Long-term tadalafil once daily in Chinese men with erectile dysfunction:a 2-year final analysis of a post-marketing,multicenter,randomized,open-label trial

The long-term safety and effectiveness of once-daily tadalafil is crucial,but limited data are available in Chinese patients witherectile dysfunction(ED).In this post-marketing,multicenter,randomized,open-label trial with 2-year follow-up,635 ED caseswere randomized to receive daily oral tadalafil 2.5 mg or 5 mg for 3 months,of whom 580 continued once-daily tadalafil 5 mg for21 ***-emergent adverse events in the 12-month and 24-month period were similar,with the most common beingviral upper respiratory tract infection,upper respiratory tract infection,and *** improvement from baseline inthe International Index of Erectile Function-Erectile Function(IIEF-EF)score was detected at month 12(least squares mean[LSM]change:7.9,95%confidence interval[CI]:7.5–8.4,P<0.001)and was maintained to month 24(LSM change:8.6,95%CI:8.1–9.0,P<0.001).The proportions of patients regaining normal erectile function(IIEF-EF score≥26)were 43.7%and 48.0%at months 12 and 24,*** Assessment Questionnaire results showed improved erection function in 97.5%ofpatients and improved ability to engage in sexual activity in 95.9%of patients at month 12;these values were 96.1%and 95.0%at month 24,*** quality of sexual life score based on the Sexual Life Quality Questionnaire(SLQQ)was increased by52.2%at month 12 and by 55.3%at month 24(both P<0.001).The treatment satisfaction score determined by SLQQ(mean±standard deviation)was 62.4±21.0 at month 12 versus 65.9±20.2 at month ***-year daily application of tadalafil 5mg in Chinese men with ED showed a favorable safety profile and durable improvement in sexual performance and satisfaction.

Diagnosis and evaluation of gastric cancer by positron emission tomography

Gastric cancer is the second leading cause of cancer mortality worldwide. The diagnosis of gastric cancer has been significantly improved with the broad availability of gastrointestinal endoscopy. Effective technologies for accurate staging and quantitative evaluation are still in demand to merit reasonable treatment and better prognosis for the patients presented with advanced disease. Preoperative staging using conventional imaging tools, such as computed tomography (CT) and endoscopic ultrasonography, is inadequate. Positron emission tomography (PET), using 18F-fluorodeoxyglucose (FDG) as a tracer and integrating CT for anatomic localization, holds a promise to detect unsuspected metastasis and has been extensively used in a variety of malignancies. However, the value of FDG PET/CT in diagnosis and evaluation of gastric cancer is still controversial. This article reviews the current literature in diagnosis, staging, response evaluation, and relapse monitoring of gastric cancer, and discusses the current understanding, improvement, and future prospects in this area.

Comparison of Topping-off and posterior lumbar interbody fusion surgery in lumbar degenerative disease： a retrospective study

Background Topping-off surgery is a newly-developed surgical technique which combines rigid fusion with an interspinous process device in the adjacent segment to prevent adjacent segment degeneration. There are few reports on Topping-off surgery and its rationality and indications remains highly controversial. Our study aims to investigate the short-term and mid-term clinical results of Topping-off surgery in preventing adjacent segment degeneration when mild or moderate adjacent segment degeneration existed before surgery. Methods The 25 cases that underwent L5-$1 posterior lumbar interbody fusion （PLIF）＋ L4-L5 interspinous process surgeries between April 2008 and March 2010 formed Topping-off group. The 42 cases undergoing L5-$1 PLIF surgery formed PLIF group. Both groups matched in gender, age, body mass index and Pfirrmann grading （4 to 6）. The patients were evaluated with visual analogue scale （VAS） and Japanese orthopaedic association （JOA） scores before surgery and in the last follow-up. Modic changes of endplates were recorded. Results The follow-up averaged 24.8 and 23.7 months. No symptomatic or radiological adjacent segment degeneration was observed. There was no significant difference in intraoperative blood loss or postoperative drainage. VAS and lumbar JOA scores improved significantly in both groups （t=-12.1 and 13.5, P 〈0.05）. Neither anterior nor posterior disc height was significantly changed. Segmental lordosis of L4-L5 and total lordosis were all increased significantly （Topping-off group： t=-2.30 and -2.24, P 〈0.05; PLIF group： t=--2.76 and -1.83, P 〈0.01）. In the hyperextension and hyperflexion view, Topping-off group＇s range of motion （ROM） and olisthesis in the L4-L5 segment did not significantly change in flexion, but decreased in extension. In PLIF group, ROM （t=--7.82 and -4.90, P 〈0.01） and olisthesis （t=--15.67 and -18.58, P 〈0.01） both significantly increased in extension and flexion. Changes of Hemoglobin and Hematocrit in Elderly Patients Receiving Lower Joint Arthroplasty without AIIogeneic Blood Transfusion

