An unusual cause of cholecystitis:Heterotopic pancreatic tissue in the gallbladder

Gallbladder localization of heterotopic pancreas （HP） is uncommon and very rarely gives rise to symptoms. Herein we report a case of HP found in the gallbladder neck presented with signs and symptoms of cholecystitis. The patient was a 40-year old male, suffering from epigastric pain, abdominal fullness and fever. On physical examination, the right upper abdomen was tender with a positive Murphy＇s sign. Ultrasonographic examination showed a hydropic gallbladder without stones and he underwent a cholecystectomy. Pathological examination revealed an intramural nodule （9 ram） in the neck region which is consisted of acini, ducts and islet cells of an aberrant pancreatic tissue. Although HP is encountered rarely in the gallbladder and is found incidentally during pathological studies, this case emphasizes that HP might cause symptoms and present clinically as cholecystitis. For this reason, in patients presenting with symptomatic gallbladder diseases, including cholecystitis without any other pathology, HP should be taken into consideration before it is diagnosed as ＂idiopathic＇：

Objective:To investigate granulomatous inflammation etiology based on clinical history and ancillary ***:Children aged<18 years with biopsy proven granulomatous lesions in any tissue specimens between January 2014 and January 2022 were included in the *** diagnosis was based on the results of immunohistochemical staining,molecular tests,culture,serology,radiological and other auxiliary laboratory *** were categorized into infectious and noninfectious ***:In total,174 patients with granulomatosis inflammation confirmed by histopathology were *** 59.2%patients were males,and the median age was 4.48(IQR 2.36-6.39)years(range:16 months-18 years).The tissues/organs that were most commonly biopsied were lymph node,bone,skin,and lung(51.1%,17.8%,9.2%,and 5.7%,respectively).Infectious and non-infectious causes were identified in 73.0%and 12.6%patients,respectively,in terms of granulomatosis inflammation etiology;however,no cause was identified in 14.4%*** most common infectious cause was tuberculosis(in 51.7%patients),followed by toxoplasmosis,aspergillosis,mucormycosis,leishmaniasis,and cat-scratch disease(in 8.6%,5.7%,1.7%,1.7%,and 1.1%patients,respectively).The common non-infectious cause was chronic granulomatous *** evaluation revealed granulomatosis inflammation in 33.3%patients,necrotizing granulomatosis inflammation in 30.5%patients,and caseating granulomatosis inflammation in 12.1%*** the pathology results of patients with and without tuberculosis were compared,the incidence of caseating granulomatosis inflammation(P=0.003)and necrotizing granulomatosis inflammation(P=0.005)was higher in patients with ***:Chronic granulomatous disease is the most common non-infectious cause in ***,primary or secondary immune deficiencies may cause granulomatosis inflammation,especially in pediatric patients.

Epiretinal membrane related vascular changes in diabetic eyes evaluated with optical coherence tomography angiography

AIM:To evaluate the retinochoroidal microvascular circulation and anatomical structure of diabetic and nondiabetic patients with epiretinal membrane(ERM)with the help of optical coherence tomography angiography(OCT-A)and compare them with healthy control ***:In this prospective,cross-sectional study,a total of 165 eyes were evaluated,including 50 eyes of patients with diabetic ERM,54 eyes of idiopathic ERM(iERM)patients,and 61 eyes of healthy *** and disc angiography was performed by *** vessel density(VD)ratio was evaluated by dividing the VD of the foveal region by the VD of the parafoveal *** calculations were evaluated at the 95%confidence ***:Macula superficial VD values of ERM cases were lower than that in the control group,while foveal VD was higher in ERM *** deep VD values of ERM cases were lower in all quadrants,except the *** width of the foveal avascular zone(FAZ)area was significantly lower in the ERM groups,and the FAZ width was lowest in iERM *** superficial VD ratio was significantly higher in the ERM groups,but there was no significant difference between ERM *** deep VD ratio was significantly higher in the iERM group than in the control ***:Diabetic and idiopathic ERMs differ in their mechanism of formation and clinical presentation,as well as their effect on retinal vascular *** the relationship of increase of retinal thickness with vascular integrity can be demonstrated with OCT-A,then,OCT-A can be used as a guide for ERM prognosis.

Ocular findings in children with thalassemia major in Eastern Mediterranean

AIM: To investigate ophthalmologic findings in children with thalassemia major(TM) and compare the findings with healthy controls. ·METHODS: In a cross-sectional study,43 children with thalassemia major from pediatric hematology outpatient clinics from two university hospitals and age/sex matched 47 healthy children were included in the study. After a complete ophthalmic examination,tear function tests including the Schirmer test,fluorescein tear breakup time(BUT),ultrasound pachymetry,and axial length measurement were performed. Obtained data was recorded for statistical analysis and the values of right eyes were compared between groups. ·RESULTS: The mean best corrected visual acuity was 1.34 ±0.75 in TM and 1.08 ±0.28 in controls. It was found lower than 0.1 logMAR unit in 10(23.2%) children with TM and 2(4.2%) in controls,and the difference was statistically significant(P 0.05). The mean axial length was 22.53±0.50 in TM and 22.57±0.43mm in the control group. The mean Schirmer test score was 19.94±6.91 in TM and 24.22±3.95mm in the control group(P 0.05). ·CONCLUSION: In TM,while corneal thickness,axial length,and BUT are close to controls,the Schirmer scores are less than normal. The study revealed that TM may be affected by the tear function and visual acuity.

