toxic organic solvents

A new Gram-positive bacterium, Rhodococcus erythropolis IBBPo1（KF059972.1） was isolated from a crude oil-contaminated soil sample by enrichment culture method. R. erythropolis IBBPo1 was able to tolerate a wide range of toxic compounds, such as antibiotics（800–1000 μg/mL）,synthetic surfactants（50–200 μg/mL）, and organic solvents（40%–100%）. R. erythropolis IBBPo1 showed good tolerance to both alkanes（cyclohexane, n-hexane, n-decane） and aromatics（toluene, styrene, ethylbenzene） with logPOW（logarithm of the partition coefficient of the solvent in octanol–water mixture） values between 2.64 and 5.98. However, alkanes were less toxic for R. erythropolis IBBPo1 cells, compared with aromatics. The high organic solvent tolerance of R. erythropolis IBBPo1 could be due to the presence in their large genome of some catabolic（alkB, alkB1, todC1, todM, xylM）, transporter（HAE1） and trehalose-6-phosphate synthase（otsA1, KF059973.1） genes. Numerous and complex physiological cellular responses and adaptations involved in organic solvent tolerance were revealed in R. erythropolis IBBPo1 cells exposed 1 and 24 hr to 1% organic solvents. R. erythropolis IBBPo1 cells adapt to 1% organic solvents by changing surface hydrophobicity, morphology and their metabolic *** modifications in otsA1 gene sequence were also observed in cells exposed to organic solvents（except ethylbenzene）.

A qualitative correlation between friction coefficient and steel surface wear in linear dry sliding contact to polymers with SGF

In this paper we tried to present a qualitative correlation,based on extensive experimental determinations between the value and the evolution of the friction coefficient,wear,and contact temperature,in the case of linear dry contact,for thermoplastic material reinforced with short glass fibers(SGF)and various steel *** aim was to highlight the evolution of the wear process depending on the evolution of the friction *** a result,it was possible to graphically illustrate the evolution of the friction coefficient and the change of the wear process,emphasizing the abrasive,adhesive and corrosive *** evolution of the plastic material transfer function of the contact temperature,namely of the power lost by friction(product between the contact pressure and sliding speed,p and v)was aimed and it was *** has been demonstrated that in the case of a 30%SGF content it can reach and even exceed contact temperatures very close to the flow limit of the plastic *** tried,believing successfully,the graphic illustration of the evolution of the steel surface wear and of the contact temperature,depending on the friction *** influence of the normal load and sliding speed was evaluated in detail,but also the influence of the metallic surface roughness on the friction coefficient was discussed.

A Price-elastic Approach for Optimal Scheduling of Small-scale Storage Devices in Smart Houses with Short-term and Long-term Constraints

Consecutive charging and discharging of storage devices(SDs)might deem beneficial from the perspective of short-term ***,it highly impacts the life span of the embedded battery and render restrictions on energy storage *** investigate short-term and long-term constraints of SDs through a three-stage price-elastic approach to the optimal operation of small-scale SDs in smart *** first stage deals with data and scenario characterization where the data for determining short-term and long-term operation constraints of SD are *** number of scenarios are generated to represent uncertain parameters such as long-term demand forecasting,daily load profile,electricity price,and photovoltaic(PV)*** second stage optimizes the long-term operation of SD using the envisioned scenarios subject to the long-term operation constraints and the installment costs of *** outputs of this stage are two indicators referred to as price elasticity and price offset coefficients,which are used as the inputs for the third *** third stage is responsible for decision-making on short-term operation of *** outputs of the second stage along with short-term forecasting for daily electricity price,daily load and daily PV generation are *** on the acquired data,proper price elasticity and price offset are determined for optimal *** simulations are performed for different demand forecasting and electricity *** results confirm the effectiveness of the proposed approach.

Minimally invasive surgery for gastric cancer is a rela-tively new research field, with convincing results mostly stemming from Asian countries. The use of the robotic surgery platform, thus far assessed as a safe procedure, which is also easier to learn, sets the background for a wider spread of minimally invasive technique in the treatment of gastric cancer. This review will cover the literature published so far, analyzing the pros and cons of robotic surgery and highlighting the remaining study questions.

Uncertainties in wind power forecasting,dayahead and imbalance prices for the next day possess a great deal of risks for the profit of generation companies participating in a day-ahead electricity *** companies are exposed to imbalance penalties in the balancing market for unordered mismatches between associated day-ahead power schedule and real-time *** of wind and thermal power plants alleviates the risks raised from wind *** paper proposes a novel optimal coordination strategy by balancing wind power forecasting deviations with thermal units in the Turkish day-ahead electricity *** main focus of this study is to provide an optimal trade-off between the expected profit and the risk under wind uncertainty through conditional value at risk(CVa R)*** problem is formulated as a two-stage mixed-integer stochastic programming problem,where scenario-based wind power approach is used to handle the stochasticity of the wind *** programming approach is utilized to attain the commitment status of thermal *** of the coordination with different day-ahead bidding strategies and trade-off between expected profit and CVa R are examined with comparative scenario studies.