Background：It has rarely been reported about the changes of hemoglobin （Hb） and hematocrit （Hct） in elderly patients receiving total knee arthroplasty （TKA） or total hip arthroplasty （THA）.This study aimed to evaluate the changes of Hb and Hct after TKA or THA in elderly patients,and analyze its relationship with sex and type of arthroplasty.Methods：This is a prospective cohort study,including 107 patients receiving TKA or THA without allogeneic blood *** were 54 males and 53 females,with a mean age of 69.42 *** of Hb and Hct were examined preoperatively and during the 6 months follow-up after operation.Results：Levels of Hb and Hct decreased postoperatively and reached their minimum points on postoperative day ***,Hb and Hct recovered to their preoperative levels within 6-12 *** significant differences in the levels of Hb and Hct were noticed between different *** patients showed significantly greater drop in Hb and Hct than TKA patients in the first 4 days postoperatively （P 〈 0.05）.Conclusions：Levels of Hb and Hct decreased during the first 4 days after arthroplasty and gradually returned to their normal levels within 6-12 weeks *** may be associated with higher postoperative blood loss than TKA.

Once-daily tadalafil administration has been well established; however, studies about tadalafil once-daily treatment in the Chinese population are lacking. In this phase 4, postmarketing study, we ascertained the long-term safety and effectiveness of tadalafil 2.5 mg and 5.0 mg once daily in Chinese men with erectile dysfunction （n = 635）. The primary endpoint of the study was safety at 12 months as assessed by the proportion of patients experiencing at least one treatment-emergent adverse event （serious or nonserious）. The secondary endpoints included safety and effectiveness, measured by the International Index of Erectile Function-Erectile Function （IIEF-EF） domain scores. Similar adverse events to the known safety profile of tadalafil, such as nasopharyngitis, upper respiratory tract infection, headache, and dizziness, were detected. No new cardiovascular safety concerns were observed. After 3 months of treatment, significant increases in IIEF-EF domain scores were detected for both 2.5-mg （least squares [LS] mean change： 6.3; 95% confidence interval [CI]： 5.4-7.1; P 〈 0.001） and 5.0-mg （LS mean change： 7.4; 95% CI： 6.8-7.9; P 〈 0.001） tadalafil doses, and significance was maintained up to 12 months. In addition, approximately 40% of patients regained normal erectile function （IIEF-EF 〉26） following 1 year of tadalafil once-daily treatment. The findings in this study provide evidence for the extended effectiveness and tolerability of tadalafil, demonstrating no new safety concerns, in a Chinese population and make once-daily tadalafil administration a viable option for improving sexual performance and satisfaction in Chinese men with erectile dysfunction.

A Comparative Study of the Power-law Relationship between the Pulse width and Energy of Precursor and Main Burst

In gamma-ray burst prompt emission,there is still no consistent conclusion if the precursor and main burst share the same *** this paper,we try to study this issue based on the relationship between pulse width and energy of the precursor and main *** systematically search the light curve data observed by Swift/BAT and Fermi/GBM,and find 13 long bursts with well-structured precursors and main *** fitting the precursor light curve of each different energy channel with the Norris function,we find that there is not only a power-law relationship between precursor width and energy,but also a power-law relationship between the ratio of the rising width to the decaying width and *** comparing the relationship between the precursors and the main burst pulses,we find that the distribution of the precursors and the relationship between the power-law indices are roughly the same as those of the main *** addition,it is found that the precursor width distribution as well as the upper limit of the pulse width ratio does not exceed 1 and both are asymmetric,which are also consistent with the main *** indicate that the precursor and the main burst are indistinguishable,and the precursor and the main burst may have the same physical origin.

