BACKGROUND Esophagojejunal anastomotic leakage(EJAL)is a serious and potentially crucial complication of total gastrectomy and represents the major cause of postoperative death,with a mortality rate of up to 50%.However,treatment remains challenging and *** report here the case of a patient whose intrathoracic EJAL was successfully treated with computer tomography(CT)-guided negative pressure drainage *** SUMMARY A 69-year-old male patient complained of difficulty swallowing within the last six *** was diagnosed with esophagogastric junction carcinoma,Siewert II,cT3N0M0 stage *** gastrectomy and Roux-en-Y esophagojejunostomy were *** fever,left chest pain and dyspnea appeared on postoperative day 5,and EJAL was confirmed by CT,gastroscopy and oral blue-dimethylene *** treatment measures were applied immediately,including antibiotics,nasojejunal tubes,and repeated thoracic puncture and drainage under ultrasound ***,without sufficient and effective drainage,the thoracic infection and systemic condition continued to *** the cooperation of multiple departments,percutaneous CT-guided drainage(24 Fr 7 mm)in the thoracic cavity was successfully placed near the anastomotic *** of continuous negative pressure suction,the infection symptoms were effectively controlled and the general situation gradually *** follow-up examination showed that the patient was in good *** Negative pressure drainage via CT may represent an effective minimally invasive approach to treating intrathoracic EJAL.

In this paper, a new pressure reducing valve （PRV） with an orifice plate is proposed. The main objective is to explain the mechanisms of pressure reduction and energy conversion in the new PRV. A numerical simulation method was used to investigate the PRV internal flow field and to analyze the throttling effects of the orifice plate and the transform of thermal parameters as outlet pressure, outlet temperature, velocity, and superheat. A structure improvement method for the valve body and orifice plate is put forward to reduce energy loss. The governing equations for internal flow numerical simulation are composed of the continuity, momentum, energy and k-e transport equations, based on isotropic eddy viscosity theory. Different valve plug displacement models were built to describe the double throttling process. Our analysis shows that the steam pressure drops twice and the degree of superheat increases. There are also lots of eddies which clog the flow channel and disturb the steam flow in the valve cavity after the valve plug and the outlet cavity. After modifying the structure, the numerical results show a better performance of steam flow.

In fusion reactor or spallation neutron source environment,damage of tungsten components begins at the *** enhance the surface toughness,strength,and hardness of tungsten,a nano-to-micro sc ale lamellar gradient surface layer(thickness up to~50μm)was produced on as-rolled pure tungsten using a dry-sliding surface *** dry-sliding temperature and time were established by observing the grain-size evolution from room temperature to 750℃and the coefficient of friction on the surface layer after sliding from 10 to 50 *** grains changed into a slender fibrous structure at 250℃,which is higher than the ductile-brittle transition temperature but lower than the dynamic recrystallization temperature(750℃).The coefficient of friction of the surface layer decreased with sliding time and stabilized at approximately 50 min,indicating near-s aturation of the surface *** repetitive thermal loads of 10,20,and 30 MW·m^(-2)on the gradient modified layer,it exhibited mainly recrystallization and grain *** proportion of low-angle grain boundaries(<10°)reached50.6,42.2,and 17.3%,respectively,and the hardness decreased to 602,582,and 488HV.A heat load of 30 MW·m^(-2)was the threshold for the fast grain *** with unmodified tungsten,the recrystallization and grain growth are relatively ***,the gradient layer could suppress the formation of pits and protrusions in the as-rolled tungsten sample.

AIM： To observe the effects of acid and bile acid exposure on cell proliferation and the expression of extracellular signal-regulated protein kinase （ERK） and peroxisome proliferator-activated receptor γ （PPARγ） in normal human esophageal epithelial cells in vitro. METHODS： In vitro cultured normal human esophageal epithelial cells were exposed to acidic media （pH 4.0-6.5）, media containing different bile acid （250 μmol/ L）, media containing acid and bile acid, respectively. Cell proliferation was assessed using MTT and flow cytometry. The expressions of phosphorylated ERK1/2 and PPAR~ protein were determined by the immunoblotting technique. RESULTS： Acid-exposed （3 min） esophageal cells exhibited a significant increase in proliferation ratio, S phase of the cell cycle （P〈0.05） and the level of phosphorylated ERK1/2 protein. When the acid-exposure period exceeded 6 min, we observed a decrease in proliferation ratio and S phase of the cell cycle, with an increased apoptosis ratio （P〈0.05）. Bile acid exposure （3-12 min） also produced an increase in proliferation ratio, S phase of the cell cycle （P〈0.05） and phosphorylated ERKx/2 expression. On the contrary, deoxycholic acid （DCA） exposure （〉 20 rain） decreased proliferation ratio. Compared with bile acid exposure （pH 7.4）, bile acid exposure （pH 6.5, 4） significantly decreased proliferation ratio （P〈0.05）. There was no expression of PPARy in normal human esophageal epithelial ***： The rapid stimuli of acid or bile acid increase proliferation in normal human esophageal epithelial cells by activating the ERK pathway.

Providing accurate predictions of extreme water levels through numerical simulation has become essential for disaster prevention and damage mitigation in coastal wetland *** study applies the FVCOM model to simulate storm surges caused by several typhoons in the Bohai Sea and the North Huanghai *** vegetation drag force caused by salt marsh plants is inserted into the FVCOM model for model improvement with vegetation effect by integrating RS and GIS technologies.A parametric typhoon model is coupled with background wind fields derived to acquire the spatio-temporal variations of wind and pressure fields in the computational *** simulation results reproduce the extreme storm surges induced by typhoon events very *** modeling results are compared by validating with literature results to examine the effect of vegetation on tidal waves in tidal mud ***,the coupled model is also applied to explore storm surge attenuation and land intrusion during Typhoon Winnie in the wetlands of the Liao River *** simulation results indicate that salt marsh plants can reduce the flow current with little impact on tide flooding/ebbing in vegetated ***,the results show that typhoon presence increases the inundation depth and extendes the flood time in the tidal wetlands of the study *** FVCOM model incorporating the method with vegetation drag force can provide new insights to understand the comprehensive impact of tidal wetland plants on hydrodynamic characteristics in the Bohai Sea and other waters,hence presents a more accurate quantification of the hydrological process of storm surge in the tidal wetlands.

A new resonance scattering method was proposed for the determination of chlorite, basing on the resonance scattering effect of rhodamine dye. In HCl-sodium acetate buffer solution, chlorite oxidizes 1- into 12 and the reaction of 12 and excess 1- results in If. It is respectively combined with rhodaminc dyes, including rhodamine B （RhB）, butyl rhodamine B （b-RhB）, rhodamine G （RhG） and rhodamine S （RhS）, to form association complex particles, which exhibit stronger resonance scattering （RS） effect at 400 nm. The chlorite concentration of ClO2 in the range of 0.00726-0.218 μg/ml, 0.0102-0.292 μg/ml, 0.00726-0.145 μg/ml and 0.0290- 0.174 μg/ml is respectively linear to the RS intensity of association complex particle systems at 400 nm for the RhB, b-RhB, RhG and RhS. The detection limits of the four systems were respectively 0.00436, 0.00652, 0.00580 and 0.01450μg/ml ClO2^-. In the four systems, the RhB system possesses good stability and high sensitivity. It has been applied to the analysis of chlorite in wastewater with satisfactory results.
