Performances of glaucoma operations with Kahook Dual Blade or iStent combined with phacoemulsification in Japanese open angle glaucoma patients

AIM:To compare surgical outcomes of phacoemulsification combined with glaucoma surgical techniques performed with either Kahook Dual Blade(KDB)or iStent for Japanese patients with either primary open angle glaucoma or exfoliation ***:We retrospectively evaluated the surgical outcomes of 129 eyes of 84 Japanese patients with glaucoma who underwent KDB or 44 eyes of 34 patients who underwent phacoemulsification with iStent procedures combined with cataract *** primary outcome was surgical success or failure[with surgical failure being indicated by18 mm Hg as criterion A;IOP>14 mm Hg as criterion B on two consecutive study visits;or reoperation requirement].In addition,we assessed the number of postoperative glaucoma medications and the resulting ***:The probability of success was significantly higher in the KDB group than in the iStent group for criterion A(60.2%vs 46.4%,P=0.019).In the KDB group,the mean preoperative IOP of 19.8±7.3 mm Hg decreased significantly to 13.0±3.1 mm Hg(P<0.01),and the mean number of glaucoma medications at 2.5±1.4 decreased significantly to 1.6±1.6(P<0.01)12 mo *** the iStent group,the mean preoperative IOP of 17.8±2.9 mm Hg significantly decreased to 14.3±2.3 mm Hg(P<0.01),and the mean number of glaucoma medications at 2.2±1.1 decreased significantly to 0.9±1.4(P<0.01)12 mo *** overall IOP reduction percentage was higher in the KDB group(26.2%)than in the iStent group(19.0%)12 mo postoperatively(P=0.03).Hyphema occurred significantly more frequently in the KDB group(16.3%)than in the iStent group(2.3%,P=0.017).CONCLUSION:KDB and iStent procedures combined with cataract surgery both result in significant IOP and glaucoma medication reductions after the 12-month *** patients in the KDB group have a higher success rate for the target IOP of less than 18 mm Hg and a higher complication rate than those

Heart failure(HF)is a major global public health concern,and one of the less commonly known risk factors for HF development is metabolic dysfunction-associated steatotic liver disease(MASLD),as they share a similar pathophysio-logical *** this article,we evaluated a recently published review article by Arriola-Montenegro et *** article briefly summarizes the common pathophysiology of HF and MASLD development and evaluates the available therapeutic options to treat both *** practice guidelines highlight the importance of initiating and titrating guideline-directed medication therapy(GDMT)for patients with HF with reduced ejection *** is comprised of the four pillars currently proposed in most clinical practice guidelines,namely angiotensin-converting enzyme inhibitors(ACEIs),angiotensin receptor blockers(ARBs),angiotensin receptor-neprilysin inhibitors,beta-blockers,mineralocor-ticoid receptor antagonists,and sodium-glucose co-transporter 2 inhibitors(SGLT-2i).Given the similarity of pathophysiology and risk factors,recent studies for GDMT regarding ACEIs,ARBs,mineralocorticoid receptor antagonists,and SGLT-2i have shown beneficial effects on ***,other medications for both conditions and novel therapies require more robust data and well-designed clinical studies to demonstrate their efficacies in both conditions.

Ethical decision-making in older drivers during critical driving situations: An online experiment

The present study examined the impact of aging on ethical decision-making in simulated critical driving scenarios.204 participants from North America,grouped into two age groups(18–30 years and 65 years and above),were asked to decide whether their simulated automated vehicle should stay in or change from the current lane in scenarios mimicking the Trolley *** participant viewed a video clip rendered by the driving simulator at Old Dominion University and pressed the space-bar if they decided to intervene in the control of the simulated automated vehicle in an online *** hierarchical models were used to analyze participants’responses,response time,and acceptability of utilitarian ethical *** results showed significant pedestrian placement,age,and time-to-collision(TTC)effects on participants’ethical *** pedestrians were in the right lane,participants were more likely to switch lanes,indicating a utilitarian approach prioritizing pedestrian *** participants were more likely to switch lanes in general compared to older *** results imply that older drivers can maintain their ability to respond to ethically fraught scenarios with their tendency to switch lanes more frequently than younger counterparts,even when the tasks interacting with an automated driving *** current findings may inform the development of decision algorithms for intelligent and connected vehicles by considering potential ethical dilemmas faced by human drivers across different age groups.

Laparoscopic approach to suspected T1 and T2 gallbladder carcinoma

AIM To evaluate a laparoscopic approach to gallbladder lesions including polyps, wall-thickening lesions, and suspected T1 and T2 gallbladder cancer(GBC).METHODS We performed 50 cases of laparoscopic whole-layer cholecystectomy(LCWL) and 13 cases of laparoscopic gallbladder bed resection(LCGB) for those gallbladder lesions from April 2010 to November 2016. We analyzed the short-term and long-term results of our laparoscopic approach. RESULTS The median operation time was 108 min for LCWL and 211 min for LCGB. The median blood loss was minimal for LCWL and 28 ml for LCGB. No severe morbidity occurred in either procedure. Nine patients who underwent LCWL and 7 who underwent LCGB were postoperatively diagnosed with GBC. One of these patients had undergone LCGB for pathologically diagnosed T2 GBC after LCWL. All of the final surgical margins were negative. Three of these 15 patients underwent additional open surgery. The mean follow-up period was 26 mo, and only one patient developed *** LCWL and LCGB are safe and useful procedures that allow complete resection of highly suspected or earlystage cancer and achieve good short-term and longterm results.