The abused ciprofloxacin antibiotics have caused significant environmental *** oxidative degradation of ciprofloxacin exhibits promising efficacy,it often entails excessive energy ***,it is necessary to explore an effective and ecologically sustainable degradation ***,we demonstrated that g-C_(3)N_(4) decorated with the coordinated CeO_(2)and Co_(3)O_(4)(CeO_(2)-Co_(3)O_(4)/CN)exhibited effective ciprofloxacin photodegradation via in situ H_(2)O_(2) production and activation *** indicate that the introduced CeO_(2) enhances the transference of photogenerated electrons to O_(2) by adjusting the oxygen vacancy of photocatalyst,thereby increasing the generation of superoxide radicals,which in turn generate H_(2)O_(2),resulting in a 22.4-fold increase in H_(2)O_(2) generation over g-C_(3)N_(4).Moreover,the in situ H_(2)O_(2)generation facilitated by CeO_(2) is confirmed to be essential for ciprofloxacin degradation via CeO_(2)-Co_(3)O_(4)/CN,as it provides enough oxidant for Co_(3)O_(4) to activate into hydroxyl radicals for the pollutants ***,CeO_(2)-Co_(3)O_(4)/CN achieves a ciprofloxacin degradation ratio of 97.7%within 80 *** study introduces a novel approach that combines H_(2)O_(2) generation and activation,offering an innovative perspective for achieving clean and efficient purification of antibiotic-contaminated water.

Butterfly taxonomic and functional diversity in the urban green spaces of Hefei city

Urbanization has profound impacts on ecological environments. Green spaces are a vital component of urban ecosystems and play a crucial role in maintaining ecological balance and enhancing sustainability. This study aimed to investigate the community composition characteristics of butterflies in urban green spaces within the context of rapid urbanization. Simultaneously, it explored the status and differences in butterfly taxonomic diversity, functional diversity, and functional traits among different types of urban green spaces, regions, and urban gradients to provide relevant insights for further improving urban green space quality and promoting biodiversity conservation. We conducted a year-long survey of 80 green spaces across different urban regions and ring roads within Hefei City, Anhui Province, with monthly sampling intervals over 187 transects. A total of 4822 butterflies, belonging to 5 families, 17 subfamilies, 40 genera, and 55 species were identified. The species richness, Shannon, Simpson, functional richness, and Rao's quadratic entropy indices of butterflies in urban park green spaces were all significantly higher than those in residential and street green spaces (P < 0.05). Differences in butterfly diversity and functional traits among different urban regions and ring roads were relatively minor, and small-sized, multivoltine, and long flying duration butterflies dominated urban green spaces. Overall, these spaces offer more favorable habitats for butterflies. However, some residential green spaces and street green spaces demonstrate potential for butterfly conservation.

Effect of Selective Dorsal Neurectomy on Erectile Function in Rats

Objective This study aimed to investigate the effect of penile selective dorsal neurectomy(SDN)on erectile function in *** Twelve adult male Sprague-Dawley rats(15 weeks old)were divided into three groups(n=4 per group):in control group,rats received no treatment;in sham group,rats underwent a sham operation;in SDN group,rats underwent SDN with half of the dorsal penile nerve *** mating test was performed,and the intracavernous pressure(ICP)assessed six weeks after the surgical *** At postoperative six weeks,the mating test revealed no significant difference in mounting latency and mounting frequency among the three groups(P>0.05),while the ejaculation latency(EL)was significantly longer and ejaculation frequency(EF)lower in the SDN group than in the control and sham groups(P0.05).Conclusion SDN does not adversely affect the erectile function and sexual desire of rats,and at the same time it can reduce EL and EF,providing an application basis for SDN in the clinical treatment of premature ejaculation.
