Shengmai San for Treatment of Cardiotoxicity from Anthracyclines:A Systematic Review and Meta-Analysis

Objective:To systematically evaluate the efficacy of Shengmai San in patients with cardiotoxicity of anthracyclines.Methods:Randomized controlled trials(RCTs)were identified by searching China National Knowledge Infrastructure(CNKI),Wanfang Database,Chinese Biomedical Literature Database(CBM),Pub Med,Cochrane Library,and Embase Databases from the inceptions until December 2020.The Cochrane Handbook was used to evaluate the risk of bias in the included studies.Data analysis was conducted using Rev Man 5.3 software.Results:Totally 19 RCTs with 2,331 participants were included in this review.Results showed that in improving arrhythmia(13 RCTs,n=1,877,RR=0.37,95%CI 0.25 to 0.52,P<0.00001),the treatment group was superior to the control group.In terms of reducing left ventricular end-diastolic diameter(LVEDD,2 RCTs,n=128,MD=-0.79,95%CI-0.93 to-0.65,P<0.00001)and left ventricular end systolic diameter(LVESD,2 RCTs,n=128,MD=-0.58,95%CI-0.82 to-0.35,P<0.00001),the treatment group was also better than the control group.In reducing myocardial enzymes such as creatine kinase(CK)[(3 RCTs,n=256,SMD=-0.80,95%CI-1.16 to-0.44,P<0.0001),(2 RCTs,n=126,SMD=-0.62,95%CI-0.98 to-0.26,P=0.0007)],the treatment group was superior to the control group.Conclusion:Shengmai San has a positive effect on the treatment of cardiotoxicity from anthracyclines.However,in the future,it is still necessary to conduct high-quality RCTs to verify its efficacy.

目的研究右美托咪定复合瑞芬太尼对心内直视手术心肌缺血再灌注后血流动力学、心肌肌钙蛋白I(cTnI)和肌酸激酶-同工酶(CK-MB)的影响。方法选取2017年1月到2021年12月在郑州市第七人民医院接受心内直视手术的患者60例,根据随机数字表法,将患者分为对照组和研究组,各30例。对照组患者采用瑞芬太尼麻醉,而研究组患者采用右美托咪定联合瑞芬太尼麻醉,比较两组患者麻醉给药10 min后(T1)、切皮时(T2)、纵断胸骨时(T3)、体外循环(CPB)结束时(T4)和手术结束时(T5)心率和平均动脉压,比较两组患者手术结束时(T5)、体外循环(CPB)结束后12 h(T6)和体外循环(CPB)结束后24 h(T7)血浆cTnI、CK-MB水平。结果研究组患者麻醉效果良好率显著高于对照组患者(90.00%vs 66.67%,P<0.05)。研究组患者T1、T2、T3、T4和T5时间点心率和平均动脉压均显著低于对照组患者(P<0.05)。两组患者在T5、T6和T7时间点研究组患者血浆cTnI和CK-MB水平均显著低于对照组患者(P<0.05)。结论在心内直视手术中应用右美托咪定复合瑞芬太尼进行麻醉不仅可以取得较好的麻醉效果,而且保障患者在心肌缺血再灌注后血流动力学更稳定和心肌损伤更低。

Numerical investigation on thermomechanical behavior of alumina–calcium hexaluminate refractories for purging plug

Alumina–spinel refractories used in slit-type purging plugs are susceptible to cross-sectional damage,resulting in a serious mismatch between their service life and that of ladle.Alumina–calcium hexaluminate refractories have gradually become the new trend in purging plug materials with the development of refining technology.The thermomechanical damage of slit-type purging plugs with alumina–calcium hexaluminate refractory was investigated by the thermo-solid coupling simulation.Combined with the polynomial fitting and design of experiments methods,the influence of thermophysical parameters on temperature and thermal stress of alumina–calcium hexaluminate refractories for purging plugs was systematically analyzed.The results show that the maximum thermal stress of the purging plugs appears during the stages of steel transporting and stirring,and the vulnerable parts are located above Y=0.323 m.The thermal conductivity and the coefficient of thermal expansion of the material are the most sensitive parameters to the temperature and thermal stress inside the structure,respectively.The addition of more calcium hexaluminate can relieve the stress concentration and large deformation around the slits.Consequently,when the content of calcium hexaluminate is 47 wt.%and in the form of aggregate-binder,the temperature and thermal stress distribution inside the refractory are optimal,which can effectively improve the service life of the slit-type purging plug.

Dear Editor,Tuberculosis(TB)is an infectious disease which has been considered as a global public health emergency problem during the past 25y.The World Health Organization has estimated that approximately one-third of the world’s population are infected by TB.However,only 10%of patients with TB is symptomatic.TB can affect multiple organs throughout the body.Eighty percent of those infected are pulmonary TB.Ocular TB can occur with or without lung or other systemic TB.It is a rare extrapulmonary form of the disease and occupied about 3.5%–5%of all of the TB.It should not be disregarded as its potential influence on vision loss.The intraocular and/or extraocular structures could be impacted by ocular TB[1].Uvea was the most common affected structure[2].

