Functional expression of a proliferation-related ligand in hepatocellular carcinoma and its implications for neovascularization

AIM： To detect the expression of a proliferation-related ligand on human hepatocellular carcinoma （HCC） cell lines （SK-Hepl, HLE and HepG2） and in culture medium. METHODS： APRIL expression was analyzed by Western blotting in HCC cell lines. Effects of APRIL to cell count and angiogenesis were analyzed, too. RESULTS： Recombinant human APRIL （rhAPRIL） increased cell viability of HepG2 cells and, in HUVEC, rhAPRIL provided slight tolerance to cell death from serum starvation. Soluble APRIL （sAPRIL） from HLE cells increased after serum starvation, but did not change in SK-Hepl or HepG2 cells. These cells showed down-regulation of VEGF after incubation with anti-APRIL antibody. Furthermore, culture medium from the HCC cells treated with anti-APRIL antibody treatment inhibited tube formation of HUVECs. CONCLUSION： Functional expression of APRIL might contribute to neovascularization via an upregulation of VEGF in HCC.

Computerized Scheme for Histological Classification of Masses with Architectural Distortions in Ultrasonographic Images

Architectural distortion is an important ultrasonographic indicator of breast cancer. However, it is difficult for clinicians to determine whether a given lesion is malignant because such distortions can be subtle in ultrasonographic images. In this paper, we report on a study to develop a computerized scheme for the histological classification of masses with architectural distortions as a differential diagnosis aid. Our database consisted of 72 ultrasonographic images obtained from 47 patients whose masses had architectural distortions. This included 51 malignant (35 invasive and 16 non-invasive carcinomas) and 21 benign masses. In the proposed method, the location of the masses and the area occupied by them were first determined by an experienced clinician. Fourteen objective features concerning masses with architectural distortions were then extracted automatically by taking into account subjective features commonly used by experienced clinicians to describe such masses. The k-nearest neighbors (k-NN) rule was finally used to distinguish three histological classifications. The proposed method yielded classification accuracy values of 91.4% (32/35) for invasive carcinoma, 75.0% (12/16) for noninvasive carcinoma, and 85.7% (18/21) for benign mass, respectively. The sensitivity and specificity values were 92.2% (47/51) and 85.7% (18/21), respectively. The positive predictive values (PPV) were 88.9% (32/36) for invasive carcinoma and 85.7% (12/14) for noninvasive carcinoma whereas the negative predictive values (NPV) were 81.8% (18/22) for benign mass. Thus, the proposed method can help the differential diagnosis of masses with architectural distortions in ultrasonographic images.

Effect of Foot Self-Care Behavior According to Severity of Diabetic Neuropathy in Japanese Patients with Diabetes

PURPOSE: Diabetic patients with severe diabetic neuropathy often have hypoesthesia and numbness. This study evaluated foot self-care behavior according to severity of diabetic neuropathy. METHODS: We used a hand-held nerve conduction test device DPN check (HDN-1000, Omron) to evaluate severity of diabetic neuropathy. Foot self-care was evaluated using the Japanese SDSCA (the Summary of Diabetes Self-Care Activities measure). Foot self-care comprised visual inspection, washing, wiping interdigital areas, and checking shoes, and was scored according to the number of days self-care which was performed in the previous week. RESULTS: The study evaluated 103 diabetic patients (age: 65.7 years, diabetes duration: 13.9 years, HbA1c: 7.3%). Total scores (out of 28 points) for self-care behavior according to neuropathy severity were 11.8 (Normal: n = 54), 10.6 (Mild: n = 27), 13.3 (Moderate: n = 17), and 10.8 (Severe: n = 5). Foot self-care scores were low in all groups, with particularly low scores in those with severe neuropathy. However, no statistically significant differences were observed. CONCLUSIONS: Foot self-care is essential in diabetes because lesions are more likely to occur in severe neuropathy. Living alone and the presence of recurrent foot lesions are associated with poor survival prognosis. Accordingly, foot-care intervention must take neuropathy severity and lifestyle factors into account. The severity of diabetic neuropathy must be determined and foot-care intervention should take lifestyle factors into account.

