BACKGROUND Addison’s disease(AD)is a rare but potentially fatal disease in Western countries,which can easily be misdiagnosed at an early *** adrenal tuberculosis(TB)may lead to depression in *** SUMMARY We report a case of primary adrenal insufficiency secondary to adrenal TB with TB in the lungs and skin in a 48-year-old *** patient was misdiagnosed with depression because of her depressed *** had hyperpigmentation of the skin,nails,mouth,and *** final diagnosis was adrenal TB that resulted in the insufficient secretion of adrenocortical *** hormone test,skin biopsy,T cell spot test of TB,and adrenal computed tomography scan were used to confirm the *** patient’s condition improved after hormone replacement therapy and TB *** Given the current status of TB in high-burden countries,outpatient doctors should be aware of and pay attention to TB and understand the early symptoms of AD.

BACKGROUND Acute decompensation(AD)of cirrhosis is associated with high short-term mortality,mainly due to the development of acute-on-chronic liver failure(ACLF).Thus,there is a need for biomarkers for early and accurate identification of AD patients with high risk of development of ACLF and *** triggering receptor expressed on myeloid cells-1(sTREM-1)is released from activated innate immune cells and correlated with various inflammatory *** To explore the prognostic value of sTREM-1 in patients with AD of *** A multicenter prospective cohort of 442 patients with cirrhosis hospitalized for AD was divided into a study cohort(n=309)and validation cohort(n=133).Demographic and clinical data were collected,and serum sTREM-1 was measured at *** enrolled patients were followed-up for at least 1 *** In patients with AD and cirrhosis,serum sTREM-1 was an independent prognosis predictor for 1-year survival and correlated with liver,coagulation,cerebral and kidney failure.A new prognostic model of AD(P-AD)incorporating sTREM-1,blood urea nitrogen(BUN),total bilirubin(TBil),international normalized ratio(INR)and hepatic encephalopathy grades was established and performed better than the model for end-stage liver disease(MELD),MELD-sodium(MELD-Na),chronic liver failure-consortium(CLIF-C)ACLF and CLIF-C AD ***,sTREM-1 was increased in ACLF and predicted the development of ACLF during first 28-d *** ACLF risk score incorporating serum sTREM-1,BUN,INR,TBil and aspartate aminotransferase levels was established and significantly superior to MELD,MELD-Na,CLIF-C ACLF,CLIF-C AD and P-AD in predicting risk of ACLF *** Serum sTREM-1 is a promising prognostic biomarker for ACLF development and mortality in patients with AD of cirrhosis.

婴儿运动发育影响因素和交互作用分析的巢式病例对照研究

目的分析婴儿运动发育影响因素的层次关系和交互作用，为婴儿运动发育迟缓的早期预测和防治提供科学依据。方法通过“北京市妇幼信息系统”儿童保健系统综合查询模块，导出并整理2020年1月1日—2021年8月31日在北京市通州区21家社区卫生服务中心进行“0～1岁运动发育筛查”的婴儿相关体检数据，应用巢式病例对照研究的方法，将筛查结果“阳性”的416例婴儿作为病例组，队列中按照婴儿月龄1∶1匹配选择筛查结果“阴性”的416例婴儿作为对照组。通过χ2检验、Logistic回归分析、Exhaustive CHAID分类树模型分析婴儿运动发育的影响因素和可能存在的交互作用，并计算相加和相乘交互作用。结果多因素Logistic回归显示：出生体重<2 500g（OR=3.28）、剖宫产（OR=1.63）、户外活动时长≤1h/d（OR=3.07）,未补充维生素D剂（OR=2.68）是婴儿运动发育筛查阳性的危险因素（P<0.01）。Exhaustive CHAID分类树分层显示，低出生体重是运动发育筛查阳性的首要危险因素，其次是户外活动时长、服用维生素D情况和分娩方式等。交互作用分析显示，未补充维生素D剂与低出生体重对婴儿运动发育筛查阳性存在相乘交互作用（OR=13.76,95%CI:1.20～158.08）和相加交互作用（RERI=10.22,95%CI:5.05～15.38）。结论应关注低出生体重、剖宫产婴儿的早期运动发育，重视其维生素D补充剂量和户外活动时长，加强婴儿运动发育的早期筛查和分层管理。

【目的】探讨乙型肝炎病毒(HBV)暴露儿童免疫后血清学检测(PVST)的重要性,并分析影响PVST进展的因素。【方法】以2020年1月1日—2022年3月31日在北京市各助产机构分娩且居住在北京市通州区的乙型肝炎表面抗原(HBsAg)阳性产妇及其所生活产儿为研究对象,由助产机构和社区卫生服务中心于儿童接种完3针乙型肝炎疫苗(HepB)后1~2个月进行随访。【结果】北京市通州区新生儿乙型肝炎免疫球蛋白(HBIg)接种率为100.00%(800/800),PVST成功随访率为85.88%(687/800),初次免疫无应答率为0.29%(2/687),但重新免疫后均成功。乙型肝炎母婴传播率为0。未进行PVST的儿童占14.13%(113/800),主要原因有受新型冠状病毒感染(COVID⁃19)疫情影响推迟检测;家长觉得幼儿年龄过小,拒绝静脉采血;幼儿跟随父母迁回原籍导致失访。logistic回归分析显示,高危儿(OR=30.009,P=0.001)、家庭居住北京(OR=2.218,P=0.002)和母亲<35岁(OR=1.687,P=0.020)的HBV暴露儿童PVST比例更高。【结论】PVST对于评估新生儿接种HepB后的HBV免疫应答状态是必要的。COVID⁃19疫情下,HBV暴露儿童PVST受影响。加强非高危儿、居住北京外、母亲≥35岁HBV暴露儿童管理,可能会提高通州区PVST率。

当代青年与短视频共融共生,短视频的价值表达深刻影响着当代青年的价值观。短视频改写了传统价值表达范式,促成了价值表达主体和客体的平等化,短视频因此具有了青年价值观教育的积极效应,但同时在显性和隐性两层面对青年价值观教育造成了不同程度的负面影响。青年短视频使用中的价值困境治理,需要结合短视频特性针对青年实际确定理性的治理原则,以社会主义核心价值观为基本内容,分别通过法律约束、技术赋能、平台监控以及青年自律等方式实现对不良短视频的有效规避。
