Measures of Abdominal Adiposity and Risk of Stroke:A Dose-Response Meta-analysis of Prospective Studies

Objective Waist circumference, waist-to-hip ratio and waist-to-height ratio, which are the indicators or measures of abdominal adiposity, have long been hypothesized to increase the risk of stroke; yet evidence accumulated till date is not conclusive. Here, we conducted a dose-response meta-analysis to summarize evidences of the association between these measures of abdominal adiposity and the risk of stroke. Methods PubMed and Web of Science databases were searched from inception to May 2015. Two investigators independently conducted the study selection and data extraction. Dose-response relationships were assessed by the generalized least squares trend estimation, while the summary effect estimates were evaluated by the use of fixed- or random-effect models. Subgroup and sensitivity analyses were performed to assess the potential sources of heterogeneity and the robustness of the pooled estimation. Publication bias of the literature was evaluated using Begg＇s and Egger＇s test. Results Altogether 15 prospective cohort studies were identified in this study. The summary of relative risks （95% confidence intervals） of stroke for the highest versus the lowest categories was 1.28 （1.18-1.40） for waist circumference, 1.32 （1.21-1.44） for waist-to-hip ratio, and 1.49 （1.24-1.78） for waist-to-height ratio. For a 10-cm increase in waist circumference, the relative risk of stroke increased by 10%; for a 0.1-unit increase in waist-to-hip ratio, the relative risk increased by 16%; and for a 0.05-unit increase in waist-to-height ratio, the relative risk increased by 13%. There was evidence of a nonlinear association between waist-to-hip ratio and stroke risk, Pnonlinearity=0.028, Conclusion Findings from our meta-analysis indicated that waist circumference, waist-to-hip ratio, and waist-to-height ratio were positively associated with the risk of stroke, particularly ischemic stroke.

目的观察个体化孕期营养指导对妊娠期糖尿病患者糖脂代谢及脂肪因子的影响。方法选取2022年1月-2023年1月我院收治的76例妊娠期糖尿病患者为研究对象，采用随机数字表法分为对照组和观察组，各38例。对照组给予常规饮食指导及正常产检，观察组在此基础上给予个体化孕期营养指导。比较两组血糖指标、血脂指标[总胆固醇（TC）、甘油三酯（TG）、低密度脂蛋白胆固醇（LDL-C）、高密度脂蛋白胆固醇（HDL-C）]、孕期并发症、妊娠结局及焦虑和抑郁评分。结果两组空腹血糖、糖化血红蛋白、餐后2 h血糖均低于治疗前，且观察组低于对照组（P<0.05）；两组TC、TG、LDL-C、HDL-C均低于治疗前，且观察组低于对照组（P<0.05）；观察组孕期并发症发生率（10.53%）低于对照组（21.05%）(P<0.05)；观察组不良妊娠结局发生率为13.16%，低于对照组的28.95%(P<0.05)；两组焦虑、抑郁评分低于治疗前，且观察组低于对照组（P<0.05）。结论个体化孕期营养指导治疗对妊娠期糖尿病患者糖脂代谢及脂肪因子具有积极的促进作用，可降低其妊娠期并发症，改善妊娠结局和不良心理，具有较理想的效果。

Intralobar sequestration （ILS） is an uncommon abnormality that accounts for 75% of all pulmonary sequestrations. Over the years there have been several reports of various presenting signs of which hemoptysis was commonly described, however, massive hemoptysis and hemothorax is extremely rare in literature. We present a case of a 45-year-old man who died of fatal complication from an ILS. This case report shows an uncommon presentation of ILS with massive hemoptysis and hemothorax resulting in a dramatic course of disease and a fatal outcome, and for this reason in the absence of trauma or other causes for massive hemoptysis, hemothorax, or lung hematoma, this possibility should be kept in mind so as to avoid misdiagnosis, and resection of the sequestered tissue should be considered in all patients.

洪水预报是流域防洪减灾的重要非工程措施之一。目前我国中小河流暴雨洪水灾害频发,但应对短历时强降雨的洪水预报能力仍不强。以安徽省东部山区中小流域为研究对象,引入长短时记忆网络建立流域降雨径流模型,探讨其在山区中小流域的洪水模拟效果。结果表明,考虑降雨输入的空间差异可提升深度学习模型降雨径流模拟预测性能,且长短时记忆网络能够取得优于传统人工神经网络的精度;长短时记忆网络模型有效建立了流域降雨与径流间的复杂非线性关系,模型在所选流域内场次洪水的峰值模拟效果较好,训练、测试集场次洪水峰值合格率均在90%以上;长短时记忆网络内部结构特征与流域水文过程具有较好的相似性,对山区中小流域暴雨洪水非线性关系拟合效果突出。

An Efficient Multi-objective Approach Based on Golden Jackal Search for Dynamic Economic Emission Dispatch

APFA:Ameliorated Pathfinder Algorithm for Engineering Applications

Pathfinder algorithm(PFA)is a swarm intelligent optimization algorithm inspired by the collective activity behavior of swarm animals,imitating the leader in the population to guide followers in finding the best food *** algorithm has the characteristics of a simple structure and high ***,PFA faces challenges such as insufficient population diversity and susceptibility to local optima due to its inability to effectively balance the exploration and exploitation *** paper proposes an Ameliorated Pathfinder Algorithm called APFA to solve complex engineering optimization ***,a guidance mechanism based on multiple elite individuals is presented to enhance the global search capability of the ***,to improve the exploration efficiency of the algorithm,the Logistic chaos mapping is introduced to help the algorithm find more high-quality potential solutions while avoiding the worst ***,a comprehensive following strategy is designed to avoid the algorithm falling into local optima and further improve the convergence *** three strategies achieve an effective balance between exploration and exploitation overall,thus improving the optimization performance of the *** performance evaluation,APFA is validated by the CEC2022 benchmark test set and five engineering optimization problems,and compared with the state-of-the-art metaheuristic *** numerical experimental results demonstrated the superiority of APFA.

目的分析不孕症患者慢性子宫内膜炎(CE)与输卵管性疾病的相关性。方法回顾性分析2019年1月至2022年1月在潍坊医学院附属医院生殖医学中心因单纯输卵管因素不孕接受IVF/ICSI-ET助孕的761例不孕症患者资料。根据子宫输卵管造影(HSG)的结果,将患者分为输卵管积水组(积水组,n=174)、输卵管梗阻组(梗阻组,n=224)以及输卵管通而不畅组(对照组,n=363)。比较不同分组患者间CE的发生率;利用Logistic回归分析CE发病的危险因素。结果3组患者在异位妊娠史、盆腔炎性疾病史、宫腔镜诊断CE以及内膜活检CD138阳性方面差异有统计学意义(P<0.05);积水组、梗阻组患者中CE的患病率(分别为29.89%和29.02%)高于对照组(19.28%),且差异有统计学意义(P<0.0167);Logistic回归分析结果显示,输卵管梗阻[OR=1.84,95%CI(1.24,2.73),P<0.05]、输卵管积水[OR=1.92,95%CI(1.26,2.93),P<0.05]是CE发生的独立危险因素。结论输卵管梗阻、输卵管积水不孕患者中CE的患病率明显增高。因此,对于因输卵管因素行辅助生殖技术助孕的患者,建议在胚胎移植前行宫腔镜检查及子宫内膜活检术,排除CE对胚胎移植的影响,以提高其助孕的成功率。
