Short-and long-term outcomes following laparoscopic vs open surgery for pathological T4 colorectal cancer: 10 years of experience in a single center

AIM To evaluate the short-term and long-term outcomes following laparoscopic vs open surgery for pathological T4(pT 4) colorectal *** We retrospectively analyzed the short-and long-term outcomes of proven p T4 colorectal cancer patients who underwent complete resection by laparoscopic or open surgery from 2006 to 2015 at Guangdong General *** A total of 211 p T4 colorectal cancer patients were included in this analysis, including 101 cases in thelaparoscopy(LAP) group and 110 cases in the open surgery(OPEN) group [including 15(12.9%) cases of conversion to open surgery]. Clinical information(age, gender, body mass index, comorbidities, American Society of Anesthesiologists score, etc.) did not differ between the two groups. In terms of blood loss, postoperative complications and rate of recovery, the LAP group performed significantly more favorably(P < 0.05). With regard to p T4 a/b and combined organ resection, there were significantly more cases in the OPEN group(P < 0.05). The 3-and 5-year overall survival rates were 74.9% and 60.5%, respectively, for the LAP group and 62.4% and 46.5%, respectively, for the OPEN group(P = 0.060). The 3-and 5-year disease-free survival rates were 68.0% and 57.3%, respectively, for the LAP group and 55.8% and 39.8%, respectively, for the OPEN group(P = 0.053). Multivariate analysis showed that ⅢB/ⅢC stage, lymph node status, and CA19-9 were significant predictors of overall survival. PT4 a/b, ⅢC stage, histological subtypes, CA19-9, and adjuvant chemotherapy were independent factors affecting disease-free *** Laparoscopy is safely used in the treatment of p T4 colorectal cancer while offering advantages of minimal invasiveness and faster recovery. Laparoscopy is able to achieve good oncologic outcomes similar to those of open surgery. We recommend that laparoscopy be carried out in experienced centers. In current neural network algorithms for nuclide identification in high-background,poor-resolution detectors,traditional network paradigms including back-propagation networks,convolutional neural networks,recurrent neural networks,etc.,have been limited in research on γ spectrum analysis because of their inherent mathematical *** is difficult to make progress in terms of training data requirements and prediction *** contrast to traditional network paradigms,network models based on the transformer structure have the characteristics of parallel computing,position encoding,and deep stacking,which have enabled good performance in natural language processing tasks in recent ***,in this paper,a transformer-based neural network (TBNN) model is proposed to achieve nuclide identification for the first ***,the Geant4 program was used to generate the basic single-nuclide energy spectrum through Monte Carlo simulations.A multi-nuclide energy spectrum database was established for neural network training using random matrices of γ-ray energy,activity,and *** on the encoder–decoder structure,a network topology based on the transformer was built,transforming the 1024-channel energy spectrum data into a 32×32 energy spectrum sequence as the model *** experiments and adjustments of model parameters,including the learning rate of the TBNN model,number of attention heads,and number of network stacking layers,the overall recognition rate reached 98.7%.Additionally,this database was used for training AI models such as back-propagation networks,convolutional neural networks,residual networks,and long shortterm memory neural networks,with overall recognition rates of 92.8%,95.3%,96.3%,and 96.6%,*** indicates that the TBNN model exhibited better nuclide identification among these AI models,providing an important reference and theoretical basis for the practical application of transformers in the qualitative and quantitativ

The focusing modified Korteweg-de Vries(mKdV)equation with multiple high-order poles under the nonzero boundary conditions is first investigated via developing a Riemann-Hilbert(RH)*** begin with the asymptotic property,symmetry and analyticity of the Jost solutions,and successfully construct the RH problem of the focusing mKdV *** solve the RH problem when 1/S_(11)(k)has a single highorder pole and multiple high-order ***,we derive the soliton solutions of the focusing mKdV equation which corresponding with a single high-order pole and multiple high-order poles,***,the dynamics of one-and two-soliton solutions are graphically discussed.

