Association of anastomotic leakage with long-term oncologic outcomes of patients with esophagogastric junction cancer

BACKGROUND Despite improvements in surgical procedures and peri-operative patients management,the postoperative complications in esophagogastric junction(EGJ)cancer remain high because of technical *** studies have indicated the negative influence of postoperative infectious complications on long-term survival after gastrointestinal ***,no study has shown the association between postoperative complications and long-term survival of patients with EGJ *** To elucidate influence of postoperative complications on the long-term outcomes of patients with EGJ *** A total of 122 patients who underwent surgery for EGJ cancer at the Keio University were included in this *** examined the association between complications and long-term oncologic *** In all patients,the 3-year overall survival(OS)rate was 71.9%,and the recurrencefree survival(RFS)rate was 67.5%.Compared with patients without anastomotic leakage,those with anastomotic leakage had poor median OS(8 mo vs not reached,P=0.028)and median RFS(5 mo vs not reached,P=0.055).Among patients with cervical anastomosis,there were not significant differences between patients with and without anastomotic ***,among patients who underwent intrathoracic anastomosis,patients with anastomotic leakage had significantly worse OS(P=0.002)and RFS(P=0.005).CONCLUSION Anastomotic leakage was significantly associated with long-term oncologic outcomes of patients with EGJ cancer,especially those who underwent intrathoracic *** anastomosis with subtotal esophagectomy may be an option for the patients who are at high risk for anastomotic leakage.

Introduction:?Ovarian borderline clear cell tumor is a very rare tumor accounting for less than 1% of borderline ovarian tumors. Currently, only 30 cases of borderline clear cell tumors have been reported.?This borderline ovarian clear cell tumor often arises from endometriosis. But there are no definitive guidelines for the management of endometriosis with malignant transformation due to a lack of understanding of the mechanistic details that mediate the transformation of endometriosis into ovarian cancer. Case: A?52-year-old woman (G2 P2) who periodically underwent ultraso-nography over the course of 12 years in order to follow the endometrioma present in her left ovary. The diameter of the lesion was 46 mm and it remained largely unchanged. However, a solid region within the ovarian cyst was detected. After the surgery, the final pathological diagnosis was cystic clear cell tumor of borderline malignancy.?Conclusion: It reported that risk factors of malignant transformation are older age, postmenopausal status, and larger tumor diameters were associated with a higher prevalence of cancer development. But it is recommended that patients with endometriosis, regardless of patient age or tumor size, undergo careful monitoring to detect changes in growth or the formation of solid regions for as long as the lesion is present.

Endoscopy is now being used for breast cancer surgery. Though it is used for mastectomy, lymph node dissection, and breast reconstruction, its prime use is for mastectomy. Because an incision can be placed inconspicuously in the axillary site, a relatively largeincision can be created. A retractor with an endoscope, CO2, and an abrasion device with the endoscope are used for operation space security. It is extremely rare that an endoscope is used for lymph node dissection. For breast reconstruction, it may be used for latissimus muscle flap making, but an endoscope is rarely used for other reconstructions. Endoscopic mastectomy is limited to certain institutions and practiced hands, and it has not been significantly developed in breast cancer surgery. On the other hand, endoscopic surgery may be used widely in breast reconstruction. With respect to the spread of robotic surgery, many factors remain uncertain.

The Efficacy and Safety of Continuous Popliteal Sciatic Nerve Block for the Relief of Pain Associated with Critical Limb Ischemia: A Retrospective Study

Background: Patients with critical limb ischemia (CLI) often suffer from severe pain. A continuous peripheral nerve block has been shown to provide effective analgesia for patients having lower limb surgery. We have been administering continuous sciatic nerve block (CSNB) for patients with CLI whose pain could not be relieved by other analgesic tools. The aim of this retrospective study was to investigate the efficacy and safety of CSNB for patients with CLI. Method: We retrospectively investigated 99 patients who received CSNB for the relief of severe pain in the lower limb associated with CLI. Patient demographics, neurological history, complications, and subjective evaluation of the effectiveness of CSNB were investigated from their clinical records. The distal tips of 108 catheters were cultured. Result: One hundred and seventy-two catheters were placed in 99 patients. More than 90% of the patients enjoyed considerable relief of severe pain. The analgesic effect of CSNB was greater in patients with older age and hemodialysis. Thirty-one catheters had positive bacterial colonization. However, no severe infectious complication was found. There was no relationship between the co-existence of diabetes and positive bacterial colonization. We encountered a patient with ASO and diabetes who suffered from persistent motor weakness and hypesthesia even after 3 months of CSNB placement. Conclusions: CSNB provided good pain control for patients with severe pain caused by CLI. Although catheters were frequently found to be colonized, infection at the catheter site was self-limiting even in patients with diabetes.

