Perineural invasion of hilar cholangiocarcinoma in Chinese population: One center’s experience

BACKGROUND Hilar cholangiocarcinoma(HCCA)often produces perineural invasion(PNI)extending to extra-biliary sites,while significant confusion in the incidence of PNI in HCCA has occurred in the literature,and the mechanism of that procedure remains unclear.AIM To summarize the incidence of PNI in HCCA and to provide the distribution of nerve plexuses around hepatic portal to clinical surgeons.METHODS Reported series with PNI in HCCA since 1996 were reviewed.A clinicopathological study was conducted on sections from 75 patients with HCCA to summarize the incidence and modes of PNI.Immunohistochemical stains for CD34 and D2-40 in the cancer tissue were performed to clarify the association of PNI with microvessel and lymph duct.Sections of the hepatoduodenal ligament from autopsy cases were scanned and handled by computer to display the distribution of nerve plexuses around the hepatic portal.RESULTS The overall incidence of PNI in this study was 92%(69 of 75 patients),while the rate of PNI in HCCA in the literature ranging from 38%to 100%.The incidence of PNI did not show any remarkable differences among various differentiated groups and Bismuth-Corlette classification groups.Logistic regression analysis identified the depth of tumor invasion was the only factor that correlated significantly with PNI(P<0.01).In spite of finding tumor cells that could invade microvessels and lymph ducts in HCCA,we did not find tumor cells invaded nerves via microvessels or lymph ducts.Three nerve plexuses in the hepatoduodenal ligament and Glisson’s sheath were classified,and they all surrounded the great vessels very closely.CONCLUSION The incidence of PNI of HCCA in Chinese population is around 92%and correlated significantly with a depth of tumor invasion.It also should be considered when stratifying HCCA patients for further treatment.

Optimal Micro-PMU Placement for Improving State Estimation Accuracy via Mixed-integer Semidefinite Programming

Micro-phasor measurement units(μPMUs)with a micro-second resolution and milli-degree accuracy capability are expected to play an important role in improving the state estimation accuracy in the distribution network with increasing penetration of distributed generations.Therefore,this paper investigates the problem of how to place a limited number ofμPMUs to improve the state estimation accuracy.Combined with pseudo-measurements and supervisory control and data acquisition(SCADA)measurements,an optimalμPMU placement model is proposed based on a two-step state estimation method.The E-optimal experimental criterion is utilized to measure the state estimation accuracy.The nonlinear optimization problem is transformed into a mixed-integer semidefinite programming(MISDP)problem,whose optimal solution can be obtained by using the improved Benders decomposition method.Simulations on several systems are carried out to evaluate the effective performance of the proposed model.

Chronic obstructive pulmonary disease(COPD)is a heterogeneous disease characteristic of small airway inflammation,obstruction,and emphysema.It is well known that spirometry alone cannot differentiate each separate component.Computed tomography(CT)is widely used to determine the extent of emphysema and small airway involvement in COPD.Compared with the pulmonary function test,small airway CT phenotypes can accurately reflect disease severity in patients with COPD,which is conducive to improving the prognosis of this disease.CT measurement of central airway morphology has been applied in clinical,epidemiologic,and genetic investigations as an inference of the presence and severity of small airway disease.This review will focus on presenting the current knowledge and methodologies in chest CT that aid in identifying discrete COPD phenotypes.

Optimization of an annular wave energy converter in a wind-wave hybrid system

A hybrid system of a spar-type floating offshore wind turbine and a heaving annular wave energy converter(WEC)provides a promising solution for collocated ocean renewable energy exploitation.The performance of the hybrid system depends on the dimensions of the WEC.Here an optimization method is proposed to determine the outer radius and the draft of the WEC under the wave condition in a randomly chosen operational site.First,three candidate models are selected based on three operational conditions of energy harvest:(1)The natural frequency of the system is matched with the peak wave frequency in the target site(referred to as synchronized mode),where the wind turbine and the WEC nearly heave together in a near-resonance condition,(2)The natural frequency of the WEC is matched with the peak wave frequency(ring mode),(3)The maximum wave power is harnessed under the peak wave frequency(target mode).Then the candidate modes are evaluated to obtain an optimum.Results show that the extracted wave power under the above operational conditions has an upper bound that can hardly be surpassed by enlarging the dimensions of the WEC only.The optimal annual wave energy production is achieved in the synchronized mode because of the superior performance of WEC over a wide bandwidth of effective energy conversion.

Research and development of advanced structured packing in a rotating packed bed

As the core component of the rotating packing bed,packing is a place for efficient gas–liquid mixing and mass transfer.In this paper,a 3D structured packing composed of a mesh structure and a support structure was designed.The mesh structure is a ring-shaped mesh surrounded by triangular meshes,which is stable in structure and can achieve a high degree of dispersion and aggregation of the liquid phase.The support structure is composed of ring-shaped structural units arranged at a certain angle along the axial direction,which can enhance the turbulence of the airflow while constructing regular gas-phase channels.Circumferential steel meshes of different diameters and supporting structures are alternately combined to form 3D packing,which is loaded in a layered cross-flow rotating packing bed.The results show that under the same operating conditions,the mass transfer performance of 3D packing and wire mesh packing are equivalent,and both are better than pall ring packing.Moreover,the pressure drop of 3D packing is significantly lower than that of pall ring packing and wire mesh packing.The design and implementation of packing the development presented in this paper can be used to develop special structured packing for rotating bed,which can further improve the performance of rotating packed bed(RPB).

