The emergence of Middle Ea.t respira.ory syndrome corona.irus （MERS-CoV） in 2012 brought ba.k memories of the occurrence of severe a.ute respira.ory syndrome corona.irus （Sa.S-CoV） in 2002. More tha. 1500 MERS-CoV ca.es were recorded in 42 months with a.ca.e fa.a.ity ra.e （CFR） of 40%. Mea.while, 8000 ca.es of Sa.S-CoV were confirmed in six months with a.CFR of 10%. The clinica. presenta.ion of MERS-CoV ra.ges from mild a.d non-specific presenta.ion to progressive a.d severe pneumonia. No predictive signs or symptoms exist to differentia.e MERS-CoV from community-a.quired pneumonia.in hospita.ized pa.ients. a. a.pa.ent heterogeneity wa. observed in tra.smission. Most MERS-CoV ca.es were seconda.y to la.ge outbrea.s in hea.thca.e settings. These ca.es were seconda.y to community-a.quired ca.es, which ma. a.so ca.se fa.ily outbrea.s. Tra.el-a.socia.ed MERS infection rema.ns low. However, the virus exhibited a.clea. tendency to ca.se la.ge outbrea.s outside the a.a.ia. Peninsula.a. exemplified by the outbrea. in the Republic of Korea. In this review, we summa.ize the current knowledge a.out MERS-CoV a.d highlight tra.el-rela.ed issues.

Infection with Middle Ea.t respira.ory syndrome corona.irus (MERS-CoV) emerged in 2012 a. a. importa.t respira.ory disea.e with high fa.a.ity ra.es of 40%—60%. Despite the increa.ed number of ca.es over subsequent yea.s, the number of pedia.ric ca.es rema.ned low. a.review of studies conducted from June 2012 to a.ril 19, 2016 reported 31 pedia.ric MERS-CoV ca.es. In this pa.er, we present the clinica. a.d la.ora.ory fea.ures of seven pa.ients with pedia.ric MERS. Five pa.ients ha. no underlying medica. illnesses, a.d three pa.ients were a.ymptoma.ic. Of the seven ca.es, four (57%) pa.ients sought medica. a.vice within 1-7 da.s from the onset of symptoms. The three other pa.ients (43%) were a.ymptoma.ic a.d were in conta.t with pa.ients with confirmed dia.nosis of MERS-CoV. The most common presenting symptoms were fever (57%), cough (14%), shortness of brea.h (14%), vomiting (28%), a.d dia.rhea.(28%). Two (28.6%) pa.ients ha. pla.elet counts of a.d one pa.ient ha. a. underlying end-sta.e rena. disea.e. The rema.ning pa.ients presented with norma. blood count, liver function, a.d urea.a.d crea.inine levels. The documented MERS-CoV Ct va.ues were 32 38 for four of the seven ca.es. Two pa.ients (28.6%) ha. a.norma. chest ra.iogra.hic findings of bila.era. infiltra.ion. One pa.ient (14.3%) required ventila.or support, a.d two pa.ients (28.6%) required oxygen supplementa.ion. a.l the seven pa.ients were discha.ged without complica.ions.

We a.so sea.ched MEDLINE a.d Pub Med for the keywords: Middle Ea.t respira.ory syndromecorona.irus, MERS-Co V in combina.ion with pedia.ric, children, childhood, infa.cy a.d pregna.cy from the initia. discovery of the virus in 2012 to 2016. The retrieved a.ticles were a.so rea. to further find other a.ticles. Releva.t da.a.were pla.ed into a. excel sheet a.d a.a.yzed a.cordingly. Descriptive a.a.ytic sta.istics were used in the fina. a.a.ysis a. deemed *** From June 2012 to a.ril 19, 2016, there were a.tota. of 31 pedia.ric MERS-Co V ca.es. Of these ca.es 13(42%) were a.ymptoma.ic a.d the ma.e to fema.e ra.io wa. 1.7:1. The mea. a.e of pa.ients wa. 9.8 ± 5.4 yea.s. Twenty-five(80.6%) of the ca.es were reported from the Kingdom of Sa.di a.a.ia. The most common source of infection wa. household conta.t(10 of 15 with reported source) a.d 5 pa.ients a.quired infection within a.hea.th ca.e fa.ility. Using rea. time reverse tra.scripta.e polymera.e cha.n rea.tion of pedia.ric pa.ients revea.ed tha. 9 out of 552(1.6%) wa. positive in the Kingdom of Sa.di a.a.ia. CONCLUSION Utilizing serology for MERS-CoV infection in Jorda. a.dSa.di a.a.ia.did not revea. a.y positive pa.ients. Thus, the number of the pedia.ric MERS-CoV is low; the exa.t rea.on for the low preva.ence of the disea.e in children is not known.

Middle Ea.t respira.ory syndrome corona.irus(MERS-Co V) ha. emerged in the a.a.ia. Gulf region, with its epicentre in Sa.di a.a.ia. the host of the ‘Ha.j' which is the world's the la.gest ma.s ga.hering. Tra.smission of MERS-Co V a. such a. event could lea. to its ra.id worldwide dissemina.ion. Therefore, we studied the frequency of viruses ca.sing influenza.like illnesses(ILI) a.ong pa.ticipa.ts in a.ra.domised controlled tria. a. the Ha.j 2013. We recruited 1038 pilgrims from Sa.di a.a.ia. a.stra.ia.a.d Qa.a. during the first da. of Ha.j a.d followed them closely for four da.s. a.na.a. swa. wa. collected from ea.h pilgrim who developed ILI. Respira.ory viruses were detected using multiplex RT-PCR. ILI occurred in 112/1038(11%) pilgrims. Their mea. a.e wa. 35 yea.s, 49(44%) were ma.e a.d 35(31%) ha. received the influenza.va.cine pre-Ha.j. Forty two(38%) pilgrims ha. la.ora.ory-confirmed vira. infections; 28(25%) rhinovirus, 5(4%) influenza.a. 2(2%) a.enovirus, 2(2%) huma. corona.irus OC43/229 E, 2(2%) pa.a.nfluenza.virus 3, 1(1%) pa.a.nfluenza.virus 1, a.d 2(2%) dua. infections. No MERS-Co V wa. detected in a.y sa.ple. Rhinovirus wa. the commonest ca.se of ILI a.ong Ha.j pilgrims in 2013. Infection control a.d a.propria.e va.cina.ion a.e necessa.y to prevent tra.smission of respira.ory viruses a. Ha.j a.d other ma.s ga.herings.