Objective:This study explores the mechanism of action of Danhongqing formula(DHQ),a compoundbased Chinese medicine formula,in the treatment of cholestatic liver fibrosis.Methods:In vivo experiments were conducted using 8-week-old multidrug resistance protein 2 knockout(Mdr2-/-)mice as an animal model of cholestatic liver fibrosis.DHQ was administered orally for 8 weeks,and its impact on cholestatic liver fibrosis was evaluated by assessing liver function,liver histopathology,and the expression of liver fibrosis-related proteins.Real-time polymerase chain reaction,Western blot,immunohistochemistry and other methods were used to observe the effects of DHQ on long non-coding RNA H19(H19)and signal transducer and activator of transcription 3(STAT3)phosphorylation in the liver tissue of Mdr2-/-mice.In addition,cholangiocytes and hepatic stellate cells(HSCs)were cultured in vitro to measure the effects of bile acids on cholangiocyte injury and H19 expression.Cholangiocytes overexpressing H19 were constructed,and a conditioned medium containing H19 was collected to measure its effects on STAT3 protein expression and cell activation.The intervention effect of DHQ on these processes was also investigated.HSCs overexpressing H19 were constructed to measure the impact of H19 on cell activation and assess the intervention effect of DHQ.Results:DHQ alleviated liver injury,ductular reaction,and fibrosis in Mdr2-/-mice,and inhibited H19expression,STAT3 expression and STAT3 phosphorylation.This formula also reduced hydrophobic bile acid-induced cholangiocyte injury and the upregulation of H19,inhibited the activation of HSCs induced by cholangiocyte-derived conditioned medium,and decreased the expression of activation markers in HSCs.The overexpression of H19 in a human HSC line confirmed that H19 promoted STAT3 phosphorylation and HSC activation,and DHQ was able to successfully inhibit these effects.Conclusion:DHQ effectively alleviated spontaneous cholestatic liver fibrosis in Mdr2-/-mice by inhibiting H19 upregulation in chol

Thermoplastic polycarbonate polyurethanes(PCUs) are multiblock copolymers that have been applied for medical devices for long time. Aliphatic diols are common chain extenders(CE) involved in the composition of the hard segments of PCU. However, limited knowledge was discovered about how the chemical structure of CE affects the hydrogen bonding organization within PCUs and their mechanical properties.To investigate this problem, a group of PCUs were synthesized respectively by extending the polymer chain with 1,4-butanediol(BDO),aminoethanol(MEA), ethanediol(EO) as three kinds of chain extenders. Tiny differences in the CE chemical structure results in remarkable variations in phase separation, condensed morphologies, thermal and mechanical properties, which are characterized by Fourier transform infrared spectrometer, atomic force microscopy, small-angle X-ray scattering, differential scanning calorimetry, and tensile tests. The microstructural evolution during unilateral deformation and the different mechanism induced by the different CEs was probed and unveil by in situ wide-and small-angle X-ray diffraction. Symmetry of CE can improve the organization of the hydrogen bonding. The coherence strength of the urethane/urea group also plays a key role by comparing the two PCUs with ethanediol and aminoethanol.

The most critical part of a neutron computed tomography(NCT) system is the image processing algorithm,which directly affects the quality and speed of the reconstructed images.Various types of noise in the system can degrade the quality of the reconstructed images.Therefore,to improve the quality of the reconstructed images of NCT systems,efficient image processing algorithms must be used.The anisotropic diffusion filtering(ADF) algorithm can not only effectively suppress the noise in the projection data,but also preserve the image edge structure information by reducing the diffusion at the image edges.Therefore,we propose the application of the ADF algorithm for NCT image reconstruction.To compare the performance of different algorithms in NCT systems,we reconstructed images using the ordered subset simultaneous algebraic reconstruction technique(OS-SART) algorithm with different regular terms as image processing algorithms.In the iterative reconstruction,we selected two image processing algorithms,the Total Variation and split Bregman solved total variation algorithms,for comparison with the performance of the ADF algorithm.Additionally,the filtered back-projection algorithm was used for comparison with an iterative algorithm.By reconstructing the projection data of the numerical and clock models,we compared and analyzed the effects of each algorithm applied in the NCT system.Based on the reconstruction results,OS-SART-ADF outperformed the other algorithms in terms of denoising,preserving the edge structure,and suppressing artifacts.For example,when the 3D Shepp–Logan was reconstructed at 25 views,the root mean square error of OS-SART-ADF was the smallest among the four iterative algorithms,at only 0.0292.The universal quality index,mean structural similarity,and correlation coefficient of the reconstructed image were the largest among all algorithms,with values of 0.9877,0.9878,and 0.9887,respectively.

BACKGROUND Gallstones are common lesions that often require surgical intervention.Laparo-scopic cholecystectomy is the treatment of choice for symptomatic gallstones.Preoperatively,the anatomical morphology of the cystic duct(CD),needs to be accurately recognized,especially when anatomical variations occur in the CD,which is otherwise prone to bile duct injury.However,at present,there is no optimal classification system for CD morphology applicable in clinical practice,and the relationship between anatomical variations in CDs and gallstones remains to be explored.AIM To create a more comprehensive clinically applicable classification of the morphology of CD and to explore the correlations between anatomic variants of CD and gallstones.METHODS A total of 300 patients were retrospectively enrolled from October 2021 to January 2022.The patients were divided into two groups:The gallstone group and the nongallstone group.Relevant clinical data and anatomical data of the CD based on magnetic resonance cholangiopancreatography(MRCP)were collected and analyzed to propose a morphological classification system of the CD and to explore its relationship with gallstones.Multivariate analysis was performed using logistic regression analyses to identify the independent risk factors using variables that were significant in the univariate analysis.RESULTS Of the 300 patients enrolled in this study,200(66.7%)had gallstones.The mean age was 48.10±13.30 years,142(47.3%)were male,and 158(52.7%)were female.A total of 55.7%of the patients had a body mass index(BMI)≥24 kg/m2.Based on the MRCP,the CD anatomical typology is divided into four types:Type I:Linear,type II:n-shaped,type III:S-shaped,and type IV:W-shaped.Univariate analysis revealed differences between the gallstone and nongallstone groups in relation to sex,BMI,cholesterol,triglycerides,morphology of CD,site of CD insertion into the extrahepatic bile duct,length of CD,and angle between the common hepatic duct and CD.According to the multivariate analysis,female,BMI(≥24 kg/m2)
