Comparison of different lymph node staging systems in prognosis of gastric cancer:a bi-institutional study from Hungary

Objective： The Union for International Cancer Control （UICC） Node （N） classification is the most common used staging method for the prognosis of gastric cancer. It demands adequate, at least 16 lymph nodes （LNs） to be dissected; therefore different staging systems were invented. Methods： Between March 2005 and March 2010, 164 patients were evaluated at the Department of General Surgery in the Ken^zy Gyula Hospital and at the Department of General, Thoracic and Vascular Surgery in the Kaposi M6r Hospital. The 6th, 7th and 8th UICC N-staging systems, the number of examined LNs, the number of harvested negative LNs, the metastatic lymph node ratio （MLr） and the log odds of positive LNs （LODDS） were determined to measure their 5-year survival rates and to compare them to each other. results： The overall 5year survival rate for all patients was 55.5% with a median overall survival time of 102 months. The tumor stage, gender, UICC N-stages, MLr and the LODDS were significant prognostic factors for the 5-year survival with univariate analysis. The 6th UICC N-stage did not follow the adequate risk in comparing N2 vs. NO and N3 vs. NO with multivariate investigation. Comparison of performances of the residual N classifications proved that the LODDS system was first in the prediction of prognosis during the evaluation of all patients and in cases with less than 16 harvested LNs. The MLr gave the best prognostic prediction when adequate （more than or equal to 16） lymphadenectomy was performed. Conclusions： We suggest the application of LODDS system routinely in western patients and the usage of MLr classification in cases with extended lymphadenectomy.

In Hungary, black locust （robinia pseudoacacia L.） is one of the most important exotic species, forming entire stands. Its importance is increasing in many other countries. As a result of a new selection program eight black lo- cust clones have been improved for establishing clone trials and a seed orchard. In this study juvenile growth and stem quality of micropropagated clones of black locust were evaluated on a marginal site in central Hungary. At ten years old, the clones ＇MB17D3/4＇ and ＇PV201E2/4＇ appeared to be especially promising for high quality wood production under arid hydrological conditions. A tissue culture method can be considered a suitable tool of propagating superior individual trees and offers new prospects for the rapid cloning of selected genotypes.

The study of human-environment relationships in mountain areas is important for both theoretical and practical reasons, as many mountain areas suffer similar problems, such as depopulation, unemployment and natural hazards. Medium mountains constitute a special case within mountains, because they are more populated but less attractive as tourist destinations than high mountains. In this context, the Apuseni Mts （romania） are considered as a ease study. In this paper, we apply GIS-based, quantitative methods to characterize the strength and dynamics of human-environment interactions, taking into consideration some environmental factors （elevation, relative height, slope, river distance, lithology, land cover, natural attractions） as well as historical population and recent tourism data. We found that population density has strong （r2〉0.8） relationships with all relief factors （elevation, relative height, slope, river distance）, and that best-fit functions are nonlinear. We outlined the varying demographic scenarios by elevation zones and interpreted the historically switching sign of population change versus elevation relationship. We demonstrated that lithology also has an impact on the spatial distribution of population, although it is not independent from the relief effect. The land cover of the mainly cultural landscape is very strongly correlated with relief parameters （especially slope）, which suggests good adaptation. We pointed out the dominance of karst objects in the natural tourism potential of the Apuseni Mts and also explored further components of real tourism （spas, heritage, towns）. Finally, we concluded that the environmental settings investigated do in fact constrain the spatial framework of society, but soeio-economic changes in history can be explained from the side of society, which conforms to the theory of cultural possibilism.

