美罗华治疗原发性皮肤B细胞淋巴瘤

Background: Ri.uxi.ab i. a geneti.ally engi.eered anti.ody di.ected agai.st the CD20 anti.en. i.travenous admi.i.trati.n of ri.uxi.ab has been used for the treatment of pati.nts wi.h low-, i.termedi.te-, and hi.h-grade B-cell non-Hodgki.’s lymphomas and i. a regi.tered treatment modali.y for thi. i.di.ati.n. Treatment of pri.ary cutaneous B-cell lymphoma (CBCL)wi.h i.tralesi.nally o rsystemi.ally admi.i.tered ri.uxi.ab has been descri.ed only i. a few cases. Objecti.e: Our purpose was to assess the effi.acy of ri.uxi.ab i. the treatment of CBCL. Methods: We performed a retrospecti.e study on 9 pati.nts wi.h CBCL who were treated wi.h i.tralesi.nal or systemi. admi.i.trati.n of ri.uxi.ab. Results: Two pati.nts treated wi.h systemi. ri.uxi.ab achi.ved complete remi.si.n. Complete remi.si.n could be observed i. 6 of 7 pati.nts after 1 to 8 cycles of i.tralesi.nal treatment wi.h ri.uxi.ab. i. one pati.nt one of two lesi.ns showed a parti.l remi.si.n after 4 cycles of treatment, whereas the second showed complete remi.si.n. A local recurrence was observed i. one pati.nt after 27 months of follow-up and i. two pati.nts recurrences developed at other body si.es after 12 and 14 months of follow-up. No severe si.e effect occurred except for sli.ht pai. duri.g i.tralesi.nal i.jecti.n. Conclusi.n: Ri.uxi.ab therapy i. a well-tolerated and effecti.e treatment for pri.ary CBCL. i. compari.on to i.travenous admi.i.trati.n, i.tralesi.nal appli.ati.n of the drug allows the use of lower dosages. i.tralesi.nal therapy wi.h ri.uxi.ab deserves further i.vesti.ati.n and compari.on to systemi. admi.i.trati.n of the drug i. controlled multi.enter studi.s.

Background: Based on the dermoscopi. classi.i.ati.n of acqui.ed melanocyti. naevi. si. di.ferent dermoscopi. types can be di.ti.gui.hed by morphology (globular, globular-reti.ular, globular-homogeneous, reti.ular, reti.ular-homogeneous, homogeneous) and by pi.ment di.tri.uti.n (uni.orm, central hyperpi.mentati.n, central hypopi. gmentati.n, peri.heral hyperpi.mentati.n, peri.heral hypopi.mentati.n, multi.ocal hyper/ hypopi.mentati.n). i. has been suggested that most i.di.i.uals harbour one predomi.ant dermoscopi. type among thei. naevi. Objecti.es: To evaluate whether the age of the pati.nt i.fluences the predomi.ant naevus pattern observed i. i.di.i.uals wi.h multi.le acqui.ed melanocyti. naevi. Methods: i.di.i.uals were recrui.ed from the pi.mented ski. lesi.n cli.i. i. Graz between July 2000 and February 2001. i.di.i.uals wi.h at least 10 melanocyti. naevi.were selected consecuti.ely unti. a total of 10 i.di.i.uals i. each of fi.e age groups was obtai.ed. Age groups were: 0-15 years, 16-30 years, 31-45 years, 46-60 years and > 60 years. Di.i.i.ed i.ages of acqui.ed melanocyti. naevi. defi.ed as beni.n melanocyti. proli.erati.ns havi.g a di.meter of at least 5 mm wi.h a macular component and whi.h were not apparent wi.hi. the fi.st year of li.e, were evaluated by dermoscopi. cri.eri.. The associ.ti.ns of dermoscopi. features as a functi.n of pati.nt age were analysed. We calculated absolute numbers and frequenci.s, gi.en as percentages, as well as predomi.ance of the dermoscopi. types of naevi.i. the di.ferent age groups. Results: Analysi. of 1268 naevi.revealed that the globular pattern predomi.ated i. the youngest age group. By contrast, the reti.ular and/or homogeneous patterns were i.creasi.gly exhi.i.ed i. naevi.from older i.di.i.uals (older than 15 years). Background: i. a pi.ot study, the three-poi.t checkli.t of dermoscopy has been shown to represent a vali. and reproduci.le tool wi.h hi.h sensi.i.i.y for the di.gnosi. of ski. cancer i. the hands of a small group of nonexperts. Objecti.es: To reevaluate these preli.i.ary results i. a large number of observers i.dependently from thei. professi.n and experti.e i. dermoscopy. Methods: The study was conducted vi. the i.ternet to provi.e worldwi.e access for parti.i.ants. After a short webbased tutori.l, the parti.i.ants evaluated dermoscopi. i.ages of 165 (116 beni.n and 49 mali.nant) ski. lesi.ns (15 trai.i.g and 150 test lesi.ns). For each lesi.n parti.i.ants scored the presence of the three-poi.t checkli.t cri.eri. (asymmetry, atypi.al network and blue-whi.e structures). Kappa values, odds rati.s, sensi.i.i.y, speci.i.i.y and li.eli.ood rati.s were esti.ated. Results: Overall, 150 parti.i.ants joi.ed the study. The three-poi.t checkli.t showed good i.terobserver reproduci.i.i.y (kappa value: 0.53). Sensi.i.i.y for ski. cancer (melanoma and basal cell carci.oma) was 91.0%and thi. value remai.ed basi.ally uni.fluenced by the observers’professi.nal profi.e. Only 20 parti.i.ants lacki.g any experi.nce i. dermoscopy performed si.ni.i.antly more poorly, but the sensi.i.i.y was sti.l remarkably hi.h (86.7%) when consi.eri.g that they were untrai.ed novi.es i. dermoscopy. The speci.i.i.y was 71.9%and was si.ni.i.antly i.fluenced by the professi.n, wi.h dermatologi.ts performi.g best. Conclusi.ns: Our study confi.ms that the three-poi.t checkli.t i. a feasi.le, si.ple, accurate and reproduci.le ski. cancer screeni.g tool.

