The backreaming operation plays a significant role in safe drilling for horizontal wellbores, while it may cause severe stuck pipe accidents. To lower the risk of the stuck pipe in backreaming operations, the mechanism of cuttings transport needs to be carefully investigated. In this research, a transient cuttings transport with multiple flow patterns model is developed to predict the evolution of cuttings transported in the annulus while backreaming. The established model can provide predictions of the distribution of cuttings bed along the wellbore considering the bulldozer effect caused by large-size drilling tools(LSDTs). The sensitivity analyses of the size of LSDTs, and backreaming operating parameters are conducted in Section 4. And a new theory is proposed to explain the mechanism of cuttings transport in the backreaming operation, in which both the bit and LSDTs have the “cleaning effect” and “plugging effect”.The results demonstrate that the cuttings bed in annuli is in a state of dynamic equilibrium, but the overall trend and the distribution pattern are obvious. First, larger diameters and longer drilling tools could lead to a higher risk of the stuck pipe. Second, we find that it is not the case that the higher flow rate is always better for hole cleaning, so three flow-rate intervals are discussed separately under the given conditions. When the “dangerous flow rate”(maller flow rate instead facilitates borehole cleaning. If the flow rate is large enough to be in the “high flow rate” interval(>35 L/s in Case 4),the higher the flow rate, the better the cleaning effect of cuttings beds. We report the discovery of a peculiar X-ray transient,EP240408a,by Einstein Probe （EP） and follow-up studies made with EP,Swift,NICER,GROND,ATCA and other ground-based multiwavelength *** new transient was first detected with Wide-field X-ray Telescope （WXT） on board EP on April 8th,2024,manifested in an intense yet brief X-ray flare lasting for *** flare reached a peak flux of 3.9×10-9erg cm-2s-1in 0.5-4 keV,300 times brighter than the underlying X-ray emission detected throughout the *** and more precise follow-up observations by EP/FXT,Swift and NICER confirmed the finding of this new *** X-ray spectrum is non-thermal in 0.5-10 keV,with a power-law photon index varying *** X-ray light curve shows a plateau lasting for4d,followed by a steep decay till becoming undetectable10 d after the initial *** on its temporal property and constraints from previous EP observations,an unusual timescale in the range of 7-23 d is found for EP240408a,which is intermediate between the commonly found fast and long-term *** counterparts have been found in optical and near-infrared,with the earliest observation at 17 h after the initial X-ray detection,suggestive of intrinsically weak emission in these *** demonstrate that the remarkable properties of EP240408a are inconsistent with any of the transient types known so far,by comparison with,in particular,jetted tidal disruption events,gamma-ray bursts,X-ray binaries and fast blue optical *** nature of EP240408a thus remains an *** suggest that EP240408a may represent a new type of transients with intermediate timescales of the order of10 *** detection and follow-ups of more of such objects are essential for revealing their origin.

目的总结深圳市新生儿脑卒中发病率、临床特征及预后等情况,提高脑卒中新生儿的早期诊治能力。方法由深圳市儿童医院牵头,收集2020年1月—2022年12月深圳新生儿数据协作网组织21家单位收治的新生儿脑卒中36例,分析深圳市新生儿脑卒中发病率、临床特征、治疗、预后等情况。结果深圳市21家医院2020、2021、2022年新生儿脑卒中的发病率分别为1/15137、1/6060、1/7704。缺血性脑卒中占75%(27/36),男性患儿占64%(23/36)。86%(31/36)的患儿在生后3d内起病,53%(19/36)以惊厥起病。颅脑影像学结果显示,22例(61%)大脑左侧发生梗死,13例(36%)基底节梗死;12例完成磁共振血管成像检查,9例(75%)累及大脑中动脉。29例患儿完成脑电图检查,21例(72%)出现尖波,10例(34%)出现电发作。不同医院对症支持治疗不同。12例(33%)完善新生儿行为神经测定评分,平均得分(32±4)分。共随访27例患儿12月龄左右的预后情况,44%(12/27)的患儿预后良好。结论新生儿脑卒中主要为缺血性脑卒中,多以惊厥为首发症状,多累及大脑中动脉,脑电图多出现尖波,神经发育评分较低,以对症支持治疗为主,部分患儿预后不良。

