Comparison of different lymph node staging systems in prognosis of gastric cancer:a bi-institutional study from Hungary

Objective： The Union for International Cancer Control （UICC） Node （N） classification is the most common used staging method for the prognosis of gastric cancer. It demands adequate, at least 16 lymph nodes （LNs） to be dissected; therefore different staging systems were invented. Methods： Between March 2005 and March 2010, 164 patients were evaluated at the Department of General Surgery in the Ken^zy Gyula Hospital and at the Department of General, Thoracic and Vascular Surgery in the Kaposi M6r Hospital. The 6th, 7th and 8th UICC N-staging systems, the number of examined LNs, the number of harvested negative LNs, the metastatic lymph node ratio （MLR） and the log odds of positive LNs （LODDS） were determined to measure their 5-year survival rates and to compare them to each other. Results： The overall 5year survival rate for all patients was 55.5% with a median overall survival time of 102 months. The tumor stage, gender, UICC N-stages, MLR and the LODDS were significant prognostic factors for the 5-year survival with univariate analysis. The 6th UICC N-stage did not follow the adequate risk in comparing N2 vs. NO and N3 vs. NO with multivariate investigation. Comparison of performances of the residual N classifications proved that the LODDS system was first in the prediction of prognosis during the evaluation of all patients and in cases with less than 16 harvested LNs. The MLR gave the best prognostic prediction when adequate （more than or equal to 16） lymphadenectomy was performed. Conclusions： We suggest the application of LODDS system routinely in western patients and the usage of MLR classification in cases with extended lymphadenectomy.

Effect of Normative and Socioeconomic Power Bases on Japanese Marital Decision-Making Authority

According to normative-resource theory,the balance of marital power is influenced by the effect of socioeconomic resources and subcultural *** purpose of our study is to empirically test the effects of Japanese married individuals’comparative socioeconomic variables—education,age,occupation status,personal income,family income,urban/rural living area,years married,and the presence/number of children;and normative resources—the four dimensions of sex role orientation—on decision-making authority across the stages of automobile *** data of 500 demographically heterogeneous married individuals are analyzed by using a Lasso(least absolute shrinkage and selection operator)regression to select variables and to improve the accuracy of prediction and *** findings reveal that wives’authority is significantly influenced across all stages by their personal income and specific dimensions of their sex role *** the other hand,husbands’authority is not affected by their perception of role norms,but only by their *** to expectations,the effects of education,occupation,and employment status are not significant in any of the cases,opening a discussion for cross-cultural *** findings are interpreted and transformed into actionable items for managerial implications.

Iterative signal reconstruction of deliberately clipped SMT signals

Staggered MultiTone(SMT)is a modulation technique showing significantly reduced Adjacent Channel Leakage Ratio(ACLR)resulting in a more compact Power Spectrum Density(PSD)for the transmitted signal,than the well-known and already widely adopted Orthogonal Frequency Division Multiple Access(OFDMA)***,the unique spectral properties of an SMT signal could be degraded by a non-linear element(e.g.a Power Amplifier(PA))in the *** baseband clipping can be applied to the transmitted signal,reducing the notable high Peak-to-Average Power Ratio(PAPR).The objective of this paper is to give a brief introduction to the SMT scheme,with a special emphasis on deliberate clipping efects and their *** paper introduces two receiver-oriented iterative methods aiming at the restoration of the baseband Bit Error Rate(BER)performance of a non-clipped *** methods are evaluated and compared based on numerical *** paper concludes with the selection of a possible candidate for use in systems applying deliberately clipped SMT signals.

Genetic background of phenotypic variation

A noteworthy feature of the living world is its bewildering variability. A key issue in several biological disciplines is the achievement of an understanding of the hereditary basis of this variability. Two opposing, but not necessarily irreconcilable conceptions attempt to explain the underlying mechanism. The gene function paradigm postulates that phenotypic variance is generated by the polymorphism in the coding sequences of genes. However, comparisons of a great number of homologous gene and protein sequences have revealed that they predominantly remained functionally conserved even across distantly related phylogenic taxa. Alternatively, the gene regulation paradigm assumes that differences in the cis-regulatory region of genes do account for phenotype variation within species. An extension of this latter concept is that phenotypic variability is generated by the polymorphism in the overall gene expression profiles of gene networks. In other words, the activity of a particular gene is a system property determined both by the cis-regulatory sequences of the given genes and by the other genes of a gene network, whose expressions vary among individuals, too. Novel proponents of gene function paradigm claim that functional genetic variance within the coding sequences of regulatory genes is critical for the generation of morphological polymorphism. Note, however, that these developmental genes play direct regulatory roles in the control of gene expression.