Background: Dermoscopy i.proves the di.gnosti. accuracy i. pi.mented ski. lesi.ns, but i. i. also useful i. the evaluati.n of nonpi.mented ski. tumours as i. allows the recogni.i.n of vascular structures that are not vi.i.le to the naked eye. Bowens di.ease (BD) or squamous cell carci.oma i. si.u i. usually nonpi.mented, but may also rarely be pi.mented. Objecti.e: To descri.e the dermoscopi. features i. a seri.s of pi.mented and nonpi.mented BD. Methods: Dermoscopi. i.ages of 21 hi.topathologi.ally proven BD were evaluated for the presence of vari.us dermoscopi. features. Each lesi.n was photographed usi.g the Dermaphot (Hei.e Optotechni., Herrschi.g, Germany), at 10-fold magni.i.ati.n, and the colour sli.es were scanned to di.i.al format usi.g a Kodak Photo CD system. Results: The majori.y of cases of BDrevealed a peculi.r dermoscopi. pattern characteri.ed by glomerular vessels (90%) and a scaly surface (90%). i. addi.i.n, i. pi.mented BD small brown globules regularly packed i. a patchy di.tri.uti.n (90%), and structureless grey to brown pi.mentati.n (80%) were observed. Conclusi.ns: Dermoscopy can be helpful for di.gnosi.g BD because of the presence of repeti.i.e morphologi.al fi.di.gs such as glomerular vessels and a scaly surface. i. pi.mented BD, small brown globules and/or homogeneous pi.mentati.n can be seen as well.

Acrodermati.i. chroni.a atrophi.ans, the characteri.ti. cutaneous mani.estati.n of late Lyme borreli.si., typi.ally occurs i. elderly women. To our knowledge, only 4 cases of acrodermati.i. chroni.a atrophi.ans i. chi.dren have been descri.ed. Prompt di.gnosi. and treatment are i.portant to prevent progressi.n of di.ease and extracutaneous compli.ati.ns. We descri.e a 15-year-old gi.l wi.h acrodermati.i. chroni.a atrophi.ans of the left leg that had been mi.di.gnosed as chroni. venous i.suffi.i.ncy for 6 years. Because of the long-standi.g di.ease course, ski. changes expanded and progressed to marked atrophy. The correct di.gnosi. was fi.ally based on cli.i.al, hi.topathologi., and serologi. fi.di.gs. The gi.l was treated wi.h oral doxycycli.e for 6 weeks, but her ski. changes di. not fully normali.e. Thi. case i.lustrates the possi.i.i.y of acrodermati.i. chroni.a atrophi.ans appeari.g i. chi.dhood and the di.fi.ulti.s i. di.ferenti.ti.g vascular di.orders from acrodermati.i. chroni.a atrophi.ans on the basi. of the cli.i.al appearance alone.
