Familial Mediterranean Fever: Clinical and Genetic Characteristics among Lebanese Pediatric Population

Objective: The aim of our study was to evaluate the clinical features, to define the frequency of mutation type, to assess genotype-phenotype correlation and the response to colchicine in childhood-onset Familial Mediterranean fever (FMF) in Lebanon. Methods: The characteristics of 550 children, presenting with FMF symptoms between January 2003 and January 2013 and having a positive Mediterranean fever gene (MEFV gene) mutation, were prospectively investigated. The clinical and genetic characteristics as well as the response to colchicine and its side effects were studied in 321 FMF children. The mutations were correlated with clinical presentation and disease severity. Results: Out of the 321 patients (183 males and 138 females), abdominal pain was the most common presenting feature documented in 84.7%. Mutational analysis detected simple heterozygotes, compound heterozygotes and homozygotes in 56.4%, 30.9% and 11.2% patients respectively. The most frequent mutation was M694V (37.2%), followed by E148Q mutation (27.4%). 71% patients received colchicine therapy;only 33.3% of them showed complete response. Genotype-phenotype correlation showed that M694V followed by E148Q was associated with moderate to severe disease form (71.6% and 62.7% respectively, P = 0.005). There was no association between mutation type and colchicine response. Conclusion: The most important features were the predominance of the M694V and E148Q. The M694V subgroup, followed by E148Q subgroup had a high disease severity score. Our data indicate an enhanced expression of the disease with E148Q mutation.

Microstructures, hardening and tribological behaviors of tin matrix composites reinforced with SiC and Zn particles

In order to strengthen pure tin and improve its dry sliding resistance, Sn/SiC and Sn/Zn composites were fabricated via a powder metallurgy route. Microstructure,hardness and pin-on-disk wear resistance of pure tin and the fabricated composites were compared to those of Sn–7.5 Sb–3.5 Cu Babbitt alloy. The dominant wear mechanism at different applied loads was determined by analyzing worn surfaces and wear debris in each case. The results showed that the hardening effect of Zn was much higher than that of SiC. The hardening role of Zn in the tin matrix was ascribed to the direct load transfer mechanism. Microscopic studies of the worn surfaces revealed that the pure tin was susceptible to surface fatigue wear and plowing damage, depending on the normal load applied during the wear test. In the case of Sn/SiC composite and the Babbitt alloy, delamination wear mechanism resulting from subsurface crack propagation controlled the wear rate. While the highest hardness and the lowest coefficient of friction were obtained for the Babbitt alloy, the Sn/Zn composite exhibited the highest wear resistance at a constant applied load, indicating the importance of asperity contact type in the wear process.

Vitamin D supplementation improves sustained virologic response in chronic hepatitis C (genotype 1)-nave patients

AIM: To determine whether adding vitamin D, a potent immunomodulator, improves the hepatitis C virus (HCV) response to antiviral therapy. METHODS: Seventy-two consecutive patients with chronic HCV genotype 1 were randomized into two groups: the treatment group (n = 36, 50% male, mean age 47 ± 11 years) received Peg-α-2b interferon (1.5 μg/kg per week) plus ribavirin (1000-1200 mg/d) together with vitamin D3 (2000 IU/d, target serum level > 32 ng/mL), and the control group (n = 36, 60% male, mean age 49 ± 7 years) received identical therapy without vitamin D. HCV-RNA was assessed by realtime polymerase chain reaction (sensitivity, 10 IU/mL). The sustained virologic response (SVR) was defined as undetectable HCV-RNA at 24 wk post-treatment. RESULTS: Clinical characteristics were similar in both groups. The treatment group had a higher mean bodymass index (27 ± 4 kg/m2 vs 24 ± 3 kg/m2, P F2: 42% vs 19%, P < 0.001) than the controls. At week 4, 16 (44%) treated patients and 6 (17%) controls were HCV-RNA negative (P < 0.001). At week 12, 34 (94%) treated patients and 17 (48%) controls were HCV-RNA negative (P < 0.001). At 24 wk post-treatment (SVR), 31 (86%) treated patients and 15 (42%) controls were HCV-RNA negative (P < 0.001). Viral load, advanced fibrosis and vitamin D supplementation were strongly and independently associated with SVR (multivariate analysis). Adverse events were mild and typical of Peg-α-2b/ribavirin. CONCLUSION: Adding vitamin D to conventional Peg-α-2b/ribavirin therapy for treatment-na■ve patients with chronic HCV genotype 1 infection significantly improves the viral response.

