糖尿病神经病变是糖尿病最常见的慢性并发症,是糖尿病足溃疡的重要危险因素之一。早期诊断和早期防治糖尿病神经病变,对于基层医师尤为重要。因此,中华医学会糖尿病学分会神经并发症学组和国家基本公共卫生服务项目基层糖尿病防治管理办公室共同制定了《国家基层糖尿病神经病变诊治指南(2024版)》,该指南在我们撰写的《糖尿病神经病变诊治专家共识(2021年版)》基础上,重点介绍了糖尿病神经病变的概述、分型与临床表现、筛查与诊断方法、防治、患者随访和转诊等内容,旨在指导和帮助基层医师对糖尿病神经病变进行规范化的综合管理,早期防治糖尿病神经病变,延缓疾病进展。

BACKGROUND: It has been demonstrated that mild hypothermia has obvious protective effect on both whole and local cerebral ischemia. However, the definite mechanism is still unclear for the brain protection of mild hypothermia on cerebral edema, inhibiting inflammatory reaction, stabilizing blood brain barrier, etc. OBJECTIVE: To investigate the effect of mild hypothermia on the expression of vascular endothelial growth factor and the infarct volume after cerebral ischemia in rats, and analyze the brain protective mechanism of mild hypothermia. DESIGN: A randomized grouping and controlled animal trial. SETTING: department of Neurology, People’s Hospital of Yunyang Medical College. MATERIALS: Twenty adult male SD rats of clean degree, weighing (250±30) g, were provided by the animal experimental center, school of medicine, Wuhan university. The kits for SP immunohistochemistry were purchased from Beijing Zhongshan Golden Bridge Biotechnology Co., Ltd. METHODS: The experiments were carried out in the laboratory of department of Neurology, Renmen Hospital of Wuhan university from May to July 2005. ① The 20 rats were divided randomly into normal temperature group (n =10) and mild hypothermia group (n =10). Models of permanent middle cerebral artery occlusion were established with modified nylon suture embolization. The rats were assessed with the Longa standards: 0 point for without nerve dysfunction; 1 for mild neurological deficit (fore claws could no extend completely); 2 for moderate neurological deficit (circling towards the affected side); 3 for severe neurological deficit (tilting towards the affected side); 4 for coma and unconscious; 1-3 points represented that models were successfully established. The rats of the normal temperature group were fed at room temperature, and those in the mild hypothermia group were induced by hypothermia from 2 hours postoperatively, and the rectal temperature was kept at 34-35 ℃ for 72 hours. ② Measurement of infarct volume: All t

Septum pellucidum cysts are rare clinical *** their optimal treatment remains controversial,large cysts causing hydrocephalus or neural compression should be treated ***, spontaneous resolution can also ***,we present a case of a 38-year-old woman suffering from intermittent headache associate with an expanding cyst of the septum *** surgical intervention was performed as hydrocephalus was not *** 16 months follow up,her symptom disappeared and MRI revealed that the cyst had *** the best of our knowledge,this is only the third reported case of spontaneous resolution of a septum pellucidum cyst,and suggests that a symptomatic septum pellucidum cyst is not absolute indication of surgical treatment.A conservative approach with close follow up with CT and MRI is strongly recommended unless the cyst causes obvious hydrocephalus or progressive neurological deterioration.

＜正＞Objective Neuroprotection is a major therapeutic approach for ischemic brain *** investigated the neuroprotective effects induced by transplantation of human embryonic neural stem cells into the cortical penumbra 24 h after focal cerebral *** Neural stem cells were prepared from embryonic human brains obtained at 8 weeks of *** cerebral ischemia was induced in adult rats by permanent occlusion of the middle cerebral artery. Animals were randomly divided into two groups:neural stem cell-grafted group and medium-grafted group（control）. Infarct size was assessed at 28 d after transplantation by hematoxylin and eosin *** severity scores were evaluated before ischemia and at 1,7,14,and 28 d after *** assay and immunohistochemical analysis of Bcl-2 and Bax were performed at 7,14,and 28 d after *** Physiological parameters of the two groups were comparable,but not significantly *** stem cell transplantation significantly improved neurological function（P ＜ 0.05） but did not reduce the infarct size significantly（P ＞ 0.05）.Compared with the control, neural stem cell transplantation significantly reduced the number of TUNEL-positive cells and Bax-positive cells in the penumbra at 7 ***,the number of Bcl-2 positive cells in the penumbra after neural stem cell transplantation was significantly higher than that after medium transplantation（P ＜ 0.05）.Conclusions The results indicate that neural stem cell transplantation has antiapoptotic activity and can improve neurological function;these effects are mediated by the up-regulation of Bcl-2 expression in the penumbra.

Essential tremor(ET) is one of the most common neurological disorder with no disease-causing gene has been ***,genetic variants in two members of the leucine-rich repeat and lg domain containing nogo receptor-interacting protein genes(LINGO1 and LINGO2) were reported to be associated with ET susceptibility in certain *** investigate whether the LINGO4 gene(a homologous gene of the LINGO1 and the LINGO2 genes plays a role in ET susceptibility in Chinese Han population,we performed genetic analysis of coding region and flanking sequence of LINGO4 in 100 Chinese Han patients with essential *** variants were identified including an A>T transition(rs61746299),predicted to lead to the amino acid change at residue 444 from Thr to Ser,and a C>T transition(rs1521179),located 12 bp downstream from the coding region and did not affect splicing *** analysis was conducted with a total of 150 ET patients and 300 sex,age and ethnicity matched healthy controls. No significant differences in genotypic and allele distribution between patients and controls for rs61746299 and rs1521179(p = 0.531 and p = 0.867 for genotype distribution;p=1.000 and p= 0.844 for allele distribution) were observed,suggesting variants in the LINGO4 gene may play litter or no role in ET susceptibility.

SELECTION OF SURGICAL APPROACH TO TREAT TRAUMATIC INSTABILITY OF SUBAXIAL CERVICAL SPINE

Objective To evaluate the clinical outcomes of surgical therapy in treating traumatic instability of subaxial cervical spine through either anterior or posterior approach. Methods According to the Allen-Fergurson's classification, we retrospectively studied 42 cases of traumatic instability of subaxial cervical spine through either anterior or posterior surgical reconstruction. Patients requiring approach for either reduction or decompression were not included. Results The average follow-up interval was 3 years and 2 months. The anterior and posterior reconstructions were 24 and 18 cases, respectively. Before operation, the average scores of JOA and VAS were: 12.1 and 6.9 for anterior group, and 12.3 and 7.2 for posterior group. At the final assement, the scores of JOA and VAS improved to 16.0 and 2.2 for anterior group, and 15.7 and 2.6 for posterior group. The average ASIA motor scores of anterior and posterior group improved to 68.2 and 65.5 at the final follow-up from 58.4 and 59.7 before operation, respectively. The ASIA grade (A-E) was converted to a numeric score. The average scores before operation in the anterior and posterior group were 3.3 and 3.4, and increased to 3.8 and 3.7 at the final follow-up. After operation, there were different extent improvements of average radiological parameter, such as Cobb angle, vertebral body translation and disc height ratio. The average operation time and blood losing were 122 min and 125 mL for anterior group, and 153 min and 287 mL for posterior group. Fusion was achieved in all patients and 4 and 2 complications occurred at the anterior or posterior group. Conclusion The results showed that there were no obvious difference in parameters, such as neurological assements, functional grades, fusion rate, operation time and blood losing, between anterior and posterior group, except the virtues of anterior group in reconstruction and maintaining physiologic cervical lordosis and intervertrbal disc height occurred.