The research on the multiphase flow characteristics of hydrate slurry is the key to implementing the risk prevention and control technology of hydrate slurry in deep-water oil and gas mixed transportation *** paper established a geometric model based on the high-pressure hydrate slurry experimental *** model was used to carry out simulation research on the flow characteristics of gas-liquid-solid three-phase *** specific research is as follows:Firstly,the effects of factors such as slurry flow velocity,hydrate particle density,hydrate particle size,and hydrate volume fraction on the stratified smooth flow were specifically *** test obtained particle size has the most influence on the particle concentration *** slurry flow velocity is gradually increased based on stratified smooth *** flow patterns were observed and their characteristics were ***,increasing the slurry velocity to 2 m/s could achieve the slurry flow pattern of partial hydrate in the pipeline transition from stratified smooth flow to wavy *** the flow rate increases to 3 m/s,a violent wave forms throughout the entire *** on wave flow,as the velocity increased to 4 m/s,and the flow pattern changed to slug *** the particle concentration was below 10%,the increase of the concentration would aggravate the slug flow trend;if the particle concentration was above 10%,the increase of the concentration would weaken the slug flow trend,the increase of particle density and liquid viscosity would weaken the tendency of slug *** relationship between the pressure drop gradients of several different flow patterns is:slug flow>wave flow>stratified smooth flow.

Cervicogenic headache(CEH)has been recognized as a unique category of headache that can be difficult to diagnose and *** China,CEH patients are managed by many different specialties,and the treatment plans remain ***,there is a great need for comprehensive evidence-based Chinese experts’recommendations for the management of *** Chinese Association for the Study of Pain asked an expert panel to develop recommendations for a series of questions that are essential for daily clinical management of patients with CEH.A group of multidisciplinary Chinese Association for the Study of Pain experts identified the clinically relevant topics in CEH.A systematic review of the literature was performed,and evidence supporting the benefits and harms for the management of CEH was ***-four recommendations were finally developed through expert consensus voting for evidence quality and recommendation *** hope this guideline provides direction for clinicians and patients making treatment decisions for the management of CEH.

The Omicron variant of the severe acute respiratory syndrome coronavirus 2(SARS‑CoV‑2)infected a substantial proportion of Chinese population,and understanding the factors underlying the severity of the disease and fatality is valuable for future prevention and clinical *** recruited 64 patients with invasive ventilation for COVID-19 and performed metatranscriptomic sequencing to profile host transcriptomic profiles,plus viral,bacterial,and fungal content,as well as virulence factors and examined their relationships to 28-day mortality were *** addition,the bronchoalveolar lavage fluid(BALF)samples from invasive ventilated hospital/community-acquired pneumonia patients(HAP/CAP)sampled in 2019 were included for *** analysis revealed that all Omicron strains belong to BA.5 and BF.7 sub-lineages,with no difference in 28-day mortality between *** to HAP/CAP cohort,invasive ventilated COVID-19 patients have distinct host transcriptomic and microbial signatures in the lower respiratory tract;and in the COVID-19 non-survivors,we found significantly lower gene expressions in pathways related viral processes and positive regulation of protein localization to plasma membrane,higher abundance of opportunistic pathogens including bacterial Alloprevotella,Caulobacter,Escherichia-Shigella,Ralstonia and fungal Aspergillus sydowii and Penicillium *** analysis further revealed significant associations between host immune responses and microbial compositions,besides synergy within viral,bacterial,and fungal *** study presents the relationships of lower respiratory tract microbiome and transcriptome in invasive ventilated COVID-19 patients,providing the basis for future clinical treatment and reduction of fatality.