Quantification of Antibiotic Residues and Determination of Antimicrobial Resistance Profiles of Microorganisms Isolated from Bovine Milk in Lebanon

The rapid growth of dairy sectors in the Middle East, particularly in Lebanon, led to extensive use of antibiotics to enhance the health and productivity of animals. Prolonged usage may lead to antibiotic residues in foods of animal origin;hence, the emergence of antimicrobial resistant microorganisms. Accurate data on the antibiotic usage in livestock treatment, antibiotic residues and antimicrobial resistances in raw milk in Lebanon are lacking. This study aimed to investigate the types and usages of antibiotics in cattle, their residual levels and the potential microbial resistances in raw milk samples. A questionnaire-based survey identified Gentamicin and Streptomycin as the most frequently used antibiotics. Selected raw milk samples from main dairy farms were then analyzed in duplicate by quantitative ELISA for the antibiotics residual levels. The mean residual levels of Gentamicin and Streptomycin were 90 and 80 μg/L, respectively;which are below the allowable maximum residue limit of 200 μg/L as set by the FAO/WHO. Staphylococcus aureus, Listeria monocytogenes, E. coli and total aerobic microorganisms isolated from the milk samples were then tested for resistance against Gentamicin and Streptomycin by the disc agar diffusion method. All the S. aureus, E. coli, and L. monocytogenes isolates showed high resistance to Gentamicin. However, 95% of S. aureus, 60% of E. coli and 58% of L. monocytogenes isolates were resistant to Streptomycin. The obtained results provide evidence that antimicrobial resistant strains of the above pathogens have become remarkably widespread in raw milk. This requires better management for antibiotic usages among livestock farmers to control sources of food contamination and reduce the health risks associated with the development of resistant microbial strains.

Sex steroids, also known as sex hormones are synthesized naturally by the gonads (ovaries or testes). The two main classes of sex steroids, androgens and estrogens, are crucial hormones for the proper development and function of the body;they regulate sexual differentiation, the secondary sex characteristics, and sexual habits. They are also well known to influence many common cancers, especially hormonally driven cancers such as breast and prostate cancer. In this report, we review the association of sex steroids with cancer and the potential use of endocrine manipulation in cancer therapy as well as the limitations and challenges faced in this field.

Uncertainties of ＂in v/vo＂ Dosimetry Using Semiconductors

The purpose of the present study is to evaluate the uncertainties of in vivo dosimetry measured with diode detectors for conformal radiation therapy techniques in order to define appropriate tolerance levels for pelvis and breast treatment in MEIH （Middle East Institute of Health）. The present work is carried out on 30-472 and 30-473 diode detectors irradiated by 4 and 15MV photon beams of a medical linear accelerator Synergy from ELEKTA. Dose computation is performed with Pinnacle 7.4 k treatment planning system. First, an estimation of the uncertainties in a simple geometric case, using a water-equivalent solid phantom is done. Secondly, each treatment parameter such as field size, beam angle, beam modifiers and source-skin distance is evaluated in order to simulate the conformal radiation treatments used in the present institution for the main anatomical sites. Interpretation of entrance dose in-vivo measurements requires the determination of appropriate tolerance levels. Indeed, the authors found that the 5% action level proposed in the literature underestimates the uncertainties in the treatment process. A method for the evaluation of tolerance/action levels related to the different anatomical sites is developed. By the end of the present study the authors have developed an integrated monitoring system that offers accurate information about the dose received by patients.