The mutual dependence of extensive land-use and conservation management has become appar-ent in Europe in the last 20-30 *** land-use often survives in protected areas only,in the form of conservation *** of extensive herding and that of conservation management are parts of two knowledge systems(traditional and scientific)which often leads to conflicts between locals and *** studied two herding/conservation systems(salt steppes and wood-pastures),and devel-oped an inventory on the common/similar and conflicting/different objectives and pasture management practices of herders and *** were collected by participatory knowledge co-production in teamwork of the co-authors(herders,conservation managers,and scientists).Data were analyzed and discussed in teamwork *** and conservationists identified 23 objectives and 29 management *** found a number of common interests with respect to herding,the ideal state of pastures,legal provisions,and *** resolution recommendations(e.g.,on time and place of grazing,pasture improvements)were also *** argue that by co-production of knowledge,and establish-ment of a herder“school”the mitigation of the existing conflicts would be more *** conclusion is that a new profession is needed:that of the conservation *** conservation herder shall be an indi-vidual knowledgeable about herding and pasture management,trained in conservation and ecology,able to design management experiments,and develop novel but tradition-based management *** such,he/she could facilitate adaptation of extensive herding in the changing socio-economic environment.

Formability Investigations of High-Strength Dual-Phase Steels

Car manufacturing is always regarded as the key industry behind sheet metal forming, and thus, the requirements of and developments in car manufacturing play a decisive role in the development of sheet metal forming. The automotive industry is faced with contradictory demands and requirements： better performance with lower consumption and less harmful emissions, more safety and comfort; these are extremely difficult to supply simultaneously with conventional materials and conventional manufacturing processes. The fulfillment of these often contradictory requirements is one of the main driving forces in the automotive industry and thus in the material and process developments in sheet metal forming, as well. In recent years, significant developments can be observed in the application of high-strength steels. In this respect, the application of various dual-phase steels is one of the best examples. However, the application of these highstrength steels often leads to formability and manufacturing problems. One formability problem is the springback occurring after sheet metal forming. In the current research, we have dealt mainly with advanced high-strength steels, primarily with dual-phase steels. When applying them, the springback phenomenon is one of the most critical issues. To reduce the tremendous amount of experimental work needed, we also applied numerical simulation using isotropic–kinematic hardening rules. The isotropic–kinematic hardening behavior of a given material in the applied Auto Form numerical package may be characterized with three independent material parameters c, v and K（a detailed explanation of their meaning will be given in the main part of this paper）. However, we found that the material data included in simulation packages for these new high-strength steels are not fully adequate. Helicobacter pylori infection: New pathogenetic and clinical aspects

Helicobacter pylori (H. pylori) infects more than half of the world’s human population, but only 1% to 3% of infected people consequently develop gastric adenocarcinomas. The clinical outcome of the infection is determined by host genetic predisposition, bacterial virulence factors, and environmental factors. The association between H. pylori infection and chronic active gastritis, peptic ulcer disease, gastric cell carcinoma, and B cell mucosa-associated lymphoid tissue lymphoma has been well established. With the exception of unexplained iron deficiency anemia and idiopathic thrombocytopenic purpura, H. pylori infection has no proven role in extraintestinal diseases. On the other hand, there is data showing that H. pylori infection could be beneficial for some human diseases. The unpredictability of the long-term consequences of H. pylori infection and the economic challenge in eradicating it is why identification of high-risk individuals is crucial.

Changing human-environment interactions in medium mountains:the Apuseni Mts(Romania)as a case study

The study of human-environment relationships in mountain areas is important for both theoretical and practical reasons, as many mountain areas suffer similar problems, such as depopulation, unemployment and natural hazards. Medium mountains constitute a special case within mountains, because they are more populated but less attractive as tourist destinations than high mountains. In this context, the Apuseni Mts （Romania） are considered as a ease study. In this paper, we apply GIS-based, quantitative methods to characterize the strength and dynamics of human-environment interactions, taking into consideration some environmental factors （elevation, relative height, slope, river distance, lithology, land cover, natural attractions） as well as historical population and recent tourism data. We found that population density has strong （r2〉0.8） relationships with all relief factors （elevation, relative height, slope, river distance）, and that best-fit functions are nonlinear. We outlined the varying demographic scenarios by elevation zones and interpreted the historically switching sign of population change versus elevation relationship. We demonstrated that lithology also has an impact on the spatial distribution of population, although it is not independent from the relief effect. The land cover of the mainly cultural landscape is very strongly correlated with relief parameters （especially slope）, which suggests good adaptation. We pointed out the dominance of karst objects in the natural tourism potential of the Apuseni Mts and also explored further components of real tourism （spas, heritage, towns）. Finally, we concluded that the environmental settings investigated do in fact constrain the spatial framework of society, but soeio-economic changes in history can be explained from the side of society, which conforms to the theory of cultural possibilism.

Hypokalemic myopathy in a patient with gluten-sensitive enteropathy and dermatitis herpetiformis Duhring: A case report

The case of a 22-year-old patient with symptomatic hypokalemia caused by rhabdomyolysis is presented as a rarely reported complication of gluten-sensitive enteropathy (GSE) and dermatitis herpetiformis Duhring. The patient's myopathy ceased on potassium supplementation and her other complaints resolved while on gluten-free *** was otherwise uneventful with a rapid decline in serum CPK level. At the time of her last follow-up a few months later, she was free of symptoms and CPK remained stable. Patients with GSE may present with hypokalemia in association with diarrhea and emesis, and if potassium loss is rapid, rhabdomyolysis may occur.
