Trabeculectomy with large area mitomycin-C application as a first-line treatment in advanced glaucoma: retrospective review

AIM: To evaluate the outcomes of trabeculectomy with large area mitomycin-C(MMC) application as a first line treatment in advanced glaucoma. ·METHODS: The records of 55 patients with severe visual field defects undergoing trabeculectomy were retrospectively reviewed. The patients were classified as first-line therapy to either early trabeculectomy(initial trabeculectomy-Group 1) or long term medical therapy followed by trabeculectomy(primary trabeculectomy-Group 2). Trabeculectomy was performed with large-area MMC application. Intraocular pressure(IOP) values,visual acuities,mean deviations,morphology and function of the blebs,necessity for anti-glaucomatous medications and surgical complications were reported. ·RESULTS: There were 20 eyes of 18 patients in Group 1 and 37 eyes of 37 patients in Group 2. The mean preoperative IOPs in Groups 1 and 2 were 40.2±10.0mmHg(27-68mmHg) and 29.0±4.4mmHg(21-41mmHg),respectively(P=0.001). Average preoperative mean deviations(MD) in Groups 1 and 2 were 17.4±2.8dB(13.3-23dB) and 17.9± 2.4dB(13.7-23.2dB),respectively(P=0.441). Postoperative IOPs significantly decreased and were comparable in both Groups. The mean number of medications was significantly higher in Group 2(P =0.005). No cystic bleb formation was observed in Group 1,whereas 4 patients from Group 2(10.8%) developed cystic bleb(P =0.040). No visually devastating complication has occurred in both Groups. ·CONCLUSION: Initial trabeculectomy with large areaMMC application might be applied in patients with advanced glaucoma with low complication rates. Long-term topically applied anti-glaucomatous medications seem to increase the risk of cystic bleb formation.

Retinal vessel diameters and their correlation with retinal nerve fiber layer thickness in patients with pseudoexfoliation syndrome

AIM: To compare retinal artery-vein diameters(RAVDs)of patients with pseudoexfoliation(PSX) syndrome with healthy controls and investigate the correlations between retinal nerve fiber layer(RNFL) thickness parameters and ***: Seventeen eyes with PSX and 17 eyes of age-matched controls were included in the study. All participants underwent routine ophthalmological examination, Humphrey visual field and RNFL examination by using Stratus OCT. Retinal images were obtained by using a retinal camera(Topcon 501X).RAVDs were measured from inferior nasal, inferior temporal, superior nasal and superior temporal arcuates by using IMAGEnet software. Superior, inferior, nasal,temporal and average RNFL thicknesses were *** and RNFL parameters in groups and correlations were analyzed by Mann-Whitney U and Spearmann correlation ***: Only inferior quadrant and average RNFL thickness were detected thinner in the PSX group compared with control group(P =0.009, P =0.038,respectively). No statistically significant difference regarding RAVDs was found between two ***: RAVDs seems to be comparable in the PSX and control group. RNFL is thinner in the inferior quadrant in the PSX group. RNFL thickness and RAVDs show significant correlations in both groups. This correlation doesn’t seem to be specific to PSX.

Neuroprotective effect of systemic and/or intravitreal rosuvastatin administration in rat glaucoma model

AIM： To evaluate the neuroprotective effect of rosuvastatin, in a rat experimental glaucoma model. METHODS： Ocular hypertension was induced in right eyes of Long-Evans rats （n=30） by cauterization of three episcleral veins. Left eyes were defined as controls. Rats were divided into five groups： oral rosuvastatin, intravitreal rosuvastatin, oral ＋intravitreal rosuvastatin, intravitreal sham and glaucoma without intervention. Rats were sacrificed at day 14. Retinal ganglion cell （RGC） number was assessed by histopathological analysis. Terminal deoxynucleotidyl transferase-mediated dUTP-nick end-labeling （TUNEL） staining and the expression of glial fibrillary acidic protein （GFAP） in RGC layer was also examined. RESULTS： A significant intraocular pressure （lOP） elevation was seen （P=0.002）. Elevated lOP resulted in a significant decrease in number of RGCs in group 5 （70.33 ±8.2 cells/mm2） when compared with controls （92.50 ±13.72 cells/mm2; P=0.03）. The RGC number in group 1 （92.4±7.3 cells/mm2） was significantly higher than group 5 （ρ=0.03）. The numbers of RGC in groups 2, 3 （57.3±8.2 cells/mm2, 60.5±12.9 cells/mm2） were comparable with that of group 5 （ρ=0.18 and P=0.31）. The apoptosis rates with TUNEL staining were also parallel to RGC number. Animals with experimentally induced glaucoma showed an increase in retinal GFAP immunoreacUvity. CONCLUSION： Decrease in RGC loss and apoptosis suggest the neuroprotective potential of oral rosuvastatin treatment in a rat model of ocular hypertension. Howeverintravitreal rosuvastatin showed a contrary effect and further studies are required.