Black locust(robinia pseudoacacia L.) was the first North-American tree species imported to Europe at the beginning of the seventeenth century. It is commonly planted worldwide because of its adaptability to environmental stresses, its valuable wood, easy propagation, frequent and abundant seed production, excellent coppicing,high seedling survival, and relatively high wood yield. In Europe, romania and Hungary have the most highly-developed black locust growing techniques and *** a result of increasing interest in black locust in many countries, this review aims to provide a comprehensive overview of state-of-the-art site requirements, propagation, improvement and management(including growth and yield as well as use in energy plantations).

recent studies demonstrated that the Painted Lady(Vanessa cardui),a cosmopolitan diurnal butterfly performs long-range migration between subtropical Africa and north-western Europe,covered by individuals belonging to up to six *** we analyze temporal patterns of complete annual migratory activity of the Painted Lady in Hungary,located in its Central European migratory route,almost completely unstudied *** do so,we used field occurrence data collected between 2000 and 2019 and estimated temporal patterns in migratory activity by fitting kernel density functions on the daily mean number of individuals and observation *** temporal distributions of kernel density estimates were analyzed as a function of time and key climatic predictors of the study *** found that(i)the timing of spring arrivals has been advancing;(ii)the relative intensity of the first and last migratory peaks of the Painted Lady significantly increased during the past decades;and(iii)intensity of the last migratory peak is related to the mean temperature of the previous month,inferring that the migration is shifting to earlier dates and their volume of the migration has substantially intensified,evoking mutually nonexclusive,competing *** study indicates the strengthening migration activities of a southerly distributed,long-distance migrant diurnal butterfly,most probably linked to the northward shift of wintering areas induced by warming trends of the southern parts of ***,the complexity of the likely processes leading to changing migratory strategies calls up for further research in both breeding and wintering areas.

The case of a 22-year-old patient with symptomatic hypokalemia caused by rhabdomyolysis is presented as a rarely reported complication of gluten-sensitive enteropathy (GSE) and dermatitis herpetiformis Duhring. The patient's myopathy ceased on potassium supplementation and her other complaints resolved while on gluten-free *** was otherwise uneventful with a rapid decline in serum CPK level. At the time of her last follow-up a few months later, she was free of symptoms and CPK remained stable. Patients with GSE may present with hypokalemia in association with diarrhea and emesis, and if potassium loss is rapid, rhabdomyolysis may occur.

AIM:To assess the efficacy of intravenous cyclophos-phamide pulse therapy for refractory inflammatory boweldisease(IBD).METHODS:We included in our cohort eight patientswith(moderate/severe)steroid refractory IBD(4 withulcerative colitis and 4 with Crohn's disease).They allreceived 6 cycles of intravenous cyclophosphamide(800mg)per ***:Patients entered into remission after the sec-ond/third cyclophosphamide *** activity *** were no side effects and *** thepatients went into long lasting *** Crohn's diseasepatients and 3 of 4 ulcerative colitis patients achievedcomplete *** patient with ulcerative colitisshowed an impressive clinical response but did not enterinto *** the maintenance,patients with Crohn'sdisease were treated with methotrexate(15 mg/wk)andpatients with ulcerative colitis were treated with azathio-prine(2.5 mg/kg body weight/d).CONCLUSION:remission was maintained in all pa-tients for 6 mo on the *** drug was well *** findings suggest that aggressive immuno-suppressive therapy may be useful in some refractorypatients and further controlled study should be consid-ered in order to fully evaluate this type of treatment as apotential therapy for IBD.

AIM： To study the effect of proton pump inhibitor （PPI） treatment on patients with reflux esophagitis and its in vivo effect on apoptosis, p53- and epidermal growth factor receptor （EGFr） expression. METHODS： After informed consent was obtained, gastric biopsies of the antrum were taken from patients with reflux oesophagitis prior to and after 6 mo of 20 mg omeprazole （n = 24） or 40 mg esomeprazole （n = 22） therapy. Patients did not take any other medications known to affect the gastric mucosa. All patients were Helicobacter pylori negative as confirmed by rapid urease test and histology, respectively. Cell proliferation, apoptosis, EGFr, and p53 expression were measured by immunohistochemical techniques. At least 600 glandular epithelial cells were encountered and results were expressed as percentage of total cells counted. Was considered statistically significant. rESULTS： Although there was a trend towards increase of cell proliferation and EGFr expression both in omeprazole and esomeprazole treated group, the difference was not statistically significant. Neither apoptosis nor p53 expression was affected. CONCLUSION： Long-term PPI treatment does not significantly increase gastric epithelial cell proliferation and EGFr expression and has no effect on apoptosis and p53 expression.