Radiofrequency ablation as treatment for pulmonary metastasis of colorectal cancer

Radiofrequency ablation (RFA) causes focal coagulation necrosis in tissue. Its first clinical application was reported in 2000, and RFA has since been commonly used in both primary and metastatic lung cancer. The procedure is typically performed using computed tomography guidance, and the techniques for introducing the electrode to the tumor are simple and resemble those used in percutaneous lung biopsy. The most common complication is pneumothorax, which occurs in up to 50% of procedures; chest tube placement for pneumothorax is required in up to 25% of procedures. Other severe complications, such as pleural effusion requiring chest tube placement, infection, and nerve injury, are rare. The local efficacy depends on tumor size, and local progression after RFA is not rare, occurring in 10% or more of patients. The local progression rate is particularly high for tumors > 3 cm. Repeat RFA may be used to treat local progression. Short- to mid-term survival after RFA appears promising and is approximately 85%-95% at 1 year and 45%-55% at 3 years. Long-term survival data are sparse. Better survival may be expected for patients with small metastasis, low carcinoembryonic antigen levels, and/or no extrapulmonary metastasis. The notable advantages of RFA are that it is simple and minimally invasive; preserves pulmonary function; can be repeated; and is applicable regardless of previous treatments. Its most substantial limitation is limited local efficacy. Although surgery is still the method of choice for treatment with curative intent, the ultimate application of RFA may be to replace metastasectomy for small metastases. Randomized trials comparing RFA with surgery are needed.

A child with severe inner ear malformations with favorable hearing utilization and balance functions after wearing hearing aids

Infants with congenital deafness caused by severe bilateral inner ear malformations frequently suffer from severe hearing loss and poor balance. Unfortunately, the use of hearing aids is usually ineffective in recovering hearing, necessitating cochlear implants. We report a case of a 6-year-old boy with congenital deafness and bilateral inner ear malformations(right side, incomplete partition type I [IP-I]; left side, common cavity deformity). Hearing aids had a remarkable effect in this patient, enabling sufficient and favorable hearing recovery such as to allow the patient to engage in daily conversations. Per-rotatory nystagmus was recorded on an electronystagmogram for both right and left rotations in a damped rotational chair test. It is rare for deaf children with severe bilateral inner ear malformation to demonstrate favorable development in hearing and good equilibrium function. Our findings suggest that auditoryevestibular hair cells in this patient may have been partially preserved despite IP-I in the right ear and common cavity deformity of the left ear.

Holographic augmented reality display with conical holographic optical element for wide viewing zone

In this study, we propose a holographic augmented reality (AR) display with a wide viewing zone realized by using a special-designed reflective optical element. A conical holographic optical element (HOE) is used as such a reflective optical element. This conical HOE was implemented to reconstruct a diverging spherical wave with a wide spread angle. It has a sharp wavelength selectivity by recording it as a volume hologram, enabling augmented reality (AR) representation of real and virtual 3D objects. The quality of the generated spherical wave and the spectral reflectivity of the fabricated conical HOE were investigated. An optical superimposition between real and virtual 3D objects was demonstrated, thereby enhancing the validity of our proposed method. A horizontal viewing zone of 140° and a vertical viewing zone of 30° were experimentally confirmed. The fabrication procedure for the conical HOE is presented, and the calculation method of the computer-generated hologram (CGH) based on Fermat’s principle is explained in detail.

Catalytic selectivity is a central issue in the efficiency of catalytic processes.A large number of strategies have been developed to improve the catalytic selectivity of metal catalysts at the atomic and molecular levels,for instance,alloying secondary elements,fabricating metal-support interactions,and introducing surface ***,macro/mesoscopic pores and cavities have been demonstrated as an alternative route to promote catalytic selectivity of metal nanocrystal *** promotion effects of continuous crystalline porosity include(1)more catalytically active sites that accelerate the favorable catalytic routes to targeted products,(2)confined spaces that increase the retention time of key intermediates and remarkably promote the selective catalysis toward desired products,and(3)an optimized electronic structure and coordination environment of active metal sites that tailor the reaction trends of selective catalysis toward desired *** this minireview,we summarize recent advances in porosity-enabled catalytic selectivity of metal nanocrystal catalysts with focused discussions of CO_(2) reduction electrocatalysis and selective hydrogenation *** mechanisms that allow for the continuous porosity that enables the catalytic selectivity of metal nanocrystal catalysts are discussed in *** end this minireview by proposing current challenges and offering future opportunities in this research field.