An Effective Discrete Artificial Bee Colony Based SPARQL Query Path Optimization by Reordering Triples

Semantic Web has emerged to make web content machine-readable,and with the rapid increase in the number of web pages,its importance has *** description framework(RDF)is a special data graph format where Semantic Web data are stored and it can be queried by SPARQL query *** challenge is to find the optimal query order that results in the shortest period of *** this paper,the discrete Artificial Bee Colony(dABCSPARQL)algorithm is proposed,based on a novel heuristic approach,namely reordering SPARQL *** processing time of queries with different shapes and sizes is minimized using the dABCSPARQL *** performance of the proposed method is evaluated on chain,star,cyclic,and chain-star queries of different sizes from the Lehigh University Benchmark(LUBM)*** results obtained by the proposed method are compared with those of ARQ(a SPARQL processor for Jena)query engine,the Ant System,the Elitist Ant System,and MAX-MIN Ant System *** experiments demonstrate that the proposed method significantly reduces the processing time,and in most queries,the reduction rate is higher compared with other optimization methods.

Ionizing radiation （IR） is a potential carcinogen. Evidence for the carcinogenic effect of IR radiation has been shown after long-term animal investigations and observations on survivors of the atom bombs in Hiroshima and Nagasaki. However, IR has been widely used in a controlled manner in the medical imaging for diagnosis and monitoring of various diseases and also in cancer therapy. The collective radiation dose from medical imagings has increased six times in the last two decades, and grow continuously day to day. A large number of evidence has revealed the increased cancer risk in the people who had frequently exposed to x-rays, especially in childhood. It has also been shown that secondary malignancy may develop within the five years in cancer survivors who have received radiotherapv, because of IR-mediated damage to healthy cells. In this article, we review the current knowledge about the role of medical x-ray exposure in cancer development in humans, and recently recognized epigenetic mechanisms in IR-induced carcinogenesis.

Novozyme 435,which is the commercially available immobilized form of Candida antarctica lipase B,has been successfully conducted ring opening polymerization of lactones in organic *** this paper,it was aimed to introduce an alternative biocatalyst for Novozyme *** antarctica lipase B immobilized onto rice husk ashes via physical adsorption(with a specific activity of 4.4 U/mg)was prepared in previous studies and used as a biocatalyst for poly(δ-valerolactone)synthesis in the present *** reactions were proceeded at various reaction temperatures and periods via both two immobilized enzyme *** resulting products were characterized spectroscopically and *** highest molecular weight(Mn=9010 g/mol)was obtained via Novozyme 435 catalysis at 40℃and 24 *** performance of home-made lipase,which resulted in a molecular weight of 8040 g/mol,was close to commercial one.

Solving vehicle routing problem with time windows using metaheuristic approaches

Purpose-Drugs are strategic products with essential functions in human *** optimum design of the pharmaceutical supply chain is critical to avoid economic damage and adverse effects on human *** vehicle-routing problem,focused on finding the lowest-cost routes with available vehicles and constraints,such as time constraints and road length,is an important aspect of *** this paper,the vehicle routing problem(VRP)for a pharmaceutical company in Turkey is ***/methodology/approach-A mixed-integer programming(MIP)model based on the vehicle routing problem with time windows(VRPTW)is presented,aiming to minimize the total route cost with certain *** the model provides an optimum solution for small problem sizes with the GUROBI®solver,for large problem sizes,metaheuristic methods that simulate annealing and adaptive large neighborhood search algorithms are proposed.A real dataset was used to analyze the effectiveness of the metaheuristic *** proposed simulated annealing(SA)and adaptive large neighborhood search(ALNS)were evaluated and compared against GUROBI®and each other through a set of real problem ***-The model is solved optimally for a small-sized dataset with exact algorithms;for solving a larger dataset,however,metaheuristic algorithms require significantly lesser *** the problem addressed in this study,while the metaheuristic algorithms obtained the optimum solution in less than one minute,the solution in the GUROBI®solver was limited to one hour and three hours,and no solution could be obtained in this time ***/value-The VRPTW problem presented in this paper is a real-life *** vehicle fleet owned by the factory cannot be transported between certain suppliers,which complicates the solution of the problem.

Effect of exercise on colorectal cancer prevention and treatment

In recent years, because of improved cancer screening, detection and treatment modalities, a rapid increase in the population of colorectal and other cancer survivors has been observed. The increasing population has justified the requirement of preventive strategies such as lifestyle modifications with regard to obesity, physical activity, diet and smoking. Physical activity may prevent approximately 15% of the colon cancers. Furthermore, several observational studies have demonstrated the efficacy and dose-dependent and anti-cancer effects of exercise on decreasing the mortality and risk of recurrence before and after the colorectal cancer(CRC) diagnosis. However, the required exercise dose,type and intensity are yet unclear. The results of randomised prospective studies are expected to determine the optimal amount, type and intensity of exercise and formulate the most appropriate exercise plan and guidelines, according to the requirements and comorbidities of the patients. In addition, recent studies have focused on the molecular and genetic mechanisms underlying the effect of physical activity on disease outcomes and recurrence rates. This review aimed to investigate the effects of physical activity and the biological basis of these effects in preventing the risk and recurrence of CRC and decreasing the hazards of cancer and cancer treatment.