Background: Increasing the number of DC (Death Conference) experiences, nurses are expected to improve the quality of nurses’ terminal care and developing nursing minds. Therefore, there is a need for further evidence on the impact of repeated DC experiences on nurses’ terminal care. Aim: This study aims to clarify the impact of repeated DC experiences on nurses’ terminal care and nursing in DC using the prepared Death Conference Nursing Self-Assessment Sheet. Method: The study is approved by the Ethics Review Board of St. Mary’s College, Chukyo Gakuin University, and the hospital where the survey was conducted. This study was an observational and cross-sectional study using an anonymous, self-completed questionnaire composed of the newly created Death Conference Nursing Self-Assessment Sheet. Its evaluation items in the Death Conference Nursing Self-Assessment sheet consist of four categories (Cooperation, Future direction of nursing, Specific measures for nursing, and Feeling of the nurse) and 14 questions. The questionnaires were conducted on 144 nurses in the ER, ICU, and Palliative Care Ward (Hospice Ward) in a general hospital in Japan. For analysis, JMP ver.14.2 was used to perform Fisher’s exact test, Wilcoxon/Mann Whitney test, univariate, and multivariate logistic regression analysis. Results: Thirty-four participants (23.6%) who answered the number of DC experiences were included in the final analysis. Seventeen nurses had DC experiences, and 17 had no experience. There were significant differences in all of the above categories impacting nurses’ terminal care and nursing in DC. Conclusion: Repeated DC affected nurses’ terminal care and nursing in DC using the Death Conference Nursing Self-Assessment Sheet. Antioxidant Capacity and Total Phenol Content of Commonly Consumed Indigenous Foods of Asian Tropical Regions

In order to screen the antioxidant capacity of commonly consumed indigenous foods of tropical region, vegetables, fruits, tea, oil seeds, legumes, spices and cereal were analyzed. Among the vegetables, green chili showed the highest total oxygen radical absorbance capacity （ORACFL） and lipophilic ORAC （L-ORACFL）; whereas, bean showed highest hydrophilic ORAC （H-ORACFL） content. The H-ORACFL of vegetable samples varied between 2.26 to 23.08 ~tmol of Trolox equivalent （TE）/g of fresh weight （FW） and L-ORACFL content widely varied from 0.20 （sweet pumpkin） to 16.95 （green chili）. The range of H-ORACFL values of fruits showed large variation （0.45-178 Ixmol of TE/g FW）. All four different types of tea showed high H-ORACFL （1,295.84-2,709.11 lamol of TE/g）, in contrast to negligible amount of L-ORACFL. In legumes, H-ORACFL content ranged from 16.22 to 140.54; whereas, L-ORACFL ranged from 4.43 to 25.78. Porso millets showed higher L-ORACFL value （19.4） as compared to H-ORACFL （0.8）. In case of spices, L-ORACFL content varied from 29.75 ± 0.00 to 1,575.24 ± 39.63, which is considerably higher as compared to H-ORACFL （61.68 ± 1.71 to 272.82± 1.05） content. Comparatively, spices showed much higher H-ORACFL values as compared to fruits analyzed. In case of selective vegetables, fruits and tea samples, the total phenol content was highly correlated with the H-ORACFL content.

Geospatial Area Embedding Based on the Movement Purpose Hypothesis Using Large-Scale Mobility Data from Smart Card

With the deployment of modern infrastructure for public transportation, several studies have analyzed movement patterns of people using smart card data and have characterized different areas. In this paper, we propose the “movement purpose hypothesis” that each movement occurs from two causes: where the person is and what the person wants to do at a given moment. We formulate this hypothesis to a synthesis model in which two network graphs generate a movement network graph. Then we develop two novel-embedding models to assess the hypothesis, and demonstrate that the models obtain a vector representation of a geospatial area using movement patterns of people from large-scale smart card data. We conducted an experiment using smart card data for a large network of railroads in the Kansai region of Japan. We obtained a vector representation of each railroad station and each purpose using the developed embedding models. Results show that network embedding methods are suitable for a large-scale movement of data, and the developed models perform better than existing embedding methods in the task of multi-label classification for train stations on the purpose of use data set. Our proposed models can contribute to the prediction of people flows by discovering underlying representations of geospatial areas from mobility data.