Hemophagocytic lymphohistiocytosis(HLH)is a life-threatening clinical syndrome characterized by aberrantly activated macrophages and cytotoxic T cells,leading to mul-tiple organ failure[1].The main manifestations included recurrent fever,cytopenia,liver dysfunction,and sepsis-like syndrome.Physicians should be fully aware of HLH since early diagnosis and management may prevent irreversible organ failure and subsequent death.Treatment protocols,such as HLH-94 based on etoposide(VP 16),are commonly used in clinical practice,especially in familial HLH[2]and malignancy-associated HLH[3].

Radioactive 125I seed implantation for pancreatic cancer with unexpected liver metastasis:A preliminary experience with 26 patients

BACKGROUND Preoperative diagnosis rate of pancreatic cancer has increased year by year.The prognosis of pancreatic cancer patients with unexpected liver metastasis found by intraoperative exploration is very poor,and there is no effective and unified treatment strategy.AIM To evaluate the therapeutic effect of radioactive 125I seed implantation for pancreatic cancer patients with unexpected liver metastasis.METHODS The demographics and perioperative outcomes of patients who underwent 125I seed implantation to treat pancreatic cancer with unexpected liver metastasis between January 1,2017 and June 1,2019 were retrospectively analyzed.During the operation,125I seeds were implanted into the pancreatic tumor under the guidance of intraoperative ultrasound,with a spacing of 1.5 cm and a row spacing of 1.5 cm.For patients with obstructive jaundice and digestive tract obstruction,choledochojejunostomy and gastroenterostomy were performed simultaneously.After operation,the patients were divided into a non-chemotherapy group and a chemotherapy group that received gemcitabine combined with albumin-bound paclitaxel treatment.RESULTS Preoperative imaging evaluation of all patients in this study showed that the tumor was resectable without liver metastasis.There were 26 patients in this study,including 18 males and 8 females,aged 60.5±9.7 years.The most common tumor site was the pancreatic head(17,65.4%),followed by the pancreatic neck and body(6,23.2%)and pancreatic tail(3,11.4%).Fourteen patients(53.8%)underwent palliative surgery and postoperative pain relief occurred in 22 patients(84.6%).The estimated blood loss in operation was 148.3±282.1 mL and one patient received blood transfusion.The postoperative hospital stay was 7.6±2.8 d.One patient had biliary fistula,one had pancreatic fistula,and all recovered after conservative treatment.After operation,7 patients received chemotherapy and 19 did not.The 1-year survival rate was significantly higher in patients who received chemotherapy than in those who did not(68.6%vs 15.

Developing electrochemical energy storage devices with high energy and power densities,long cycling life,as well as low cost is of great significance.Hybrid metal-ion capacitors(MICs),commonly consisting of high energy battery-type anodes and high power capacitor-type cathodes,have become a trade-off between batteries and supercapacitors.Tremendous efforts have been devoted to searching for high-performance electrode materials due to poor rate capability of anodes,low capacity of cathodes,and interior sluggish kinetic match.Carbon materials with large surface area,good electrical conductivity and stability have been considered to be ideal candidates for electrodes of MICs.In this review,the advanced carbon materials directly as cathodes and anodes of MICs are systematically summarized.Then,the key structural/chemical factors including the structure engineering,porous characteristics,and heteroatom incorporation for improving electrochemical performance of carbon materials are highlighted.Additionally,the challenges and opportunities for future research on carbon materials in MICs are also proposed.

Model of Underground Ant Nest Structure Using Static and Dynamic Finite Element Analysis

This paper focuses on the structural characteristics of ant nests,which are complex structures.Natural underground ant nests generally have good air circulation,pressure resistance, waterproof properties,thermal insulation and a favorable temperature and relative humidity. Additionally,ant nests are often surrounded by trees and other natural barriers.In this study, the natural underground ant nests of Iridomyrmex anceps were gathered from different collection sites.Manual cutting and frozen computer numerical control milling were performed on the ant nests in a laboratory.The internal structure of each nest was measured and recorded,and then, the 2D and 3D numerical structure models of the Iridomyrmex anceps nest were created.The static and dynamic simulation analysis of an underground ant nest structure was performed by using finite element analysis software (ABAQUS),and the mechanical properties of the ant nest were discussed.The underground ant nest structure effectively resisted the additional stress due to external static and live loads,and the ant nest was not completely destroyed.

Enhanced energy density in polyetherimide nanocomposite film at high temperature induced by electrospun BaZrTiO_(3) nanofibers

Polymer dielectrics which possess excellent dielectric properties such as high breakdown strength,flexibility,and facile processability are considered as promising materials for advanced electrostatic capacitors.However,most dielectric polymers have unsatisfactory energy storage performances at high-temperature environments.Here,polyetherimide(PEI) nanocomposite films contained with electrospun Ba(Zr_(0.79)Ti_(0.21))O_(3) nanofibers(BZTNFs) are fabricated by common solution casting method.The dielectric properties,especially the breakdown strength of the BZTNFs/PEI nanocomposites,are characterized,yet improvement is only in the small loading ones.The energy storage performance of the 0.5 vol% and1.0 vol% BZTNFs content nanocomposite is further investigated from 25 to 150℃.With the introduction of small loading BZTNFs,the dielectric permittivity and electric displacement of the nanocomposite are improved at all evaluated temperatures.The 1.0 vol% BZTNFs/PEI possesses a maximal discharged energy density of6.05 J·cm^(-3) with high efficiency of 94.9% at 25℃,then falls to 3.34 J·cm^(-3) with efficiency of 54.6% at 150℃ for the larger remnant displacement.Apparently,the relaxation ferroelectric nanofller of BZTNFs is much effective in increasing the dielectric permittivity of nanocomposite,but its capacity to restrict the migration of the charge carriers at high temperatures is weaker than that of the nanofillers with wider bandgap.The complementation of both kinds of the nanofillers probably provides an approach to available high-temperature dielectric films.