Elastography is a method which determines the stiffness of tissues with the help of ultrasound technology and exhibits more quantitative data according to palpation that made during physical examination. Elastography has relatively entered to routine use in the breast evaluation with imaging techniques. Although palpation has a very long history, elastography has been used since 1990s. Elastography is used as an adjunct to conventional gray scale ultrasound and can identify the stiffness of the tissues non-invasively. Today, it is used as two separate technological modalities: strain elastography and shear wave elastography. The aims of these modalities are to increase the sensitivity of the separation between malignant and benign lesions, reduce the unnecessary biopsy processes and to provide a more accurate Breast Imaging Reporting and Data System (BIRADS) categorization of the breast lesions. In this article, we aimed to review the clinical utilization and benefits of elastography in differantial diagnosis of breast lesions, BIRADS categorization, and biopsy decision making in the light of current literature.

Crescentic glomerulonephritis in children:a single centre experience

Background:Crescentic glomerulonephritis(CsGN)is characterized by crescents in 50%or more of glomeruli and clinically by a sudden and progressive decline in renal ***:We evaluated the etiology,clinical features,prognostic factors and long-term outcome of *** January 2000 and December 2010,45 children(26 girls,19 boys)with biopsy-proven CsGN(>50%crescents)were investigated ***:The mean age of the patients was 130.86±33.77 *** mean duration of symptoms prior to diagnosis was 26±12 days(4-40 days).Most of the children had hypertension(62.2%),macroscopic hematuria(73.3%),oligoanuria(44.4%),edema(51.1%)and purpuric rash(40%)at *** final clinical status of the patients was complete remission(n=21),partial remission(n=5)or chronic kidney disease(n=19).Adverse outcomes were significantly associated with a long duration between the onset of symptoms and treatment(P=0.038),the presence of oligoanuria(P=0.006),a severe decreased glomerular filtration rate(GFR75%)(P=0.03),and the ratio of fi brous crescents(P=0.015).Conclusion:The outcome of CsGN in children continues to be poor,and it should be treated as a renal emergency.

A New Mathematical Modeling of the COVID-19 Pandemic Including the Vaccination Campaign

In a short time, many illustrative studies have been conducted on the mathematical modeling and analysis of COVID-19. There are not enough studies taking into account the vaccine campaign among these studies. In this context, a mathematical model is developed to reveal the effects of vaccine treatment, which has been performed recently, on COVID-19 in this study. In the proposed model, as well as the vaccinated individuals, a five-dimensional compartment system including the susceptible, infected, exposed and recovered population is constructed. Moreover, besides the positivity, existence and uniqueness of the solution, biologically feasible region are provided. The basic reproduction number known as expected secondary infection which means that expected infection among the susceptible populations caused by this infection is evaluated. In the numerical simulations, the parameter values taken from the literature and estimated are used to perform the solutions of the proposed model. Fourth-order Runge-Kutta numerical scheme is applied to obtain the results.

Inflammatory myofibroblastic tumor (IMT) is an uncommon myofibroblastic neoplasm that was formerly included within the broad category of inflammatory pseudotumor (IPT).IMT is rarely encountered in the *** to IMT of other organs,the interchangeable use of the terms IMT and IPT in liver has made the analysis of these lesions *** this review,clinical and pathological features of IMT of the liver are reviewed and the differential diagnosis of IMT is discussed,with emphasis on IPT and the other entities included in this large *** can mimic malignant *** are no known unique diagnostic clinical,laboratory,or radiological *** definitive diagnosis of IMT depends on careful pathological *** histopathological evaluation of hepatic IMT reveals that,the myxoid/vascular pattern is the most frequently observed,followed by,in decreasing frequency,fibrous histiocytomalike pattern and hypocellular fibrous *** IMT of the liver,anaplastic lymphoma kinase (ALK) expression reliably predicts the presence of an ALK gene *** diagnosis of hepatic IMT depends on the dominant histopathological pattern,and the management of the disease is still *** the liver is a distinctive neoplasm of intermediate biological potential,and should be distinguished from the variety of lesions that are included under the broad category of ***,to avoid confusion regarding the true incidence and behavior of hepatic IMT,the term IPT should not be used interchangeably with *** rarity of IMT in liver should not minimize its consideration in the differential diagnosis of liver tumors,especially in patients with tumor markers in normal range.