Reperfusion following cerebral ischemia causes both structural and functional damage to brain tissue and could aggravate a patient's condition;this phenomenon is known as cerebral ischemia-reperfusion *** studies have elucidated the neuroprotective role of the sirtuin protein family(Sirtuins)in modulating cerebral ischemia-reperfusion ***,the potential of utilizing it as a novel intervention target to influence the prognosis of cerebral ischemia-reperfusion injury requires additional *** this review,the origin and research progress of Sirtuins are summarized,suggesting the involvement of Sirtuins in diverse mechanisms that affect cerebral ischemia-reperfusion injury,including inflammation,oxidative stress,blood-brain barrier damage,apoptosis,pyroptosis,and *** therapeutic avenues related to Sirtuins that may improve the prognosis of cerebral ischemia-reperfusion injury were also investigated by modulating Sirtuins expression and affecting representative pathways,such as nuclear factor-kappa B signaling,oxidative stress mediated by adenosine monophosphate-activated protein kinase,and the forkhead box *** review also summarizes the potential of endogenous substances,such as RNA and hormones,drugs,dietary supplements,and emerging therapies that regulate Sirtuins *** review also reveals that regulating Sirtuins mitigates cerebral ischemia-reperfusion injury when combined with other risk *** Sirtuins show promise as a potential target for the treatment of cerebral ischemiareperfusion injury,most recent studies are based on rodent models with circadian rhythms that are distinct from those of humans,potentially influencing the efficacy of Sirtuinstargeting drug ***,this review provides new insights into the role of Sirtuins in the pathology and treatment of cerebral ischemia-reperfusion injury.

The off situ accurate reconstruction of the core neutron field is an important step in realizing real-time reactor *** existing off situ reconstruction method of the neutron field is only applicable to cases wherein a single region changes at a specified location of the ***,when the neutron field changes are complex,the accurate identification of the individual changed regions becomes challenging,which seriously affects the accuracy and stability of the neutron field ***,this study proposed a dual-task hybrid network architecture(DTHNet)for off situ reconstruction of the core neutron field,which trained the outermost assembly reconstruction task and the core reconstruction task jointly such that the former could assist the latter in the reconstruction of the core neutron field under core complex ***,to exploit the characteristics of the ex-core detection signals,this study designed a global-local feature upsampling module that efficiently distributed the ex-core detection signals to each reconstruction unit to improve the accuracy and stability of *** experiments were performed on the simulation datasets of the CLEAR-I reactor to verify the accuracy and stability of the proposed *** results showed that when the location uncertainty of a single region did not exceed nine and the number of multiple changed regions did not exceed ***,the reconstructed ARD was within 2%,RD_(max)was maintained within 17.5%,and the number of RD≥10%was maintained within ***,when the noise interference of the ex-core detection signals was within±2%,although the average number of RD≥10%increased to 16,the average ARD was still within in 2%,and the average RD_(max)was within 22%.Collectively,these results show that,theoretically,the DTHNet can accurately and stably reconstruct most of the neutron field under certain complex core changes.

Since then, additional supporting data have greatly strengthened the proposal. The drug, HHT (homoharringtonine, or omacetaxine), is readily available and inexpensive as it has been approved since 2012 for treating leukemia.

There have been reports of potential health risks for people from hydrophobic organic pollutants,such as polycyclic aromatic hydrocarbons(PAHs),polychlorinated hydrocarbons(PCHs),and organophosphate flame retardants(OPFRs).When a contaminated site is used for residential housing or public utility and recreation areas,the soil-bound organic pollutantsmight pose a threat to human *** this study,we investigated the contamination profiles and potential risks to human health of 15 PAHs,6 PCHs,and 12 OPFRs in soils from four contaminated sites in *** used an in vitro method to determine the oral bioaccessibility of soil *** PAHswere found at concentrations ranging from26.4 ng/g to 987 ng/***(0.27-14.3 ng/g)and OPFRs(6.30-310 ng/g)were detected,but at low levels compared to earlier *** levels of PAHs,PCHs,and OPFRs released from contaminated soils into simulated gastrointestinal fluids ranged from 1.74%to 91.0%,2.51%to 39.6%,and 1.37%to 96.9%,*** on both spiked and unspiked samples,we found that the oral bioaccessibility of pollutantswas correlated with their logKow andmolecularweight,and the total organic carbon content and pH of *** in 13 out of 38 contaminated soil samples posed potential high risks to *** considering oral bioaccessibility,nine soils still posed potential risks,while the risks in the remaining soils became *** contribution of this paper is that it corrects the health risk of soil-bound organic pollutants by detecting bioaccessibility in actual soils from different contaminated sites.