Learning from the Film “Mitorishi (Transition Doula)”—Application to Understanding the End-of-Life in a Gerontological Nursing Practice Course in Japan

Purpose: This study aims to detail what nursing students learned from watching the film “Mitorishi”, which was used among the teaching materials in a gerontological nursing practice course in nursing education in Japan, and demonstrate the applicability of using the film as an element relevant to the teaching of end-of-life care in Japan. Methods: The participants were fourth-year nursing students at University A who had watched the film “Mitorishi” during the course, had submitted a report on what they learned, and provided informed consent to the study participation. The reports submitted by the participants were used as data and analyzed qualitatively using the content analysis approach. Results: The analysis identified the following five categories (number of codes): Characteristics of life during the final days (9), Importance of views on life and death (3), Importance of end-of-life care (3), Assistance in preparation for the final days of life (13), and Understanding the profession of transition doula (9). Discussion: The five categories show that the students have learned about the “role of nurses at the time of end-of-life care” while “visualizing end-of-life care” and learning “preparedness for end-of-life care”. These findings suggest that using the film “Mitorishi” in the teaching curriculum is effective for learning about end-of-life care for older people.

Homology modeling and structural analysis of human <i>γ</i>-glutamylcysteine ligase catalytic subunit for antitumor drug development

Homology modeling and structural analysis of human glutamate cysteine ligase catalytic subunit (hGCLC) were performed with a software package the Molecular Operating Environment. A yeast GCLC (yGCLC;PDB code: 3LVV) was selected as a template for the 3D structure modeling of hGCLC. The modeled hGCLC showed significant 3D similarities at the ligand biding site (LBS) to the yGCLC structure. The contact energy profiles of the hGCLC model were in good agreement with those of the yGCLC structure. Ramachandran plots revealed that only 1.4% of the amino acid residues were in the disfavored region for hGCLC. The molecular electrostatic potential (MEP) map of the hGCLC model exhibited that the model was slightly different from the yGCLC model electrostatically at the LBS. Further, docking simulations revealed the similarity of the ligand-receptor bound location between the hGCLC and yGCLC models. The different binding orientations between the glutathione (GSH)-hGCLC and GSH-yGCLC complexes reflected the different MEP maps at the LBSs between the hGCLC and yGCLC models. These results indicate that the hGCLC model was successfully modeled and analyzed. To the best of our knowledge, this is the first report of a hGCLC model with detailed analyses, and our data verify that the model can be utilized for application to target hGCLC for the development of anticancer drugs.

Analysis of the Approach on Usability and Ergonomics in Handling and Transporting Packaging

The purpose of this research is to explore the principle of the man-object relationship to identify the issues shown when handling packaging and emphasizing the usability aspect. For that, the theoretical reference of the research involves the analysis of the parameters according to the aspects that involve the use interactions of packaging in agreement with the definition and scientific concepts of ergonomics, allied to the development of product projects, discussing forms, handling, load transportation and the support of the product. Therefore, improperly handling of the packaging may cause damages to the products and even potential accidents. Coding and decoding are significant to facilitate the use recognition within the individual＇s repertoire. The packaging, within the macro-ergonomics universe, may be the object of ergonomics, whose contribution to solve problems is related to labor activities, by dealing with packaging, transportation and load resulting from the man-object interaction. The incorrect identification of information may lead to problems of interpretation and misuse, from the handling to the disposal of packaging. For this reason, this research suggests the adjustment of the man-object interface which may lead users to make mistakes regarding use and important choices, through adequate handling actions.

Conventional therapy for moderate to severe inflammatory bowel disease: A systematic literature review

BACKGROUND Despite the advent of biological drugs, conventional therapy continues to be used in moderate to severe inflammatory bowel disease(MS-IBD). This study hypothesized that as a standard of treatment and the primary alternative to biologics, conventional therapy should present robust effectiveness results in IBD *** To investigate the effectiveness of conventional therapy for *** A systematic review with no time limit was conducted in July 2017 through the Cochrane Collaboration, MEDLINE, and LILACS databases. The inclusion criteria encompassed meta-analyses, systematic reviews, randomized clinical trials, observational and case-control studies concerning conventional therapy in adult patients with MS-IBD, including Crohn's disease(CD) and ulcerative colitis(UC). Corticosteroids(prednisone, hydrocortisone, budesonide, prednisolone,dexamethasone), 5-aminosalicylic acid(5-ASA) derivatives(mesalazine and sulfasalazine) and immunosuppressants [azathioprine(AZA), methotrexate(MTX), mycophenolate, cyclosporine, tacrolimus, 6-mercaptopurine(6-MP)] were considered conventional therapy. The exclusion criteria were sample size below50; narrative reviews; specific subpopulations(e.g., pregnant women,comorbidities); studies on postoperative IBD; and languages other than English,Spanish, French or Portuguese. The primary outcome measures were clinical remission(induction or maintenance), clinical response and mucosal healing. As secondary outcomes, fecal calprotectin, hospitalization, death, and surgeries were analyzed. The quality of the evidence was assessed using the Grading of Recommendations Assessment, Development and Evaluation *** The search strategy identified 1995 citations, of which 27 were considered eligible(7 meta-analyses, 20 individual studies).
